Астахова — 19-й Конгресс Общероссийской общественной организации «Ассоциация врачей-офтальмологов». Это ежегодное мероприятие, являющееся наиболее крупным не только для России, но и для всей Северной Европы, посвящено диагностике и лечению заболеваний органа зрения и проводится в соответствии с приказом Министерства здравоохранения нашей страны.


 

"Ophtalmology Reports" — the new parallel title of the journal to be used in citations and references

The journal has changed its parallel title in English to "Ophtalmology Reports" to ensure the uniqueness of the name among scientific journals with similar research fields.

Editorial team ask the authors to use the parallel title "Ophthalmology Reports" when citing published articles to indicate the journal correctly, even if the cited articles were published in earlier issues of the journal. This will allow not to lose the citations of the journal and better identify references.

All articles' publication imprint are kept unchanged, including DOI. Changing the parallel title is not associated with changes in ISSN and eISSN (remained the same) and does not change the scientometric indicators of the journal, but is intended to eliminate incorrect citation counting, which may occur when there are several different journals with the same title.

The effect of changes in retinal perfusion on postoperative recovery of foveal function in full-thickness macular holes

Abstract

BACKGROUND: Data on the effect of changes in retinal perfusion on retinal functional recovery at surgical treatment of full-thickness macular holes are limited.

AIM: To investigate the effect of changes in retinal perfusion on postoperative recovery of fovea function in full-thickness macular holes.

MATERIALS AND METHODS: A prospective study included data of 93 patients (93 eyes) with full-thickness macular holes. Optical coherence tomography-angiography (OCT-A), visual acuity testing, microperimetry, and multifocal electroretinography were performed before surgical treatment, after 1 and 6 months. We studied the relationship between changes in foveal avascular zone area, vessel density in the superficial capillary plexus (SCP) and deep capillary plexus (DCP) with changes in best corrected visual acuity (BCVA), retinal sensitivity (RS) at the fixation point and P1 amplitude in the fovea at different periods after treatment.

RESULTS: After surgical hole closure, a significant decrease in foveal avascular zone area, increase in vascular density in SCP and DCP was found within 1 month after treatment (p < 0.001). Significant increase in BCVA, RS and P1 amplitude was observed 1 and 6 months after hole closure (p < 0.001). The most pronounced correlation was found in the long-term postoperative period between the change in vessel density in the SCP and the change in BCVA, RS and P1 amplitude (r = 0.42, r = 0.26 and r = 0.3, p < 0.05), as well as between the change in vessel density in the DCP and the change in BCVA, RS and P1 amplitude (r = 0.41, r = 0.34 and r = 0.43, p < 0.05).

CONCLUSIONS: In the treatment of patients with full-thickness macular holes, there is a significant relationship between changes in retinal perfusion and the recovery of visual acuity, retinal sensitivity and bioelectrical activity in the foveal area, and it is more pronounced in the period from 1 to 6 months after macular hole closure. The obtained results suggest a possible prognostic role of OCTA results in the surgical treatment of full-thickness macular holes.

Biomechanical properties of lacrimal drainage pathways in dacryostenosis

Abstract

BACKGROUND: In the literature, there is a deficit of data regarding clinical and morphological correlations in characterizing lacrimal drainage pathways. However, changes in the biomechanical properties of the lacrimal drainage pathways may influence the clinical picture of dacryostenosis, and may also have a predictive value in the context of prognosis of surgical procedures.

AIM: The aim of this study is to study of changes in the viscoplastic properties of the lacrimal sac wall in dacryostenosis

MATERIALS AND METHODS: The study included 38 patients with dacryostenosis. All patients underwent a biometric examination of lacrimal drainage pathways to determine the average area of their section. All observations were divided into cases with stenotic changes (at an average section area of the lacrimal drainage pathways less than 0.18 mm2 — 26 observations) and with ectatic changes (at an average section area of lacrimal drainage pathways equal to or more than 0.18 mm2 — 12 observations). The biomechanical properties of lacrimal sac wall samples obtained during dacryocystorinostomy were analyzed. The peak value of the viscosity of the lacrimal sac wall and the integral viscosity of the lacrimal sac wall (AUC) were determined.

