role of sotalol in the treatment of heart rhythm disturbances: modern approaches

Abstract

Sotalol is a mixture of sinistrorotatory (has the properties of beta-blocker, increases the duration of the action potential of cardiomyocytes) and dextrorotatory (characterized by the properties of class III antiarrhythmics) stereoisomers. Several studies showed sotalol efficacy for reduction of atrial fibrillation and maintenance of sinus rhythm, comparable to amiodarone. In long-term monitoring, sotalol is less effective than amiodarone, but has a better safety profile. In addition, sotalol and amiodarone are major antiarrhythmics applied for the secondary prevention of sudden cardiac death.

Clarithromycin as inherent component of anti-helicobacter therapy

Abstract

The article is devoted to the review of current treatment regimens of Helicobacter pylori infection and place them clarithromycin. Current recommendations of international consensus Maastricht II-III are presented. We discussed the possible ways to solve the main problems associated with increasing antibiotic resistance of H. pylori. The basic reasons for its development are stated. Particular attention is paid to the prospects of new regimens, in particular the eradication treatment regimen with clarithromycin administration, and to the quality of antibacterial drugs.

The general principles of drug therapy of hepatobiliary system pathology based on the correction of gall bladder and sphincter apparatus function, hepatic fibrosis and injury reduction. The use of Silybum marianum flavonoids in the treatment of hepatobiliary system pathology is proved. The results of study of Hepabene (fumitory extract + Silybum marianum fruit extract) efficacy for hepatobiliary disorders are presented.

Abstract

The paper presents the review data for the Meksidol pharmacological effects and pharmacokinetics. We considered the neuroprotective, anti-ischemic, nootropic, antistress, anxiolytic, anticonvulsant, anti-alcohol, antiatherogenic, and geroprotective effects of Meksidol. Special attention is given to the Meksidol effects in the treatment of neurodegeneration diseases, especially acute and chronic cerebral circulation disorders. The data for multicomponent mechanism of Meksidol action with important antioxidant and membranotropic effects, the ability to modulation of the receptors and membrane-bound enzymes enzymes function, and restoration of neuromediator balance are presented.

Potential for Pantogam use in medical practice

Abstract

The main effects and classification of nootropics, such as Pantogam (hopantenic acid) which affect the system of gamma-aminobutyric acid (GABA), are presented. As contrasted with GABA, hopantenic acid cross well the blood-brain barrier and has a pronounced effect on the functional CNS activity. We consider a diverse range of Pantogam effects that provided use of the drug in the somatic, neurological and psychiatric practice, including children. It is emphasized that the most important sphere of Pantogam application are asthenic conditions; it is also actively used in cerebrovascular pathology. The successful experience of Pantogam administration in children with epilepsy, attention-deficit hyperactivity syndrome, and cognitive impairment is presented.

Pantoprazole use in the treatment of symptomatic (stress) gastro-duodenal ulcerations

Abstract

Stress ulcers (SU) - symptomatic gastroduodenal erosions with stomach predominance. They arise due to extensive surgical interventions on the abdominal cavity organs and aorta, and myocardial infarction. The main pathogenetic mechanisms of SU are: ischemia of gastric and duodenum mucosa, hypovolemia, hypotension and apoplasmia; increase of ACTH, corticosteroids, catecholamines, and histamine production, which has an adverse effect on the mucous membrane, and enhancing the acid-peptic factor; gastroduodenal motility disturbanses (gastroparesis and enteroparesis, duodenogastric reflux). In modern therapy of SU, proton pump inhibitors with minimal potential for drug interactions having a dosage form for parenteral and oral administration are often used. SU management with pantoprazole, its efficacy for standard doses administration in comparison with other proton pump inhibitors are presented.

Abstract

Acute low back pain (LBP) - a common pathological condition associated with a high incidence of temporary disability. LBP - a benign condition requiring a detailed instrumental assessment only in the presence of symptoms, suggested the secondary nature of pain. An important task is pain relief in patients with LBP. NSAIDs, including meloxicam (Movalis) can be used for this reason. The data for drug efficacy in various pain syndromes are presented. We have observed good tolerability and low risk of gastrointestinal complications. Given the current data we discuss the problem of low risk of atherothrombotic complications in Movalis administration.

Migraine is a chronic disease that breaks conventional lifestyle, performance efficiency, leading to patients disadaptation. The paper discusses the problem of therapy and prevention of migraine attacks. We considered drugs of different pharmacological groups used for the treatment of acute pain attacks and preventive therapy. It is emphasized that the prevention of migraine is one of the most effective ways to disease control. The requirement for preventive therapy is determined not only the frequency, duration and intensity of migraine attacks. The indications for preventive treatment are: high level of patients disadaptation, the presence of comorbid disorders, complications of migraine and high risk disease "chronification".

