The Military Medical Journal is the oldest national periodical devoted to medical issues; it has been published since January 1823.

The initiative to establish the Military Medical Journal belongs to a prominent figure in military medicine in the first half of the 19th century, Chief Military Medical Inspector of the Russian Army, President of the Medical and Surgical Academy Yakov Vasilievich Villie (1768–1854).

For many years, the Military Medical Journal was the only representative of Russia's periodical scientific medical literature. Over the years, he has turned into a priceless treasury of domestic medicine, which has absorbed many generations of physicians' vast experience. Until 1917, the magazine was headed, as a rule, by well-known military doctors, professors, and prominent organizers of the military medical service: Ya.V.Villie, P.A.Charukovskii, S.F.Khotovitskii, P.A.Naranovich, E.V.Pelikan, Ya.A.Chistovich, N.I.Kozlov, S.P.Lovtsov, N.P.Ivanov, A.I.Belyaev, A.S.Tauber, I.F.Rapchevskii.

In the 20th century until 1947, the responsible editors of the magazine were M.I.Baranov, E.I.Smirnov, N.I.Zavalishin. Subsequently, the Military Medical Journal was headed by full-time editors: in 1949–1955. - V.V.Skvortsov, in 1955-1960. - D.I.Troitskii, in 1960-1968. - I.E.Karpov, in 1968-1978. - A.V.Voropai, in 1978-1992. - V.V.Belozerov, in 1992-2007. - L.L. Galin. From May 2007 to the present, the journal is headed by M.V. Poddubny.

Since 1992, the journal is an organ of the Ministry of Defense of the Russian Federation. Frequency - 12 issues per year, volume - six printer’s sheets. 

Subject area description. Among the regular headings of the journal «Organization of medical support of the Armed forces,» «Medicine of emergencies,» «Military medicine,» «Medical and preventive issues,» «Epidemiology and infectious diseases,» «Hygiene and physiology of military labor,» «Through the pages of foreign medical Press,» «News Feed,» etc. In recent years, the subject area of publications, as a rule, has been focused on the issues of providing medical care in the context of local wars, anti-terrorist operations, technological disasters, and natural disasters. The problems of rehabilitation of the wounded and injured in the areas of conflict are widely covered. Themed issues of the journal are regularly published.

Organization of medical support for the military establishment

Development of the concept of providing medical care to the wounded with injuries to the musculoskeletal system in modern conditions.

Abstract

A modern concept of providing medical care to the wounded with injuries to the musculoskeletal system has been developed and implemented, tested during special tasks in Syria and implemented during a special military operation. The concept is based on the following principles: early and effective treatment of shock and traumatic illness, including correction of regional circulatory and microcirculation disorders; early saving (organ-preserving) primary surgical treatment of gunshot musculoskeletal wounds during the first 4–12 hours, followed by repeated surgical treatment for 24–72 hours; early stabilization of damaged segments with external fixation devices (within 24 hours); early air ambulance evacuation to the stage of providing specialized medical care; early closure of integumentary tissue defects (up to 3 weeks) using portable and stationary systems for vacuum drainage of wounds, physical methods and acute shortening techniques; early performance of reconstructive operations in specialized centers using plastic surgery techniques, external and minimally invasive internal, as well as sequential osteosynthesis within a period of 3 to 5 weeks; application of additive technologies; early comprehensive and individual rehabilitation with the return of military personnel to duty.

Relevance of the problem of chemical injury

Abstract

The historical stages in the development of ideas about chemical injury are characterized. For the first time, a detailed etiopathogenetic classification of chemical poisonings is presented according to the causes and conditions of their occurrence, modern statistical data on the frequency of cases of acute chemical poisonings, as well as related information that allows for a critical assessment of the influence of chemical factors on the population of Russia. Modern challenges for medical toxicology and military field therapy are assessed with an emphasis on current practical problems that require solutions in the near future. An important direction should be considered the implementation of round-the-clock information and advisory support, including the format of telemedicine, in the interests of medical units and primary care organizations in the provision of medical care on the territory of each constituent entity of Russia. It is advisable to resume teaching the issues of providing medical care for chemical trauma in specialty programs within the educational disciplines “Military Field Therapy” and “Military Toxicology, Radiology and Medical Protection”. Promising areas of research have been identified to improve the means and methods of preserving the lives of military personnel when exposed to chemical damaging factors.

