Tubulo-interstitial involvement in kidney disease in patients with infective endocarditis
Аннотация
Aim. Assessment of incidence and severity of renal tubulo-interstitial damage in patients with infective endocarditis (IE) with kidney involvement.
Methods. 41 patients with IE were included into the study. In all patients albuminuria, urine level of α-2-microglobulin, activity of γ-glutamiltransferase (γ-GT) - enzyme of the brush border of tubular epithelium, mitochondrial enzyme lactatdehydrogenase (LDH), as well as markers of systemic inflammation - serum C-reactive protein and circulating immune complexes concentrations were determined.
Results. Patients with IE were characterized by significant elevation of enzymuria and urine α-2-microglobulin level; this parameters were highest in those patients, who received aminoglicozydes or vancomycin. Signs of glomerular and tubulo-interstitial involvement correlated with intensity of systemic inflammatory responce.
Conclusion. Patients with IE are characterized with renal tubulo-interstital involvement, associated with systemic inflammatory response, glomerulonephritis and decrease of renal function.

LEFT VENTRICULAR REMODELING IN PERITONEAL DYALISIS PATIENTS
Аннотация
Aim Study of myocardial remodeling type and risk factors of left ventricular hypertrophy (LVH) in patients with chronic renal failure (CRF) on peritoneal dialysis (PD).
Methods. 84 patients with CRF on PD (duration 6 - 64 months) were included into the study. Echocardiography, office blood pressure (BP) measurement, 24-hour BP monitoring were performed, hemoglobin, parathyroid hormone, and blood biochemical parameters were measured. 49 patients were examined iteratively (25 on PD, 14 - after renal transplantation, 10 - transferred to program hemodialysis).
Results. LVH was found in 79,8% patients. Left ventricular myocardial mass index was independently determined by duration of arterial hypertension, (р=0,04), office mean BP (р=0,03), daytime and night systolic (р=0,007 and р=0,006), diastolic (р=0,006 and р<0,001) and mean BP (р=0,007 and р<0,001) and their time indices (р=0,03 and р<0,001 for systolic BP, р<0,001 and р<0,001 for diastolic BO), as well as hemoglobin level (р<0,001) and serum concentrations of albumin (р=0,003) and total protein (р=0,004).
Conclusion. Systolo-dyastolic arterial hypertension, 24-hour BP curve abnormalities, anemia, hypalbuminemia and hypoproteinemia are main risk factors for LVH in PD patients

Left ventricular diastolic function in nephrogenic arterial hypertension in patients with preserved renal function
Аннотация
Aim. Assessment of diastolic function of left ventricle (LV) in hypertensive patients with chronic glomerulonephritis and preserved renal function.
Methods. 136 hypertensive patients with chronic glomerulonephritis and preserved renal function were included into the study. Doppler-echocardiography was preformed in all patients.
Results. E/A ratio in hypertensive patients with chronic glomerulonephritis was significantly higher, than in healthy controls. Time of isovolumetric relaxing increased according to the arterial hypertension stage, while time of delay of early diastolic filling decreased.
Conclusion. Hypertensive patients with chronic glomerulonephritis and preserved kidney function have diastolic dysfunction of left ventricle, which progresses according to the acceleration of arterial hypertension

Role of sulodexide in treatment of diabetic nephropathy in patients with type 1 diabetes mellitus
Аннотация
Aim. Determination of efficacy of sulodexide in diabetes mellitus (DM) type 1 patients with diabetic nephropathy (DN).
Methods. 108 patients (19 male, 17 female, age 24-27 years, mean duration of DM 7 - 16 years) were included into the study. 62 patients at baseline had microalbuminuria, 46 - proteinuria. The duration of treatment course with sulodexide was 4 weeks.
Results. Patients with microalbuminuria demonstrated significant decrease of albuminuria, in 50% of them - to normalbuminruia. In patients with baseline proteinuria changes of albuminuria were not significant.
Results. Sulodexide demonstrates antiproteinuric effect in DM patients with DN

Role of R990G polymorphism of calcium-sensing receptor gene in regulation of parathyroid hormone level in children with chronic kidney disease stage III-V
Аннотация
Aim. Determination of frequency of A986S, R990G and Q1011E polymorphisms of calcium-sensing receptor gene in children with chronic kidney disease (CKD) stage II - V and assessment of influence of these polymorphisms on parathyroid hormone (PTH) level.
Methods. 80 children (44 boys and 36 girls, age 11,5±4,2 (1,6-17,3) years) with CKD stage II - V were included into the study. 40 subjects (14 male, 36 female, age 32,9±11,5 (22-45) years) without CKD served as controls. Presence of A986S, R990G и Q1011E polymorphisms of calcium-sensing receptor gene was assessed. PTH level was determined in all patients: in 70 children (PTH>72 pg/ml) secondary hyperparathyroidism was diagnosed, 10 children has hypoparathyroidism (PTH<62 pg/ml).
Results. Frequency of calcium-sensing receptor gene alleles did not differ significantly between groups of children with hyperparathyroidism, hypoparathyroidism and healthy controls. In children with hypoparathyroidism frequency of G allele and RG990 genotype increased in comparison with control group and children with hypertparahyroidism. G allele frequency was 25% in hypoparathyroidism group, 6,2% in control group (χ2=6,25, р=0,01) and 9,3% in hyperparathyroidism group (χ2=4,33, р=0,04). RG genotype was found in 50% of children with hypoparathyroidism, 12,5% in control group (χ2=7,03, р=0,008) and 18,6% in hyperparathyroidism group (χ2=4,96, р=0,03).
Conclusion. Presence of G990 allele of calcium-sensing receptor gene in children with CKD is associated with lower PTH level and susceptibility to hypoparathyroidism