RESULTS: In patients with stenotic changes, a correlation was determined between the duration of lacrimation and the average section area of lacrimal drainage pathways (r = –0.537, p = 0.018), in patients with ectatic changes, a correlation was determined between the average section area of lacrimal drainage pathways and the peak value of the viscosity of the lacrimal sac wall (r = 0.662, p = 0.019). No correlations of biometric parameters with the integral viscosity of the lacrimal sac wall (AUC) were found.

CONCLUSIONS: At stenotic changes in lacrimal drainage pathways, the average area of their section depends on the duration of lacrimation; this dependence is absent when a critical level of ectasia is reached. With an increase in the average cut area of lacrimal drainage pathways, the value of the peak viscosity of the lacrimal sac wall increases.

Clinical aspects of the application of endoscopic laser cyclodestruction

Abstract

BACKGROUND: Glaucoma remains one of the leading causes of irreversible vision loss worldwide, due to the steadily increasing incidence with age. In recent years, minimally invasive glaucoma surgery combined with cataract extraction has gained significant popularity as an alternative to traditional filtering surgeries. Among the methods of hypotensive action included in the minimally invasive glaucoma surgery concept, endoscopic laser cyclodestruction plays a special role, allowing for effective reduction of aqueous humor production.

AIM: The aim of this study is to determine the optimal conditions for achieving maximum efficiency and safety of endoscopic laser cyclodestruction.

MATERIALS AND METHODS: The present study analyzed the results of 110 combined procedures (phacoemulsification and endoscopic laser cyclodestruction) performed in our clinic. The study included 110 patients (56 men and 54 women) aged from 56 to 89 years (mean age 73.7 years) diagnosed with stage I–IV glaucoma and initial complicated cataract. All patients underwent anterior endoscopic laser cyclodestruction using the E2 videoendoscopic laser ophthalmic device (Endo Optiks Inc., USA) after phacoemulsification and intraocular lens implantation. Dynamic follow-up included visual acuity testing, perimetry, tonometry, instillation regimen assessment, anterior segment OCT, and registration of complications over 60 months.

RESULTS: The data analysis showed no hemorrhagic complications and significant inflammatory reactions in the vast majority of patients. The average dynamics of intraocular pressure reduction over 60 months was 24.6%, indicating the high efficacy of endoscopic laser cyclodestruction in intraocular pressure lowering and in stabilizing the condition in glaucoma patients.

CONCLUSIONS: Endoscopic laser cyclodestruction demonstrates high efficacy and safety, with virtually no contraindications, except in cases of technical difficulties. The optimal efficiency of the procedure is achieved by destructing the ciliary body processes over 180–200°, significantly reducing intraocular pressure and decreasing dependence on hypotensive therapy.

Ischemic maculopathy at the preproliferative stage of diabetic retinopathy: epidemiology, clinical picture and diagnosis

Abstract

BACKGROUND: One of the leading causes of central vision loss in patients with diabetic retinopathy is ischemic maculopathy, the incidence of which in diabetic retinopathy varies depending on the stage of the disease from 20 to 77% according to fluorescein angiography results. More accurate diagnosis of ischemic maculopathy is possible using the technique of optical coherence tomography angiography (OCTA).

AIM: To study the prevalence and severity of ischemic maculopathy in patients with diabetes mellitus type 1 and 2 with preproliferative stage of diabetic retinopathy using OCTA.

MATERIALS AND METHODS: 43 patients (72 eyes) of diabetic retinopathy levels 47 and 53, according to ETDRS criterions, were included in the study. The exclusion criterion was the presence of diabetic macular edema with involvement of the center of the macula. Patients were divided into 3 groups according to the ETDRS classification of ischemic maculopathy grade. Each of them was subject to standard ophthalmologic examination, OCT with determination with determination of central retinal thickness in macula zone and OCTA to evaluate the status of the foveolar avascular zone.

