The information presented is about the life and creative activity of the researcher of Experimental Medicine Institute, who is a prominent Russian pathophysiologist Peter Nikolayevich Veselkin, that includes the main research areas of his activity, social environment, personal features, and life interests. Work has been prepared for the monograph «Scientists of Saint-Petersburg Medical Academy of Postgraduate Study» and is published in the journal format for the first time.

HIV INFECTION AND MENTAL DISORDERS: MODERN VIEW OF THE PROBLEM

Abstract

According to developing of a system providing specialized treatment and preventive care in order to improve the quality of care, all major psychiatric disorders in HIV-infected patients were reviewed. Mental disorders were systemated by following types: substance abuse disorders, somatogenic mental disorders, psychogenic mental disorders, endogenous mental disorders. Material presented in this review is important for proper diagnosis of mental status of patients with HIV infection, including patients with psychiatric and substance abuse disorders. The implementation of the principle of providing care to HIV-infected patients suggests that multidisciplinary teams including psychiatrists be organized for observing and treating this group of patients.

APPLICATION OF CALBINDIN IMMUNOCYTOCHEMISTRY FOR STUDY OF STRUCTURAL ORGANISATION OF THE PURKINJE CELL DENDRITES OF THE HUMAN CEREBELLUM

Abstract

The calbindin immunocytochemistry-based fluorescent technique is proposed for revealing the dendrites of the Purkinje cells in the human cerebellum. By improving the sensitivity of the immunocytochemical reaction and application of embedding medium with a high refractive index, fluorescence level was reached sufficient for the high resolution laser confocal microscopy and the informative three-dimensional reconstructions. Despite the use of the fixed material and the paraffin sections the presented protocol provides high-quality visualization of the structures studied.

INFLUENCE OF POSTURAL CORRECTION HEMODYNAMICS ON HEART RATE PARAMETERS

Abstract

We have studied the application of postural effects (PE) in rehabilitation purposes and characteristics of reactions to PE. We did a survey of publications devoted to the study of the processes occurring in body dynamically changing position. The results are well-known research on separate fragmented systems and estimated parameters, and sometimes controversial and that the physiological basis of these processes is not fully understood, despite numerous studies in this area. We have learned that the hemodynamic responses are associated with duty cycle with a significant redistribution of blood in the body and thus largely opposed to orthostatic and antiorthostatic PE under which alternate deposition in blood vessels of the lower and upper parts of the body is accompanied by a vasoconstrictor and vasodilator vascular reactions. It is substantiated the use of dynamic PE to achieve certain positive hemodynamic responses. They are compared ways to use PE for therapeutic purposes. This article describes a prototype of the computerized system for dynamic PE and patient assessment. The reactions of the cardiovascular system to simple dynamic PE (HRV in particular) are researched. Our studies show regular variation indices of cardiac rhythm after PE.

MECHANISMS OF CONTRACTILE ACTIVITY CHANGES IN DIFFERENT PARTS OF THE GASTROINTESTINAL TRACT UNDER PSYCHOGENIC STRESS

Abstract

Psychogenic stress in rabbits caused inhibition of contractile activity (CA) in antral and pyloric parts of the stomach. This reaction is preserved after blockade of a2-adrenergic receptors, β1/β2-adrenergic receptors, M or N-cholinergic receptors. We conclude that inhibition of gastric motility under stress is substantially «non-adrenergic non-cholinergic» and only in the initial phase of the reaction it appears to be «α-adrenergic». The latter mechanism also determines the initial transient inhibition of CA in duodenum. The subsequent strengthening of CA in the proximal duodenum is preserved after blockade of M or N-cholinergic receptors and is the result of direct exciting action of the endocrine stress factor on the smooth muscle of the gut. Strengthening of the CA in the distal duodenum is abolished by blockade of M or N-cholinergic receptors, as well as β1/β2-adrenergic receptors and is a consequence of the endocrine action of catecholamines on stimulating β-adrenergic receptors of enteric cholinergic neurons. The same mechanism is responsible for the initial increase of CA in jejunum, which is replaced by its inhibition, which is «non-adrenergic non-cholinergic». Inhibition of CA in the initial phase of the stress-induced response of the ileum, cecum and colon is due to «α-adrenergic» mechanism, and subsequent period of the CA inhibition is the result of the «non-adrenergic non-cholinergic» mechanism. Strengthening of the CA in the initial part of the distal colon is abolished by the blockade of M or N-cholinergic receptors, and is caused by the centrogenic stimulation of preganglionic neurons by endocrine stress factor with subsequent activation of effector cholinergic neurons of the enteric nervous system.

THE IMPACT OF PROTEOLYTIC ENZYMES IN THE QUALITY OF LIFE AND OPTIONS REMODELING OF THE LEFT VENTRICLE AFTER ACUTE MYOCARDIAL INFARCTION

Abstract

The aim of this study was to investigate clinical and pathogenetic features of myocardial infarction, medical correction of this pathology using proteolytic enzymes. The use of proteolytic enzymes (drug Phlogenzym) in the early stages contributes to correction and inhibition of structural-functional restructuring of the heart. Concentric remodeling is the predominant type of structural transformation of the myocardium of the left ventricle. Cardioprotective effect of systemic enzyme therapy is most pronounced in the early post-infarction period (up to 3 months) that is combined with a higher quality of life for patients.