Legal aspects of the prescription of drags for off-label-use

Abstract

The article is dedicated to the problem of drugs administration out of the instructions for use (off-label-use). It is emphasized that the off-label-use of drugs applies not only for indications but also for others formal parameters, such as dosage form, dosage, mode of administration, etc. We discuss the practice of off-label-use in foreign countries, particularly in Germany. In Russia, there is no normative document giving guidance for action if off-label-use of drug required. Today, however, the legal practice of off-label-use may occur as individual therapy for health reasons when performing the necessary requirements for scientific substantiation of the potential benefit to the patient, decision-making by council of physicians, adequate informing the patient (or his representative) and patient voluntary informed consent.

Abstract

The problems associated with the use of beta-blockers (BAB) in essential arterial hypertension (AH) are discussed; in particular, we consider the possibility of using these drugs in patients with multiple metabolic risk factors, including metabolic syndrome and its components. It is emphasized that high cardioselektive BAB, such as bisoprolol, have no negative adverse effects of first generation BAB and can be widely used in the treatment of cardiovascular diseases, including hypertension. The results of numerous clinical studies confirming the high antihypertensive efficacy of bisoprolol and its favorable safety profile, including patients with carbohydrate metabolism disorders and broncho-obstructive pathology.

Pharmacoeconomic analysis of patients with the management of arterial hypertension with the use of bisoprolol

Abstract

The comparative pharmacoeconomic analysis of the treatment of arterial hypertension (AH) by medications bisoprolol (Bisogamma and Concor) based on study conducted by Shostak N. et al. (2008) showed that the Bisogamma administration is scientifically well-founded approach to the treatment of hypertension in middle age patients from an economic point of view. Compared with Concor, Bisogamma clinical benefits (efficiency, estimated by the number of cases achieving the target blood pressure) is accompanied by substantial economic benefits, which amounted to 22 984 rubles in the treatment of 100 patients within 60 days, including hospital and outpatient phases. Based on the these data, the transition from Concor on Bisogamma will allow further treat 2.5% of patients, that has great economic importance, given the high prevalence of hypertension in the population.

Abstract

The article is dedicated to the problem of lipid metabolism disturbances and its role in the pathogenesis of metabolic syndrome (MS) and its complications. We review the numerous international studies have shown the importance of dyslipidemia in the development and progression of cardiovascular disease, evaluating lipid-lowering and pleiotropic properties of statins and fibrates. Data for own comparative analysis of case histories of 66 patients with MS treated with lipid-lowering therapy by micronized fenofibrate (Tricor) and atorvastatin are presented. According to the analysis results, fenofibrate can be recommended for use in patients with low HDL cholesterol level, high triglyceride level, and normal or moderate elevated low density lipoprotein cholesterol level. It is submitted that fibrates have a wide range of favorable nonlipid effects, among which anti-inflammatory, antioxidant, anticoagulant properties, the effect on the functional state of the endothelium of the vasculature, which may play a key role in decrease of risk of cardiovascular complications of metabolic syndrome.

Clinical significance of dornase alfa (Pulmozyme) in complex therapy of early age children suffering cystic fybrosis

Abstract

We considered the pathogenic mechanisms, clinical presentation and principles of treatment of cystic fibrosis. Data for retrospective study of clinical efficacy and safety of long-term administration of recombinant human DNase (dornaza alpha; Pulmozim) in infants with cystic fibrosis are presented. The results confirm the Pulmozim safety and efficacy in this group of patients. The frequency of respiratory episodes is decreased by 42.37 due to Pulmozim administration. It is emphasized that early Pulmozim administration helps in the prevention of respiratory episodes, reduction of inflammatory activity in the lungs, improvement of nutritional status, and in the future - improvement of quality and extended life-span in patients with cystic fibrosis.

Clinical experience of Berlitione® use in liver diseases

Abstract

In a study including 52 patients assessed the Berlition ® 300 (Berlin-Chemie AG/Menarini Group company, Germany) efficacy in the treatment of chronic liver diseases. The results of treatment confirm the positive effect of the drug Berlition ® on the functional state of the liver in patients with chronic lesions of different etiology, which was seen as significant improvement in clinical condition, the normalization of the functional "hepatic indices, decrease of cholesterol and glucose levels, significant improvement of quality of life in patients. Only one patient had moderate changes in some biochemical parameters after treatment.

Encephabol in the treatment of patients with moderate cognitive disturbances

Abstract

We conducted the complex study of the efficacy and safety of the drug Encephabol (piritinol) in patients with chronic cerebrovascular insufficiency with mild cognitive impairment. The daily dose was 200 mg thrice daily, course of treatment - 60 days. For the year held two piritinol treatments - for 1st and 6th month of follow-up. Piritinol at a dose of 600 mg daily leads to significant improvement of cognitive function and reduction of asthenic syndrome and depression severity in patients. Piritinol administration causes the decrease of progression rate of chronic cerebrovascular insufficiency. The drug in this dose is well tolerated, including elderly patients groups.
