New targets of treatment of patient from the risk group for vascular complications of atherosclerosis

Abstract

Based on the results of recent studies of statins (JUPITER, etc.) discussed the potential positive therapeutic effects (anti-inflammatory, antithrombotic) of this group medicine that could be grounds for revision of guidance for their use. Emphasized the importance of the statins impact on C-reactive protein (CRP), given the fact that the latter is an active participant in atherothrombosis. We showed that the relatively healthy people with high CRP levels treated with active statin, had not only myocardial infarction or stroke risk reduction, but also frequency reduction of thromboembolic complications and death from cancer. The author believes that the statins administration in the normalization of CRP should be another factor tracked during treatment, in addition to the level of low density lipoprotein.

Nonselective beta-adrenoblocker carvedilol with vasodilating properties in the therapy of cardiovascular diseases

Abstract

Beta-blockers (BAB) widely used in clinical practice. Large clinical studies have shown that BAB can improve the prognosis in patients with cardiovascular diseases. However, recent years have increasingly raised the issue that BAB reduce the stroke risk in patients with arterial hypertension (AH) less effectively compared with other antihypertensive drugs, and are metabolically unsafe drugs. Carvedilol - nonselective beta1- and beta2- adrenoreceptors blocker with antioxidant action and vasodilating properties due to the additional blockade of alfa1-adrenergic receptors, has proven clinical efficacy in patients with hypertension, ischemic heart disease, congestive heart failure. In addition, large clinical studies of carvedilol safety demonstrated its application is not accompanied by disturbances in lipid and carbohydrate metabolism that creates additional benefits for the use of this drug in clinical practice.

Abstract

Ischemic stroke (IS) is an important medical and social problem due to high level morbidity as well as significant temporary disablement and disability rates in the population. In terms of secondary prevention of IS and patient's quality of life improvement, well-founded pathogenic management of patients experienced acute cerebral circulation disorder at out-patient stage is the most significant and economically justified. Current principles of treatment of patients with IS at out-patient stage based on competent and individualized antihypertensive therapy, lipid-lowering therapy, correction of the manifestations of diabetes mellitus, and antithrombotic therapy. This article provides an overview of the main classes of drugs used for stroke secondary prevention.

Cardiologic complications of acromegaly

Abstract

A frequent complication of acromegaly in any age patients is akromegalic cardiomyopathy, characterized by the formation of left ventricle myocardial hypertrophy (LVMH), and the occurrence of heart rhythm disorders. In cases with no adequate controls of hormonal disturbances in acromegaly, cardiomyopathy leads to the formation of diastolic heart failure. Surgical and pharmacological (using somatostatin analogues) treatment of acromegaly leads to the improvement of the heart condition and regress LVMH, especially in young patients (younger than 40 years) with a small duration of disease (no more than 5 years). Meaningful morpho-functional and hemodynamic parameters improvement observed in middle-aged patients.

Abstract

Milgamma and Milgamma compositum administration provide a wide range of favorable therapeutic effects: neuropathic pain relief, prevention of retinopathy progression, the elimination of oxidative stress in retina, brain, kidneys tissues, reducing endothelial dysfunction and endothelial apoptosis, lipid metabolism improvement, glucose metabolism normalization in patients with diabetes mellitus. This defines a favorable effect Milgamma and Milgamma compositum on the development of macro- and microvascular complications of diabetes. In addition to diabetes, these drugs administration favorably influence on course of uremia, including patients on dialysis, and may be useful in a number of other states, accompanied by thiamine deficiency.

Abstract

The present article is concerned with pathogenetic mechanisms of gastrointestinal tract (GIT) motor function disturbances. Both primary and secondary motor disturbances accompanied by the emergence or intensification of dyspeptic symptoms. Domperidone is widely used in complex therapy of many digestive diseases, accompanied by motor-evacuation disorders of the upper digestive tract. It is emphasized that the domperidone use in patient with gastroesophageal reflux disease, functional dyspepsia, gastric ulcer and duodenal ulcer and other diseases involving stomach and duodenum motor disorders, thus enable reduce diseases exacerbation time/period and significantly improves the patients quality of life due to the rapid and effective dyspeptic symptoms relief. Domperidone is also used successfully for the prevention and symptomatic treatment of nausea and vomiting of various origins. We give current clinical practice guideline for domperidone use.

