Abstract

The estimation of intestinal dysbiosis by identification of the O-antigens of pathogens of acute intestinal infections (AII), markers of Clostridium difficile toxins A and B and Shiga toxin in 78 patients with erysipelas who received antibiotic therapy for the underlying disease was performed. Coagglutination reaction on glass and plates was used. On admission, O-antigens were identified in 75 % of patients, C. difficile A and B toxins - in 72,5 %, Shiga toxin - in 45 %, moreover, high titers - in 20 % of patients. Growth of C. difficile toxins A and B antigen titers in stool was detected in half of patients treated with cefazolin and gentamicin, and in one third of patients treated with benzylpenicillin. The results indicated the presence of intestinal dysbiosis (subclinical) in patients with erysipelas, which is aggravated against the background of antibiotic treatment; this fact should be considered during the treatment of erysipelas, and during follow-up.

The article presents the current data on the pathogenesis of gastroesophageal reflux disease (GERD). The role of the “acid pocket" as a source of gastroesophageal reflux is demonstrated. The approaches to the treatment of the disease from the perspective of impact on the “acid pocket" are discussed. Antireflux mechanism of action of alginates, and their importance in the diagnosis and treatment of GERD are disclosed.

Abstract

Migraine and epilepsy are most common neurological diseases, manifested with paroxysmal disorders. The prevalence of epilepsy in people with migraine varies from 1 to 17% with a median of 5.9%, that is significantly higher than the average in the population (0.5%), and the prevalence of migraine in epileptic patients is 8-15%. Clinical manifestations of migrainous and epileptic attacks may be similar, making difficult differential diagnosis, for which the most important tool is history-taking. Electroencephalography (EEG) may be useful for patients with headache and signs of epilepsy, such as atypical aura or episodes of unconsciousness. Prolonged video-EEG monitoring is used as a differential diagnosis for migrainous and epileptic aura and objectification a comorbide migraine and epilepsy or migralepsy syndrome. In case of combination of migraine and epilepsy, drugs effective in both disorders (valproate and topiramate) should be preferred

Abstract

The benefits of selective cyclooxygenase-2 (COX-2) inhibitors in the treatment of acute and chronic pain syndromes include the following: significantly fewer adverse events associated with the gastrointestinal tract, kidney, liver; no negative effect on the synthesis of the major components of articular cartilage as contrasted with nonselective NSAIDs. Nimesulide (Nise) in therapeutic doses is a selective reversible competitive COX-2 inhibitor. NSAIDs implement analgesic effect via endocannabinoid system, apart from the traditional well-known pathways. Nimesulide affects the cannabinoid receptors type 1 without affecting anandamide or 2-arachidonoyl glycerol.

Abstract

Prevention of stroke and recurrent cerebrovascular events is the actual problem of modern medicine. Diabetes mellitus increases the risk of stroke by 2-4 times. Atherothrombosis is one of the key points for the development of stroke, and the presence of diabetic throm-bocytopathy and angiopathy accelerates the formation of blood clots. According to the European Stroke Association recommendations, antiplatelet (antiaggregant) therapy is the first line of prevention of this disease. As reported by international studies, clopidogrel is the drug of choice in high-risk patients, including patients with diabetes mellitus.

Abstract

Unstable angina and myocardial infarction without ST-segment elevation are the most common forms of acute coronary syndrome (ACS). At the pre-hospital stage of emergency medical services (EMS), optimizing of antiplatelet therapy in this actual pathology is pathogeneti-cally justified and necessary. The aim of study was to evaluate the effectiveness of early administration of acetylsalicylic acid (ASA) and magnesium hydroxide (Cardiomagnyl) in patients with ACS without ST-segment elevation at the pre-hospital stage. A retrospective analysis of 3858 patients with ACS without ST elevation who sought emergency medical care in 2008-2010 was performed. With the increasing frequency of aspirin use in ACS without ST-segment elevation at the pre-hospital stage from 2008 to 2010 (from 46.4 to 56.7 and 93.8%, respectively), the incidence of myocardial infarction in the hospital has decreased from 134 (10%) to 111 (8.9%) and 86 (6.8%) cases, respectively; in addition, hospital mortality in this disease has reduced from 11.7 to 11.5% and 9.6% in the dynamics by years. Due to the effectiveness of early use of ASA, this drug should be administered in patients with complaints of chest pain during reception of call by dispatcher of ambulance care.

