TREATMENT FOR ACUTE CORONARY SYNDROME WITH ST-SEGMENT ELEVATION: PROGRESS AND PERSPECTIVES
Abstract
The main line of treatment for acute coronary syndrome with ST-segment elevation is to restore blood flow along infarct-related artery by thrombolytic therapy or percutaneous coronary intervention (PCI) as soon as possible. The choice of a reperfusion technique is determined by the capacities of a specific health care facility or a possibility of referring a patient to an institution that is able to perform PCI. The time of the onset of reperfusion can be reduced by prehospital thrombolysis. Due to the complexity of performing emergency PCI in our country, this strategy may become saving to an enormous number of patients.

Сapacity for work in patients with dilated cardiomyopathy during three-year atenolol therapy
Abstract
Addition of atenolol to therapy for severe chronic heart failure in patients with dilated cardiomuopathy leads to a marked significant improvement of capacity for work throughout the follow-up. The p-adrenoceptor blocker was given in patients with persistent circulatory insufficiency for at least 6 weeks and during unchanged therapy with angiotensin-converting enzyme inhibitors and diuretics.

Сhoice of antithr0mb0lytic therapy for patients of different ages and warfarin therapy adherence in atrial fibrillation patients aged over 75 years
Abstract
Atrial fibrillation (AF) is the most common cardiac arrhythmia. In many patients with both paroxysmal and persistent AF, it is a cause of serious complications due to hemodynamic disorders, requires preventive therapy to reduce a risk for thromboembolic events, including stroke, of which this arrhythmia is a pathogenetic factor. A number of randomized studies have provided evidence that the risk of thromboembolic events, stroke in particular, can be reduced by anticoagulant and antiaggregatory therapy. This investigation has studied the frequency of prescribing warfarin and aspirin to patients of different ages and anticoagulant therapy adherence in persons aged over 75 years.

METABOLIC SYNDROME AND NON-ALCOHOLIC ADIPOSE HEPATOSIS
Abstract
The paper considers the basic pathogenetic mechanisms of development of the metabolic syndrome and its association with non-alcoholic adipose hepatosis. It presents the current classifications of the metabolic syndrome and non-alcoholic adipose hepatosis. The leading role in the pathogenesis of these conditions is played by the insulin resistance phenomenon. The present-day recommendations are given on the treatment of the above pathology. The data of the study aiming at examining the liver in patients with the metabolic syndrome and the evaluation of the impact of metformin therapy on this cohort of patients are shown.

USE OF PEPSAN-R IN THE TREATMENT OF GASTROESOPHAGEAL REFLUX DISEASE
Abstract
The anti-inflammatory effect of Pepsan-R that is new to the Russian Federation, used as monotherapy in 50 patients with 0-first degree gastroesophageal reflux disease (GERD), was evaluated. The drug contains 2 active ingredients: guaiazulene and dimeticone. The mechanism of its action is to suppress mast cell degranulation and to generate a protective layer on the gastroesophageal mucosa. The efficacy of the agent was clinically shown in 100% of the patients (10% of them needed larger doses of the drug). Four-week positive changes were morphoendoscopically seen in 50% of the patients. No side effects were recorded

LONG-TERM RESULTS OF ERADICATION THERAPY FOR H. pylori IN ATROPHIC GASTRITIS
Abstract
Atrophic gastritis is an indication for compulsory eradication therapy for H. pylori, which should be considered as a means of active prophylaxis of gastric cancer. The paper discusses outcomes 18 months after successful therapy against Helicobacter pylori. A histological study indicated that regression of atrophy was not achieved and no progression of atrophy and intestinal metaplasia was seen. Gastrin 17 levels and pensinogen Vll ratio significantly increased as compared with these in the control group (receiving no eradication therapy).

Abstract
The development of Helicobacter pylori resistance to antibiotic therapy, the high proportion of reinfection and relapses, and side effects after eradication therapy require new approaches to treating peptic ulcer. To restore of psychovegetative balance and neurohormonal regulation, a complex of therapy for peptic ulcer was added by Grandaxin having an autonomic modulator effect, which potentiated the effect of the traditional therapy.

ABDOMINAL PAIN IN CHILDREN WITH IMPAIRED FUNCTIONS OF THE DIGESTIVE TRACT
Abstract
The most common causes of abdominal functional pain are functional diseases: functional dyspepsia, irritable bowel syndrome, functional abdominal pain. In infants of the first year of life, the condition having the similar symptoms is called infantile colic. The paper considers the treatment of colic that is observed in almost 90% of the infants and disappears by 3-4 months of age in most cases.

COMPARATIVE EFFICACY OF ORAL BISPHOSPHONATES IN POSTMENOPAUSAL OSTEOPOROSIS(according to the results of the VIBE retrospective study)
Abstract
The VIBE retrospective observational cohort study demonstrated that that risks for extravertebral and femoral neck fractures tended to reduce in patients given oral bisphosphonates for a year. This study confirmed that ibandronate was as effective as other bisphosphonates, such as alendronate and residronate. The risk of vertebral fractures turned out to be much less in compliant patients who received ibandronate in a dose of 150 mg once monthly than those who took other bisphosphonates weekly

SPECIFIC FEATURES OF THERAPY FOR ERECTILE DYSFNCTI0N IN PATIENTS WITH HYPOGONADISM
Abstract
Since erectile dysfunction is frequently accompanied by hypogonadism, this study has been undertaken to define indications for combined therapy with phosphodiesterase type V inhibitors in combination with androgens. Androgen monotherapy have turned out to be effective in 67.6% of patients. It is advisable to use combined therapy to patients with severe and moderate erectile dysfunction, diabetes mellitus and/or coronary heart disease shortly after these conditions are diagnosed. However, there is an earlier occurrence of benefits from therapy with phosphodiesterase type V inhibitors if androgens have been previously used.

OPTIMIZATION OF A PROCEDURE FOR TROCAR LAPAROSCOPIC FIBR0CH0LED0CH0SC0PY AND LITHOEXTRACTION IN SURGERY FOR CH0LED0CH0LITHIASIS
Abstract
Due to a progressive rise in the incidence of cholelithiasis (CL) and in the number of patients with choledocholithiasis, it is necessary to improve the diagnosis and treatment of this pathology. For this, a trocar was designed and a procedure was developed for trocar fibrocholedochoscopy and lithoextraction that was used in the video-assisted endolaparoscopic surgical treatment in 16 patients with choledocholithiasis-complicated CL. Trocar fibrocholedochoscopy and lithoextraction simplify this stage of surgical intervention and improve both the diagnosis of choledocholithiasis and the results of surgical treatment.

ASSESSMENT OF THE TREND IN PRIMARY HEALTH CARE MANPOWER RESOURCES AND THEIR INFLUENCE ON THE POPULATION'S HEALTH
Abstract
The manpower resources that provide primary health care in the Sverdlovsk Region were estimated. In the past 3 years, the number of local physicians has increased; their qualification improved. However, these continue to be as high as 47.6% of the required number; significant intermunicipal differences remain in the provision with local therapists and pediatricians. It is concluded that the measures implemented within the framework of the priority national Health project have yielded no expected effect so far.