Abstract

We outlined current understanding about the clinical presentation and psychopharmacotherapy management of patients with anxious-phobic (APD) and anxiety-depressive disorders in general practice. The present article is dedicated to the issues of classification, APD diagnosis, given special attention to panic disorder. We emphasized not only the high comorbidity presence of anxiety disorders specific variants, but their frequent combination with depression. Presented main groups of drugs used to treat anxiety-depressive disorders: benzodiazepine tranquilizers, antipsychotics, selective serotonin reuptake inhibitors group of antidepressants, tricyclic antidepressants.

Hysteria in the therapeutic practice

Abstract

In clinical practice, hysterical, neurological and somatic symptoms are often observed. Until now, these patients were classified as "difficult patients". The article presents the current classification of hysterical disorders, according to International Classification of Diseases, 10th Edition. We described the typical hysteria neurological manifestations: seizures, paresis, walking disturbances and hyperkinesis. We distinguished features of hysterical symptoms observed in therapeutic practice. We presented diagnostic criteria for psychogenic pain, characteristics of hysterical gastro-intestinal and respiratory disorders, psychogenic fever and hemorrhagic syndromes. The possibilities of pharmaco-and psychotherapeutic approaches to treatment of hysterical syndromes are discussed.

Evaluation of rifaximin efficacy in complicated course of intestinal diverticular disease

Abstract

The results of the study that included 17 patients with diverticular disease (DD) complicated by diverticulitis, treated with rifaksimin in combination with symptomatic therapy are presented. Patients received antibiotic therapy in hospital within 10 days, and then patients were encouraged to continue to receive rifaksimin for 7 days within 3 months in the outpatient setting. Control group consisted of 16 patients with diverticulitis treated with ciprofloxacin for a similar regimen in conjunction with symptomatic therapy. Dynamic assessment results in 3 months later confirmed sufficient rifaksimin efficacy in patients with DD complicated by diverticulitis. Patients treated with rifaksimin had decrease frequency of abdominal pain on a background of moderate intestinal dysfunction. Rifaksimin therapy was accompanied by a reduction of inflammatory changes in the intestinal mucosa, colon microecology improvment. We observed positive dynamics in the number of mast cells as a marker of abatement of acute inflammation in the rifaksimin group. Thus, the study showed a high therapeutic efficacy of selective intestinal antibiotic in patients with diverticulitis.

Abstract

The results of the study of environmental influences and xenobial load on the features of the formation of a pathological process in 159 children with chronic Helicobacter pylori- associated gastroduodenitis and living in areas of large industrial settlements are presented. Based on these data, we formulated principles for choosing drug for eradication therapy and performed clinical evaluation of recommended treatment regimen. Enterofuril (nifuroxazide) in suspension in a daily dose of 800 mg in combination with eliminative action drugs was most effective and safety treatment.

Abstract

The results of a comparative study evaluated the intake of Magne B6 various forms (solution for drinking and tablets), which contains an organic salt of magnesium, and Berokka Plus, which contains inorganic salt of magnesium in 16 volunteers are presented. These data suggest that the Magne B6 statistically significantly raises levels of magnesium in plasma and erythrocytes, while the comparison drug are not lead to statistically significant differences in magnesium levels before and after use. Drug absorption dynamics in the intestine of various forms Magne B6 are different. Thus, liquid form of Magne B6 more effectively raises the levels of magnesium in plasma compared with the control within 2-3 hours. At the same time, Magne B6 in tablets contribute to a more prolonged retention of high concentrations of magnesium in red blood cells within 6-8 hours.