Clinical and diagnostic features of the poisoning in the bites of poisonous snakes and approaches to the provision of medical care

Abstract

The article is devoted to the analysis of modern approaches to the provision of medical care for the bites of poisonous snakes. The differential diagnostic signs of bites of asps and viper aspis are given. Research on the development of non-specific means and schemes for the treatment of poisoning and saving the lives of victims remains an urgent task. An analysis of literature materials devoted to the search for approaches to providing medical care for snake bites made it possible to determine a complex of therapeutic areas, including the use of analgesics, antihistamines, anti-inflammatory, anticholinesterase, anticholinergic and anticoagulant drugs, and in severe intoxications, also means for combined infusion therapy. An information and analytical review of domestic and foreign clinical guidelines for the diagnosis and treatment of snake venom poisoning made it possible to formulate an algorithm for providing medical care to the affected.

The effectiveness of the central non-opioid analgesic nefopam in the treatment of acute pain syndrome in participants of a special military operation.

Abstract

The goal is to evaluate the effectiveness and safety of the use of nefopam in the provision of medical care. The study of the effectiveness of nefopam was carried out in 2 groups: the 1st group (n=53) included patients who, because of dissatisfaction with analgesia, were additionally prescribed a non-steroidal anti-inflammatory drug and trimeperidine at a dose of 20 mg within 30 minutes after the administration of nefopam; Group 2 (n=201) received only nefopam. The administration of nefopam was accompanied by a decrease in pain intensity, normalization of heart rate, and respiratory movements in all 254 patients. The reason for the ineffective relief of pain in group 1 was the development of an initially severe pain syndrome. The likelihood of developing side effects (nausea, vomiting, tremors, sweating, subcutaneous erythema) was 2–9 times lower in group 2. Nefopam is recommended for the relief of pain of moderate intensity; its effectiveness for the relief of severe pain is insufficient.

Surgery for pulmonary tuberculoma

Abstract

With new and effective anti-tuberculosis drugs in the arsenal of conservative therapy and the development of a modern chemotherapy regimen for tuberculosis patients, discussions arise between phthisiatricians, therapists and surgeons about the advisability of using a surgical method of treatment. It is known that blood vessels are located only in the tuberculoma capsule and do not penetrate the caseosis, i.e., the foci of encysted caseosis that makes up the tuberculoma are avascular. In this regard, anti-tuberculosis drugs practically do not penetrate caseosis. Therefore, the principal method of treating pulmonary tuberculoma is surgical. The authors, using a large amount of their own clinical material, presented indications and contraindications for surgical treatment of patients with pulmonary tuberculoma, as well as modern methods of surgical technique for unilateral and bilateral pulmonary tuberculoma.

Complications of pleural drainage in the practice of a military doctor

Abstract

Chest injury is the most common type of combat injury. With the introduction of new approaches and methods of medical care in combat conditions, mortality rates from this injury have noticeably decreased, which is confirmed by the results of the operations of the US armed forces and their allies «Iraqi Freedom» and «Enduring Freedom». However, at the stages of medical evacuation, the number of deaths from wounds and chest trauma, in comparison with the Vietnam war, increased from 3 to 10%. The advanced stages of evacuation are characterized by early complications – inadequate choice of the drainage site, subcutaneous placement of drainage, intraoperative damage to the vessels, organs of the thoracic and abdominal cavity, in high-tech centers – late – coagulated hemothorax, pneumonia, pleural empyema, and several others. The study of clinical and diagnostic approaches to the provision of medical care to wounded with chest trauma is an urgent problem requiring, among other things, structuring the complications of pleural cavity drainage, optimization of diagnostic algorithms, and improvement of training methods for medical specialists.

