ALISKIREN - NEW DATA OF EVIDENCE-BASED MEDICINE AND PROSPECTS OF APPLICATION

Abstract

The article presents the review of new scientific data on the mechanism of action of a direct renin inhibitor - aliskiren, and the difference of its impact on the basic components of the renin-angiotensin-aldosterone system (RAAS) in comparison to other RAAS blockers. New data of meta-analyses of the efficacy and safety of aliskiren are discussed. Meta-analyses have shown comparable efficacy and tolerability of aliskiren with angiotensin receptor antagonists and other antihypertensive drugs. Long-term studies on the organoprotective effects of aliskiren suggest the achievement of regression of left ventricular hypertrophy in patients with arterial hypertension and proteinuria in patients with arterial hypertension and diabetic nephropathy; there was no increased risk of total and cardiovascular mortality. Evaluation of possible use of aliskiren in other cardiovascular disease showed no benefits of the drug to the existing ones.

Abstract

Optimal physical development of children and adolescents involved in sports require high concentration of vitamin D. It is well known that prevention of numerous diseases, comorbid with deficiency of vitamin D, is effective at a concentration of 25(OH)D more than 30.0 ng/mL. At the same time, the optimum functioning of the muscles and reliable improvement of sports results achieve at a concentration of 25(OH)D more than 40.0-50.0 ng/mL. The article deals with the molecular and physiological mechanisms of vitamin D effects on the muscles and the results of clinical studies of athletes.

Abstract

In 2015, the Kyoto Consensus was conducted. It was aimed to the systematization of the data on the etiology, diagnostic tactics and treatment of inflammatory changes in the mucous membrane of the stomach and duodenum accumulated over the past few years. Expert council has widely discussed issues related to the updating of the current systems of classification of chronic gastritis, duodenitis, analyzed the role of this nosological units in the formation and progression of precancerous changes of the mucous membrane of the stomach, as well as the possibility of eradication therapy as a method of cancer prevention. Kyoto Consensus has played an important role in the modern understanding of the problem of inflammatory gastroduodenal pathology and identification the ways to resolve it. The Consensus statements concerned mostly the classification, diagnosis and treatment of H. pylori-associated gastritis.

MODERN APPROACHES TO TREATMENT OF CHRONIC VEIN DISEASES: PLACE OF AESCUSAN IN FLEBOLOGICAL PRACTICE

Abstract

The beginning of XXI century was marked not only by significant progress in the development of medicine, but also an increase in the incidence of various diseases, including chronic venous diseases of the lower extremities (CVD). To date, epidemiological analysis of the situation indicates that the frequency of occurrence of different types of CVD reaches 60% or more among the adult population of the industrialized countries. To improve the treatment of these patients, not only concerted actions of physicians of all specialties are required, but also, above all, understanding of the problems and knowledge of modern approaches to the treatment of CVD. Russian and international clinical guidelines for the diagnosis and treatment of CVD reflect all aspects, from pathogenesis of this disease to the approaches and treatment schemes at all stages of the disease. The use of the modern algorithms of management can provide quality adequate treatment of patients with chronic venous diseases. Along with the surgical methods, administration of conservative therapy (eg, Aescusan) has a prominent place and is a mandatory treatment of CVD.

MANAGEMENT OF PATIENTS WITH NON-VALVULAR ATRIAL FIBRILLATION IN THE SETTINGS OF ANTICOAGULANT THERAPY CONTROL OFFICE: PROLONGED EFFECTIVE AND SAFE USE OF ANTICOAGULATION

Abstract

The study was aimed to the identification of adverse factors of use of anticoagulant therapy (ACT) for the selection of patient’s groups at high risk of bleeding during treatment with oral anticoagulants. The research is conducted on the basis of the University Clinical Hospital № 1 First MSMU n.a. I.M. Sechenov and includes 325 patients with non-valvular atrial fibrillation receiving various ACT for the prevention of thromboembolic complications (TEC). Mean duration of follow-up period - 24 months. Among 325 patients included in the study, more than 95% of patients continue to receive anticoagulants and continue monitoring in anticoagulant therapy control office. A total of 37 bleeds were recorded, including 19 «small» and 18 «major». In most cases, bleeding complications were detected in patients with CHA2DS2-VASc scores= 1, 4 and 8, and HAS-BLED scores=4. In patients with history of acute cerebrovascular accidents (ACVA), bleedings have occurred significantly more often than in patients without ACVA. Number of major bleedings in patients on warfarin was significantly higher than in patients on oral anticoagulants, and the number of small bleedings in patients on warfarin was significantly lower (p<0.05). It should be noted that the frequency of major bleedings on warfarin was significantly higher than on any of the new oral anticoagulants - dabigatran, rivaroxaban, and apixaban. During follow-up period, TECs were observed only in 3 patients (0.9% of the total number receiving ACT). It is emphasized that the effective and safe ACT requires systematic monitoring of patients receiving this type of therapy. The most optimal way to monitor this patient cohort is considered to be anticoagulation therapy control office.

