Thiazide diuretics and arterial hypertension: the history is going on

Abstract

The present article is concerned with indications and contraindications to the use of thiazide diuretics (TD) such as hydrochlorothiazide, in the treatment of arterial hypertension (AH). The mechanism of action of the TD are considered, the results of major clinical trials with their use are presented. Authors discuss possible restrictions on the administration of TD in AH. Problem of type 2 diabetes mellitus associated with the use of TD, and the indications for the use of TD in AH are in the focus. It is emphasized that hydrochlorothiazide is one of the several antihypertensive drugs, which can be used for long-term treatment of AH, and for the relief of hypertensive crisis.

Discirculatory encephalopathy: the main targets of therapy

Abstract

The article presents review of current data for clinical manifestations and pathophysiology of various forms of dyscirculatory encephalopathy (DE). The main goals in the treatment of DE are correction of risk factors, the compensation of chronic cerebral ischemia, and the prevention of recurrent cerebrovascular events. The present article considers the main guidelines in the treatment of DE. It is emphasized that the principal direction in the prevention of ischemic cerebral lesions is antiplatelet therapy. Various options of antiplatelet therapy with acetylsalicylic acid (ASA), clopidogrel, ticlopidine, and dipyridamole are discussed. The advisability of a combination of ASA + dipyridamole is proved. This combination should be used as first-line secondary prevention of ischemic stroke, especially in patients with unstable course of arterial hypertension, predisposition to hypertensive crisis, gastric ulcer and duodenal ulcer, and high risk of bleeding.

Rituximab in the current therapy of rheumatoid arthritis

Abstract

The article is dedicated to the problem of treatment of rheumatoid arthritis (RA), and the current concepts of pathogenesis and treatment of this serious disease are illustrated. The modern genetically engineered biological preparations used for treatment of RA, including inhibitors of tumor necrosis factor α (TNF-alpha) and group of chimeric monoclonal antibodies, with particular focus on to rituximab, are considered. The article presents review of studies devoted to evaluation of the effectiveness of rituximab for the treatment of patients with RA. Results of studies allow us to consider rituximab as drug for wide practical use and actively competing with inhibitors of TNF-alpha.

Moderate cognitive disturbances: diagnostics and treatment

Abstract

Cognitive impairments secondary to various neurological, psychiatric, somatic and endocrine-metabolic diseases very often develop in older persons. The main stages of the evolution of cognitive failure in the elderly are mild, moderate and severe cognitive impairments. Moderate cognitive impairments (MCI) are clinically delineated syndrome of derangement of memory and other higher brain functions that are beyond the scope of age norms, but do not cause disadaptation in daily living. Prevalence of MCI is 11-17% in persons aged 65 years and older, and 44% in neurological outpatients. The clinical performance may be represented in the form of memory impairment or other cognitive impairments, as determined by the underlying disease. MCI is eventually transforms into dementia more than in half of the cases. Treatment includes correction of cerebral ischemia, optimization of microcirculation, neurometabolic therapy, and compensation of available neurotransmitter deficiency.

Lisinopril in the treatment of arterial hypertension in patients with concomitant liver pathology

Abstract

Lisinopril is one of the most famous and well-studied angiotensin-converting enzyme (ACE) inhibitor, considered as drug-prototype. Like other members of this group, the main mechanism of action of lisinopril is founded on the blockade of active Zinc-containing domain of ACE that leads to a decrease in the synthesis of angiotensin II and activity of the renin-angiotensin-aldosterone system (RAAS) in general. This effect defines the main indications for use of ACE inhibitors. They are used for conditions in the pathogenesis of which may play a role of RAAS activation, including arterial hypertension. The article is dedicated to the basic pharmacokinetic properties of lisinopril compared with ACE inhibitors. The antihypertensive efficacy of lisinopril as monotherapy and in the combination with other drugs is considered. The available data for the use of this drug in patients with chronic hepatic disorders are analyzed.

Probiotics in the treatment of inflammatory intestine diseases in children

Abstract

The development of inflammatory bowel diseases (IBD), such as Crohn's disease (CD) and nonspecific ulcerative colitis, is associated with disruption of the interaction of the intestinal microbiota and the intestinal immune system. Taking into account the beneficial effect of probiotics on the protective intestinal barrier and the formation of immunological tolerance, these drugs can be used for the treatment of IBD. Only few reports were dedicated to this problem in children. Results of the study of effectiveness of Saccharomyces boulardii (Enterol) in remission maintenance in children with CD are presented. Twenty-four children after remission induction were randomized into 2 equal groups. The patients of group 1 have received maintenance therapy with mesalazine (50 mg/kg), and patients of group 2 in addition to this therapy received Enterol (500 mg / day) within first 3 months of treatment. Dynamics of clinical presentation and the contents of calprotectin in feces were evaluated 6 months after treatment. The exacerbation was observed in 8.3% of children receiving Enterol, and in 25% of children not receiving this drug. Level of calprotectin in feces was significantly lower in patients receiving Enterol.

