Experience of organization of surgical aid for gunshot wound of large bowel in the armed conflicts on Northern Caucasus

Abstract

The article presents generalization of experience of organization of surgical aid for gunshot wound of large bowel in the armed conflicts on Northern Caucasus. Were studied results of treatment of 272 wounded persons. Were detected 249 wounds of segmented intestine, 23 wounds of straight intestine. 78% of injured had multisystem injuries. All wounded persons had fire peritonitis, which is a logical result penetrating wound of stomach. On the base of results of treatment were proposed algorithms of surgical tactic in condition of wound of large bowel, also were proposed original methods of closing of major softgranulating wounds of perineum. The article presents a generalized experience of work in sphere of treatment of given category of wounds in coloproctologic center CMCH by Vishnevsky A.A.

Atopic dermatitis by teen-agers as medical-social problem of the Armed Forces

Abstract

The analyze of data of official statistics of Department of Health and Social Development speaks for veridical differences in prevalency and morbidity of atopic dermatitis by teen-agers in comparison with children (younger then 14 years old). These indexes are higher in 2-3 times, then these indexes among children. For the first time was effectuated the analyze of health survey. It has been found that in structure of dermatological pathology in general 5/6-3/4 is the group with patients with new-onset diagnosis, and only 1/5-1/4 - is a dispensary group. For children, persons older then 18 and population in general the index of health survey is practically identic and is equal to 0,5. For teen-agers this index is higher - 0,6 and more. The highest abundance and morbidity is: among constituents of the Russian Federation - Privolzhsky Federal district, among it’s regions - Ul’yanovsk region.

About standardization and control of quality of dental aid

Abstract

On the base of our experience, it’s possible to recommend following methods of controlling of quality of dental aid: daily control of fullness and correctness of filling all the documents by dentist; daily control of quality of restoration installation, fixing of interchannel post and obturation of channel on the base of analyze of roentgenograph; monthly control of precise accomplishment of tooth-treatment methods by dentist; weekly and monthly analyze of comments on filling of documents and accomplishing of treatment methods on medical conferences. The maximum circumstantiation of standarts give an opportunity to calculate the real cost of service and move from cost-sheet principle of keeping up of medical institutes to paying for rendered volume and quality of medical aid.

Clinics, diagnostics and treatment of acute intoxication by baclofen

Abstract

For last few years in Russia and in the Armed Forces of RF is marked a growth of number of acute intoxications by GABA-agonists and by muscle relaxants, particularly by baclofen. Manifestations of intoxication by this specimen include cerebral and somatic-vegetal disorders, in grave cases of illness could evolve psychic excitement, delusions and coma. Treatment of baclofen intoxication compares of gastric lavage using a thick pathfinder, accompanied by insertion of adsorbent and apocathartic means, maintaining of impaired vital functions of body. Hereafter these injured patients should be hospitalized in a treatment institute, where exists a possibility of effectuating of reanimation and intensive care as well as extracorporal detoxication (hemosorption, hemodialysis).

Peculiarities of the course of the repeated outhospital pneumonia by compulsory-duty servicemen

Abstract

The article presents data of peculiarities of the course of the repeated outhospital pneumonia of 68 compulsory-duty servicemen, which were ill for 2-5 times during the service time. In the given group was detected a high percent of ill persons with background pathology of upper and lower respiratory tracts (69%). In the group also were marked: clinical, laboratorial and instrument peculiarities of course of disease, increase of percent of patients with restrictive damages of respiratory function, increase of quantity of patients with complications and of term of hospital stay. Retrogression of vegetative securing of activity before discharge from the hospital after cases of secondary disease by pneumonia brings to light unpreparedness of patients even to minimal physical activity and causes to create individual rehabilitation programs on outhospital base.

Ethiology, clinics and nosological structure of acute nephritic syndrome by compulsory-duty servicemen

Abstract

On example of 110 compulsory-duty servicemen of Far Eastern military region, were studied etiological, provocative factors, peculiarities of nosological structure, clinic, clinical courses and outcomes in conditions of acute nephritic syndrome (ANS). Streptococcal infection took precedence of progress of ANS in conditions of acute glomerulonephritis (AGN) in 96,2% of cases, in conditions of chronic glomerulonephritis (CGN) - in 71,9% of cases. Veridical AGN is detected by 27 patients (24,54%), CGN - 39 patients (35,46%). To remaining 44 patients the diagnosis of AGN also was detected, but for a variety of reasons they were not examined for the second time after stick leave. Peculiarity of ANS among compulsory-duty servicemen is difficultness of determination of it’s nosological belonging to AGN or CGN. It is dictated by speed of regression of clinical and laboratorial manifestations of ANS, by absence hyposthenuria in condition of CGN. In connection with this for detecting of right diagnosis and right decision of questions of expertise, it’s supervised an increase of role of early nephrobiopsia and obligatory repeated examination by persons, survived ANS.

