Planning and organisation of allo-grafting performed on the soldier’s face

Abstract

According to the experience of military conflicts facial trauma with the loss of person’s social function occurs in 4,5-5% of patients with such injury. In most cases, these injuries can be fixed with the help of reconstructive plastic surgery using their own tissue. However, some of the wounded to various reasons the use of autologous tissue is not possible or does not allow adequately restoring the appearance and function of the person. In such cases the only way is a donor allograft. There are just over 3 dozens of such successful operations, which indicates of their uniqueness and complexity. In May 2015 at the S.M.Kirov Military Medical Academy for the first time in our country made a successful transplantation of a difficult graft to a conscript, who was injured during the duty. The article presents the basic directions of the planning and organization of this unique operation.

The article presents current views on the classification of chronic viral hepatitis. The characteristic of methodical diagnostic approaches is presented. The attention to the etiological pathology verification is paid. The results of study on clinical characteristics of patients with chronic viral hepatitis are used as the basis for the developed criteria for evaluating fitness for military service.

Abstract

The results of comparative analysis of domestic and foreign means of the first aid on the battlefield and in the epicentre of emergency situation are presented. It was found that the first aid means used in the Armed Forces are effective and allow performing specified activities, regardless of the qualifications and assisting skills. Modern innovative means of providing emergency and urgent care may be used on the stages of medical evacuation of military and military hospitals.

Experience of the treatment of gunshot wounds and pancreas traumas during domestic armed conflict in the North Caucasus (1994-1996, 1999-2002)

Abstract

The paper presents our own experience of surgical treatment of gunshot wounds, and pancreatic damage. Pathophysiological mechanisms of development of fire pancreatitis are presented. Tasks for surgery and their solutions are defined. The reasons for reoperation are analysed. It was found that the surgical approach depends on the nature and localization of pancreatic damage. The features of the surgical intervention are shown.

Assessment of the influence of the balneo-physiotherapeutic procedures on microcirculatory blood flow in patients with occlusive diseases of the great arteries

Abstract

A comprehensive program of rehabilitation treatment of patients with atherosclerosis of the arteries of the lower extremities, including over venous laser light, gradient magnetic field on the collar region and calf muscles, dry carbon dioxide baths alternating with baths with horse chestnut. The program is pathogenetically justified the use of its high-performance, regardless of the predominance of a clinical syndrome. This allows recommending it for widespread use in clinical practice, including sanatorium conditions. Some methods that are included in a comprehensive program, in accordance with the proposed algorithm can be used in patients with atherosclerosis of the arteries of the lower extremities with the predominance of individual symptom complex.

Effectiveness of infrared-terahertz radiation with acupuncture in the initial period of ischemic stroke

Abstract

This article presents the data of observational retrospective clinical investigation aiming at generalization of the experience of usage of infrared terahertz radiation combined with acupuncture for treatment of the patients in the initial period of ischemic stroke. The experimental group contained 30 patients which had various clinical problems at the initial period of hospitalization. The attempts to dissolve these problems within the acceptable period of time employing domestic and international standard treatment approaches failed. The control group consisted of 31 patients with uncomplicated course of stroke. The patients of the experimental group in addition to the standard treatment received infrared terahertz treatment (device «IК-Dipol») combined with acupuncture. Despite the fact that the condition of the patients in the experimental group at admission was much serious than in the control group, at the time of discharge the level of functioning of the first group was definitely higher than the second. In 6 months that superiority increased and has been retained during two years of following up. Despite the fact that the employed in the current investigation method has been used for dissolving different clinical problems, it has been resulted in increase of the general level of functioning proved by the dynamics of such an integral criterion as index Barthel. This means that employment of infrared terahertz radiation in the stroke patients possesses the systemic effect. To recommend the discussed method for treatment of all patients in the initial period of stroke as well as its repeated employment during rehabilitation the separate prospective randomized clinical trials should be undertaken.