Abstract

It was shown that cystic fibrosis (CF) is related to chronic rhinosinusitis (CRS) and nasal polyposis. It is emphasized that the presence of CRS greatly aggravate the course of CF, leads to a decline in lung function, increase of colonization of Pseudomonas aeruginosa and its chronic infection. The pathogenesis, clinical symptoms and diagnostics of polypous CRS are considered. Conservative and surgical methods of treatment of this disease, including the development CRS secondary to CF are discussed in detail.

Aztreonam: pharmacological properties and experience of clinical use of monobactame antibiotic

Abstract

Aztreonam is monobaktam, antibiotic with selective bactericidal action in regard to aerobic gram-negative bacteria. The spectrum of activity includes such problematic pathogens as Pseudomonas aeruginosa and Klebsiella spp. Synergy of action with antibiotics of other groups is the specific characteristic of aztreonam. Clinical efficacy of aztreonam is well studied. Monobaktam has been successfully used in the treatment of moderate and severe gram-negative infections, including infections of the CNS, respiratory tract, abdominal space, kidney and urinary tract infections, infections of soft tissues, febrile neutropenia, and others. The possibility of use of aztreonam in patients with allergy to other β-lactams (penicillins, cephalosporins) is emphasized, as well as good tolerability in adults and children with minimal risk of adverse drug reactions. Since 2009, aztreonam (invented name Aztreabol) is available in Russia.

Early therapy of Parkinson's disease with rasagiline (Azilect) slows down disease progression: results of the study ADAGIO

Rasagiline (azilect) is next generation inhibitor of monoamine oxidase type B. Its potential to reduce the severity of main parkinsonian symptoms in patients with Parkinson's disease (PD) was previously demonstrated in some controlled trials (TEMPO, PRESTO, LARGO). Design and results of the ADAGIO study - first clinical trial with a delayed start for BP, which confirmed the neuroprotective effect of rasagiline in this disease - are discussed. It is important to emphasize the safety of rasagiline, confirmed in a study ADAGIO: tolerability of rasagiline and placebo was comparable.

About appropriateness of macrolides use for the treatment of patients with comorbide infectious inflammatory diseases of upper and lower segments of respiratory system

Abstract

The functional linkage between the nasal cavity and paranasal sinuses and other organs and systems, primarily with bronchopulmonary system, is emphasized. The number of respiratory diseases, particularly chronic rhinosinusitis, increases in proportion to the number of lower respiratory tract diseases. Streptococcus pneumoniae, Haemophilus influenzae, less frequently Moraxella catarrhalis and atypical flora play a major role in the etiology of bacterial exacerbation of rhinosinusitis and chronic obstructive pulmonary disease. The beta-lactams (aminopenicillins, cephalosporins), macrolides-azalides and modern fluoroquinolones with antipneumococcal activity are promising drugs for the treatment of respiratory infections. Azalides (azithromycin) from the group of macrolides have highest spectrum of activity in regard to pathogens.

Abstract

Hyperprolactinemia is one of the most important syndromes of reproductive endocrine diseases. The frequency of endocrine infertility due to hyperprolactinemia is estimated at 40 to 80%. The mechanisms of formation of excessive secretion of prolactin in various forms of the disease, and the features of effect of hyperprolactinemia on the reproductive system of women are discussed. The fundamental principle of drug therapy of hyperprolactinemia is the restoration of action of dopamine, which suppress excessive activity of lactotrophics. The present article considers the tactics of treatment of different variants of the disease with the use of dopamine agonists - bromocriptin, quinagolide hydrochloride and cabergoline.

Diabetic gastroparesis: possibilities of prokinetic therapy

Abstract

Diabetic gastropathy, accompanied by delay of food evacuation from the stomach, with symptoms of gastro-intestinal dyspepsia, or gastroparesis, occur in 50% of patients with diabetes mellitus. In most cases hypoglycemic, angio-and neuroprotective therapy can not reverse the motor dysfunction of the stomach, which in turn embarrass the effective correction of glycemia. The clinical and post-marketing surveillance studies provide to consider itopride (Ganaton) as first-line drug in the treatment of motor disorders of the stomach. Itopride is an effective and safe prokinetic with antiemetic effect, and use of this drug in patients with diabetic gastroparesis is clinically and pathogenetically substantiated.

In recent years, there are more and more evidences that the disease of old age and oxidative stress are associated with significant cerebrovascular changes. Peroxidation is one of the most important parts of gene activation reactions coding apoptosis, which suggests therapeutic benefit of antioxidants in the acute and chronic stages of cerebral ischemia. At least three main effects of Mexidol - antioxidant, antihypoxic and hypolipodemic - may be useful in the treatment of ischemic brain damage. The groups of patients with dyscirculatory encephalopathy from the perspective of advisability of mexidol administration are discussed.

