DIURETIC ABUSE: A NEPHROLOGIST’S VIEW

Abstract

Diuretic abuse is frequently observed in individuals who strictly control their weight, in young women in particular. Patients with anorexia nervosa often use diuretics and laxatives to detoxify the body. The uncontrolled use of diuretics (commonly in large doses) is associated with serious somatic abnormalities, such as secondary hyperaldosteronism, electrolyte imbalance, pseudoidiopathic edemas, secondary gout, osteoporosis, and interstitial nephritis. Diuretic abuse should be differentiated from Gitelman’s syndrome, Bartteґs syndrome, and idiopathic edemas. The basis for treatment for diuretic abuse is psychiatric care and diuretic discontinuation via spironolactone suppression of renin-angiotensin-aldosterone system activity.

The role of radiofrequency ablation in the treatment was assessed in 41 patients with liver metastases from colorectal cancer. The immediate and late results of treatment and the impact of the number and sizes of hepatic foci on its late results were studied. Three- and five-year overall survival was 32,4 and 16,2%, respectively. The worst prognosis was noted in patients with 6 hepatic foci or more and with those measuring more than 5 cm.

Abstract

In the period 2005 to 2010, the N.N. Burdenko Faculty Surgery Clinic carried out gastrectomy in 178 patients, of whom 65 (35,6%) had cardioesophageal cancer (CEC). Transhiatal operations were performed in all patients with CEC. Postoperative complications developed in 23 (35,4%) patients. The pattern of postoperative complications showed a preponderance of extraabdominal complications, including cardiopulmonary ones, identified in 13 patients; esophageal anastomotic incompetence was noted in 6. In addition, there was acute pancreatitis (n=4), intra-abdominal abscess (n=2), cardiovascular events (n=3), adhesive obstruction (n=2), postoperative wound suppuration (n=2), and peritonitis (n=1). A fatal postoperative outcome occurred in 2 (3,1%) patients

ANEMIA TREATMENT IN PATIENTS WITH STAGES III-V CHRONIC RENAL DISEASE

Abstract

The paper gives the results of using the original drug Venofer® to treat anemia in 146 patients with chronic renal disease, of whom 25 were on programmed hemodialysis (HD). The patients were followed up during a year, over which they received the original epoetin preparation Recormon subcutaneously and the orgininal iron drug Venofer ® intravenously. All the 146 patients with nephrogenic anemia who used epoetin preparations (Recormon) and intravenous iron (Venofer ®) could achieve the goal hemoglobin level of 110-120 mg/dl, thereafter the weekly dose of Recormon was halved compared to its starting dose of 60 IU/kg/week and the constancy of the body's iron level (TSAT>20%) in all the patients could be maintained with Venofer® 100 mg once every two weeks. In the patients on programmed HD, Venofer® was administered in a dose of 40 mg after each session or 100-200 mg once weekly. No serious complications were noted with the standard doses of Venofer ®.

Abstract

Potassium and magnesium enter, in an important manner, into the regulation of cardiovascular and central nervous system functions, afford the normal course of numerous metabolic biochemical processes, control neuromuscular transmission, positively affect phosphorus and calcium metabolism, and maintain electrolyte balance in the body. Potassium and magnesium imbalance belongs to the most common electrolyte disorders. So the preparations of these macroelements are used in a wide range of indications in both the treatment of primarily cardiovascular diseases and the prevention of hypokalemia and hypomagnesemia particularly in patients taking diuretics (thiazide and especially loop ones) and/ or cardiac glycosides (digitalis preparations). It is appropriate to coadminister the salts of potassium and magnesium since the metabolism of the macroelements is closely related. Asparaginic acid potassium and magnesium salts have been widely used in clinical practice. Panangin is one of the most effective drugs that have a proper balance of potassium and magnesium.

An association was studied between endothelin-1 and resistin levels and insulin resistance in patients with varying degrees of carbohydrate metabolic disturbances. There was an elevated resistin level in women and in those with glucose intolerance (GI) or type 2 diabetes mellitus (DM) and in men with GI only. The most marked increase in endothelin-1 level was found in women with GI. The level of endothelin-1 was directly related to that of resistin in insulin-resistant men.
