Abstract

Lumbal pain syndrome caused by degenerative vertebral column pathology is one of the most often reasons for applying for both outpatient and inpatient care; also it is one of the most common temporary disability reason. One of the basic ground for lumbal pain syndrome occuring is osteoartritis, damaging mainly the small joints of vertebral column. Modern possibilities of treatment lumbal pain patients by means of slow-acting medicaments' use (SYSADOA) for symptomatic treatment of osteoarthritis are considered. Data concerning the efficacy of medicine Artra use for lumbal pain patients are also considered.

Analgetic and anti-inflammatory treatment: ketoprofen in novel delivery systems

Abstract

The article focuses on the recent data on ketoprofen compound perspectives of application, as a propionate non-steroidal anti-inflammatory drug. The data is shown, on the drug properties update by October this year, and specifics of usage in some special populations. The data of current year, 2017, is provided on ketoprofen studies, e.g. novel delivery systems for local and systemic treatment, as in chronopharmacotherapy. The studies results are shown, on possible usage of the drug in children with chronic pain. The article demonstrates directions of therapeutical approaches with non-steroidal anti-inflammatory drugs.

Musculoskeletal pain accounts for about a third of all acute and chronic pain syndromes and is one of the most common reasons for patients to seek help from specialists of various profiles. However, the use of non-steroidal anti-inflammatory drugs does not always eliminate pain in the myofascial pain syndrome, which necessitates the use of co-analgesics. In the article the possibilities of muscle relaxant tolperisone in therapeutic and surgical practice in the treatment of myofascial pain syndrome at the stage of providing primary health care are considered.

Complex regional pain syndrome in the practice of the neurologist as a rare variant of neuropathic pain

Abstract

Complex regional pain syndrome is the least studied syndrome of chronic neuropathic pain, manifesting itself by constant intensive pain in extremity in combination with local vegetative disorders and trophic disturbances. Mostly it arises after different peripheral traumas. Therapy of pain syndrome is one of the most significant and difficult aspects of complex regional pain syndrome therapy. There are not so many scientific data, proving the efficacy of complex regional pain syndrome pharmacotherapy. From the viewpoint of evidentary medicine, for such a purpose could be effective anticonvulsant drugs, corticosteroids, biophosphonates and ketamine. Recommendations for opioids use, use of calcitonine, anti-depressants and also sympathetic blocks are ambiguous. No one of the separately used methodics of pharmacological, physical and psychological treatment could not be compared for its efficacy with interdisciplinary complex approach of curation patients with chronic pain syndromes. Complex biopsychosocial rehabilitation is more effective for restarting daily and working activity of patients.

Role of В group vitamins in the therapy of diabetes melitus complications

Abstract

Diabetic neuropathy (DNP) is the most often complication of diabetes melitus which could be conventionally devided into peripheral DNP touching peripheral nerves, autonomous - with vegetative innervation of organs disorders, and central - with cerebrocranial nerves damage. Often DNP goes with chronic pain syndrome, sharply decreasing life quality of patients and demanding a special therapy. Pathogenetic DNP therapy is performing by means of neurometabolic medicaments, among which vitamin B-based medicaments (thiamine (В1) first of all) occupy the important place. Mechanism of therapeutic effect of thiamine in case of DNP is connected with its ability of slowing glycolysis, lactate generating and final product of glycation, thereby decreasing the toxic effect of hyperglycemia. Moreover, it is zhown, that vitamin B1 independently by itself or in combinations with B6 and B12 vitamins is able to reduce neuropathic pain.

Abstract

Chronic pain is one of the most challenging issues of modern neurology, which is caused by the widespread prevalence of pain syndrome accompanying various diseases, especially among the working age population, negatively affecting patients' quality of life. The pathophysiology of chronic pain has been well studied and a wide range of drugs is currently being used both for relief of acute pain and for the treatment of chronic pain. Nevertheless, there is a need for the use of analgesics with high analgesic activity, which are capable of providing a continuous analgesic effect, having a narrow spectrum of adverse effects and simple to use in practice. A powerful opioid analgesic fentanyl meets these requirements. For many years fentanyl TTS has been used primarily for relief of cancer chronic pain. Now the use of fentanyl TTS is increasingly considered for non-cancer chronic pain. This article presents the results of using fentanyl TTS for chronic vertebral pain syndrome and osteoarthritis.

Sex hormones in pain management

Abstract

The problem of the role of sex hormones (SG) in pain perception and their possible place in pain syndromes of different genesis treatment is discussed. The majority of research works describe the correlation between SG blood level and pain severity in male and female patients. The overwhelming majority of chronic pain syndromes in females (migrainous headache, pain in maxillotemporal joint dysfunction, сhronic pelvic pain, etc.) have similar clinical course during menstrual cycle period: the lowest pain threshold and maximal pain sensibility under low levels of estrogen. Also it is becoming obvious the influence of blood testosterone deficiency at antiinflammatory status intensity in males, taking into account coexisting insufficient supression of the expression and inflammatory mediators' release - cytokines and chemokines. In such a way, SG balance disorder in male and female organisms helps pronocyceptive effects' generation. Early correction of SG level in neurologist's practice increases the efficaххcy of pain syndromes' therapy.

Article introduces the results of analysis of peptide medicament Laennec, pointing out at the molecular action mechanisms of the medicine at mitochondrial function. There are found Laennec peptides helping to slow down cellular apoptosis in conditions of oxidative/toxic stress, chronic inflammation and hyperinsulinemia due to Bcl-2 antiapoptotic protein activation, PPARA receptor, G6 adhesive G-protein receptor, МАР-kinases, В proteinkinase and pyruvate dehydrogenase kinases.

If it is possible to prevent metabolic remodelling possibilities of trimetazidine use

Abstract

Trimetazidine is related to those medicaments, which mechanism of action is aimed at myocardium metabllism improvement. The main mechanism of action of the medicine is connected with its possibility to suppress long-chain mitochondrial 3ketoacyl-coenzyme A-tiolaze, due to which fatty acids' pinch and oxydation are suppressed with sumultaneuos activation of glucose metabolism. Trimetazidine farmacological characteristics are considered, results of its clinical trials are discussed. Obtaind data prove, that the medicine can be used for curation of patients with different diseases, firstly cardiovascular diseases, in particular for stable stenocardia treatment, and also in case of stenocardia and diabetes melitus combination. The ability of trimetazidine to normalyze metabolic processes is fixed. That, possibly, slows the heart remodelling down.

Selected pages of the history of the ambulance se of the departmental medicine

Abstract

The review presents a unique material about the history of the departmental Medical-Sanitary Administration (Lechsanupr) established in 1918 to ensure the highest quality of medical care to the top state authorities. In 1953 the Kremlin Medical-Sanitary Administration was reorganized into the Fourth Main Directorate under the Ministry of Health of the USSR and measures were taken to improve the outpatient health care by organizing departments of emergency medical care (DEMC) to provide urgent care to those requiring immediate medical assistance. More than the 50 years' experience of these departments in the system of the Government Medical Center (GMC is the successor of the 4th Main Directorate since 1992) confirmed the relevance of their creation. Sufficient supply of all necessary medical equipment, the availability of modern ambulance vehicles and high qualification of medical staff allow DEMCs to provide professional pre-hospital care.
