Abstract

Complex comparative clinico-morphological (histological, histochemical, immunohistochemical) research of 1041 hospital surgical cases of a progressive gastric carcinoma (GC) (1995-2004) with group allocation of three basic (intestinal, diffuse, mixed) histological forms of a tumor are established diagnostically significant general and particular (within histological form) morphological (tissue, cellular) and morpho-functional features of organo-specifically variable parenchymal compartment that testifies the rationality of mixed histological form of growth addition for classification system WHO-P.Lauren (2000).

Lifetime anatomy and topography of the retroperitoneal spaces fascias

Abstract

The purpose of work is studying lifetime computer-tomographic anatomy of the retroperitoneal spaces fascias. As object of research the computer tomograms received from 140 patients without a pathology of organs of a retroperitoneal spaces have served. As a result of carried out research are received morphometrical parameters of all retroperitoneal spaces fascias of the person. Opportunities of their visualization are found out from persons of three age groups, sexual distinctions are studied. The alternative anatomy and topography of everyone fascias of the retroperitoneal spaces is described. Results of work can be useful to surgeons, urologists, morphologists.

INDIVIDUAL DIFFERENCES OF TARGET ORGANS REACTION OF WISTAR RATS TO ACUTE BACTERIAL ENDOTOXICOSIS

Abstract

Individual morphofunctional differences of target organs of Wistar rats to acute endotoxicosis, induced by lypopolisaccharide (LPS) administration in high dose were investigated. By cluster analysis of target organs and cytokines characteristics Wister rats were divided into two groups - with high and low production level of Th1 cytokines. In 24 hours after LPS administration hepatic dystrophic changes and lung inflammation in rats with low level of Th1 cytokines (IL-2, IF-γ) were more expressed than in rats with high level of Th1 cytokines. In 7 days individual morphofunctional differences were less expressed and mainly were found out on functional level

Case management with severe out-patient form of pneumonia audit at the in-patient

Abstract

The aim of this paper is to evaluate the validity of case management of severe out-patient form of pneumonia using the indicators of quality at the in-patient departments in Ryazan. This audit was carried out on the basis of 102 patients data hospitalized to the in-patient departments in Ryazan in 2005-2009 years period. The mean age of patients was 51,85±1,68 years, 61 patients were males and they predominated over females (59,8%). The indicators of quality were taken by us because they have already proved their reliability in Russia and abroad. Initial antibacterial therapy (70,6%) didn't meet modern clinical recommendations. In less than half cases IV antimicrobial injections were used. Only in 24% of cases we gave step-down antimicrobial therapy. The method of clinical scaling to estimate the severity of patients condition was not applied, only in 24,5% of cases we used pulseoxymetry to find out the stage of hypoxemic abnormality

Sudden Coronary death frequency, detectability and statistical recording

Abstract

Specification of Sudden Coronary Death (SCD) prevalence, diagnostics quality and statistical recording in outpatients' clinics. Materials and methods. Comparative analysis of SCD prevalence, recorded in outpatients' clinics and qualified per study algorithm, was performed in 285 736 population (76.4% of them ≥18 лет, 46.0% - males). Results. Qualified SCD frequency accounted 156 per 100 000 male population and 72 per 100 000 female population, that in 2,3 и 2,8 times more then recorded in outpatients' clinics level (р<0,001). 55.6% SCD cases in males and 66.5% - in females were not diagnosed and/or recorded in outpatients' clinics. The reasons of SCD statistical loss are diagnostics defects (45.4%) and medical documents recording mistakes (55.6%). Conclusion. ½ SCD cases in males and 2/3 - in females are not revealed in outpatients' clinics, that causes underestimate of SCD in population.

Comparative characteristics of induced sputum cellular composition in 11mild and 12 moderate stable COPD patients, 30 patients with chronic bronchitis and 13 smokers have been studied. Patients with mild stable COPD were found to have increased absolute number of all cells, including neutrophils and macrophages in the induced sputum in comparison with patients with chronic bronchitis. SPIROMETRY SCREENING IN EVALUATION OF CHRONIC OBSTRUCTIVE PULMONARY DISEASE AT PRIMARY CARE

Abstract

Chronic obstructive pulmonary disease is an acute medical and social problem. Spirometry is a "gold standard" of diagnostics of the disease. In our investigation we studied respiratory function in rural population. A high prevalence of obstruction: FEV1 less than 80% of normal, recorded in 23.5% of the cases and FEV1/FVC <0,7 in 15,1% of the surveyed adult rural population was revealed. Among the males the corresponding figures were respectively 33.6% and 29.9%.

SHORT-TERM AND LONG-TERM RESULTS OF RADICAL R0 MULTIORGAN RESECTIONS FOR ADVANCED STOMACH CANCER

Abstract

The results of surgical treatment of 169 patients with R0 combined resections performed in the period from 2000 to 2005 due to advanced stomach cancer were analyzed. The postoperative morbidity rate was 31,4 ± 3,3%, postoperative mortality rate was- 7,1 ± 1,0%. The overall 5-year survival rate was 24,1 ± 3,1%, median of survival was 16 months. The categories of patients with R0 resections which have not improved the treatment long-term results were identified. This requires a differentiated surgical treatment for patients with advanced stomach cancer

The system aspects of the safety of the operating treatment patients of the high anesthetical risk group using laparoscopic access

In the given research from the point of the system analyses is studied the haemodynamicy as an integral and one of the most leading factors affecting the safety of operating period by invasions under the circumstances of carboxyperitoneum by patients with heart problems. It is shown that similar nosological forms combine heterogenic patients' groups with different system reactions for carboxyperitoneum and surgical stress. EXPERIENCE OF A VARIABLE MAGNETIC FIELD FOR REHABILITATION OF PATIENTS WITH HIGH-condylar fractures PROCESS OF LOWER JAW

Abstract

In the clinic of maxilla-facial surgery of 1st Moscow State Medical University IM Sechenov, a surveillance consisting of 34 patients, who were hospitalized with the diagnosis of high fracture of condylar process of the mandible, was conducted. Application of physical therapy by an alternating magnetic field with a frequency corresponding to the natural frequency of the pulsating microvascular bed, patients with high fractures of the condylar ridge of the mandible showed high efficiency to this technique. The usage of alternating magnetic field is to optimize of the microcirculation in the zone of fracture, which generally contributes to the local healing processes, a favourable postoperative course, the absence of inflammatory complications.
