Abstract

The article discusses the approaches to examination of patients with fever of unknown origin (FUO), which can masks life-threatening diseases (tuberculosis, malignancies, systemic diseases, etc.). The classification of FUO is presented; underlying causes and options for the clinical picture are considered. It is emphasized that FUO is one of the most topical problem of clinic of internal diseases, which requires not only improvement the management with application of new technologies, but also the cooperation of clinical centers, summarizing their experience in the multicenter clinical trials.

Abstract

Depression is one of the factors that can impair the functional outcome in patients after stroke. The article highlights the problems of diagnosis, clinical manifestation and approaches to the treatment of poststroke depression. Currently, selective serotonin reuptake inhibitors (SSRIs) are more widely used for the treatment of depression in patients with stroke. These include citalopram, escitalopram, fluoxetine, etc. Some authors consider prophylactic use of antidepressants from SSRI group for prevention of development of poststroke depression as reasonable method of treatment, particularly in patients with depressive disorders in anamnesis.

Abstract

The article is dedicated to the features of cognitive impairment in cerebrovascular diseases. Pathogenetic heterogeneity of these states is emphasized. Cerebral vasculitis is the one of the underdiagnosed causes of vascular dementia. An algorithm for evaluating patients with suspected cerebral vasculitis is presented. The therapeutic aspects of cerebrovascular insufficiency, including the use of the drug mexidolum, are discussed.

PROBLEMS OF DISCOGENIC DORSALGIA: PATHOGENESIS, CLINICAL PICTURE AND TREATMENT

Abstract

Despite the wide variety of drugs, the problem of drug therapy of back pain (dorsalgia) remains one of the most topical issues in modern medicine. According to WHO, back pain occurred in nine of ten individuals just once in one's life, each year in about one in four persons, and in 5% of the population dorsalgia lead to prolonged disability. This type of pain is the most frequent reason for seeking care at outpatient level. The feature of back pain is peak incidence in working age (40 years). Its frequency is not increased with age and is not correlated with age-related degenerative changes of the spine. The most frequent location of back pain is the lumbo-sacral region, less frequently it occurs in the thoracic spine. Acute dorsalgia is characterized by duration of pain syndrome about 6 weeks; subacute dorsalgia is characterized by persisting pain during 6-12 weeks; the case of back pain lasting more than 12 weeks is called chronic dorsalgia.

APPLICATION OF NSAIDS IN THE TREATMENT OF PAIN SYNDROMES

Abstract

A fundamental aspect of the problem of pain is its division into two types - acute and chronic; mechanisms of development and approaches to treatment of these types of pain are fundamentally different. To date, more than 30 compounds involved in the mechanisms of excitation and inhibition of nociceptive neurons in the central nervous system are identified. Reduction of synthesis of inflammatory mediators leads to significant analgetic effect. Non-narcotic analgesics and nonsteroidal anti-inflammatory drugs (NSAIDs) have such mechanism of action. In modern medicine, priority is given to NSAID that selectively inhibits cyclooxygenase-2 (COX-2); in contrast to the nonselective COX inhibitors, they have significantly less side effects.

Tension-type headache (TTH) is common pain syndrome in the population. The frequent episodic and chronic tension-type headaches are most important. For the treatment of frequent episodic tension-type headache, non-steroidal anti-inflammatory drugs (NSAIDs) are drugs of choice. The combination of NSAIDs with antispasmodics (Novigan) potentiates the analgesic effect of the drug and does not contribute to the development of drug dependence, which can occur against the background of use of codein-containing preparations

Abstract

In radiculopathy, impact on nerve root leads to focal demyelination, that causes a block or slow impulse conduction along nerve fibers. B vitamins (thiamin, pyridoxine and cyanocobalamin), which action is directed at restoration and maintenance the normal structure of myelin sheath of peripheral nerves are the basis of neuroprotective therapy. The therapeutic effect of B vitamins is also important in the treatment of pain syndromes due to a positive effect on the antinociceptive nervous system, and reduction of intensity of nociceptive impulses in the posterior horns of the spinal cord.

IBUPROFEN IN THE TREATMENT OF INFLAMMATORY DISEASES OF JOINTS IN CHILDREN

Abstract

Treatment of acute and chronic inflammatory disease of joints in children is a challenge for the pediatrician and rheumatologist. Traditionally, therapeutic algorithms of treatment of arthritis in children include non-steroidal anti-inflammatory drugs, which have a unique combination of anti-inflammatory, analgesic and antipyretic action. The article summarizes the available results of use of ibuprofen in juvenile arthritis in children. Application of ibuprofen is characterized by good tolerability and rare development of adverse events. Permit for use of ibuprofen from the age of 3 months and older allows to consider it as drug of choice for initial therapy of inflammatory diseases of joints in children.

