Improvement of dermatovenereologic care in the armed forces.

Abstract

Authors presented data about primary incidence of diseases of the skin and subcutaneous tissue in the Armed Forces of the Russian Federation over the past 6 years and gave information about basic peculiarities of the delivery of medical dermatovenereologic care to servicemen in peacetime. A list of dermatovenereologic diseases, which have to be treated by the medical service at the troop level. Main factors affecting the level of skin and venereal diseases among servicemen are revealed. Authors considered typical errors occurring in the process of medical care delivery to patients suffering from dermatovenereologic diseases at the troop level. These errors are usually of organizational and methodical nature.

The first experience of transporting a patient with severe respiratory failure in conditions of extracorporeal membrane oxygenation.

Abstract

This publication is the first case report in the Russian Federation aircraft evacuation of a patient with severe respiratory failure due to pneumonia of viral and bacterial etiology with the use of extracorporeal membrane oxygenation. The use of this method helped to stabilize the patient’s condition and safely carry out transportation to a specialized medical institution for further treatment.

Diagnostics and antimicrobial therapy of severe community-acquired pneumonia.

Abstract

In the current paper authors presented the latest information concerning etiology of severe community-acquired pneumonia. Most cases are caused by a relatively small number of pathogenic bacterial and viral natures. The frequency of detection of various pathogens of severe community-acquired pneumonia may vary greatly depending on the region, season and clinical profile of patients, availability of relevant risk factors. Authors presented clinical characteristics of severe community-acquired pneumonia and comparative evaluation of a number of scales to assess the risk of adverse outcome of the disease. Diagnosis of severe community-acquired pneumonia includes the following: collecting of epidemiological history, identification of pneumonia, detection of sepsis and identification of multiple organ dysfunction syndrome, detection of acute respiratory failure, assessment of comorbidity. Authors gave recommendations concerning evaluation of the clinical manifestations of the disease, the use of instrumental and laboratory methods for diagnosis of severe community-acquired pneumonia. To select the mode of antimicrobial therapy is most important local monitoring antimicrobial resistance of pathogens. The main criteria for the effectiveness of treatment are to reduce body temperature, severe intoxication, respiratory and organ failure.

The use of navigation system in the process of hip replacement.

Abstract

Endoprosthesis replacement is the leading surgical procedures among the whole range of procedures performed on hip joint. The optimal position of the acetabular component of prosthesis is an important factor determining the success of the operation. In the Center of Traumatology and Orthopedics of the Burdenko Main Military Clinical Hospital of the Russian Ministry of Defense was developed a hip replacement navigation system. In the given prospective and randomized clinical study acetabular component prosthesis was fixed with the help of «image-free» navigation system (the control group, n=56) and with the help of navigation system developed by the authors (experimental group, n=50). An evaluation of the cup position after the operation was carried out by means of computed tomography of the pelvis. An average value of anteversion in the experimental group was 14±3°, in the control group -13±4°. The average value of the inclination of the experimental group was 44±5°, in the control was 45±4°. Developed navigation system allows us orienting acetabular component of prosthesis properly, reducing time of surgical procedure and obtaining good and excellent functional results after the surgery.

First experience of a polyurethane foam composition «Locus» use to stop intra-abdominal hemmorage as a result of liver damage of V degree. (An experimental study)

Abstract

Today self-expanding polymers are considered as the most promising as means for intracavitary hemostasis in case of continuing bleeding after trauma. Testing of domestic open-cell polyurethane foam composition «Locus» was carried out on the developed experimental model simulating liver trauma of V degree. After damaging 6 experimental rabbits were injected intraperitoneally with 80 ml of the composition. 5 experimental rabbits were included into to control group (haemostatic agent was not given). Estimated blood loss was 111-124 ml. The two-hour survival rate didn’t differ significantly: 3 animals survived in the experimental group; 2 animal survived in the control. Despite the 3-4-fold widening of the foam, due to open cells it absorbed 72,6±8,3 g of blood. Thus, open-cell polyurethane foam intraperitoneal administration of the composition didn’t provide a temporary intra-abdominal hemostasis in liver. In order to enhance the hemostatic effect it requires changing the formulation of the polyurethane composition. For a more accurate assessment of the results it is neccessary to perform additional researches on larger animals.

