Methodological bases of rehabilitation institutions design and organization

Abstract

In this article the concept of rehabilitation institution creation is stated. The method of rehabilitation activities evaluation is proposed. For effective planning and analysis activities of rehabilitation institutions introduced the concept of rehabilitation services, which is a unified unit of work of the main personnel involved in the rehabilitation and measured in relative units. The algorithm of creation of rehabilitation centers in any subject of the Russian Federation is proposed. This algorithm consists of the following stages: analysis of needs of rehabilitation in the region (or in the specific health organization); determination of the main consumers rehabilitation services; analysis of the existing conditions for the creation of rehabilitation; development of standards of rehabilitation for each category of consumers; calculation of rehabilitation manning table; calculation of the necessary facilities for rehabilitation and their justification; determination of necessary additional rehabilitation equipment; selection of an optimal model of rehabilitation services in medical organizations; calculation of financial indicators for rehabilitation. Practical examples for each step of creating a rehabilitation facility are suggested. The question of rehabilitation financing possible sources in the Russian Federation is discussed.

Method for objective movement detectection in the shoulder joint

Abstract

Introduction: The main manifestation of post-stroke neurological disorders that lead to disability of patients, is a marked reduction of upper extremity function. This disturbance reduces work, complicating self-service process, and cardinally changes quality of life of the patient. In this regard, objective research of movements in a humeral joint is of special value. The gyroscopic method - well represents the qualitative party of movement, doesn't demand an external recorder, has high precision of registration of kinematic parameters. Compactness and low-invasiveness of a method allows to use it together with electromyography devices at various medical tasks and trainings with biological feedback. The purpose of work is development of a technique of objective registration of movements in a humeral joint focused on patients with disturbance of a cerebral circulation in the acute period. Materials and methods: Research was conducted on 15 healthy examinees who didn't have in the anamnesis of injuries and diseases of a locomotorium. Examinees in process were offered to carry out movements of a flexion/extension, abduction/reduction, to 90 degrees, and external/internal rotation by the top extremities in humeral joints in the sagittal, frontal and vertical planes respectively. Movements were carried out at the same time by two arms and each arm separately in situation sitting. Each exercise was carried out two times. Movement was carried out freely, without additional indicating's. Results of research: For all main movements we received pattern when the maximum of amplitude comes in half of a movement phase. Any movement in a humeral joint which is perceived as isolated has essential amplitudes of movements in the accompanying planes. The volume of information received provides a comprehensive and multi-evaluate each movement, its spatial characteristics. Proposed method of recording the movements of the shoulder joint may be of interest to physicians LFK, neurologists, orthopedists and trauma surgeons and other specialists.

Our 20 years of experience with the product showed that the most informative indicator is the maximum torque (peak torque), other characteristics of highly variable and do not have significant clinical value. Percutaneous minimally invasive surgery of athletes and ballet dancers with lumbosacral pain

Abstract

The article presents the treatment experience of patients, athletes and ballet dancers with lumbosacral pain caused by disc protrusion, facet syndrome or discopathy by percutaneous surgical interventions. 135 patients underwent laser vaporization disc 114 - radiofrequency denervation of the intervertebral joints, 14 - radiofrequency denervation of the intervertebral discs. Considered indications and contraindications for these operations, techniques of surgery are shown as well. Abstract

Aim of this study was to assess the effectiveness of comprehensive rehabilitation to include biofeedback machine MBN-Parastobilo in wheelchair. The pilot study consists of 43 disabled people aged 20-47 years with cervical and thoracic traumatic spinal cord injury (n = 33) and infantile cerebral paralysis (n = 10), which were hospitalized to the rehabilitation center «Preodolenie» for passage of a 30-day course of comprehensive rehabilitation. Computer Stabia biofeedback training was performed on adapted for people with limited mobility (paraplegics) on the unit MBN-Parastabilo, integration into a chair. Before and after the individual biofeedback training balansoterapii conducted for 10-15 minutes, carried out in 20-second test with open (OE) and closed eyes (CE) with the analysis stabilometric indicators: 1. Rombergs factor - ratio of the tatokineziogrammy area OE to the same indicator at CE; 2. speed deviation of center of pressure at OE and CE; 3. area statokineziogrammy at OE and CE; 4 Stability at OE and CE. The article presents the clinical observation of the child with the 1st type of Gaucher's disease, which was carried out a total hip replacement of the left hip joint due to the development of coxarthrosis.