Epileptic seizures in servicemen

Abstract

The analysis of the survey data and treatment of 73 soldiers with a diagnosis «Epilepsy. Status epilepticus». A large part of soldiers when calling-up concealed the disease at the call-up commission or were wrongly found fit for military service. In some soldiers the onset of epilepsy coincided with the adaptation period (first three months), and was associated with the performance of military duties. Revealed insufficient general-purpose modern anti-epileptic drugs. When conscription case of dispute, it is recommended to use EEG-video monitoring and electronic data outpatients.

Experience of the use of complex liquid pyo bacteriophage as a mean of prophylaxis of the acute tonsilitis in a military collective

Abstract

In 2015-2016 was evaluated the effectiveness of an integrated liquid pyo bacteriophage in the outbreak of acute tonsillitis among conscripts as a mean of emergency prevention, reduction of disease incidence and communication prevention. The authors have studied an epidemiological efficacy and safety of the drug , and determined an accuracy of the cause-and-effect link on the epidemic process. Based on the results of the study, it has been concluded that the use of the complex liquid pyo bacteriophage in acute tonsillitis focuses for emergency prevention of diseases is effective, has been proved a significant reduction of military morbidity. The use of bacteriophages, including pyo bakteriopahge, is a promising direction of prevention of infectious diseases of bacterial aetiology in military units.

According to numerous chemical-toxicological and forensic researches it have been sildenafil and tadalafil in a variety of dietary food supplements. Falsified dietary food supplements are essentially a substitute for drugs - phosphodiesterase type 5 inhibitor, but are produced with unknown composition and in unknown dose of synthetic pharmaceutical substances. This can cause consumers unpredictable negative consequences, including death.

Evaluated the clinical and immunological effectiveness of spa treatment 49 patients with bronchial asthma and obesity by cranial electrotherapy stimulation and baths with biolong. The changes revealed immune and cytokine status indicate a decrease in systemic inflammation and activation of the immune system response to an increase in the reserve capacity of the organism compared to patients treated with standard spa treatment.

Paraneoplastic skin itch in case of lymphoproliterative diseases

Abstract

Studied the incidence and nature of paraneoplastic pruritus in patients with lymphoproliferative diseases. Paraneoplastic pruritus was observed in patients with Hodgkin’s lymphoma, diffuse large cell lymphoma, follicular lymphoma, chronic lymphocytic leukaemia and multiple myeloma. Among these haematological malignancies skin itch was detected in patients with Hodgkin’s lymphoma (23,9%), and other lymphoproliferative diseases - from 4 to 9,8% of cases.

Use of silicone membranes for the treatment of postoperative wounds in the vestibule of the mouth

Vestibuloplasty of the postoperative wound in the vestibule of the mouth with silicone complex in 20 patients with edentulous jaw allowed forming a deep arches of the mouth, which was at the time of removal of percutaneous sutures 9,5±3,7 mm. The technique is easy to perform, has predictable results and can be used for preorthopedic surgical preparation.

Abstract

The history of development of the policlinic of the branch N 6 of the Federal Government Budgetary Institution «the 3rd Vishnevskiy Central Military Hospital» of the Ministry of Defence, which dates back to 1976, when the 9th clinic of the Moscow garrison was formed. Over the years, repeatedly changed the organizational and staff structure and the name of the clinic (9th consultative-diagnostic polyclinic, polyclinic of the 52nd Consultative Diagnostic Centre MF), continuously improved patient care technology. Currently, the clinic deployed 26 medical-diagnostic branches and offices, with more than 200 employees. Among them, 1 Honoured doctor, 5 candidates of medical sciences, 50% of doctors and 65% of nurses have the highest qualification category. To health care is more than 22 thousand people.

Abstract

The Medical Service of the Central Military District has organized medical support during lead-time and conduct of the strategic command post exercise «Center-2015». At the area of medical responsibility of the branch N 3 of the 426th military hospital of the Ministry of Defence is organized the interaction with the Ministry of Orenburg region and the regional centre of disaster medicine in order to ensure that medical activities carried out at the training area «Donguz». Was organized a medical service forces setting, designed to ensure medical teaching. Well-planned activities and training of personnel allowed organizing high-level medical care, preventing host of diseases and injuries among military personnel being involved into training.
