Diagnostical value of intestinal microbiota as one of the leading factors of the development of ulcerative colitis in type 2 diabetes mellitus patients

Incidence of ulcerative colitis (UC) in the structure of gastroenterological pathology is steadily increasing. Presence of comorbid conditions, including type 2 diabetes mellitus (T2 DM2), contributes to the development of UC complicated forms. The progressive course and resistance to the main groups of drugs lead to disability of the working part of population. The search for new diagnostic markers could help to the timely initiation of therapy, which will shorten time periods for clinical and endoscopic remission.

The aim: to assess the biodiversity of the gut microbiota in patients with UC and T2 DM.

Material and methods. 16S rRNA sequencing of the intestinal microbiota in 100 patients (80 patients with UC and T2 DM2, 20 patients with UC, 20 somatically healthy patients) was performed. Diagnosis of UC was verified using laboratory and instrumental research methods. The average age of the examined groups of patients was 50.1 ± 8.3 years.

Results. The study group was consisted mostly by patients with mild UC. When assessing the intestinal microbiota in patients with UC and T2 DM2, a significantly important increase of quantitative amount of bacterial specimens involved in carbohydrate metabolism (Akkermansia, Dorea, Collinsella, Lachnospira) was fixed. Correlation between representatives of methane producers group and the presence of an inflammatory process in UC, as well as an increase in the opportunistic and pathogenic cluster were found.

Conclusion. An increase of the amount of Firmicutes representatives was found in UC patients comparatively to the control group. At the same time, an increase of the amount of methane-producing bacteria can be one of the non-invasive criteria for assessing the severity of UC with underlying T2 DM2. The obtained data, indicating new potential mechanisms for UC development, can serve as the basis for improving the diagnosis and treatment of this pathology in the presence of comorbid conditions.

Peculiarities of lipid metabolism and vascular wall rigidity in ulcerative colitis patients

Nowadays, lots of data concerning the higher incidence of cardiovascular diseases (CVD) in patients with ulcerative colitis (UC) comparatively to the general population, despite the significantly lower incidence of traditional cardiovascular risk factors among them are collected. Existing methods for assessing CVD risk do not cover all aspects through which inflammatory bowel diseases (IBD) potentiate cardiovascular risk, therefore, a targeted study of the pathophysiological links between UC and CVD is necessary to improve methods for assessing the risk of cardiovascular events in this group of patients for their timely prevention.

The aim: to estimate the characteristics of the lipid profile and severity of arterial wall rigidity in UC patients.

Material and methods. A comparative analysis of the lipid profile and cardio-ankle index CAVI was performed in 120 patients with UC. All of them were kept for observation at the Republican Clinical Hospital of the Ministry of Healthcare of Chuvashia.

Results. Patients with UC had lower levels of total cholesterol, low-density and high-density lipoproteins, higher values of cardio-ankle index CAVI than in the control group, as well as a stronger correlation between the rate of increase of this index and age. A high correlation between the degree of excess of CAVI index and the duration and age of onset of the disease was found.

Conclusion. Patients with UC associated with long-term persistent chronic inflammation, have changes in the structure of the vascular wall and endothelium, which is confirmed by detected higher values of CAVI index. Also in this case, qualitative changes in the activity of lipoproteins are observed, which, in combination with the identified changes in lipid profile indexes, determines the presence in this group of patients of a special type of dyslipidemia with a specific combination of quantitative and qualitative indexes that determine its high atherogenicity.

Analysis of the dynamics of electrocardiograms of patients who died from COVID-19 on admission, during treatment in a hospital and before death

In addition to damage of respiratory system, infection caused by SARS-CoV-2 is accompanied by changes in cardiovascular system with electrocardiographic (ECG) declinations.

The aim: to study the ECGs of deceased COVID-19 patients upon admission, during hospital treatment and before death.

Material and methods. From 01.06.2020 to 01.06.2021, 2641 patient with COVID-19 was examined and treated in the temporary COVID hospital in Kazan. 122 of these patients (4.6%) died. 117 patients who died (4.4%) underwent ECG testing. All ECGs were analyzed by three independent specialists in electrocardiography. Heart rate, position of cardiac electrical axis, rhythm (sinus, atrial, atrioventricular, atrial fibrillation and flutter, ventricular), extrasystoles (supraventricular and ventricular), blocks (atrial, atrioventricular, bundle branches), acute myocardial infarction (AMI) and previous myocardial infarction, ST segment and T wave abnormalities and ischemic disorders were assessed.