RESULTS: Ischemic maculopathy level 1 was detected in 23 patients — group 1 (33 eyes), level 2 was detected in 23 patients (27 eyes) — group 2, and level 3 — in 8 patients (12 eyes) — group 3. A statistically significant difference using “IBM SPSS Statistics” version 27 was found between the foveolar avascular zone area scores of groups 1 and 2 — 0.18 mm2 versus 0.32 mm2 (p < 0.001) and between groups 1 and 3 — 0.18 mm2 versus 0.98 mm2 (p < 0.001), and between groups 2 and 3 — 0.32 mm2 versus 0.98 mm2 (p < 0.008). In group 3, negative correlations were found between best-corrected visual acuity and foveolar avascular zone circumference length (r = –0.906, p = 0.02), foveolar avascular zone (r = –0.748, p = 0.033) and circularity index (r = –0.569, p = 0.141), while no such statistically significant difference was found in the other groups.

CONCLUSIONS: In patients with diabetic retinopathy levels 47 and 53, ischemic maculopathy is revealed in 100% of cases. In 83.4% of cases, level 2 of the ischemic maculopathy is detected, and in 16.6% — level 3. Ischemic maculopathy of levels 1 and 2 has no significant effect on best-corrected visual acuity. Level 3 is clinically significant, as the change of parameters characterizing foveolar avascular zone, and first of all the increase of foveolar avascular zone circumference length above 500 µm, is associated with a decrease in best-corrected visual acuity.

Biomechanical parameters of the fibrous capsule of the eyeball in pseudoexfoliative glaucoma in comparison with primary open-angle glaucoma

Abstract

BACKGROUND: Pseudoexfoliation syndrome is currently considered as a systemic disorder of the connective tissue metabolism with the accumulation in all corneal cell layers of pseudoexfoliation syndrome deposits, which disrupt corneal morphology and biomechanics.

AIM: to study the features of biomechanical parameters of the fibrous capsule of eyes in primary open-angle glaucoma (POAG) in comparison with those in pseudoexfoliative glaucoma (PEG).

MATERIALS AND METHODS: We compared 65 eyes with POAG and 77 eyes with PEG aged under 80 years. The control group consisted of 18 healthy eyes. Biomechanical indicators were compared, such as: DA Ratio, Integr. Radius, SP-A1, SSI, BGF, biomechanically corrected intraocular pressure (bIOP) obtained with Pentacam (Oculus) and CorVis ST.

RESULTS: Patients with PEG were elder (68.013 ± 0.75 years) in contrast to POAG patients (60.03 ± 1.05 years) (p = 0.001), had a thinner central retinal thickness (CRT) — 543.99 ± 3.9 µm versus 559.33 ± 4.4 µm in those with POAG (p = 0.010). The IOP level did not differ between groups, and no correlation with CRT was detected. Indicators of corneal stiffness: DA ratio Integr. Radius did not differ between POAG, PEG and control group. The SP-A1 parameter also did not differ between POAG and PEG patients, while there were differences between PEG patients and the control group (p = 0.046). Moreover, in eyes with POAG, SP-A1 directly correlates with IOP Ро (p = 0.001) and CRT (p = 0.001), in those with PEG — p = 0.001 and p = 0.001, respectively. The SSI index in PEG was higher and amounted to 1.38 ± 0.03 versus 1.27 ± 0.03 in POAG (p = 0.013), while it correlated with age only in the case of PEG (p = 0.007). A correlation between SSI and CTR was also revealed — in POAG (p = 0.018), in PEG (p = 0.001). In PEG, BGF shows higher values (25.92 ± 2.3) than in POAG (17.71 ± 2.2; p = 0.010). BGF has no correlation with age (p = 0.094 and p = 0.737 for POAG and PEG, respectively), depends on CRT (p = 0.001 and p = 0.027, respectively), on bIOP (p = 0.001 and p = 0.001, respectively), and on SP-A1 (p = 0.009 and p = 0.001, respectively). The only parameter that was higher in PEG than in POAG was SSI, which did not correlate with the BGF indicator (p = 0.642 and p = 0.327, respectively).

