Actual issues of cardiorehabilitation in a new russian healthcare reality

Abstract

Cardiac Rehabilitation in Russia at the present time is once again at the beginning; existed since 1981. The state system of phased cardiac rehabilitation has ceased to exist. Today it is possible to recreate a new system in Russia cardiorehabilitation (CR) in accordance with the law «On the basis of the health of citizens in the Russian Federation» dated 2011. At the same time help in medical rehabilitation is depending on the severity of the patient in three stages. The first (I) stage is carried out in the acute phase of the disease in the Intensive care unit specialized cardiology units of hospitals. The second (II) stage - in offices cardiorehabilitation during convalescence. The third stage (III) is carried out in the period of residual effects of the disease in outpatient centers cardiorehabilitation. KR is, regardless of the timing of the disease, provided the stability of the clinical condition of the patient and the availability of rehabilitation potential. Assistance for cardiac rehabilitation carried out by members of a multidisciplinary team who have been training for the organization and methods of rehabilitation treatment cardiology; nurses who have specialized training in physical therapy and massage, patient care, as well as specialists with higher non-medical professional education in the field of physical therapy, medical psychology, career counseling and occupational therapy. Of particular importance in the management of patients with cardiological centers attached to cardiac rehabilitation (CDC). Dispensary observation in the CCR is primarily subject to the patients who have had acute myocardial infarction, acute coronary syndrome, heart surgery and coronary arteries, including patients receiving invasive treatment interventions. CDC task - to perform a comprehensive program of medical rehabilitation and secondary prevention for 12 months after acute coronary incidents. CDC should be equipped with cardiac diagnostic equipment. Their distinctive feature - the presence of the disposal velatrenazhërnyh computerized systems with high bandwidth. The paper discusses in detail issues devoted to: 1) the potential rehabilitation of patients; 2) different stages of rehabilitation; 3) the degree of coverage of rehabilitation patients, 4) physical training program; 5) program for increasing treatment compliance.

Evaluation of long-term experience cardiac rehabilitation in a rehabilitation center «Black River» allowed to formulate general technology rehabilitation of cardiac patients in different categories. The key points of technology at the 2nd stage of rehabilitation are the risk stratification, evaluation classes severity of patients in a rehabilitation center, multidisciplinary rehabilitation. In practice cardiorehabilitation used notion of rehabilitation potential and prognosis of rehabilitation. Suggested modular design personalized rehabilitation programs. The modular structure allows individual programs to achieve optimum results of rehabilitation, facilitating current and final control effectiveness.

Increased efficiency of staged medical rehabilitation of patients with coronary heart disease following coronary bypass surgery with the use of modern organizational technology

Abstract

In the context formed the legal framework of medical rehabilitation and development of its system acquires particular importance in the improvement of organizational technologies stages of medical rehabilitation. Organization of an integrated system of specialized medical rehabilitation of patients with coronary heart disease after coronary artery bypass surgery, subject to the phasing, continuity and multi-disciplinary therapeutic measures - one of the important tasks of national health care. The results of investigations 170 patients with coronary artery disease after coronary bypass surgery in a rehabilitation center. Basis to develop ways of increasing the effectiveness of the landmark medical rehabilitation of patients after coronary artery bypass grafting in a rehabilitation center made organizational technology of interaction of the first and second stages of medical rehabilitation, standardization and organizational quality management system rehabilitation. Improving the organization of medical rehabilitation at the second stage allows to reduce the overall length of inpatient rehabilitation by 33.3% at achieving better health outcomes. Modern management level diagnostic and treatment process enhances the quality of medical rehabilitation with 65,6 ± 3,19% to 87,9 ± 1,41% (p <0.001) with increasing its efficiency by 2 times.

The influence of low intensity EMR on the state of tissue blood flow in the skin loskute of rats

Abstract

The article analyses the results of the pilot study of the impact of electromagnetic Terahertz range on the dynamics of mikrogemokrovotoka in operirovannom tissue loskute. The experiment was conducted on male rats breed lab rats Wistar in accordance with the requirements of the Geneva Convention «International Guiding Principles for Biomedical Research Involving Animals». Violations in the intravascular component were modeled using the technique of microcirculation of blanking dies dorsal skin flap that resulted in severe circulatory disorders in the distal half of the graft. Animals (22 rats) were divided into two equal groups, one of which after the operation had not been subjected to a control. In experimental animals unit daily for seven days of 1 times per day have noise electromagnetic radiation with a frequency range 110-170 GGz. The impact started in day surgery and continued daily until the seventh day, turn to continued daily until the seventh day inclusive under the supervision of clinical data with fotoarchive, the dynamics of Thermal Imaging and Laser Doppler Floumetria. Reliability of differences results in the experimental and control group tested the U-test of Mann-Whitney. For the first time identified the particular effects of the radiation regime containing broadband frequency fundamental molecule-metabolites: nitrous oxide and oxygen, on microcirculatory in the skin of rats loskute moved. Clinically in the experimental group of distal necrosis area averaged 26,3% compared with 46,5% in control, which is confirmed by the data of Thermal Imaging and Laser Doppler Floumetrii. The results have important practical significance in the development of new medical rehabilitation technologies for correction of microcirculatory disorders and associated equipment.

