Sostoyanie i problemy vosstanovitel'nogo lecheniya v munitsipal'nykh i gosudarstvennykh uchrezhdeniyakh zdravookhraneniya primorskogo kraya

Abstract

This article represents analysis of the state of carrying out medical rehabilitation, which was taken by municipal and government medical and preventive institutions in Primorsky Krai during 2006-2010 years. The analysis was made in four directions: 1. in-patient medical- clinical institutions of medical rehabilitation and their work; 2. outpatient-clinical medical and preventive institutions of medical rehabilitation and their work; 3. departments of medical rehabilitation as parts of medical and preventive institutions; 4. peopleware index of rehabilitation medicine movement. Results indicate such problems as decrease accessibility of medical treatment in the field of rehabilitation treatment both in-patient treatment and outpatient- clinical treatment, because of reduction the number of medical and preventive institutions of medical rehabilitation, not enough specialists in the rehabilitation medicine. Natural climatic resources of Primorsky Krai is using not enough in the rehabilitation medicine

Abstract

The theoretical prerequisites of reconstruction of the motional sphere in patients with the pathology of the nervous system under the influence of application of functional programmable electrical stimulation of muscles (FPES) during walking are given in this article. Clincal efficiency of the restorative treatment by means of FPES in patients with different pathology of the central nervous system has been analyzed. The remarkable changes of neurofunctional and biomechanical parameters (electroneuromyography, three-dimensional videoanalysis of movements) have been demonstrated in groups of patiens with infantile cerebral palsy, poststroke motional disturbances and the scoliotic disease. The received results confirm the high level of the achieved reconstuctions of the locomotor stereotype in patients in the process of FPES

Robotizirovannye tekhnologii v reabilitatsii bol'nykh posle khirurgicheskoy revaskulyarizatsii mozga

Abstract

Were examined and divided into 2 groups of 43 patients. Group I included 26 patients after carotid endarterectomy, group 2 - 17 patients after balloon angioplasty with stenting. Age stroke from 3 to 6 months; prescription transactions from 1 to 3 months, the degree of paresis of the lower extremity of 3 to 4 points. To assess the rehabilitation measures we used: a 6-point scale paresis, Stroke Scale NIHSS(The National Institute of Health Stroke Scale), Barthel ADL Index, Hauser Gate Index and electrophysiological examination (impedance cardiography, Doppler ultrasound of the affected middle cerebral artery). Patients received Locomat training once a day 6 days a week for 3 weeks. Discharge weight was on average no more than 25% of body weight, treadmill speed 1.5 - 2.5 km / h. Conclusions: It was found that locomotor training with the help of Lokomat improve motor function and quality of life and it doesn’t depend on the type of surgery

Abstract

This paper presents the results of the study of prevalence of the attention deficit hyperactivity disorder (ADHD) in 4101 children (boys - 48% (n = 1968), girls - 52% (n = 2133)) of the city Vladimir and the Vladimir region. The ADHD was present in 7% of the school children and in 20% of the pre-school children. There have been detected some particular features in the health status of the children between ages 7 to 10 with ADHD (n = 68: 46 boys, 22 girls; control sample - 90 children: 43 boys and 47 girls). Among boys with ADHD (compared with the control group), speech disorders and diseases of the musculoskeletal system dominate (p <0.01). In the second place - nocturnal enuresis, the incidence of which was 2 times greater than in the control group (p <0.05). In the third place - diseases of the eye and the upper respiratory tract, the incidence of which was 5 times greater than in the control group (p <0.01). Among girls with ADHD, in first place come the speech disorders, that occur 9 times more than in the control group (p <0.01), second - Eye diseases (p <0.05), third - nocturnal enuresis (p <0.01), fourth - ENT diseases (p <0.01). The rehabilitation program of children with ADHD should be developed based on the particular features of the health status that were observed. An important role should be given to the speech correction, to the in time rehabilitation of the cases of chronic ENT diseases, to the disorders of the musculoskeletal system and to the eye diseases of the pre-school and school children The pharmacological correction of ADHD in children should be preceded by non-drug methods of rehabilitation: a) increase physical activity (exercise to improve motor coordination and to reduce hyperactivity, impulsivity), b) psycho-pedagogical correction, c) family therapy, and d) relaxation techniques (causing muscle relaxation).

Reabilitatsiya detey s neyroortopedicheskoy patologiey na etapakh khirurgicheskogo lecheniya s primeneniem robotizirovannoy mekhanoterapii

Abstract

The aim of the study was the analysis of the role of the robotized mechanotherapy in complex rehabilitation of children with neuroorthopaedic disorders. We observed 40 children (20 - with spastic cerebral palsy and 20 with spina bifida). The data of effectiveness and tolerance to the treatment were obtained. Tolerance to the robotized mechanotherapy was better in children with spina bifida as compared with cerebral palsy group at all stages of treatment. The increase of general movement activity was also more considerable in patients with spina bifida; in children with cerebral palsy robotized mechanotherapy was more effective at the late postoperative stage (more than 1 month postoperatively). The shortening of the period of rehabilitation due to using of robotized mechanotherapy was more remarkable in children with spina bifida in early and late postoperative period, as well as in children with cerebral palsy in late postoperative period

Algoritm prognozirovaniya povtornykh infarktov miokarda

Abstract

The results of 3-year observation for 298 men at the age 30-65 years old who had undergone the infarction for the first time presented. There was realized the comparison of frequency of repeated infarction in 3 year period with the frequencies of signs in early period after the first infarction. We also have compared these frequencies in groups of patients who underwent different tactics of physical rehabilitation: physical training, periodical hypobaric training in altitude chamber or absence of any training. It was performed the computation of self-descriptiveness (measure of informativeness) of mentioned signs on prognosis of probability of the repeated infarction. It were shown that the largest prognostic significance revealed for non-usage of any rehabilitation methodic just after the first infarction, pure tolerance to physical loading, possession of diabetes and small fraction of ejection of the left ventricle.