Results. Patients with COVID-19 died more often with sinus tachycardia. Predominance of sinus bradycardia in such patients was not detected. Before death, both female and male patients had a decrease in the number of supraventricular (about 2 times) and ventricular extrasystoles (more than 3 times), as well as 1st degree atrioventricular blocks. Complete blockade of the left bundle branch was not found in any patient upon admission; incomplete blockade was found in only 1 male patient (it persisted even before death). The number of cases of incomplete block of the right bundle branch before death decreased in both females and males; the number of patients with complete block of the right bundle branch remained virtually unchanged. The cases of blockade of the anterior superior branch of the left bundle branch before death in females decreased, while in male patients this number remained the same as on admission. The number of patients with myocardial ischemia in the pre-death period among females increased by more than 2.5 times, among men this figure remained almost unchanged. Changes in the T wave were observed more often. AMI was not diagnosed in any patient upon admission, while before death it was recorded somewhat more often in female patients than in male ones.

Conclusion. COVID-19 occurs with ECG abnormalities. Therefore, it is important that doctors know them well and make adequate decisions.

Clinical portrait of elderly outpatient

The increase in the cohort of elderly people throughout the world leads to a revision of the approach to medical care provision, taking into account body involutional changes.

The aim: to assess the incidence of multimorbidity, geriatric syndromes and their correlations in patients who are taking medical care at an outpatient basis.

Material and methods. Open, cross-sectional study included 371 patients (61.7% females) aged 60–93 years, observed by a general practitioner in medical consulting room for patients of older age groups with multiple chronic diseases. Clinical and laboratory parameters were analyzed, screening for senile asthenia was performed using “Age is not a hindrance” questionnaire.

Results. The average age of studied patients was 73 (68; 80) years, most of them were elderly (58.8%) and female (61.7%) persons, having more than three (70.89%) diseases. The most common disease was arterial hypertension (95%). Diabetes mellitus and obesity had 42 and 39% of them, respectively. Geriatric syndromes occurred in 80.86% of patients: decreased hearing and vision (71%), sleep disturbances (56%) and movement disorders (48%), urinary incontinence (27%) and falls during the year (20%). Factors associated with frailty were osteoarthritis (odds ratio (OR) 2.56; 95% confidence interval (CI): 1.33–4.89; p = 0.005), myocardial infarction in anamnesis (OR 3.43; 95% CI: 1.78–6.62; p = 0.001), chronic kidney disease (OR 1.91; 95% CI: 1.00–3.65; p = 0.050), female gender (OR 4.23; 95% CI: 1.73–10.31; p = 0.002). The cut-off value for the age at which the likelihood of senile asthenia developing increases was 75.5 years (95% CI: 0.68–0.85).

Conclusion. Among outpatients aged 60 years and older, elderly female patients and persons with complex pathology are predominating. Arterial hypertension, diabetes mellitus and obesity are leading diseases in the structure of multimorbidity, hearing and vision loss, sleep disturbances and mobility problems are existing among geriatric syndromes. Chronic kidney disease, history of myocardial infarction, and osteoarthritis are associated with senile asthenia, especially in females, as well as at the age of 75.5 years and older, when the probability of senile asthenia developing is highest.

Predictors of creatininemia in locomotive crew workers: Prospective observational study

Cardiorenal correlations are a key problem among many unresolved problems in nephrology and cardiology. Creatininemia (CR), as a presage of an unfavorable prognosis, is diagnosed in case of renal pathology and cardiovascular diseases (CVD). Studying the causes of its occurrence will make closer the solution to the point of pathological cardiorenal relationship arising and improve the prognosis of nephrological and cardiological patients.

The aim: to find out the reasons for CR appearance in a natural group of 7959 initially healthy male patients 18-66 years old – workers of locomotive crews (WLC) of Trans-Baikal Railway.

Material and methods. Using the materials from a 6-year prospective observation of 7959 WLC, we identified predictors of CR. For this purpose, a 4-field 2 × 2 table, multi-factor regression model was used, and relative risk (RR) of various factors was estimated.