Abstract

The article reveals the author’s concept of paternalnyh physician-patient relationship, defined by their types and substantiate the nature of their influence on the formation of internal picture of disease in cardiac patients. Presents the results of an empirical study on the impact of the image of a cardiologist on attitudes toward the disease of cardiac patients, their health, activity and mood. In the process of the emergence and development of disease mechanisms violated interaction between man and the environment, so that the processes are effective mental adaptation included compensatory mechanisms aimed at the restoration of the interaction to a new level - in the situation of the disease. In this case, the clinical manifestations can be very diverse, depression or euphoria; apathy, weakening of emotional response, the weakening of criticism, self-centeredness, etc. Most of the patients, according to our research, assess their condition without exaggeration tendency of its severity, but without underestimating her. For many of them characterized by a tendency in all actively contribute to the success of treatment and devote themselves to the areas of life that are possible with their disease. Clinical manifestations of physical illness is largely due to the peculiarities of the vision of the disease by the patient and his attitude to the new circumstances of life in the face of illness and treatment, that is, characteristics and features of existing internal picture of his illness. The article reveals the essence and content of internal picture of disease cardiac patients, determinants of its formation and development. As one of the dominant factors considered factor of professional and personal characteristics of the attending physician, its image, formed as objective characteristics of his personality and subjective evaluations of his patients as a professional -cardiac patients. The authors, determining the structure of the internal and external image, especially considering medical image formation through the prism of his relationship to the patient’s position in cooperation with him and his system of professional and personal orientations. Justifying the legitimacy and usefulness of the category «paternalnye relations» in the study of doctor-patient relationship, the authors reveal a new vision of the psychological essence of this category, reflecting the relations developing between subjects by virtue of their having need for medical care and personal support. Using cluster analysis of the empirical data revealed professionally qualified, information and business, alienated, paternalno-caring, paternalno-prescriptive and paternalno-affiliate doctors types, their characteristics and the professional and personal characteristics, as well as their impact on the subjective attitude of patients to their physical illness and somatic status.

First results and contents of rehabilitation program for patients with coronary diseases

Abstract

This article concerns two stages of cardio rehabilitation for patients after coronary revascularization in Ivanovo State Academic Hospital. The rehabilitation program was made up in accordance with multidisciplinary approach. First results of three weeks’ complex medical rehabilitation in patients after coronary revascularization are presented. This program leads to the significant increase of physical tolerance, considerable improvement of psychological status and quality of life. It also provides additional antihypertensive effect and further normalization of lipid profile. It is shown the strong need of organization and implementation of unified teaching program for different specialists participating in multidisciplinary team.

Modern methods of medical rehabilitation at the acutest cardioembolic stroke, accompanied by heart failure

Abstract

The article presents the use of medical rehabilitation in 45 patients in the acute phase cardioembolic stroke. Evaluation of the effectiveness of rehabilitation on a scale Barthel and Rankin and convincing evidence of the 0 that with increasing degree of heart failure require strict application of restrictive measures. The most common complication of rehabilitation patients in these patients was orthostatic hypotension. We have developed an approach for the management of orthostatic hypotension with verticalization of stroke patients, which included: correction of water and electrolyte balance by optimizing fluid therapy; correction medical treatment of heart failure; sparing regimen verticalization. It is proved that in the context of basic medical treatment, early verticalization on complex «ERIGO» was more effective than the control group in the application of restrictive measures. With the development of arrhythmias in these patients require additional antiarrhythmic therapy on the background of ongoing rehabilitation. Exacerbation of symptoms of heart failure in the form of paroxysmal arrhythmias, pulmonary hypertension rise requires additional use of drugs, as well as compensation for medical rehabilitation programs in the form of reducing the volume and intensity.