EXPERIENCE OF USE OF OMEGA-3 POLYUNSATURATED FATTY ACIDS IN THE INTEGRATED PREVENTION OF PREECLAMPSIA (GESTOSIS) IN PREGNANT WOMEN

Abstract

The efficacy of use of omega-3 polyunsaturated fatty acids (ω-3 PUFAs) in comprehensive prevention of gestosis (preeclampsia) in pregnant women with extragenital pathology and treatment of mild to moderate gestosis in cases of any contraindications for the appointment of antiplatelet and anticoagulant therapy was evaluated. It was found that long-term use of the ω-3 PUFAs from 1 trimester contributes to the normalization of hemostasis and significantly reduces the frequency and severity of gestosis in high risk group of pregnant women. Addition of the ω-3 PUFAs to the complex therapy of mild to moderate gestosis significantly improves pregnancy outcomes for the fetus.

MANAGEMENT OF PATIENTS WITH INJURIES AND SURGICAL INTERVENTIONS ON THE UPPER RESPIRATORY TRACT IN THE POSTOPERATIVE PERIOD: APPROACHES FROM A PERSPECTIVE OF EVIDENCE-BASED MEDICINE

Abstract

The study was aimed to the evaluation of the efficacy and safety of complex homeopathic preparations (CHP) in patients after injuries and surgeries on the upper respiratory tract. The effectiveness of drug therapy in trauma and postoperative was evaluated in dynamics in 101 patients receiving CHP and in 63 patients who received conventional drug therapy. The leading indicator for the assessment of wound healing process was a dynamics of status localis; also, microbiological examination and evaluation of the respiratory function of the nose by the front active rhinomanometry. The results of treatment were assessed at 1, 3, 5, 7, 14 day, and 1 and 6 months after injury or surgery on the basis of the dynamics of clinical symptoms, laboratory parameters and instrumental methods. The results revealed high efficiency of CHP, significantly accelerating the timing of reduction of pain, signs of local inflammation and providing faster recovery of patients after injuries and surgical interventions that can reduce the duration of hospitalization.

THE COURSE OF GASTROESOPHAGEAL REFLUX DISEASE ACCORDING TO THE RETROSPECTIVE 5-YEAR FOLLOW-UP

Abstract

The article discusses the current problems of gastroesophageal reflux disease (GERD), and the potentials for its diagnostics at the present stage. The results of a retrospective 5-year follow-up of course of GERD in patients referred to a Perm Specialized Gastroenterological Center. The features of clinical manifestations, data for 24-hour esophageal pH monitoring and endoscopic examination of the esophagus in the study group patients, and concomitant diseases of the digestive system are discussed. The examination revealed comorbidity of GERD with gastroduodenal pathology and pathology of biliary system. Relapse of GERD most often occurs against the background of worsening of gastroduodenal pathology and pathology of biliary system; in some cases, the diagnosis of GERD was not confirmed either endoscopically or by 24-hour pH monitoring of the esophagus and stomach.

Abstract

Diseases of the mucous membranes (epiteliopathies) are characterized by relapsing course, severe inflammatory tissue reactions and resistance to various therapeutic effects. Methods of traditional medicine are not always effective in diseases associated with disorders of the mucous membranes, and often have side effects. An alternative is the use of the drug Mucosa compositum - complex preparation for regeneration of the mucous membranes of the human body. The results of our own research allow us to recommend the drug Mucosa compositum against the background of basic therapy for children with diseases of the respiratory tract, out the period of exacerbation, especially in the presence of concomitant diseases of the digestive system.

Abstract

One method of the correction of uremia in patients with the chronic kidney disease (CKD) on the non-dialysis stage is the reduction of the absorption of the end toxins of the microbial metabolism to the gastrointestinal tract by using modern enterosorbents, allowing significantly to slow the progression of CKD, to delay the beginning of the dialysis, as well as to help the incurable patients, whom contraindicated the active methods of the therapy. Enterosgel is the indispensable detoksikant in patients with CKD, so it’s nessesary to include it to the complex therapy of CKD due to its proven efficiency and safety.
