Medical-and-statistic characteristics of morbidity among female military personnel of the Armed Forces of the Russian Federation (2007-2016)

Abstract

A selective statistical analysis of medical reports on the health status of the personnel and the activities of the medical service (form 3/MED) of military units in which from 2007 to 2016 served up to 60% of female servicemen to their total number in the Armed Forces of Russia. The mean annual rates were as follows: general morbidity - 1744±117‰, primary - 706±67, need for dispensary observation - 238±27, general hospitalization - 322±30, days of labor losses - 7328±509, dismissals for health reasons - 11.4±1.78‰, the death rate is 67.5±6.5 per 100 000 female servicemen. The growth during the period of observation of virtually all indicators was established, which turned out to be 1.5-2 times higher than for officers. According to the developed algorithm, a generalized structure of health disorders of female servicemen was formed, in which 83.5% accounted for diseases and traumas of 8 classes: II (neoplasms), IV (endocrine system diseases, eating disorders and metabolic disorders), IX (diseases of the system (diseases of the digestive system), XIII (diseases of the musculoskeletal system and connective tissue), XIV (diseases of the genitourinary system) and XIX (trauma, poisoning and some other effects of external causes). Prevention of the pathology of these classes will contribute to improving the professional health of female servicemen.

Experience of military medical examination of personnel working with chemically dangerous objects

Abstract

Surety for storage and destruction of chemical weapons is a multifaceted and extremely difficult problem to implement. The service at these facilities is classified as job with harmful working conditions. As a result of the studies, standardized form of dynamic health monitoring of personnel working with chemical weapons, automated software tools for entering and storing of clinical and diagnostic indicators in the database, an algorithm for the medical examination of servicemen have been developed. The necessity for the phasing of medical examination in the link «the facilities for storage and destruction of chemical weapons - a specialized hospital» is proved. The system has allowed increasing the effectiveness of hold measures and significantly increasing the quality of evaluation of personnel working at chemically hazardous object.

Soft tissue sarcoma or purulent disease of the hand: difficulties in differential diagnosis

Abstract

Due to the rarity of malignant neoplasms of soft tissues, there is a lack of experience among surgeons and a unified approach to the diagnosis of such diseases. The article presents a clinical observation of the case of soft tissue sarcoma of the hand, which was initially regarded as the hand hygroma. According to the data of the immunohistochemical study, a conclusion was made about the presence of an atypical (aggressive) hydroadenoma/hydroadenocarcinoma of low degree of differentiation. It was noted that the oncological alertness of doctors-surgeons of the general medical network, especially the outpatient stage, is increasing.

Heart contusion in case of closed chest injuries: etiology, diagnosis, severity of heart damage (literature review)

Abstract

According to the literature, the heart contusion remains a pathology with a wide range of unresolved issues, which include the absence of clear data on the prevalence of pathology due to the use of different diagnostic standards, the lack of specific methods of intravital diagnosis of a heart contusion, insufficient data on the degree the severity of heart damage, which is the basis of developing complications leading to death. The polymorphism of the clinical picture of the heart contusion complicates his early diagnosis. The most reliable early diagnostic criteria for heart contusion are arrhythmia, electrocardiographic signs on the background of a fracture of ribs or sternum. The suggested scoring of early signs of a heart contusion according to the scale «MS-SU» is a rather effective diagnostic tool and allows objectifying its diagnostics with an accuracy of up to 94.7%.

Gastroesophageal reflux disease: features of pathogenesis, clinical picture, diagnosis and examination of citizens

Abstract

At the present stage gastroesophageal reflux disease is considered as a multifactorial disease. The important role of the microbiota of the esophagus and the entire gastrointestinal tract in its emergence and progression has been established. There are 2 types of esophagus microbiome, type I microbiome is characteristic for a normal histological picture of the pseudoras, and type II is associated with the development of Barrett’s esophagitis and esophagus. The definitions of gastroesophageal reflux disease, its classification, mechanisms of development are presented. The leading pathogenetic mechanism is pathological gastroesophageal reflux. The characteristic of the clinical picture of the disease is given. Rational approaches to the diagnosis of pathology in the interests of military-medical expertise of citizens are outlined.

Prospects of using regenerative medicine technologies in the treatment of Peyronie’s disease

Abstract

Peyronie’s disease is characterized by an inflammatory reaction under the tunica albuginea with the proliferation of fibroblasts and the formation of a thickened fibrous plaque, followed by pain, abnormal penis curvature and erectile dysfunction. The results of conservative methods of treatment do not exceed the placebo, and surgical treatment does not always lead to satisfactory results. It leads to development of new therapeutic methods and technologies including cell and gene therapy. Adipose tissue derived stromal-vascular fraction and platelet-rich plasma are most promising approaches for the treatment of Peyronie’s disease among all methods or regenerative medicine.

Prospects of using ampakines for correction of unfavorable functional states in military specialists (literature review)

Abstract

The search for promising drugs for pharmacological correction of non-favorable functional states in order to maintain a high level of operability of the operator’s profile specialists is an active task of military medicine. The article is devoted to the discussion of the prospects of using ampakins - preparations from the class of allosteric AMPA receptor modulators for this purpose. The analysis of literature data showed that the nootropic effect of ampakines is due to the improvement of neurotransmission in glutamatergic synapses of the cerebral cortex, which is functionally realized in increasing attention, concentration, vigilance, improving learning and memory processes. The available data on the high efficacy and safety of this class of drugs determine the prospect of their use to correct the functional states of the specialists in the operator’s activity profile.

Abstract

The implementation of the orders of the Ministry of Health of the Russian Federation of August 31, 2016 N 646н «On approval of the rules of good practice of storing and transporting medicinal products for medical use» and No. 647н «On approval of the rules for the proper pharmacy practice of medicinal preparations for medical use» as well as the Rules of proper distribution practice within the framework of the Eurasian Economic Union, necessitated the introduction of a quality management system in the pharmaceutical organizations of the Armed Forces. The classification of the processes of the pharmaceutical organization is presented. The technology of the implementation of the process approach, the development of standard operating procedures for ensuring the continuous improvement of the quality and efficiency of the activities of pharmaceutical organizations is shown.

To the 75th anniversary of the unique issue of doctors of the S.M.Kirov Military Medical Academy

Abstract

The release of military doctors from the Military Medical Academy in 1943 can be called unique. Training course this course (600 people) began on September 1, 1939, the day of the beginning of World War II, and the issuance of diploma doctor graduates in 1943 coincided with the beginning of the Great Patriotic War. In the first year of the war, studies took place in besieged Leningrad and were accompanied by famine, cold, shelling and air strikes with the death of listeners. The course participated in the defense of Leningrad in the trenches of the outskirts of the outskirts of the city, the listeners were awarded the medal «For the Defense of Leningrad». The evacuation of the academy to Samarkand was held in unprecedented conditions. The study was accompanied by a 4-year training with the assignment of a doctor’s qualification and the issuance of a full-fledged diploma, which did not occur in any medical college of the country. Graduates were awarded military ranks of the captain of the medical service, handed the order with the indication of the front. Of those arriving in military units, 37 (10%) of one-team soldiers were killed in combat, 50% were injured and contusions.