Abstract

Due to the influence of postinfarction cardiosclerosis (PICS) on disease progression and survival in this group of patients, development of therapeutic approaches and evaluation of its effectiveness in patients with PICS is one of the most important problems of modern cardiology. The results of the analysis of the dynamics of functional and laboratory parameters in 68 patients with PICS (43 men and 25 women) who received the recommended drug treatment in real-life clinical practice are presented. Mean follow-up was 8,1 ± 5 years. All patients with PICS had comorbidity. Pharmacotherapy in patients with prior myocardial infarction included β-blockers (82,4 %), ACE inhibitors (80,9 %), antiplatelet agents (94 %), diuretics, nitrates, and statins. During treatment a decrease in blood pressure (BP) which reached the target level in 46,7 % of patients was reported. Decreased left ventricular mass index (LVMI), reduced proportion of patients with concentric and eccentric hypertrophy of the left ventricle (LV) were achieved, normal LV geometry was recovered in some patients. Renal function, glucose and lipid metabolism have not changed. Thus, pharmacotherapy in patients with prior myocardial infarction mostly corresponded to Society of Cardiology of Russian Federation recommendations; insufficient administration of lipid-lowering therapy was noted. Reduction in BP and LVMI was achieved, the proportion of patients with poor type of myocardium remodeling was decreased.

Abstract

Intestinal dysbacteriosis is a disturbance of qualitative and/or quantitative composition of the normal flora. In the treatment of dysbiosis, probiotics (active agents of positive microflora) and prebiotics (oligosaccharides that promote the growth of positive flora) are widely used. The use of new class of probiotics, a metabolic prebiotics (eg, Hylak forte) is the most promising area in the treatment of dysbacteriosis. Metabolic prebiotics represent extracts of metabolites of positive flora which stimulate the growth of positive microbiota similar to oligosaccharide probiotics. The present article for the first time demonstrates the comparative metabolomics analysis of positive microflora underlying drug Hylak forte. Based on an analysis of possible molecular components, new mechanisms of therapeutic effect of this drug is formulated, promising synergistic effect in combination with Be, B2 vitamins, selenium, and glutathione. It was found that the positive flora produces and consumes metabolites associated with the processing of bile acids, biosynthesis and biotransformation of vitamins, selenium, prebiotic sugars, and short-chain fatty acids. Metabolomics analysis suggests that the metabolites in the Hylak forte promotes the survival of positive flora and can inhibit the growth of pathogenic intestinal microflora.

Abstract

In recent decades, research on the mechanism of malignant transformation led to the identification of a number of signaling pathways responsible for the regulation of tumor cell proliferation. Initiated by growth factors, hormones or cytokines, such signaling pathways usually start with tyrosine kinase cascade and result in activation of various families of transcription factors that control gene expression in cell division. Identification of these activated signaling pathways in carcinogenesis and suppression of their activity is one of the urgent problems of modern systemic targeted therapy. A description of the key role of PI3K/Akt/mTOR signaling cascade in the pathogenesis of tuberous sclerosis, that responsible for the integration of proliferative stimuli and activation of protein translation, has allowed to plan and begin clinical studies on the effectiveness of mTOR inhibitors as drugs for pathogenetic therapy tuberous sclerosis.

Abstract

The article describes the current clinical aspects of erysipelas as a streptococcal infection. In 2011, 1580 patients with erysipelas were under observation. The disease more frequently occurred in women (56,4 % of cases). The prevalence of the disease in older patients (72,8 % of all patients with erysipelas admitted to the department were older than 50 years, with the frequent development of hemorrhagic forms), the predominant localization of inflammatory lesions at lower limbs (75,8 %), and hemorrhagic forms (61,8 %) are the modern features of the clinical course of erysipelas. Current schemes of antibacterial therapy and rehabilitation are presented.