Physical rehabilitation after heart transplantation

Abstract

In the article the own experience of dynamic supervision, since the early postoperative period and within 3 years after the orthotopic heart transplantation performed in 2011-2012 in the Republican Scientific and Practical Center «Cardiology» is stated. We developed original program of physical rehabilitation taking into account a clinical condition of the patients, complications of the early and late postoperative period, individual tolerance to physical loading according to spiroergometry bicycle stress test. We also performed the assessment of its efficiency developed rehabilitation program. We found significant improvement not only in tolerance to physical loading but also in maximal oxygen consumption at patients after heart transplantation. Above mentioned improvement was more significant in patient used developed program of physical rehabilitation. In 14-36 months after heart transplantation in 12% of the patients received the developed program of rehabilitation and at 38% of its not received we found myocardial ischemia in stress test. Appearance of myocardial ischemia may be a sign of transplant heart coronary artery disease little less often is patients received developed rehabilitation program.

Gender, type of personality and the quality of life of patients after one year of operation, coronary bypass surgery

Abstract

The study included 408 patients who underwent CABG planned for the period from 1 February 2009 to 31 January 2010. Depending on the gender and the presence of type D personality formed four groups: Group 1 - women with type D (n = 20), group 2 - women without type D (n = 51), group 3 - men with type D (n = 73), group 4 - men without type D (n = 264). All patients before CABG and one year after he assessed the quality of life using the SF-36, the level of depression and personality type D. Type D personality in the preoperative period was detected in 28.2% of women and 21.7% of men (p = 0.234). Women with type D often detected multifocal atherosclerosis (p = 0,043) and a higher risk of coronary artery bypass grafting on a scale EuroScore (p <0,001). QOL in patients with type A personalities, both before and after CABG were lower (p <0.05). At the same time one year after CABG showed a significant increase in performance of the physical component of QoL only among men, and psychological component of quality of life - in all patients (p <0.05). The presence of type D personality before surgery increased the risk of detection of a low quality of life one year after CABG - for the physical component of 1.93 times (p = 0.029) for the psychological - 1.62 times (p = 0.072). On the physical component of QoL independently influenced the level of depression and the presence of diabetes, psychological component only on the level of depression (p <0.05). Before the surgery, CABG personality type D was observed in men and women with equal frequency. Quality of life before and after CABG was lower in patients with type A irrespective of gender. Improving the psychological component of QoL after CABG was noted in both men and women, and physical - only in men. Gender patients had an independent association with the presence of a low quality of life one year after CABG.

Hock-wave therapy in the complex treatment and rehabilitation of patients with coronary heart disease with refractory angina

Abstract

Coronary heart disease (CHD) is the leading cause of death worldwide. A number of patients, despite conducted medical and surgical therapy over time reappears in the form of coronary artery disease refractory angina. One of the new treatments for refractory angina in patients who can not be carried out surgical treatment and medication is not effective, is the method of shock wave therapy (SWT). When UVT acoustic wave focuses on the periphery of the ischemic area of the myocardium. Effects on endothelial cells causing «shear effect», stimulates the synthesis of nitric oxide causes vasodilation and myocardial perfusion improvement also results in the release of several inflammatory factors promoting neoangiogenesis. The efficacy of using UHT treatment of CHD patients with refractory angina. There were 46 male patients with coronary heart disease c refractory angina III-IV functional class (FC). Age 59 to 71 years, average 65.8 + 1.4 years. Two groups of 23 people, most (OG) and control (CG). OG patients along with standard medical therapy was prescribed a course of UHT heart. The course consisted of UHT heart 9 procedures. Exposure was conducted for 3 zone 100 beats per zone at an energy level of 4.0. The acoustic wave is induced on the boundary of the identified areas of hypokinesia and normokinezii, areas of hibernating myocardium. After a course of UHT number of angina attacks requiring additional nitropreparatov receiving short-acting, decreased in OG group with 18,2 ± 1,9 to 3,2 ± 0,9 per week (p <0.01) in the CG group 15, 9 ± 2,1 to 7,3 ± 1,2 (p <0.01). 6 months after the end of UHT number of angina attacks decreased in the exhaust gas from 18,2 ± 1,9 to 2,2 ± 1,6 (p <0.01), a week in the CG group 15,9 ± 2, 1 to 10,3 ± 1,5 (p <0.05). Patients OG fewer ischemic segments with 4,3 ± 0,4 to 2,8 ± 0,1 (p <0.01).

Abstract

The article presents the results of a study of 220 patients with metabolic syndrome. Comparative analysis revealed the advantage of an integrated application of a diet, electrostatic massage, multifactor influence of «Alfa Light Led OxySpa» and «Hubert», psychological correction system «Shufrid» compared to monomethod. It was proved that the exchange application has developed comprehensive programs autonomic function and improve the mental health, improving the functional capacity of the cardiovascular system and physical health in patients with metabolic syndrome in a multidisciplinary departmental sanatorium.
