Preduprezhdenie tromboembolii u bol'nykh fibrillyatsiey predserdiy
Abstract
This review presents new data on the efficacy and safety of use of acetylsalicylic acid, warfarin, dabigatran, rivaroxaban, and apixaban for the prevention of thromboembolism in patients with atrial fibrillation. Taking into account the results of large randomized clinical trials, it can be argued that the new anticoagulants are preferable for the vast majority of patients with non-valvular atrial fibrillation compared with warfarin, because they provide better efficiency, safety and convenience of treatment. At the same time, the new anticoagulants can not yet compete with warfarin for patients with mechanical heart valves.

Vyrazhennost' kislotosupressivnogo deystviya ingibitorov protonnoy pompy i effektivnost' sovremennykh eradikatsionnykh skhem
Abstract
In many Western countries, Japan and Australia, a trend toward reduction in the incidence of peptic ulcer disease and noncardiac gastric cancer, directly correlated with a decrease in the prevalence of Helicobacter pylori infection in the population, is observed. Almost three decades of practical application of antihelicobacter therapy, a hundreds of clinical studies and more than 100 meta-analyses of controlled studies clearly suggest that there is no guarantee of success, regardless of the chosen regimen of treatment. This article considers the significance of the antisecretory treatment in eradication schemes, are discusses the prospects for improvement of efficacy of antihelicobacter therapy by increasing doses of proton pump inhibitors.

Abstract
The article discusses the reasons that lead to falls in elderly and senile patients. Multidisciplinarity of problem of prevention of falls in this category of patients is emphasized considering that there are physical and neurological causes of falls. Recommendations for neurological and physical examination of patients with the risk of falls are presented. It is emphasized that the drug therapy of patients with falls is determined by their causes. Ergot alkaloids are widely used for the treatment of vascular and primary degenerative diseases as well as dizziness.

Pleyotropnye effekty blokatorov kal'tsievykh kanalov tret'ego pokoleniya pri lechenii serdechno-sosudistykh patologiy, assotsiirovannykh s metabolicheskim sindromom
Abstract
Metabolic syndrome (MS) is a cluster of risk factors in cardiovascular continuum, and pandemic of XXI century as predicted by experts. Treatment of arterial hypertension (AH) as a component of MS is difficult for the general practitioner when it is necessary to take into account not only organoprotective and antihypertensive, but metabolic effects of drugs. This review summarizes the evidence on the effectiveness of calcium channel blockers (CCBs) in the treatment of hypertension combined with MS. Lercanidipine is III generation calcium channel blocker, characterized by a pronounced vasoselectiveness, a long period of antihypertensive action and the positive metabolic effects, has organoprotective influence with minimal side effects compared to other CCBs, which ensures a good tolerance and treatment compliance.

Nealkogol'naya zhirovaya bolezn' pecheni: novye vozmozhnosti kombinirovannoy terapii
Abstract
Nonalcoholic fatty liver disease has a high prevalence in the population worldwide. In the pathogenesis of the disease, insulin resistance and oxidative stress of hepatocytes and the subsequent development of inflammatory and necrotic changes in the liver play a leading role. Treatment is aimed at weight loss, overcoming insulin resistance, prevention of oxidative stress, and normalization of intestinal biocenosis. One promising area in the treatment is the use of combined hepatoprotectors acting on the various links of pathogenesis of the disease. An example is the combination of essential phospholipids and substances with antioxidant and metabolic action - vitamin E and L-carnitine.

The article demonstrates the need for screening of diabetes mellitus (DM) for the purpose of timely detection of early disturbances of carbohydrate metabolism. These results show the effectiveness of drug therapy with metformin in patients with impaired fasting glucose in relation to reducing the rate of conversion of impaired fasting glucose into impaired glucose tolerance and type 2 diabetes mellitus.

