Analysis of dynamics of activity of mobile rehabilitation section in multifunctional center of rehabilitation

Abstract

This article reports about the legislative and methodological prerequisites for organization department of mobile rehabilitation in a rehabilitation center for disabled. Describes the organization of the activities of mobile rehabilitation section and procedure of comprehensive rehabilitation at home. Represents by social- demographic characteristics of the service contingent in dynamics of a two-year work. We give data about the types and amount of services rendered, the methods of assessment of efficiency of mobile rehabilitation section. Indicates promising directions of activities for mobile rehabilitation section.

Correction of affective disorders, autonomic dysfunction and sleep disturbances at patients, which suffering cervical dorsopathies

Abstract

Sixty-nine patients with cervical dorsopathies (Сй) have been suffered from different clinical symptoms of this disease. We used photophoresis with bisphosphonates and heparin as an unguent in complex therapy. That characterized by a decrease of pain intensity, correction of autonomic and neuropsychological disturbances and associated sleep disorders. Regression of clinical manifestations of CD was confirmed by positive dynamics of a set of special test methods.

Correction of the gait stereotype in patients with the syndrome of central hemiparesis by the method of activation of the posterior foot impulse

Abstract

In this article we present the results of a research work dedicated to the assessment of the impact of an original method of gait stereotype correction in patients with the syndrome of central hemiparesis in the rehabilitation period of stroke. We compared the efficiency of rehabilitation between two groups of patients. The study group received standard medical treatment, apparatus physiotherapy, massage and the proposed original method of gait stereotype correction. The idea of the original method is the activation of the moment, when the paretic leg lifts from the support surface, the activation of the posterior foot impulse by means of mechanic stimulation. The patients of the control group received standard medical treatment, apparatus physiotherapy, massage, kinesiotherapy, training in the costume «Adeli» for propriocorrection. Research methods were: neurological examination, objective assessment of balance function using the Computer Stabilometry (CS), objective assessment of gait using «laser analyzer of kinematic parameters of gait» (LA-1), clinically balance function was assessed using the Berg Balance Scale, gait function was assessed using the Dynamic Gait Index. Results: In the I group after the course of treatment was a statistically significant decrease in the data of computer Stabilometry (area, length, velocity of displacement of the center of gravity, the shift of center of gravity in frontal plane), LA-1 data change (increase stride length of paretic limb, reducing the time of step in intact limb), positive dynamics of the results Berg Balance Scale and Dynamic Gait Index. In the II group also noted a positive trend in data of computer Stabilometry (displacement of the center of gravity in the frontal plane) and functional scales. The results of the study allow us to conclude that the efficiency of the method is comparable to the traditional methods of movement rehabilitation. The patients showed improved stability during standing and walking, which facilitates the formation of a new movement stereotype and lowers the risk of falling.

Evaluation of the effectiveness of treatment of patients with ischemic stroke in the early recovery period

Abstract

For personalized treatment of ischemic stroke in order to achieve maximum therapeutic effect, it is advisable not only to develop and introduce new drugs, but also better able to use some funds on the basis of monitoring their actions in the mode of on line. This feature was added due to use of microprocessor-based Registrar of the temperature difference between the biologically active point (BAP) and the intact area of skin. We have proposed a method for predicting the effectiveness of treatment of patients with ischemic stroke, based on measurement of the temperature difference between the biologically active points and intact area of skin. The method provides for the registration is not absolute temperature BATH, and the temperature difference between the BATH and the intact area of skin where you install a second thermocouple sensor, at least within 2 minutes. If it hasn't got any negative effects on the acupuncture point; the proposed method has high resolution, efficient, inexpensive, noninvasive, non-invasive, convenient and easy to use. Based on the analysis of changes in indicators thermometry BAT in the middle of a course of rehabilitation treatment in a group of patients underwent correction therapy of ischemic brain damage, and evaluated the results of treatment, allowing to optimize the treatment process and to achieve maximum results recovery of neurological deficit, psycho-emotional status and quality of life of patients after acute vascular attack. The calculated linear regression equation have revealed to define the prognostic significance of the differential thermometry BAHT and the ability to use it to modify the treatment program in real-time. On the basis of the conducted research we have developed the concept of managed treatment of ischemic brain damage, i.e. on the basis of the heterogeneity of AT BAHT to monitor the effects of medicines in real-time to assess their therapeutic effectiveness and to predict outcomes.