CONCLUSIONS: We did not find any fundamental differences in biomechanics between PEG and POAG, which could explain the significant rates of progression of PEG. Based on our data, it is obvious that the eye with PEG differs from that with POAG being more rigid, even at similar IOP values.

Efficiency ophthalmochromotherapy in the complex treatment of patients with computer vision syndrome

Abstract

BACKGROUND: One of the leading principles of asthenopia treatment is a comprehensive approach based on multidirectional methods of physical impact on the organ of vision. This provision determines the relevance of considering ophthalmochromotherapy, the use of which is determined from the position of photobiomodulation as a non-thermal biological process activated by certain wavelengths of light.

AIM: To evaluate the clinical efficacy of ophthalmochromotherapy in the complex rehabilitation treatment of patients with computer vision syndrome.

MATERIALS AND METHODS: The study involved 132 patients with computer vision syndrome, accompanied by characteristic manifestations of accommodative asthenopia — habitual excessive tension of accommodation (HETА) or asthenic form of accommodative asthenopia (AFAA). Patients were divided into four groups — two control groups, which underwent the traditional method of treating accommodative asthenopia and two main groups, which, along with the traditional method, underwent ophthalmochromotherapy (green color for HETА and red color for AFAA, respectively).

RESULTS: A significantly higher level of clinical efficacy of restorative treatment was established when ophthalmochromotherapy was included in the traditional method, which is confirmed by more pronounced dynamics of accommodation indices (p < 0.01), contrast sensitivity at low (0.5, 1.0 cycles/deg.) frequencies (p < 0.01), critical frequency of fusion and flickering (p < 0.05), and quality of life (p < 0.01).

CONCLUSIONS: The use of ophthalmochromotherapy in the complex treatment of patients with computer vision syndrome symptoms may be considered as an effective method of restorative correction of the functional state of the visual analyzer, which is confirmed (in comparison with the traditional method) by a pronounced, statistically significant improvement in the state of the accommodative system of the eye, as well as indices reflecting “sensory” fatigue and the patient’s quality of life. At the same time, in accordance with clinical standardization, the inclusion of ophthalmochromotherapy in the traditional treatment method ensured the achievement of standard indicators of quality of life, which reflects the full achievement of treatment targets from the perspective of the “medical and social” health model.

Endothelial keratoplasty in patients with endothelial dysfunction of various etiologies, combined with abnormality of the iris-lens diaphragm

Abstract

The article presents a description of a modified method of transplantation of the endothelium on descemet membrane and of a method of femtosecond laser-assisted posterior lamellar keratoplasty in patients with corneal endothelial dysfunction combined with severe defects of the iris-lens diaphragm integrity. During surgery, in both cases, a banded stromal flap was used. There were no complications in the early postoperative period. The air resorption in the anterior chamber in both cases lasted no more than 2 days. After air resorption, the stromal flap occupied an intermediate position between the iridolens diaphragm remnants and the endothelial graft. On day 3, the bandage stromal flap was removed in the operating room. It was shown that the use of a bandage stromal flap during surgical procedures for extensive defects of the iris-lens diaphragm allows minimizing the risk of dislocation of the endothelial graft into the vitreal cavity. Proposed technique is an universal solution for DMEK and Fs-DSAEK in patients with an abnormality of the iris-lens diaphragm. The preliminary insertion of banded stromal flap into the anterior chamber makes it possible to block the defects of the iris-lens diaphragm and provides conditions for prolonged air tamponade of the anterior chamber and primary graft adhesion.

