Abstract

The article presents the results of prospective cohort study included 56 women aged 20-35 years with tuboperitoneal infertility, who were planned for in vitro fertilization (IVF). The main group (29 patients) received preconceptional intravaginal therapy using gel based on Dead Sea mud for 10 days from the 7-8 th day of the menstrual cycle. The control group (27 women) withdrew from physiotherapy. Intravaginal peloidotherapy reduced the incidence of chronic endometritis by 2.5 times (51.7 vs. 20.7%), increased the chances of pregnancy by 4 times (58.6 vs. 14.8%, odds ratio=4.0), increased the effectiveness of IVF by 2.7 times (40.0 vs. 14.8%), led to a spontaneous pregnancy in 9 (31.0%) patients with proximal tubal occlusion and chronic endometritis.

Abstract

Gram-negative aerobic bacteria of the Enterobacteriaceae family remain one of the leading causative agents of puerperal infections. Apart from the most significant among them Escherichia coli, post-partum infections may be caused by other members of the family, such as Enterobacter cloacae and Klebsiella pneumoniae. A clinically important feature of Gram-negative enterobacteria is the high rate of microorganism resistance to different classes of antimicrobial agents. The study results obtained in the studied hospital were used for recommendations for changing approaches to post-partum infections’ empirical treatment. The only drug group showed to be active against all tested strains, including multiresistant ones, was carbapenem antibiotics group. Therefore, they should be recommended for empirical treatment of severe postpartum infections in the maternity hospital.

Abstract

The article presents results of the study investigating contraceptive behaviors of 754 female students (age 14-24 years) of secondary special educational institutions of the city of Samara. It was found that a significant proportion (80%) of the respondents knows about the possibility of becoming pregnant as a result of a single (first) intercourse and that condom is a reliable means of protection against both unwanted pregnancy and HIV and other sexually transmitted diseases. Half of the girls know about daily intake of hormonal tablets; this knowledge naturally increases with the onset of sexual activity. Contraception by coitus interruptus is found to be widely spread among pupils of secondary special educational institutions; it is used by 28% of girls who have sex, even though most of them know of its unreliability. Another unreliable method of contraception - menstrual cycle contraception - is practiced by7% of sexually active girls. The authors present the data on modern combined oral contraceptives, which are the most appropriate for the young and nulliparous women from the age at menarche.

Abstract

This article discusses the criteria for diagnosis, individual normal values of testosterone and hormone replacement therapy monitoring. It is shown that the individual variability of of serum total testosterone in men differs in time by several folds (the coefficient of variation varies from 5.2 to 29.6%). Variability of the total testosterone level depends on the patient age and the presence of clinical signs of hypogonadism. These facts are of importance in the application of individual approach to the treatment of men with sympto-matic androgen deficiency, and should be considered when evaluating the effectiveness of testosterone replacement therapy.

Testing VEGF production levels and KDR, FLT-1receptors, analysis of metalloproteinases and their inhibitors relationship, the role of telomeric binding protein factor-1 TRIF in clinical practice extends the feasibility of verification and differentiation of inflammatory, benign, premalignant and malignant tumors of the prostate.

Abstract

The article describes diagnosis and treatment experience of embryonal rhabdomyosarcoma in a 16-year-old patient. Detailed clinical, laboratory and instrumental findings are presented. Based on the findings, the patient underwent surgical removal of the necrotic masses and curettage of the cervical canal and the uterine cavity. The postoperative period was uneventful. The patient was scheduled for adjuvant therapy. The presented clinical case is of interest as an extremely rare pathology in adolescent children.

The article discusses the choice of treatment of gonococcal infection taking into account the antibiotic resistance of the pathogen of the disease, Neisseria gonorrhoeae. Current parameters of sensitivity of microorganisms to antibiotics in the world and in the Russian Federation are presented. The application of the oral preparation of III generation cephalosporin - cefixime (Pancef) in the treatment of gonococcal infections is justified. The basic pharmacokinetic characteristics and advantages of cefixime are discussed. The schemes of treatment of gonococcal infection according to the Federal clinical guidelines of the Russian Society of dermatologists and cosmetologists are presented.

Abstract

He article considers the role of omega-3 polyunsaturated fatty acids (PUFAs) in the prevention and treatment of obstetric complications. The results of several studies suggest that omega-3 fatty acids are most effective only in complex treatment of different diseases. First of all, we can be quite accurate to talk about the use of omega-3 fatty acids during pregnancy for the prevention of preterm labor, premature birth, thrombophilia, in some cases, pre-eclampsia, and allergic and chronic inflammatory diseases in children. Given the deficit of omega-3 PUFAs-rich food intake in the world and almost complete harmlessness of use of PUFAs at therapeutic doses, additional use of omega-3 PUFAs is justified for pregnant women and their children.

Abstract

The review presents the role of progesterone and dydrogesterone in the treatment of recurrent miscarriage. Comparative studies showed no significant differences in the efficacy of micronized progesterone and dydrogesterone in the treatment of spontaneous abortion. However, oral micronized progesterone was found to have lower bioavailability compared to dydrogesterone. It was demonstrated that dydrogesterone therapy reduces the risk of platelet aggregates’ formation and other mother-placenta-fetus system vascular disorders underlying preeclampsia pathogenesis.