Abstract
The most common complication of herpes zoster is postherpetic neuralgia (neuropathy). Pain that lasts for more than 3 months after the onset of herpes zoster infection is called postherpetic neuralgia. The incidence of postherpetic neuralgia depends on the patients age: 50-75 % of patients in the seventh decade develop postherpetic neuralgia after an infection with herpes zoster. The clinical appearance of postherpetic neuralgia is characteristic. Three different pain types can be distinguished: 1. spontaneous, constant, burning pain, 2. intermittent sharp pain, 3. pain in response to a normally non-painful stimulus. Other phenomena which can be observed are hyp- or anaesthesia, and dysaesthesia. In 20 % of all cases of herpes zoster, the innervations territory of the ophthalmic nerve is affected. Ramsay Hunt syndrome is a special form of herpes zoster which is typically characterized by peripheral facial palsy and unilateral herpetic vesicles on the ear. These symptoms are often accompanied by vestibulocochlear dysfunction and other neurological symptoms. Management options include early antiviral treatment and gabapentinoids, tricyclic antidepressants, controlled release opioids.

Dezloratadin - sovremennoe effektivnoe sredstvo lecheniya allergicheskogo rinita
Abstract
The article considers potentials for the use of desloratadine, second generation antihistamine, in the treatment of allergic rhinitis (AR). Pharmacological properties of desloratadine are presented, the results of its clinical application are discussed. It is emphasized that desloratadine is the drug of first choice in the treatment of AR, especially for the various defects of the immune system and in the case of combination of AR with other atopic conditions.

Abstract
In recent decade, gastroesophageal reflux disease (GERD) is attracting more attention due to the marked tendency to increasing the frequency of its occurrence. Significance of GERD is determined not only by its prevalence, but also by complicated course of disease. Over the last decade, severe reflux esophagitis (RE) were observed 2-3 times more frequently. Barrett’s esophagus, a pre-malignant condition, develops in 10-20 % of patients with RE. In addition, it was found that gastroesophageal reflux occupy an important place in the genesis of a number of ENT-diseases and pulmonary diseases. Substantial progress was achieved in the diagnosis and treatment of GERD. The introduction of a 24-hour pH-metry allowed to diagnose the disease at a stage when the endoscopy not reveal visible change in the esophageal mucosa. Widespread use of new drugs in clinical practice, especially proton pump inhibitors (PPIs), has significantly enhanced the treatment of the disease, even in severe GERD. At the same time, practitioners and patients themselves underestimate the importance of this disease. Patients see a doctor late, and are treated oneself even in severe symptoms. Physicians, in turn, know little about the disease, underestimate its impact, and administrate irrational therapy of RE.

Lechenie psikhicheskikh zabolevaniy v period beremennosti
Abstract
The article considers the different aspects of mental disorders during pregnancy. General principles of treatment of mental disorders in pregnant women are presented. Questions of safety of psychotropic drugs for mother and child (fetus) are discussed. It is emphasized that decision making about administration of psychotropic drugs during pregnancy should be individual in each case. Planning for pregnancy, discussion of potential risks and therapeutic opportunities is extremely important for women with mental disorders. In some cases, withdrawal of psychotropic drugs for the entire period of pregnancy, or in some critical periods is feasible. In case of need for psychopharmacotherapy, careful selection of drugs in view of the course of disease and its negative effects on the fetus and the health of the pregnant woman is necessary. Monotherapy is preferred if possible. In all cases, careful monitoring of the woman and fetus throughout the pregnancy is required.

Abstract
The effect of the drug Cytochrome C in the combination treatment of 60 patients with coronary heart disease complicated by arrhythmias and/or heart failure was examined. The study was aimed to evaluation the clinical efficacy and safety, anti-ischemic and antiarrhythmic action of this drug. The results were assessed at the hospital stage of treatment and six months after treatment with regard to occurrence of various cardiovascular events in patients. Outcome measures in study included re-hospitalization, acute myocardial infarction, acute stroke, and death.

Effektivnost' primeneniya preparata Lavenum® u bol'nykh varikoznoy bolezn'yu, perenesshikh venektomiyu nizhnikh konechnostey
Abstract
The article presents the results of the study involving 24 patients who underwent phlebectomy for the primary varicose veins of the lower limbs. Of these, Lavenum® gel was applied on the area of affected vein in 12 patients on 2 nd day after surgery for 14 days 2-3 times a day. Based on the data of the study, rationale for the use of the gel and the scheme of the use of Lavenum in the postoperative period after phlebectomy of lower extremities are provided. The clinical, physiological and laboratory results demonstrating the effectiveness of use of the gel are presented.