SORE THROAT: CAUSES AND POTENTIALS FOR RELIEF

Abstract

The article provides rationale for application of local treatment with combination products containing anesthetics and antiseptics in the therapy of pharyngitis and uncomplicated tonsillitis. These include the topical antibacterial drug Strepsils well-proven in ENT and dental practice. Strepsils has antiseptic - antibacterial and antimycotic - action, and is active against most common pathogens causing tonsillopharyngitis: group A β-hemolytic streptococcus, pneumococcus, Haemophilus influenzae, Staphylococcus aureus. The drug relieves irritation in the throat and oral cavity, resolves the pain, foreign body sensation, irritation and dry sore throat, and neutralizes bad breath. An additional important advantage of Strepsils for patients, who can not stop working during the ARVI, is the ability to resolve dysphonia.

Abstract

A comparative analysis of the effectiveness of local therapy of gonarthrosis by hyaluronic acid preparations (HA) of different molecular weight (Fermatron and Sinokrom forte) against the background of oral administration of chondroprotector Teraflex (glucosamine hydrochloride + chondroitin sulfate) was performed. Prospective follow-up of patients after a course of intra-articular injections showed the superiority of preparation of high molecular weight HA Sinokrom forte over preparation of medium molecular weight HA Fermatron for improvement of functional activity of the joints at 6 months. Sustainable pain management at rest and on movement was achieved in the group receiving Fermatron.

PATHOGENETIC TREATMENT OF OSTEOPOROTIC VERTEBRAL FRACTURES IN REAL-LIFE CLINICAL PRACTICE

Abstract

The article presents the results of evaluation of antiosteoporotic therapy (AOPT) in real-life clinical practice during the year among 40 women with low traumatic vertebral fracture. It is shown that adequate pathogenetic AOPT (bisphosphonates, strontium ranelate, calcitonin) at 4 and 12 months after osteoporotic fractures was administered only in 25 and 35% of patients, respectively. Previous studies have shown that combination of two active preparations - bisphosphonate alendronate and D vitamin - in one product (Fosavance) is a highly effective and safe pathogenetic approach to the treatment of osteoporosis that is justified in the long-term pathogenetic therapy of this chronic disease. Fosavance can be widely used for prevention of osteoporotic fractures, including the treatment of patients with low traumatic vertebral fractures, because currently pathogenetic AOPT of these patients is insufficient in real-life clinical practice.

Abstract

The article presents an analysis of the performance of the Samara regional antiepileptic center in 2010. Adequate therapy allows to achieve remission of epilepsy in most patients, especially among young people. The application of available conventional antiepileptic drugs (valproate, carbamazepine, lamotrigine) has a positive result. In pregnant women with epilepsy, further use of drug, which has the greatest therapeutic effect, is recommended.

QUALITY OF LIFE IN PATIENTS WITH PANIC DISORDER, AND ITS DETERMINANT FACTORS

Abstract

Scientific Research Centre, Department of Pathology of Autonomic Nervous System, Moscow According to the classification registers, panic disorder (PD) is a mental illness, but most of the patients refer to internal medicine specialists. Using the SF-36 questionnaire, we evaluated the quality of life (QOL) of 70 patients with PD and 50 healthy individuals. Compared with the healthy population, patients with PD have a low overall QOL - 52,4 points vs 84,7 in healthy subjects (p = 0,0001) due to reducing the physical and mental health components. In patients with severe PD (disease duration of more than five years, social maladjustment, comorbid agoraphobia), subjective measurement of deteriorating physical health component worsened. The conviction of patients that a their violation of activity is primarily caused by disorder of physical health may explain the preference that patients with PD give to general practitioners, resulting in inadequate treatment of this category of patients.

Abstract

The article considers the theory of homotoxicosis as a cause of diseases; based on this theory, methods of treatment and rehabilitation of borderline personality disorders are justified. The analysis of the most curable syndromes in the clinic of borderline mental disorders using antihomotoxic drugs is performed; some mechanisms of their action are explained; criteria for inclusion of these drugs in the treatment and rehabilitation programs in various stages of rehabilitation therapy are formulated. The results of own authors examination of clinical efficacy of antihomotoxic drugs Traumeel S and Vertigoheel C in patients with residual borderline personality disorders in the late period of craniocerebral trauma are presented.

According to randomized clinical trials ALADIN III (1999), SIDNEY (2003) and study performed by D. Ziegler et al. (2009), "cost-effectiveness" ratio in the treatment of diabetic polyneuropathy with thioctic acid and Actovegin showed an economic advantage of thioctic acid in the short-term use (21 days), while Actovegin has demonstrated clinical and economic benefit in long-term therapy (5-6 months).