Antibacterial therapy of community-acquired pneumonia

Abstract

The present article is concerned with the principles of diagnosis of pneumonia from a perspective of evidence based medicine. Data for the etiology of community-acquired pneumonia are presented. It is emphasized that the feature of the current epidemiological situation is a significant increase in the frequency of mycoplasmal and chlamydial diseases of the lower respiratory tract. The problems of choice of first-line antibiotic for treatment of pneumonia are considered. Peculiarities of spectrum of action and the use of macrolides widely used in clinics and hospitals are discussed. Advantages of dispersible form of Josamycin (Wilprafen ® solutab) are presented. The programs of antibacterial therapy of community-acquired pneumonia in outpatient and hospital settings are stated. Author presents own data for long-term experience in treatment of pneumonia.

Moderate doses of statins and combination of statin with ezetimibe in patients with carotide atherosclerosis: influence on blood lipids, inflammation markers and thickness of intima-media complex

Abstract

We investigated the effects of low doses of simvastatin (original and generic products in the average daily dose of 18 mg), atorvastatin (13 mg/day), rosuvastatin (11 mg/day) and simvastatin (12 mg/day) in combination with ezetimibe (10 mg/day) in patients with carotid atherosclerosis without marked hyperlipidemia. We assessed lipid levels, levels of markers of inflammation, and the thickness of the intima-media complex (IMT) according to duplex ultrasonography screening of brachycephalic arteries. Within 36 weeks 180 patients were observed. 150 patients received active treatment, 30 entered in control group. Treatment with low doses of statins and the use of a combination of simvastatin with ezetimibe led to decrease of low density lipoprotein cholesterol levels by at least 30% that led to the achievement of the target values in some patients. Most hypolipodemic effect was achieved against the background of combination of simvastatin and ezetimibe, and monotherapy with rosuvastatin. The level of C-reactive protein in the blood against the background of monotherapy with statins has not changed, and in the group of combination therapy with simvastatin and ezetimibe was significantly reduced. Treatment with low doses of statins, and combination of simvastatin with ezetimibe did not lead to significant change of IMT, indicating inhibition of the atherosclerosis progression, whereas IMT in the control group was significantly increased.

Current conception of functional constipations and approaches to treatment

Abstract

The problem of functional constipation is discussed, and the classification of disease, the causes of its development, diagnostic procedure and treatment guidelines are presented. The results of comparative study, devoted to examination of effectiveness of two drugs - Bisacodyl and Dulkolax - in 110 patients with chronic functional constipation, are presented. According to results of study, Dulkolax and bisakodil are recommended as effective drugs for the treatment of functional constipation, especially in elderly patients. Dulkolax have some advantages over Bisakodil in terms of soft change in stool frequency and consistency, achievement of complete depletion, more rapid and predictable time of effect onset, and significant increase in the electromotor activity of colon. Tolerability of Dulkolax and bisakodil was very good and good in 80% and 72% of cases, respectively.

Certain aspects of diagnostics and treatment of cardiovascular diseases in combination with non-alcoholic steatohepatitis

Abstract

The numerous studies have shown that cardiovascular disease and non-alcoholic fatty liver disease (NAFLD) are widespread in the general population and are linked by common pathogenetic mechanisms. Therefore their combination is typical for obesity and metabolic syndrome. The results of the study, whose goal was to study the clinical course, diagnosis and treatment of arterial hypertension (AH) in patients with non-alcoholic steatohepatitis (NASH - NAFLD stage) are presented. It was found that concomitant NASH leads to more severe course of arterial hypertension. It is emphasized that in patients with arterial hypertension and NAFLD comprehensive approach to treatment, aimed at the various components of metabolic syndrome is justified. In addition to antihypertensive treatment, drugs improving liver function (essential phospholipids, ursodeoxycholic acid), lipid-lowering drugs (statins), and, if necessary, hypoglycemic agents (metformin), should be used.

Treatment of cough in acute respiratory infections and influenza in patients of risk group

Abstract

Cough is one of the common symptoms in medical practice, and is the fifth most frequent cause forcing the outpatients to see a doctor. Data for the etiopathogenesis of cough, its classification and principles of treatment are presented. Particular attention is paid to the treatment of cough in cases of acute respiratory infections (ARI) in smokers. The results of a comparative study of efficacy and safety of a complex preparation Ascoril expectorant and standard therapy for patients with catarrhal diseases with symptoms of bronchial obstruction and cough are presented. The results showed that both regimens are effective and safe. However, against the background of Ascoril administration, duration of severity of symptoms significantly decreased, and course of treatment of ARVI was shorter.
