MODERN TRENDS FOR USE OF PROTECTED AMINOPENICILLINS IN THE TREATMENT OF RESPIRATORY TRACT INFECTIONS IN AMBULATORY PRACTICE

Abstract

The combination of amoxicillin with clavulanate is used in clinical practice for treatment of respiratory infections for over 30 years. A wide range of antimicrobial activity, including the main respiratory pathogens (Streptococcus pneumoniae, Haemophilus influenzae and Moraxella catarrhalis), and favorable pharmacokinetic and pharmacodynamic (PK/PD) parameters after oral administration provide a high clinical and microbiological efficacy of amoxicillin/clavulanate. At the same time, the occurrence and spread of penicillin-resistant pneumococcal strains have required further optimization of PK/PD parameters of amoxicillin/clavulanate, which led to the creation of new high-dosage form of amoxicillin/clavulanate with modified release of amoxicillin, such as Augmentin SR, registered in Russia in 2010. Review discusses the characteristics of the drug and the prospects its clinical application for acute respiratory infections in adult patients in ambulatory practice.

ANTIBIOTIC THERAPY OF INFECTIOUS EXACERBATIONS OF CHRONIC OBSTRUCTIVE PULMONARY DISEASE: PATIENTS WITH THE RISK OF POOR RESPONSE TO ANTIBIOTICS

Abstract

The article discusses the general and particular problems of treatment and prevention of infectious exacerbations of chronic obstructive pulmonary disease (COPD) in terms of optimizing the choice of antibiotic therapy. Applicability of use of respiratory fluoroquinolones, particularly levofloxacin, for the treatment of exacerbations of COPD and chronic bronchitis is validated. It is emphasized that the effectiveness of antibacterial drugs in a patient with infectious exacerbation of COPD, ultimately is defined by prolonged recurrence-free period of the disease. With this context, levofloxacin can be considered as optimal antibacterial drug for the treatment of exacerbations of COPD, because the main goal of antibiotic therapy in these patients - eradication of etiologically significant microorganisms. Prolonged recurrence-free period provides a low demand for antibacterial drugs, reduces the number of doctor visits and hospitalization rate, thereby improves the quality of life and reduces treatment costs.

Abstract

Based on numerous literatures, article shows a variety of clinical options for course of different forms of brucellosis infection and presents the pathogenetic basis of polymorphism of clinical manifestations. The basic principles of diagnosis of this disease are considered. Because the clinical picture is characterized by marked polymorphism, especially in the acute stage, brucellosis should be included in the range of differential diagnosis in marginal cases, particularly given appropriate epidemiological prerequisites for suspicion.

LEVOFLOXACIN (TAVANIC) IN THE TREATMENT OF PNEUMONIA

Abstract

The article is dedicated to the problems of diagnosis of pneumonia from the standpoint of evidence-based medicine. Data for the etiologic agents of pneumonia and the spectrum of action of levofloxacin (tavanic) are presented. Features of the bioavailability and pharmacokinetics allow to apply this drug once a day that provides high treatment compliance. Administration of tavanic for the treatment of more than 400 patients with pneumonia in a pulmonary unit have shown that the drug is highly effective and well tolerated. The criteria of application of tavanic, different dosage regimens for pneumonias of varying severity are considered. The schemes of administration of tavanic as step monotherapy and in the treatment of critically serious patients are presented.

PNEUMONIA IN YOUNG AND MIDDLE AGE PATIENTS: DIAGNOSIS AND TREATMENT

Abstract

The article is dedicated to the problems of diagnosis of pneumonia from point of view of evidence-based medicine. The extent of necessary examinations to confirm the diagnosis and clarification of etiologic agents is considered. The features of radiographic data in pneumonias caused by different etiological agents and data for the etiology of community-acquired pneumonia are presented. High frequency of mycoplasmal and chlamydial pneumonia is one of the features of respiratory tract disorders in young and middle age patients. Various clinical and epidemiological situations specific to different pathogens are listed. The choice of first-line antibiotic for treatment of pneumonia is considered. Most effective in suppression of various microorganisms antibacterial drugs are discussed. Programs of antibacterial therapy of pneumonia in out-patient and hospital settings are framed. The optimal time periods for antibiotic therapy are determined.

Chronic obstructive pulmonary disease (COPD) is characterized by considerable heterogeneity of clinical presentation and progression of the disease. Current diagnostic indicators (in particular, forced expiratory volume in first second) can not fully describe this heterogeneity; therefore, introduction of concept of COPD phenotype is necessary. Identification of individual COPD phenotypes not only explain the different course of the disease, but help in selection the most appropriate therapy for each patient, that in turn will improve the quality of life and prognosis in patients with COPD.

Abstract

Bronchial asthma (BA) is one of the most common diseases in modern society; ineffective treatment can significantly reduce patients' quality of life, and in some cases lead to hospitalization in the intensive care unit, and even death. In 1995, a program Global Initiative for Asthma (GINA) combining the results of various studies on the diagnosis and treatment of asthma was developed. Taking into account the recommendations of GINA-2006, the main objective of BA treatment is achievement of control of the disease, which provides absent or minimally expressed symptoms, no limitations in daily activity, no or minimal need for emergency medications, and extremely low frequency of exacerbations. The article reviews current approaches to the treatment of BA.

Abstract

The article presents a case of disseminated angiolupoid Brocq-Pautrier (DABP) associated with active visceral sarcoidosis. Patients with DABP are subjects to detailed examination aimed to diagnosis of visceral lesions, primarily involvement of intrathoracic lymph nodes and lung interstitium. The possibilities of treatment of DABP are discussed.

CEFIXIME. ROLE OF ORAL III GENERATION CEPHALOSPORINS IN EXACERBATION OF CHRONIC OBSTRUCTIVE PULMONARY DISEASE

Abstract

An open prospective study has evaluated the clinical efficacy and safety of cefixime in exacerbation of moderate chronic obstructive pulmonary disease (COPD) in 32 men. The high (84%) efficacy of drug was demonstrated. During the six-month follow-up period, recurrences of COPD exacerbation were reported in 19 % of patients that indicates reliable anti-relapsing role of cefexime. There adverse effects of cefexime required its cancellation were not observed. It is concluded that along with aminopenicillins and protected β-lactamase inhibitors, cefixime, which has a high safety profile and easy to use, can be considered as a drug of choice for treatment of exacerbations of moderate COPD.

In this article presents the results of a study of clinical efficacy of the drug Wobenzym in traditional therapy of community-acquired pneumonia in servicemen organized military groups. Based on a survey 157 patients of community-acquired pneumonia shows the benefits of combination therapy, which promotes a more rapid regression of clinical symptoms and reduce the likelihood of formation of residual radiological changes of lung tissue.

BACTERIOPHAGE PREPARATIONS IN THE TREATMENT AND PREVENTION OF URINARY TRACT INFECTIONS IN PREGNANTS AND PUERPERANTS

Abstract

The article presents the results of study, which was aimed to comparative evaluation of methods of antibiotic therapy in pregnants with urinary tract infections using standard courses of antibacterial therapy and combined treatment with antibiotics and bacteriophages. The study have demonstrated that the inclusion of bacteriophage preparations in the course of antibacterial therapy is the most effective treatment for this disease and reduces the incidence of disease recurrence. In women treated with such combined treatment, complications of pregnancy are rarely, childbirth are more favorable, newborns have higher levels of health and are less likely to develop septic complications.