Diagnostics of pulmonary-renal syndrome in young patients with suspected community-acquired pneumonia

Abstract

In patients of young age atypical clinical course of community-acquired pneumonia may mask of interstitial lung disease caused by systemic vasculitis. Lung damage occurs in the form of vasculitis with perivascular infiltration, interstitial pneumonia symptoms and various sequential processes up to necrosis of the lung tissue and pleurisy. A distinctive feature of vascular pneumonia is probability of development of plurivisceral damage with simultaneous involvement of heart, kidney, abdominal, skin, joints and hepatolienal syndromes into the pathological process. At suspicion on development of systemic vasculitis it requires to perform differential diagnosis with the use of immunological tests and X-ray examination of the chest. Lung disease in systemic vasculitis worsens prognosis and necessitates urgent therapeutic measures.

Effectiveness and safety of antiviral therapy of military personnel suffering from chronic hepatitis C

Abstract

In order to evaluate effectiveness and safety of antiviral therapy schemes examined and treated 191 patients with chronic hepatitis C were assigned standard interferon and ribavirin, pegs-lated interferon and ribavirin, the total duration of the course coput 24-48 weeks. Based on clinical and laboratory parameters evaluated the safety of antiviral therapy. Formation of sustainable viral response, depending on the genotype observed, was given at 58,9-70% of patients. In case of insufficient antiviral therapy was prescribed a second course that will improve the effectiveness of treatment to 90-95%. Correction of adverse events was held lower dosages of interferon and/or ribavirin.

Possibilities of the computed tomography considered as a part of differential diagnosis of pneumonia and pneumonia-like form bronchioloalveolar pulmonary cancer

Abstract

Authors cover a study performed with the aim to determine differential and diagnostic and, in addition, computer tomographic signs of pneumonia-like form of bronchioloalveolar pulmonary cancer and pneumonia. Data on CT and results of diagnosis and treatment of 36 patients with infiltrates in the peripheral segments of the lungs are analysed. The following features were considered as differential and diagnostic: induration of the type «frosted glass», «CT angiography» symptom, «air bronchogram» symptom, «honeycomb lung», bronchial wall thickening proximal of the affected area, trapped air in the normal lung tissue adjacent to the abnormal areas, pleural thickening and retraction at the level of the lesion.

Practical application of sylvinite speleoclimatic camera in the complex health resort treatment of chronic respiratory diseases in children of military personnel

Abstract

On the basis of health resort «Solnechniy» of the Russian Interior Ministry was researched the effect of speleotherapy on the respiratory system of children. A 29% increase of timed inspiratory capacity and a 28% increase of timed expiratory capacity on a daily 60-minute stay in the speleological chamber for 10 days was registered. It was also established a significant difference changes of functional parameters of the respiratory system on the background of speleotherapy in comparison with indicators of children who did not receive speleotherapy, but is under the influence of the same climate and other factors comparable physiotherapy. The study confirmed the efficacy of speleotherapy in the composition of complex sanatorium treatment of children on the Black Sea coast of Russia.

Medical science during the Great Patriotic War.

Abstract

Domestic science contributed significantly to the solution of problems of medicine during the Great Patriotic War. Forms of organization of scientific work in the interests of the front were different: for example, united efforts of physicians to organize a proper work at Scientific Medical Boards directed by the Head of the Main Army Medical Department of the Red Army and the Head of the Health and Sanitary Department of the Navy, as well as Scientific and Hospital boards of the People’s Commissariat of Health of the USSR. At the plenary sessions the heads of these boards considered the most important medical problems of evacuation, treatment, sanitary and disease control and also new methods of treatments of wounded, results of medical services during particular period of war, new tasks and etc. The most prominent scientists and presenters of all leading sectors of healthcare worked at these boards, that allowed developing, testing and implementing of the latest achievements of medical science.

Abstract

In the current paper authors discuss problems of marine and aerospace medicine and psychophysiology, which Georgii Zarakovskii (1925-2014), a prominent domestic experts in the field of military medicine, psychology and ergonomics, solved. Authors focused on methodological approaches and results of the study of psychophysiological characteristics and human capabilities took into account for design of tools and organization of flight crews, astronauts and military experts. Authors marked the contribution to the creation of a system integrating psychophysiological features and characteristics of the person neccessary for development, testing and maintenance of aerospace engineering and organization of its professional activities. The possibilities of using the methodology of psychophysiological activity analysis in order to improve the reliability of psychophysiological military specialists are shown.