Navigated laser treatment with preliminary anti-VEGF therapy in macular telangiectasia type 1

Abstract

Macular telangiectasia type 1 (MacTel1) is a rare condition characterized by multiple parafoveal microaneurysms, which leads to local nonperfusion, capillary ischemia, lipid and serous exudation which cause cystoid macular edema. Microaneurysms are detected by fluorescein angiography and optical coherence tomography-angiography. MacTel1 is relatively resistant to antiangiogenic therapy. Focal laser photocoagulation of microaneurysms is a pathogenetically substantiated treatment method. The use of navigation approach with topography-based planning allows increasing precision, safety and efficacy of laser effect. Clinical case. A 75-year-old patient with MacTel1, associated with a cystoid macular edema 702 µm. Treatment included intravitreal antiangiogenic therapy with consequent navigation targeted laser photocoagulation of microaneurysms. The best-corrected visual acuity and central retinal thickness were examined. After the combined treatment, best-corrected visual acuity increased from 0.3 to 0.9. OCT showed central retinal thickness reduction to 395 µm (∆307 µm). Fluorescein angiography showed decrease in the number and size of the microaneurysms, in the number of observed leakage points, in the intensity of the dye leakage. In the presented clinical case of MacTel1, the staged treatment approach including intravitreal antiangiogenic therapy with consequent navigation targeted laser photocoagulation of microaneurysms showed its effectiveness in reducing cystoid macular edema height and in improving visual functions.

Modelling of silicone oil emulsification in vitro (review)

Abstract

Silicone tamponade is one of the prevailing methods used in the surgical treatment of retinal detachments. However, one of the significant disadvantages of using silicones is their emulsification and the risk of developing associated ophthalmic conditions. In this regard, the question of ways to prevent silicone emulsification remains relevant. Emulsification does not occur in all cases of tamponade, and this indicates that there are factors that could interfere with the development of this process. It is known that emulsification of silicone oil has a multifactorial etiology, and in vitro experimental models are being developed to study these mechanisms in more detail. It is the variety of factors that determine the tendency to emulsify ophthalmic silicone that formed the basis for the creation of these models. Such factors include physicochemical and mechanical effects, saccadic eye movements, adhesion of silicone oil to eye tissues, absorption of biological substances from intraocular fluids and tissues. The development of a variety of in vitro models allows, on the one hand, to obtain new fundamental knowledge, while, on the other hand, it allows us to solve practical issues related to the prevention of silicone emulsification and may provide insight into future strategies for improved intraocular tamponade.

Retinal detachment with macular involvement: in the struggle for maximal visual acuity. Part 1

Abstract

The problem of improving visual acuity and functional outcomes in the treatment of macula-off rhegmatogenous retinal detachment remains relevant today. There is still no unified classification of this disease, which complicates the analysis and comparability of data from scientific studies. The first part of the review presents various classification options for macula-off rhegmatogenous retinal detachment, based on optical coherence tomography data of the macula. It provides data from experimental studies in this area, examines perioperative factors affecting the maximum final visual acuity, and analyzes the impact of the timing of surgical treatment of rhegmatogenous retinal detachment involving the macular area on the final maximal corrected visual acuity. Macula-off retinal detachment requires immediate surgical intervention due to better functional outcomes when the duration of macular detachment does not exceed 7 days. Despite achieving anatomical success after surgery for retinal detachment, in cases of pronounced microstructural morphological changes in the macular area, the final visual acuity may remain unsatisfactory. When predicting final visual acuity, a comprehensive assessment of the morpho-functional state of the macular retina is important.

Methods of surgical treatment of congenital ptosis (review)

Abstract

Congenital ptosis is one of the most common eyelid abnormalities in the field of pediatric ophthalmology. This condition is characterized by drooping of the upper eyelid and can lead to functional and aesthetic problems in children in the absence of timely treatment. Various surgical methods aimed at correcting blepharoptosis are described in the literature, including levator resection, frontalis sling and tarsomullerectomy. Levator resection is the most common technique that significantly improves the position of the upper eyelid and provides lasting functional results. Frontalis sling is usually used for severe ptosis or in cases where levator resection is impractical. Tarzomullerectomy surgery is also frequently used, especially in patients with sufficient levator function. An important aspect of surgical treatment is the need for preoperative diagnosis of the degree of ptosis, levator function, and the presence of concomitant abnormalities such as amblyopia and strabismus. The results of surgical procedure show a high degree of patient satisfaction, however, they require constant monitoring to identify possible complications and the need for repeated intervention. Thus, the choice of surgical treatment method should take into account the individual anatomical and physiological characteristics of the patient.