Evaluating the effectiveness of electrical stimulation and vazoselective alternating electrostatic field at patients with ischemic stroke

Abstract

Based on a survey of 62 patients with ischemic stroke who received course continued rehabilitation in neurology Department clinic «Ivanovo State Medical Academy», the analysis of the effectiveness of four modern and traditional physiotherapeutic measures: vasoselective stimulation, alternating electrostatic field of the high voltage low frequency, pneumomassage and magnetic therapy pulsed magnetic field. The parameters of physical therapy effects (time, frequency, power, current) were selected individually taking into account the functional defect and comorbidity of the patient. All subjects were patients of the Department received only one type of physical therapy effects. Assessment results use of these methods relative to motor, cognitive and somatic spheres. Evaluation of the effectiveness of the treatment was carried out on the basis of the two parts of the International classification of functioning, disability and health (ICF): «Damage to body functions and activity limitations, and participation». It was found that patients in the early rehabilitation period of ischemic stroke treated in the complex rehabilitation of physical therapy showed positive points, characterizing the degree of severity of paresis and spasticity muscle groups, all groups improved indicators on mobility, self-care, including activity, and participation, as well as under section d1 is the Ability to learn and apply knowledge. The groups of patients treated in the complex rehabilitation treatment vasoselective electrical stimulation from the apparatus Body Drain, according to the International classification of functioning, disability and health, improved indicators of participation in the exercise of the functions of the walk; and in patients who have undergone a course of treatment with application of an alternating electrostatic field apparatus «Hivamat 200», improved characteristics of cardiac function. The marked advantage of modern methods of physiotherapy treatment in comparison with the previously widely used (pneumomassage with apparatus «Pneumomassage» and magnetotherapy with «Almag»).

Clinical efficiency of dynamic electroneurostimulation in the acute period of the ischemic stroke

Abstract

Following research was carried out to study the influence of dynamic electroneurostimulation (DENS) on severity of neurological deficit and activity level in everyday life. Study measured the efficiency of early rehabilitation supplemented by inclusion of DENS among 35 patients (18 men and 17 women, the average age 64,4 ± 2,2 years) in acute period of ischemic stroke. There was simultaneous monitoring of condition of patients in control group (15 people), who underwent traditional drug therapy in combination with physical therapy and physiotherapy. Estimation of seriousness of the neurologic deficiency in both groups was done according to Lindmark scale and NIHSS. Barthel ADL index was used to estimate patient’s everyday activity after the stroke. The course of the DENS therapy began mainly on second or third day of hospitalization. 2-3 impact zones were chosen for one procedure (scalp area - every day). Monitoring was carried out on the 1st, 7th and 15th days from beginning of the therapy. On 15th day from therapy’s beginning, examined group showed an increase in the total score on Lindmark scale by 40 points (p <0.05); decrease of neurologic deficiency intensity measured 5,1 points on NIHSS (p <0.001); Barthel ADL index increased by 29 points (p <0.001) comparing to the 1st day. Parameters in the control group were much lower: 22 (Lindmark), 3,4 (NIHSS) and 25,5 (Barthel ADL index) points correspondingly. Research has shown that complex treatment supplemented by DENS in basic group of patients, compared to use of traditional therapy in the control group, increases therapy’s effectiveness, which leads to faster and more significant regress of neurologic symptoms.

Shockwave therapy in diseases and injuries of the musculoskeletal system due sports