On the structure of cardiovascular risk factors in male military personnel with myocardial infarction and chronic inflammatory lung diseases

Abstract

The structural features of cardiovascular risk factors in military personnel with chronic inflammatory pulmonary diseases during myocardial infarction were assessed to improve prevention. The study included male military personnel aged 19–60 years with type I myocardial infarction. The patients were divided into two age-matched groups: I – study group, with chronic inflammatory lung diseases – 10 patients; II – control, without them – 72 patients. A comparative analysis of the frequency of observation of main and additional cardiovascular risk factors in the selected groups was performed. Patients in the study group do not differ from the control group in the frequency of most of the main and additional risk factors for cardiovascular pathology, except for smoking (100% in the study group and 65.3% in the comparison group; p=0.01) and chronic infections of internal organs (100 and 26.4%, respectively; р<0.0001). In the study group, more pronounced disturbances of carbohydrate metabolism were revealed in the first hours of myocardial infarction and lipid metabolism at the end of the subacute period of the disease, which indicates a less favorable prognosis of the disease and the need for appropriate preventive measures .

Vaccination of viral infections: history and current problems

Abstract

The article reflects the historical evolution of approaches to solving the problem of vaccinal prevention of viral infections. The effectiveness of modern vaccine prophylaxis is associated with advances in biotechnology, immunization with attenuated viruses, purified viral proteins, specific antibodies, and activated T-lymphocytes. The success of vaccination depends on the formation of collective immunity. Immunization should cause the same type of immune response as the infection. The effectiveness of vaccination is affected by options for the introduction of vaccines, violations of the «cold chain», the age and health of the vaccinated. Prospects for vaccination are associated with new options for obtaining subunit vaccines, vaccines based on nucleic acids. They are characterized by rapid development, low-cost and safe production, and effective activation of cellular and humoral immunity. Increasing the effectiveness of vaccine prevention of viral infections will also be associated with further study of the biology of viruses and the immunopathogenesis of viral infections, taking into account the individual immune response.

On the prevention of respiratory diseases among conscripts

Abstract

The article presents the results of a comparative analysis of the incidence of respiratory diseases in two groups of conscript military personnel – those vaccinated with the Pneumo 23 vaccine and those not vaccinated with this vaccine. The results indicate a decrease in the incidence of respiratory diseases in the first group. The Pneumo 23 vaccine has low reactogenicity when used with other vaccines, as well as significant epidemiological effectiveness against other pneumococcus – acute sinusitis and acute otitis media. The drug is compatible with vaccines of the Russian vaccination calendar (live measles and mumps, ADS-M toxoid) and can be recommended for the prevention of pneumococcal infection in groups with a high risk of developing respiratory tract infections.

The feat of pharmaceutical workers in evacuation hospitals of besieged Leningrad

Abstract

The article presents characteristics of the activities of pharmaceutical workers in evacuation hospitals of besieged Leningrad, including those deployed based on the Military Medical Academy. Ways to overcome difficulties in the manufacture of medicines in conditions of lack of electricity and centralized water supply, shortage of pharmaceutical substances and excipients, glassware and closures are shown. Information is provided about the professionalism and heroism of pharmacists and pharmacists who worked in the pharmacies of evacuation hospitals, who, like all the defenders and residents of the besieged city, steadfastly and courageously overcame hardships, showed dedication and patriotism.

Abstract

The article is devoted to the history and present day of the branch of the 1029th Center for State Sanitary and Epidemiological Surveillance of the Ministry of Defense (Vladivostok), formed as a sanitary and epidemiological laboratory in 1933. During the Great Patriotic War, laboratory specialists took an active part in carrying out measures to prevent the spread of infectious diseases among army and navy personnel and the civilian population. Over the course of 90 years, the institution has trained many scientists who have contributed to the development of domestic military hygiene and epidemiology. In the COVID-19 pandemic, the institution successfully completed its tasks. Currently, the branch is distinguished by the high professionalism of its employees and their dedication to the traditions of domestic medicine.

The author told about the collection on the basis of the N.I. Pirogov Saki military clinical health-resort. The priority area today is medical rehabilitation and medical-psychological rehabilitation of active military personnel of the Armed Forces of the Russian Federation – participants in a special military operation. Some work has been carried out to restructure the activities of medical institutions of the sanatorium-resort complex, and active training of qualified rehabilitation specialists is underway. Night of Museums" at the Military Medical Museum
Abstract

At the end of May last year, the Military Medical Museum took part in the annual “Night of Museums” event. A rich program of events was prepared for visitors of different ages at five venues.

Particular emphasis in the museum’s program was placed on the 225th anniversary of the Military Medical Academy, whose representatives took an active part in preparing and holding events at the museum’s sites.