Results. The formation of CR was determined by a family history of early CVD, arterial hypertension (AH), dyslipidemia, hyperglycemia, grade I–III obesity, age 27–66 years, left ventricular hypertrophy (LVH), reduced glomerular filtration rate, grade I–II retinopathy. All CR predictors in using mathematical models had a statistically significant result, except for class II obesity, hyperglycemia, family history of early CVD, LVH, hypertension, and age 27–66 years. These factors were not included in the list of predictors of the studied outcome during the process of multivariate stepwise analysis performing. In contrast, the predictor “overweight” (OW) was included in the multivariate analysis and had a significant RR estimate, and that lead to the decision to examine the statistical quality of the risk factors in other types of statistical analysis.

Conclusion. Statistical heterogeneity of CR predictors (obesity of the second degree, OW, hyperglycemia, family history of early CVD, LVH, hypertension, age 27–66 years) is probably associated with different specific qualitative abilities of these predictors and the unique implementation of their damage effect at organ cell level-targets during the formation and progression of CR. Obtained data indicate the need to continue the study of CR predictors in other types of statistical analysis to identify their unique specific characteristics and study the implementation of their damage effect in formation of that type of pathology.

Efficacy of use of Enerion (sulbuthiamine) in asthenia treatment in patients having chronic non-infectious diseases in outpatient practice

Asthenia is a common and at the same time alarming symptom of many chronic non-communicable diseases (CNCDs).

The aim: to estimate the efficacy of drug treatment of asthenia with Enerion (sulbutiamine) medicine in CNCDs patients in outpatient conditions.

Material and methods. An open prospective clinical study was performed. Screening for asthenia was made in 2373 outpatients with chronic NCDs, of which 847 persons (326 male and 521 female patients, mean age 60.6 ± 14.1 years) met the diagnostic criteria. A survey, collection of complaints and anamnesis, physical examination, and analysis of outpatient records of patients were carried out, on the basis of which CNCDs diagnosis was made. Using MFI-20 asthenia rating scale and FSS asthenia severity scale, subjective diagnosis of asthenia syndrome was established. All the participants were divided into 3 groups: 1st – 446 patients who did not receive anti-asthenic therapy and did not change their lifestyle; 2nd – 71 patients who changed only their lifestyle; 3rd – 330 patients who received Enerion 600 mg/day. within 4 weeks.

Results. In patients of the 3rd group, at the 1st screening of asthenia, a greater degree of severity was fixed on both scales in comparison with patients of the 1st and 2nd groups. Despite this, while taking sulbutiamine in group 3, there was a decrease and complete normalization of the total score by MFI-20 and FSS questionnaires. At the same time, the severity of asthenia in groups 1 and 2 also decreased, but did not reach norm level by MFI-20 assessment. Also, in the group of patients receiving sulbutiamine therapy, normalization of asthenia severity indexes was fixed for all its subtypes, and the total score for each of the MFI-20 subscales after a month of observation was significantly lower in comparison with both groups 1 and 2. There were no side effects associated with sulbutiamine during the study.

Conclusion. Our study is the first to demonstrate the positive effect of sulbutiamine (Enerion) on the symptoms of asthenia in a fairly large group of patients with CNCDs in outpatient practice. Active, targeted diagnosis of asthenic syndrome in patients with chronic NCDs by therapists will lead to the need to resolve the point concerning appropriateness of its pharmacotherapy, including the possibility of using sulbutiamine as a highly effective and safe medicine.

Peculiarities of management of pregnant patients having inflammatory bowel diseases in hospital conditions: Literature review

Multiannual observations of pregnant patients with inflammatory bowel diseases (IBD) allow us to conclude that many of their adverse outcomes are associated with the activity of the disease itself, and not with the use of genetically engineered biological drugs or immunosuppressive therapy. Most drugs used to treat patients with ulcerative colitis (UC) and Crohn’s disease (CD) are considered to be safe during pregnancy. To minimize the risks of hospitalization of IBD patients, gastroenterologists need to advise them planning pregnancy, discussing with them not only the potential risks of exacerbation of UC or CD during gestation, but also the necessity of systematic visits to the doctor, as well as the importance of following prescribed plan of treatment, as well as regular obstetric and gastroenterological monitoring.

