Battle-field surgery in the beginning of 21st century

Abstract

The authors underwent an analysis of doctrine changes of battle-field surgery. The terminology is specified, the ways of improvement of effectiveness and quality of medical care for the wounded, taking into account the new doctrine, are formulated. In accordance with Federal law № 323-FZ of 2011.11.21 the term «reduced specialized care» is suggested for description of medical care at medical forward treatment unit instead of «qualified aid».

The article presents a comparative characteristics of domestic and foreign approaches to the static analysis of losses of surgical profile in military conflicts. The ways of improvements of statistical materials on military losses for improvement of surgical aid delivery are mapped out. The authors proved the need to establish the register of combat trauma.

Organizational and therapeutic aspects of the elimination of the medical consequences of chemical accidents

Abstract

During the liquidation of consequences of chemical accidents the main task is minimization of the population losses and the provision of effective medical care to the victims. Training experience related to chemical accidents, shows the need for constant readiness of health facilities in the area of the chemical industry accommodation, planning of the medical forces and resources reserve. The most important elements of medical care in chemical accidents are affected sorting, intensive therapy and reanimation in cases of severe injuries, the organization of the observation of persons with initial minor manifestations of intoxication. Emergency assistance should be provided in full volume as soon as possible. It is essential to provide enough oxygen and required drugs (hormonal drugs, diuretics, antibiotics, anticonvulsants, analgesics, and others). Multistage evacuation should be excluded and should be done the planned training within the medical service stuff with toxicological profile.

Recovery after deep defects of the fingers covering tissues in daily surgical hospital.

Abstract

The article describes the experience of reconstructive and restorative operative treatment of patients with defects of the skin of the fingers at the daily surgical hospital. It is shown that surgeons applying standard instruments and having additional training in hand surgery, using a technique non - free skin plastic flap on a temporary pedicle and using inpatient technology, get cosmetic and functional results compared to those of treatment in hospitals. Cited in the article data on the direct and separate treatment outcomes, quality of life of patients, the estimated non-specific and organ-specific method, clearly show that non-free skin plastic flap on a temporary pedicle - a reliable and effective method of restoration of the skin fingers defects which can be accompanied wit the use of resource-saving technologies in health care.

Zika virus infection: clinical and diagnostic aspects and organization of preventive measures

Abstract

The article presents modern data about Zika virus outbreak, the biological characteristics of the pathogen, vectors, the nature of the epidemic process, pathogenesis and diagnosis of infection. This infection has a high potential for epidemic spread because vectors are widely represented in the fauna of many climatic and geographic zones. It presents information on the main clinical manifestations of the disease. Particular attention is paid to the problem of congenital malformations of the nervous system (microcephaly and others) detected in infants born to women infected with Zika virus. The basis of therapy in this disease constitute a pathogenic agent. The package of measures for the prevention of disease caused by Zika virus, including early identification and treatment of patients, as well as measures for the destruction of the virus vectors.

Experience in health prevention work at the accident at the Chernobyl nuclear power plant.

Abstract

One of the main conditions for successful completion of liquidation tasks in the consequences of the accident at the Chernobyl nuclear power plant is of the swipe-effective sanitary and anti-epidemic (preventive) measures. The main directions of medical services in this area include the prevention of acute radiation injuries of personnel and distant effects of ionizing radiation, prevention of epidemics of acute intestinal, respiratory and other infections in the armed forces, pro-prevention of injuries, poisonings, accidents. Medical preventive measures were performed by the Medical Service, which took part in liquidation of consequences of military units, the specialists of the Operational Group of Civil Defense, the sanitary-epidemiological units and institutions formed the Belarusian, Kiev and Odessa Military District. Sanitary-epidemiological (preventive) activities carried out during the accident at the Chernobyl nuclear power plant, provided the sanitary-epidemiological well-being of the troops, which allowed to perform their tasks.

Abstract

The current article is a brief review of the 2015 foreign scientific publications dedicated to medical provision in the Arctic. The review begins from the problem of water supply. Then the publications from the field of food supply, epidemiology and infectious diseases, suicide, and telemedicine are reviewed in a shorter manner. In conclusion it is pointed out that diversity and uniqueness of the Arctic medicine problems require complex approach for the problems solution which can be accomplished by establishing a specialized scientific practical center.

Organization of medical evacuation support of the Red Army during the Great Patriotic war: lessons and conclusions

Abstract

The article analyses the experience of medical support of the Red Army troops during the Great Patriotic War. The data characterizing particular forms and methods of work of the medical service of different fronts in the dynamics of the operations in different periods of the war is given. Taken in the Red Army’s principles of medical support and the organizational and staff structure of the health service as a whole showed its viability in various conditions of defensive and offensive operations. At the same time, the activities of the medical service needed to make adjustments on the basis of the provisions of the unified military field medical doctrine. A system of medical and evacuation support was created, successfully operated and was improved during the war. The lessons and experiences of medical support in the Great Patriotic War, its comparison with modern problems of the military medicine make it possible to identify the direction of improvement of activities of the medical service in the performance of modern problems by the Armed Forces.

Abstract

The article is a report on the conference «Innovative methods of treatment of cardiac patients in a multi-hospital», which took place at the 3rd Central Military Clinical Hospital n.a. A.A.Vishnevskiy and dedicated to the 40th anniversary of the cardiology hospital center. The reports covered achievements, the latest developments introduced into the practice of medical and diagnostic work of the hospital and its affiliates with patients of cardiovascular profile.