Peculiarities of surgical aid in central regional hospital in conditions of ballistic wounds during the armed conflicts

Abstract

There was effectuated an analyze of delivery of health care to 1294 patients with ballistic wounds, admitted to Gudermess central regional hospital of Chechen Republic during the armed conflict on Northern Caucasus in 1994-2002. Age of wounded was between 12 and 67 years old, among them men - 1088 (84,1%), women - 206 (15,9%) and children - 291 (22,5%). Native population - 945 persons (73%), military servicemen from different departments - 349 persons (27%). In accord with character of destructive factor all wounds were divided in following way: ballistic wounds - 419 cases (32,4%), fragment wounds - 466 cases (36%), explosive wounds - 409 cases (31,6%). 392 persons (30,3%) had isolated wounds, 340 persons (26,3%) had multiple wounds, 513 persons (39,6%) had conjoined wounds, 49 persons (3,8%) had combined (term-mechanic) wounds. In accord with localization all wounds were divided in following way: head - 93 cases (7,2%), neck - 19 cases (1,5%), sternum - 117 cases (9%), stomach - 110 (8,5%), bowl - 89 cases (6,9%), axis - 21 cases (1,6%), upper limbs - 352 (27,2%), lower limbs - 493 cases (38,1%). 1074 were operated, term from admission to operation was: for emergency operation - 2,1±0,3h, for delayed operations - 12±0,5h. 220 patients were not operated, 6 of them died in 30-45 minutes after admission (cause - damages, not compatible with life). 214 were not pertained for first surgical aid. Postoperation complications were 8,2%, mortality - 2,6%, postoperation lethality - 1,93%, average term of stay in hospital - 24,3+0,3 days.

Treatment of cough in conditions of acute respiratory virus infections and of grippe by the patients of group of risk

Abstract

It’s necessary to use a special method to treat an acute cough by patients with acute respiratory virus infections of excessive smoker. Often they have a productive acute cough. Hyperproduction of gluey bronchial mucus requires an assignation of effective mucolytic, expectorant agents, and hyperresponsiveness of mucosa of airways and frequent bronchial obstruction - bronchial spasmolytic. Unmasked, comparative, randomized study found out a high effectiveness of a complex agent Askoril, combining the all 3 effects. Absence of hard adverse reactions, decreasing of threat of polypragmasy by patients of adult-age groups and high clinical effectiveness make this agent into an object of choice in conditions of complex treatment of acute respiratory virus infections, proceeding with manifestations of bronchial obstruction and cough by smokers.

Piggy grippe: epidemiology, diagnostics, treatment and prophylaxis

Abstract

Virus of piggy grippe is a virus of type A, which has greatly changed in it’s antigenic structure. As a result, has appeared a new variant of germ (syb-type), in relation to which vaccines, used for period 2008-2009, are unsuccessful. Virus represents a real risk for life and health of millions of people. Experts of World Health Organization are sure, that eruption can lead to a global expansion of virus. To the group of high risk refer: children younger then 5 years old, full-growns of 50 years old and older, children and teen-agers (from 6 months to 18 years), treated for a long time by aspirin, gravidas, full-growns with several chronic diseases, persons in nursing homes, hospices, requiring a long-termed hipurgia, compulsory-duty servicemen. Latent period is from 1 to 7 days (2-3 days on the average). Accountancy of clinical data: acute beginning, hyperpyretic fever, predominance of damages of upper respiratory tracts. The article presents a detailed characteristic of therapeutic and prophylactic measures in the Army and NAVY.

Abstract

Now heightened interest to a life of military men, to their sexual-reproductive problems is marked. 406 military men has been carried out for sexual-reproductive aspects of behavior estimation. It was established, that in all researched groups the low reproductive installations, an insufficient level of sexual education and the overestimated self-estimation of the knowledge in this area were marked. However, respondents specified problems of reproductive sphere, including sexological character. Thus, taking into account features of sexual-reproductive behavior of military men, the further studying this direction is required with the purpose of perfection of the treatment-and-prophylactic help.
