Trimetazidin effektiven v reabilitatsii bol'nykh posle protezirovaniya klapanov serdtsa: rezul'taty randomizirovannogo kontroliruemogo godichnogo issledovaniya

Abstract

It is known that a successful surgery for valvular heart disease increases life expectancy, reduces symptoms and improves cardiac function. Nevertheless, left ventricular dysfunction, heart failure, and impaired quality of life (QOL) may persist and worsen in the postoperative period. We hypothesized that metabolically active drug trimetazidine may improve symptoms and quality of life of patients after heart valve replacement. The study involved 150 patients with valvular heart disease at the age of 55,3 ± 3,2 years (58 % men) who were randomized 1 : 1 to were to either use the optimal standard therapy (vitamin Kantagonists, angiotensin converting enzyme inhibitors/ angiotensin II receptor antagonists, β-blockers, diuretics, statins and digoxin - if indicated) or trimetazidine at a dose of 35 mg twice a day in addition to the traditional therapy 2-4 weeks after surgical replacement of the aortic (n = 84) or mitral (n = 66) valve. Significant baseline differences between the trimetazidine group and the control group were not observed. It was shown that long-term therapy with trimetazidine in patients with mechanical prosthetic aortic/mitral valve improves the clinical status and QoL. This study is the first evidence that trimetazidine may be new additional therapy of patients after heart valve replacement.

Abstract

The article presents the results of duplex scanning of the common carotid arteries (CCA) and assessment of structural and functional state of the vasculature in 47 women with arterial hypertension (AH), dyslipidemia and estrogen deficiency in late fertile period (LFP). Using detailed duplex scanning of CCA, it was found that patients with arterial hypertension and dyslipidemia in the LFP with reduced estradiol levels had an expansion of the CCA lumen, reduced blood flow in the carotid arteries with direct course without hemody-namically relevant stenosis, indicating structural and functional changes in the vascular wall in the form of arteriosclerosis. In patients with arterial hypertension and dyslipidemia in the LFP and poor hormonal profile, during the test of postocclusive reactive hyperemia before treatment, on the background of significant reduction of vasomotor endothelial function, in almost every second case (46.7 %) inertial type of vasomotor response is revealed, indicating the contribution of dyslipidemia in the progression of arterial hypertension in dyshormonosis, primarily due to an increase of arterial stiffness and development of endothelial dysfunction. Combination therapy with Mertenil (rosuvastatin) and Ekvator (lisinopril, amlodipine) has significantly better effect on the structural and functional changes in the CCA vascular wall, endothelial dysfunction, and reduces arterial stiffness compared to the therapy with Ekvator (p < 0.05).

Abstract

The study was aimed to the investigation of the features of cerebrovascular disorders in patients with diffuse toxic goiter (Graves' disease), as well as evaluation of the effectiveness of neuroprotector Cortexin in this category of patients. Patients were divided into 2 groups: group with hyperthyroidism (38 patients) with the manifest untreated diffuse toxic goiter, and group with medically induced euthyroidism (30 patients). Euthyroid patients were treated by an endocrinologist for at least 6 months. The control group (CG) consisted of 10 healthy individuals. All patients were additionally treated with Cortexin at a dose of 10 mg 2 times a day (morning and afternoon) for 10 days. It was concluded that the use of Cortexin in patients with cerebrovascular disorders against the background of Graves' disease significantly improves neurological and neuropsychological status, which in turn contributes to shorten the rehabilitation of this category of patients and improvement of their quality of life.

The article is devoted to the pharmacotherapeutic approaches to the treatment of gastroesophageal reflux disease (GERD). The article presents the results of recent randomized clinical trials evaluating the impact of GERD on the quality of life of patients. It is conclude that improving the quality of life is one of the main goals of GERD patients and is considered as an endpoint in evaluating the success of treatment. The development of new dosage forms may contribute to improve the patient’s satisfaction of treatment. Enterosoluble tablet formulation of esomeprazole in the form of preparation Neo-zekst ® is one of these forms.

