POTREBNOST' VZROSLOGO NASELENIYa ROSSII V MEDITsINSKOY REABILITATsII

Abstract

Больные, обратившиеся в стационарные или амбулаторно-поликлинические учреждения за медицинской помощью и получившие лечение в острой стадии заболевания, должны быть сразу направлены на медицинскую реабилитацию в стационарных или амбулаторных условиях, что позволит снизить общий уровень заболеваемости и инвалидности в стране, особенно среди трудоспособного населения. В ходе данного исследования определена доля нуждающихся в подобной реабилитации от всех лечившихся амбулаторно и стационарно больных в зависимости от поставленного диагноза. С помощью знаний о коэффициенте потребности в медицинской реабилитации в амбулаторных и стационарных условиях от всех лечившихся амбулаторно и стационарно больных по каждому классу болезней и отдельным заболеваниям можно определить потребность в этом виде медицинской помощи на любой территории

Abstract

The efficiency of the use of low-frequency "running" of the magnetic field from the device "Polimag" in rehabilitation of patients with chronic venous insufficiency of the lower extremities. Conducted on 60 patients, the study showed that the inclusion in the basic treatment of such patients "running" of the magnetic field is effective, improving quality of life by affecting the main pathogenetic links of the disease (reduction of venous hypertension, improve microcirculation processes, increased lymphatic drainage, anti-edema action, anti-inflammatory effect), and can be applied in patients with chronic venous insufficiency of the lower extremities to prevent complications, and in the rehabilitation of patients with complicated forms of the disease in order to relieve their clinical manifestations and prevention of disease progression and recurrence of the disease. Abstract

The article presents the results of a comparative randomized studies on the effect of common hydrogalvanic baths on clinical and functional parameters of patients with type 2 diabetes complicated by diabetic angiopathy of lower extremities. It is shown that the clinical efficiency of the new technology is based on the combined impact of two different factors: hydro - and electro-therapy. It is proved that the galvanic baths used in diadynamic mode have expressed analgesic effect, contribute to the improvement of regional blood circulation, normalization of microcirculation, improving the quality of life of patients

Dinamicheskaya elektroneyrostimulyatsiya kak variant personalizirovannoy aktivatsionnoy elektroterapii

Abstract

Dynamic electric neurostimulation (DENS) is based on the monitoring of the surface impedance of the skin in under electrod space. At the same time with a reduction in the impedance of the skin, pointing to increased sympathetic tone, the most typical reaction is the increase in the index of stress heart rhythm (ISHR). This reaction is considered as manifestation of activation of the organism, aimed at the restriction of painful stress. After the first treatment procedure ISHR increases regardless of its original size. At low source ISHR (68 ± 8,5 c.u.) it rises to a level slightly exceeding the upper limit of the normal range of values (118 ± 24,7 c.u.), remained so until the end of the treatment course. The initial values ISHR the corresponding offset by distressed (316 ± 27,7) also has a statistically insignificant tendency to its increase, and the completion of the course, he reliably is reduced to 144 c.u.. DENS, based on the automatic adjustment of the parameters of stimulation in accordance with the individual changes to the impedance of the skin in under electrod space is an implementation of the personalized activation therapy in the format of physiotherapy

Abstract

Stroke is often associated with cognitive decline. This problem needs treatment. We have researched the perspective method of correction of cognitive impairments with the use of computer training programs. 51 participants, who took the neurorehabilitation, were randomized to either the treatment or the control group. The patients of the treatment group had additional computer training. Initial level of cognitive impairments and results of restoration were estimated with the use of neuropsychological testing. Statistically significant training effects were found on the treatment group in all neuropsychological scales and on the control group in 2 scales. Cognitive functions improved more significantly in the treatment group.

Abstract

The article presents the results of reserve capacity in patients with neuro dystonia of hypertensive type by bicycle exercise tolerance test, evaluated by biochemical parameters .. It is established that the source of this disease is characterized by the presence of biochemical status of compensated violations of neurohumoral regulation and changes in the functional state of the cell membrane by the type of stress-damaging manifestations, indicating a decrease in functional reserves. Further accumulation of stress-damaging effects with disregulatory disorders is a potential risk of developing pathological processes. It is shown that the conduct of bicycle exercise test in patients with neuro dystonia of hypertensive type reveals the decrease in reserves sympathetic nervous system and the potential hemocoagulation expressed the tension of regulatory mechanisms and metabolic processes that increase the damaging effects of stress on the cell membrane level.

Abstract

The article is dedicated to the tissue of total hip endoprothesis survival rate, as well as medico-biological, social and organizational factors influencing it and possible routes towards improvement of endoprothesis survival. Differences between healthcare systems of various countries in regards to relative and absolute contraindications for total hip arthroplasty are described, epidemiological data concerning hip endoprothesis survival rates in different countries based on literature data and hip arthroplasty register reports (where available) is provided. Information from Russian and international sources concerning medico-biological, social and organizational factors influencing the probability of revision hip arthroplasty is discussed. A clinical case where successful seven year endoprothesis survival was achieved by means of complex conservative treatment in a THA patient with severe autoimmune pathology and AVN is provided.

Abstract

The dynamic of 115 elder patients with II-III stage dyscirculatory encephalopathy and osteochondrosis of spinal cervical region were observed. The patients were divided into 2 groups: the basic group in which they received combined treatment: standard pharmacotherapy, physiotherapy exercises, reflexotherapy; and control group in which the patients got standard pharmacotherapy and physiotherapy exercises. Based on the clinical observations and the quality of life questionnaire the efficiency of the treatment was assessed. The received data have demonstrated that the complex therapy which included reflexotherapy and physiotherapy exercises significantly improves the quality of life (1,8 times better) compared to the data of the control group

Abstract

Evaluation of the functional state of athletes swimmers, confirms the feasibility of biomedical support in the training process and to determine predictors of the effectiveness of the provided methods. Individual approach, as well as the timely correction of individual programs of biomedical support, directly linked to the performance of the functional state of an athlete, the most important period of training in midlands. As a result, training camp was an increase in indicators of AST, ALT, CK in the normal range, as well as increased levels of cortisol, indicating that adequate voltage systems of the body in response to the load presented in conjunction with the use of pharmacological agents correction. Relationship was identified baseline indicators of iron in the blood and the active cell mass with a threshold of anaerobic metabolism, which further helped to solve the diagnostic task of predicting the effectiveness of biomedical support of the training process. As a result, discriminant analysis was performed, which provided the equation and the range of values ​​with varying degrees of effectiveness