Gilbert’s syndrome concerning a new look at the effects of hyperbilirubinemia

Previously, bilirubin was considered as a potentially dangerous waste product of the vital activity of the organism, and Gilbert’s syndrome, characterized by moderate increase in unconjugated bilirubin systemic level, was perceived as a pathological condition requiring correction of hyperbilirubinemia. However, recent research works show that bilirubin molecule serves as a modulator of various biological functions in the body and has powerful antioxidant and anti-inflammatory properties, immunomodulatory and antiproliferative effects, and the ability to regulate metabolism with obvious clinical and even therapeutic implications. That is why, moderate hyperbilirubinemia can be considered as protection against the development of a wide range of “diseases of civilization”. The review analyzes the results of current studies showing new properties of bilirubin and the mechanisms of their implementation, as well as the possible clinical significance of hyperbilirubinemia with formation of a new viewpoint at Gilbert’s syndrome.

Experience of use and perspectives of carbocysteine in respiratory tract pathology treatment

Medical statistics accepts respiratory system diseases to be the most common in the structure of morbidity in Russia; also they are leading in the number of cases of associated disability. Most of these diseases are acute respiratory viral infections (ARVI) and bronchitis. The causes and mechanisms of respiratory tract damage due to viral infections are varied and require the use of a wide range of medications. Using of mucoactive agents in these cases is justified by their most important characteristics. Thus, carbocisteine, in addition to its mucoregulatory, mucolytic activities and the ability to improve mucociliary transportation, also demonstrates anti-inflammatory, immunomodulatory and antioxidant properties, which determines its place in the treatment of acute respiratory viral infections and bronchitis. The article provides a review of studies on the efficacy of this mucoactive medicine and also outlines perspectives for its use in therapeutic practice.

Difficulties of curation of a patient with irritable bowel syndrome

Irritable bowel syndrome (IBS) is considered to be a polyetiological disease with complex pathophysiological mechanisms, which significantly reduces the quality of life, is often accompanied by the patient’s fear for his health, development of cancerophobia, etc. Clinical case described in the article illustrates the difficulties of supervising such kind of patients. Multiple visits to gastroenterologists, numerous examinations, unjustified expectations of a quick effect from previously prescribed therapy, fixation on their feelings, continuous keeping of a detailed food diary for two years, refusal to start a full family due to the belief that she has a serious illness – all this significantly reduced the life quality of the observed female patient. Specialist recommended a complex treatment of IBS using selective myotropic antispasmodic mebeverine in extended-release capsules as a first-line medicine. Understanding by the patient the core of the disease and her trust in the attending physician, which was achieved during their mutual communication, psychotherapy sessions, and taking mebeverine in combination with sorbents, made it possible to achieve a significant improvement in her clinical condition.

Irritable bowel syndrome in new clinical practice

Basic component of irritable bowel syndrome (IBS) is intestine and brain dysfunction initiated by a fairly large number of risk factors. Dynamism of this system determines the variety of clinical manifestations and creates significant difficulties in diagnosis and treatment of the disease. In this regard, the initial tactics of the physician should be aimed at determining IBS phenotype in a certain patient and only after that – at selecting the appropriate treatment. In case of persistent, severe and/or aggravating symptoms or in case of adiaphoria to therapy, further testing is recommended to clarify the cause of treatment resistance and develop new tactics for patient management.

Chemopreventive and anti-tumor potential of ademetionine in case of chronic hepatic disorders

Hepatocellular cancer (HCC) is a primary hepatic tumor starting from hepatocytes, is characterized by high aggressiveness and conditionally unfavorable prognosis, which makes the problem of its chemoprevention urgent. Despite the numerous drugs considered as chemopreventive agents, their use in clinical practice is limited by the small volume and unsystematic nature of evidence of their efficacy and safety. Current work considers well known metabolic and epigenetic mechanisms of action of ademetionine (S-adenosyl-L-methionine), which are laying in the basis of its chemopreventive and antitumor activity against HCC.

Extragastral clinical manifestations in patients with chronic atrophic gastritis

B12 and iron deficiency anemia, vitamin B12 deficiency and latent iron deficiency belong to the most well-known extragastric manifestations of chronic atrophic gastritis (CAG).

The aim: to assess the incidence of extragastric clinical manifestations (vitamin B12 deficiency, iron deficiency, hypoproteinemia) in patients with CAG.