Abstract

This article describes the clinical experience of management of patients with high risk for cardiovascular complications. For medical correction of arterial hypertension, tactics of combined antihypertensive therapy with angiotensin-converting enzyme inhibitor fosinopril in the starting dose of 10 mg daily and thiazide diuretic indapamide at the starting dose of 1.5 mg per day was chosen. At the end of cycle of therapy, improvement of BP levels, biochemical tests and endothelial function was revealed. This clinical case demonstrates the possibility of drug correction of endothelial dysfunction, which can improve BP control in patients with high risk of cardiovascular disease in the outpatient settings.

With specific references, the issues of risk assessment for thromboembolic complications and bleeding when selecting antithrombotic therapy for the prevention of ischemic cerebrovascular disorders are analyzed. The importance of the treatment compliance of patients treated with oral anticoagulants as a factor that largely determines the efficacy and safety of therapy is emphasized.

Abstract

Based on the literature data and our own clinical observations and research, the article considers the current theory of aging, explaining general biological mechanisms of this process that underlies the so-called age-associated and comorbid human pathology. Currently, there are about 300 theories of human aging, but the authors prefer the three of them, and through the example of the theory of oxidative stress, justify their views on the important role of free radical oxidation mechanisms of cellular aging, apoptosis and cell death, underlying social health phenomenon of aging and age - associated diseases. Excessive influence of oxidative stress, which result in age-dependent functional failure of the antioxidant system, has a more negative impact on the entire cellular and systemic metabolism, is one of the key, but wel-controlled mechanism of human aging in contrast to the age. Currently, there are unique drugs with proven efficacy correcting severity of oxidative stress - antioxidants. On the example of α-lipoic (thioctic) acid (Espa-Lipon), it is shown that correct and rational use of antioxidants with significant neuroprotective effect, such as brightest representative α-lipoic (thioctic) acid, in clinical practice allows to effective impact on the aging process, successfully implement the prevention and pathogenetic therapy of many age-related diseases.

Abstract

Acute otitis media (AOM) is the most common cause of systemic antibiotic therapy in pediatric practice. The high prevalence of the disease and the objective difficulties of adequate diagnosis of AOM dictate the need to development of systematic approach to the diagnosis and treatment strategies. In this regard, reputable pediatric communities have developed a number of guidance documents. Accumulation of new evidence leads to the revision of a number of positions, as demonstrated by the example of the Recommendations on the diagnosis and treatment of AOM in children by the American Academy of Pediatrics (AAP). According to the opinion of ADA, the most relevant criteria for diagnosing of AOM, tactics for determination of the indications for systemic antibiotic therapy, drug selection and duration of therapy are presented.

Abstract

The article is devoted to the one of the most severe pathologies in pediatric gastroenterology - ulcerative colitis (UC). Despite progress in the evaluation of the pathogenesis, diagnosis and treatment of this disease, individualized and, at the same time, a systematic approach to the therapy has not been developed. This is particularly due to the lack of algorithms for the prediction of course of disease, which largely determines the tactics of treatment. Based on a retrospective analysis, the authors have identified a mathematical predictors of recurrent and continuous course of UC, and have presented the scheme for the stage-by-stage prediction, from outpatient settings to specialized centers, taking into account the availability of information. Mathematical prediction model and differentiated therapeutic tactics based on this model will optimize therapy in these pediatric patients.

Abstract

The article describes the importance of the problem, the evolution of points of view and modern concepts of fibrogenesis in chronic liver diseases. The roles of factors such as the intrahepatic and portal vascular resistance, blood volume in the portal vascular bed of the liver, and status of cardiovascular system are considered. Basic control mechanisms of fibrogenesis by universal, and in some cases specific cytokines produced by cells of the sinusoidal compartment and regulating the inflammatory response to injury of hepatocytes are demonstrated. Main predictors of active fibrogenesis are specified. The efficacy of use of hepatoprotectors with pathogenetic antifibrotic mechanism of action is evaluated.

Abstract

In the structure of infectious diseases among children, acute intestinal infections (AII) took second place. Currently in Russia, there are no consistent approaches to therapy. The article presents the World Gastroenterological Association recommendations for the hospitalization with infectious diarrhea, describes the basic principles of therapy and evidence of the effectiveness of key therapeutic interventions. The authors note that the use of antibiotics is not always safe and effective tactic in bacterial diarrhea, and may lead to rapid development of resistance and allergic reactions. A significant role of antibacterial action of bacteriophages as a possible therapy of bacterial intestinal infections is emphasized.