Abstract

Objective: to assess the role of shockwave therapy (SWT) in holiatry of the musculoskeletal system diseases and injuries among athletes with the subsequent development of SWT optimal dosage. Material and methods: 976 athletes aged 16 to 42 years with the musculoskeletal system diseases and injuries were divided into 2 groups: 459 patients (I group) received standard conservative treatment (physiotherapy, non-steroidal anti-inflammatory drugs, cartilage protectors, exercise therapy, etc.); 517 patients (II group) additionally received 3-6 procedures of SWT for 10 minutes after 1.5-2 days (3000 beats per procedure, the average percussion frequency - 5 Hz, pressure - 2 Bar x 100kRa, energy density - 0,3 mJ/mm 2). Treatment results were evaluated for 6 months based on the visual analog pain scale and timing of the resumption of exercises. Results: SWT allowed reducing intensity of pain significantly in a short period of time, and stable positive effects of treatment were observed in long-term period. Good results of SWT in one month treatment were obtained in 81%, satisfactory - 7%, unsatisfactory - 2% of patients. After the first procedure, over 65% of patients reported significant reduction of pain. Six month from baseline, a stable positive effect was observed in 55% patients of the first group and in 80% of patients in the second group. Conclusions: SWT allows reducing pain syndrome quickly helps to prevent the recurrence of pain in the long-term period in patients with degenerative-dystrophic musculoskeletal diseases, reduces the time period of rehabilitation and disability sports.

Features of functional orthotic knee early postoperative period

Abstract

The results of motor rehabilitation in patients undergoing reconstruction of the anterior cruciate ligament of the knee joint, parallel-group using the method of functional orthotics were studied. An assessment of clinical, biometric and functional outcomes, resulting in faster recovery noted knee function, the optimal parameters of quality of life, as well as subjective perception of treatment outcome in patients of the main group. Morphological substrate of recovery of motor activity is the mechanical strength of the graft achieved intraoperatively, as well as adequate parameters of muscle tone. Rigid fixation is optimal for osseointegration in the channels of the femur and tibia, but contributes to muscular atrophy of the lower extremities, which in turn may underlie the instability of the operated joint. Functional orthotics allows metered volume limit movement in the joint, thus ensuring the optimum conditions for regeneration medium structures synovial joint, at the same time, allows the strain on the dosage painless extremity in the range that does not cause clinically significant muscular atrophy, thereby improving parameters of motor rehabilitation and quality of life of patients in the postoperative period. Thus, the comparative analysis of the method of motor rehabilitation in the postoperative period proved the advantages of functional latch HKS - 375 brand «Orlett». Were marked by positive results on subjective symptoms in patients of the main group compared with the control group, in particular, revealed earlier pain relief in conjunction with the restoration of quality of life (Fig. 7) that this was confirmed by objective evidence, namely the angles of flexion in the knee joint (Fig. 5) and stabilometry parameters (Fig. 6). Based on these results we can recommend the use of products HKS-375 brand «Orlett» as an effective, easy-to-use and secure resources in the comprehensive rehabilitation of the pathologies of the knee joint in the perioperative period. Functional orthotics modern means of external immobilization provides optimal conditions for the regeneration of bone tissue in the femoral channel and the tibia, at the same time does not lead to muscular atrophy, as evidenced anthropometric data, in particular the dynamics of the lower thigh circumference, stabilometry parameters, and subjective perception of patients the results of surgical reconstruction.

Expert solutions influence on medical rehabilitation in wounded of lower limbs

Abstract

The analysis of treatment outcome in 680 wounded limb injured in counter-terrorism operations was done. 138 people (42%) of them were found to partially fit for military service, 125 people (38%) - unfit for military service, and 67 (20%) - fit for military service with minor restrictions. A retrospective analysis showed that treatment outcomes are depend on three factors: 1) the severity of injury; 2) defects in the treatment during medical evacuation; 3) incorrect assessment of the health status of injured military medical commission premature to draw conclusions about unfit for military service. It was found that, regardless of the nature and cause of the injuries, the lower stages of medical rehabilitation of injured and runs faster than it comes in a specialized medical facility, the less the period of his stay in hospital and better treatment outcomes.

Abstract

Advances in treatment of children with hematological and oncological diseases require development of technologies of medical, neuro-cognitive and psycho-social rehabilitation of cured patients. Based on the analysis of late effects and long-term results of treatment of patients in the article proposed the concept of multidisciplinary ontogenetic rehabilitation, that provides an individual trajectory of development, education and social adaptation of sick children. The concept of rehabilitation for patients with oncological diseases is based on the data of the meta-analysis of delayed effects of the disease and aggressive treatment in all cases that require individual interdisciplinary assessment of disorders in neurocognitive, neuromuscular, physical and social spheres, on the basis of which a rehabilitation diagnosis requiring evidence based methods of correction is made.
