PROTON PUMP INHIBITORS IN THE CONTEXT OF THEIR EFFICACY AND SAFETY
Abstract
Proton pump inhibitors (PPI), or H +, K +-ATPase inhibitors, are the most commonly prescribed class of drugs to treat acid-dependent diseases. The paper unveils the mechanisms of action of PPI and considers the pharmacokinetic properties of drugs from this group and novel specific adverse reactions.

SIGNIFICANCE OF N34S MUTATION IN THE SPINK1 GENE IN CHANGING THE CLINICAL COURSE OF CHRONIC PANCREATITIS
Abstract
The investigation showed that the N34S mutation in the SPINK1 gene (a pancreatic secretory trypsin inhibitor gene) was most common in patients with toxic and idiopathic chronic pancreatitis (CP). The significantly high rate of identification of the above mutation in patients with complicated forms of idiopathic CP suggests the increased risk of disease complications.

DETERMINANTS OF A RISK FOR CHRONIC KIDNEY DISEASE IN DIABETES MELLITUS
Abstract
The influence of genetic factors on the risk of chronic kidney disease (CKD) was evaluated in patients with type 2 diabetes mellitus. A significant association was found between CKD and polymorphism of the genes (the products of which play a decisive role in the pathogenesis of renal injury) encoding the production of endothelial vasoactive factors (NOS34 gene) and lipid metabolism factors (APOB gene).

The best results were obtained in patients with stenotic bleeding pyloroduodenal ulcers during emergency surgical interventions made after short-term preoperative preparation to replenish blood loss and to correct water-electrolyte and protein-energy disorders available in the presence of pyloroduodenal stenosis.

ISOLATED ACTH INSUFFICIENCY AFTER BRAIN INJURY
Abstract
The sequel of brain injury (BI) may be hypopituitarism, the development of which potentially worsens rehabilitation and prognosis. Cohort studies have shown that the rate of posttraumatic hypopituitarism is 15 to 90%. According to the data obtained by a number of authors, secondary hypocorticism is one of the most common types of tropic insufficiency in posttraumatic hypopituitarism. The authors describe their clinical case of isolated secondary hypocorticism developing after BI.

HEPATOCYTE DYSPLASIA IN HBeAg-NEGATIVE CHRONIC HEPATITIS B WITH DIFFERENT HBsAg PHENOTYPES OF VIRAL INFECTION
Abstract
Examination of 58 patients with HBeAg-negative chronic hepatitis B (CHB) with HBsAg +- and HBsAg - phenotypes of hepatitis B virus infection revealed a considerable predominance of small liver cell dysplasia over large liver cell one, an upward trend for liver cell dysplasia in HBsAg loss, and a direct correlation of hepatocyte dysplasia with the magnitude of a necrotic/inflammatory process and the hepatocyte levels of HBcorAg in HBsAg -.

ASSOCIATION OF INFECTIOUS FACTORS WITH BILIARY TRACT DISEASES
Abstract
In order to study the relationship of biliary tract disease with markers of infection Helicobacter pylori (H. pylori) performed an ultrasound scan of the gallbladder, clinical examination and collection of serum for detection of antibodies to H. pylori in 586 patients aged 18 to 85 years; Esophagofibregastroduodenoscopy performed in 297 cases; target biopsy from antrum division of stomach mucosa with followed definition of H. pylori contamination by morphological method performed in 147 patients. Serological and morphological indicators of H. pylori contamination in patients with cholecystitis and cholelithiasis were higher than in healthy individuals.

SPECIFIC FEATURES OF GENDER ROLE BEHAVIOR IN MALES WITH PANIC DISORDER
Abstract
The Sandra Bam index was used to assess the gender type of behavior in 70 patients with panic disorder (PD) and in 50 healthy individuals. The masculine type was found in 4.3% of the patients versus 22% of the healthy population (p = 0.002). The dominant gender types were androgynous and masculine in the healthy men; on the contrary, feminine gender role behavior is most common in the men with PD. Gender behavior in the men was associated with the type of PD. PD with agoraphobia is characteristic of the men with feminine gender behavior and that without agoraphobia was more common in the men with androgynous gender behavior, in whom alcoholic excesses were a characteristic trigger of the first attacks. Inadequate masculinization is a characteristic personality trait in the patients with PD and a predisposing factor in the formation of avoidance behavior.

PROGNOSTIC VALUE OF LIPID METABOLISM MARKERS IN PATIENTS WITH ST-SEGMENT ELEVATION MYOCARDIAL INFARCTION
Abstract
Out of 133 patients diagnosed with ST-segment elevation myocardial infarction (MI), 66 were found to have early MI complications (acute heart failure, early postinfarction angina pectoris, recurrent MI, life-threatening cardiac rhythm and conduction disturbances) during hospitalization. During a year, 38 patients were fixed to have unfavorable disease outcomes, such as progressive angina pectoris, decompensated heart failure, and recurrent MI. Only 3 out of all the lipid profile parameters determined in the hospital period of MI, such as the levels of free fatty acids, oxidized and modified low-density lipoproteins and their antibodies, were established to be of prognostic value in patients with both early and late outcomes of MI.

SIMULATION OF GASTROESOPHAGEAL REFLUX DISEASE IN OLD AGE GROUPS
Abstract
A 5-year prospective cohort study was conducted in patients (mean age 69.0±5.9 years) with gastroesophageal reflux disease. The results of discriminant analysis were used to derive original formulas that are supported by a Russian Federation patent for invention and can predict the unfavorable course of the disease in elderly patients to an accuracy of 97.1%.

DIFFERENTIAL DIAGNOSIS OF HEPATITIDES AND LIVER CIRRHOSES
Abstract
The existing methods for the diagnosis of diffuse liver damages do not always allow the timely detection of these diseases. Their differential diagnosis presents even greater difficulties. A computed tomographic (CT) histographic technique has been devised and tested, which can define the pattern and magnitude of morphological changes in the liver parenchyma in its different diffuse damages. The technique involves the examination of a CT histographic spectrum based on the division of the total densitometry value in the measuring zone.

PROBLEMS IN THE DIAGNOSIS OF SUBCLINICAL HYPERCORTICISM IN ADRENAL INCIDENTALOMAS
Abstract
Subclinical hypercorticism (SH) is characterized by impaired cortisol secretion without specific clinical manifestations of hypercorticism. According to various sources, the prevalence of SH varies between 5-30% among patients with adrenal masses. Sixty-four patients with adrenal incidentalomas were examined; 11 of them were found to have SH. Surgery was performed in 1 patient. Based on the findings, the authors propose an algorithm for examination of patients with adrenal incidentalomas for SH.