Material and methods. 143 patients aged from 20 to 79 years (residents of Saint Petersburg and the Leningrad region) were examined. The average age of participants was 59 (49–66) years. All patients underwent serum determination of pepsinogen-I (P-I), pepsinogen-II (P-II), gastrin-17 and H. pylori antibodies, also esophagogastroduodenoscopy with biopsy according to the OLGA system. Confirmation of H. pylori infection was performed by assessing the presence of antigen in stool by means of immunochromatography use. Indexes of clinical and biochemical blood analysis of patients (iron levels, ferritin, vitamin B12, holotranscobalamin, total protein level) were estimated.

Results. Anemia was found in 29.6%, latent iron deficiency in 7.7%, vitamin B12 deficiency in 9.8%, and hypoproteinemia in 2.1% of patients. A weak correlation was fixed between the following indexes: P-I and iron level (ρ = 0.220, p = 0.01), P-I and B12 level (ρ = 0.250, p = 0.003), P-I/P-II index and iron level (ρ = 0.229, p = 0.007), P-I/P-II and B12 level (ρ = 0.176, p = 0.042). Ferritin level was statistically significantly higher in patients with H. pylori infection (p < 0.001). There was no correlation between anemia and stages of atrophy (p = 0.625).

Conclusion. In CAG patients with different stages of atrophy, no statistically significant differences concerning the presence of B12 deficiency and iron deficiency conditions were found. Positive correlation was revealed between P-I, P-I/P-II and deficiency of iron and vitamin B12. Presence of vitamin B12 or iron deficiency is independent of the etiology of CAG.

Obesity in medical students: What has changed in 18 years

Obesity (OB) still retains a leading role in the development of associated clinical conditions in case of cardiovascular system pathology among the population of various age groups.

The aim: to study the prevalence of risk factors for obesity and excess body weight (EBW) development and assess the dynamics of these parameters in young individuals examined in 2005 and 2023.

Material and methods. Using questionnaires specially developed by the authors, the prevalence of overweight and obesity and risk factors for their occurrence were determined, taking into account the type of eating behavior of patients. Based on the analysis of their food diaries and self-observation, the diet and regularity of eating were estimated.

Results. Comparing data from 2005 and 2023, we found that students’ nutrition is still remaining irrational and irregular, although there are some positive trends (increase in fresh vegetables and fruits in the diet, protein foods for breakfast). In 2023, the number of first-year students with EBW and OB increased comparatively to 2005, when these diseases were not encountered at all among this cohort. This may indicate an increase in the number of cases of obesity among adolescents. The range of bad habits has also changed. In 2023, in general, 6 ± 1.5% of respondents smoke, but among people with EBW and OB, the same figure is 25%. The number of beer drinkers in general has also significantly decreased, and among people with EBW, not a single person consumed this drink in 2023. The prevalence of physical inactivity among students has decreased slightly over 18 years, but this difference is not significant. Use of erenumab in real clinical practice

Migraine is a primary headache (PH), and it is the one of the leading causes of disability in the world. Currently, a division into episodic and chronic migraine is accepted, however, the boundary between these forms is arbitrary, and the presence of one or more comorbid diseases in a patient increases the risk of chronic episodic migraine and may reduce the efficacy of therapy. Tablet medications approved for the preventive treatment of migraine are nonspecific, poorly tolerated, and patient adherence to such kind of treatment is extremely low. Monoclonal antibodies to calcitonin gene-related peptide (CGRP) or its receptor (anti-CGRP mAb) have become the first targeted preventive therapy for migraine.

The aim: to determine the efficacy and safety of anti-CGRP mAb of erenumab in real clinical practice in patients with episodic and chronic migraine.

Material and methods. 76 patients diagnosed with diagnoses chronic and episodic migraine were included in the study according to the criteria of the International classification of headache disorders, 3rd revision. During 3 months all the participants were given erenumab 70 mg subcutaneously for every month. From the beginning and every month, a clinical interview, estimation of the headache diary, and completion of questionnaires for anxiety, depression, impact of headache on the quality of life, and sleep quality assessment were performed.

Results. In patients with both chronic and episodic migraine after 1 and 3 months of erenumab therapy, there was a significant reduction in the frequency of headaches in general, migraine headaches, days with analgesics, as well as positive changes in questionnaire data. Treatment-arising adverse events were of low prevalence.

Conclusion. Erenumab is an effective and safe drug when used in real clinical practice in patients with both episodic and chronic migraine, including multimorbid patients.
