Peculiarities of differential diagnostics and treatment of arterial hypertension in young people

Abstract

We consider the peculiarities of differential diagnosis and treatment of arterial hypertension (AH) in young people. We give much attention to the lack of research in this area. Importance of differential diagnosis to exclude secondary (symptomatic) hypertension, which often occurs in younger ages, is emphases. For this reason, the effectiveness of new coformulated drug Liten H (lisinopril + hydrochlorothiazide) which enhances the treatment compliance of young people with hypertension is considered.

Phytosterins and cardiovascular diseases

Abstract

It has been proved that the consumption of phytosterins in doses of 1.5-3.0 g daily in the "functional" foods leads to a decrease of plasma total cholesterol and low density lipoprotein by 10-15%. These data determine the interest of researchers to the role of phytosterins in the prevention of cardiovascular disease (CVD) and their acute complications. In this regard, there are some conflicting data obtained in studies on animal and human associated with dose-related effects of phytosterins and various accuracy and selectivity measurement techniques. In LASA (2008) epidemiological study, included random sample of 1,242 people older than 65 year in the Netherlands showed that moderate increase of plasma phytosterol levels was associated with reduction of risk for cardiovascular disease. Prospective randomized placebo-controlled clinical studies with fixed endpoints are required to confirm the effectiveness of phytosterol in the "functional" foods for the prevention of CVD and their acute complications at the level of evidence-based medicine.

Use of nimesulide in the therapy of rheumatoid diseases

Abstract

Nimesulide is widely used in 50 countries, including Russia and the EU. A number of clinical trials showed Nimesulide efficacy for correction of disorders associated with lesions of the peripheral joints, spine, periarticular soft tissues in patients with rheumatic diseases. In these studies, nimesulide was effective as other nonsteroidal anti-inflammatory drugs (NSAIDs), and caused a rapid response in patients. In addition, the pharmacokinetics of nimesulide characteristics (short half-life, lack of clinically significant accumulation) and the ability to selective cyclooxygenase-2 inhibition determine the general drug high tolerability. Analysis of the results of its use in general clinical practice shows that nimesulide significantly less likely cause adverse event on the gastrointestinal tract as compared with nonselective NSAIDs.

Place of ciclosporin A in modern therapy of rheumatoid arthritis

Abstract

The article dedicated to problem of treatment of rheumatoid arthritis (RA). At the present time, cyclosporine A (CyA) - immunosuppressant with selective effect on the T-lymphocytes especially without cytotoxic effects of immunosuppression - is effective medicine of pathogenetic therapy of RA in patients poor under the standard basic therapy or in cases ineffectiveness of treatment by methotrexate. CyA regimen in RA is well established and provides a treatment monitoring. In case of insufficient response to treatment, combined therapy including using a combination of CyA with genetically engineered biological reparations is feasible.

Abstract

Subarachnoid hemorrhage (SAH) is form of stroke, in which the development of cognitive deficits in the long term is preventable with an integrated approach to treatment and rehabilitation. This is especially important given that SAH develops mainly in younger working age patients. The review is concerned with the current understanding of the causes of cognitive deficits associated with SAH. The evaluation of the various medical and surgical treatments for SAH on cognitive outcomes is presented. Relevant data for the effectiveness of preventive and therapeutic Nimotop (nimodipine) administration, its role in preventing the development of cognitive impairment are discussed.

Abstract

Dizziness is an important interdisciplinary medical-social problem, the solution of which is aimed at improvement of pre-existing methods of diagnosis and treatment and development of new approaches of diagnosis and treatment. Severity of clinical symptoms, urgent state in patient during dizzy spell requires a doctor's good knowledge of the algorithm of diagnosis and medical care plan. The article describes basic disease, accompanied by dizziness, provides informative differential diagnostic criteria, recommendations for their practical application. The results of studies which support the usefulness of the drug Vazobral for dizziness caused by cochlear-vestibular angiogenic disorders are presented. The paper discusses Vazobral efficacy in patients of older age groups, and in the climacterical period. Modern view of the main treatment and rehabilitation and preventive measures for dizziness is presented.

Abstract

We consider the possibility of acetylcholinesterase inhibitors use, primarily ipidakrine (Aksamon), in the treatment of diseases of the peripheral nervous system (various mono- or polyneuropathy, neuralgia, etc.). It is emphasized that the ipidakrine inclusion in the remedial measure complex leads to increase of treatment efficacy, reduction of recovery period, and is not accompanied by the risk of development of muscle contractures in patients with injuries of peripheral nerves. In addition to the positive impact on the restoration of impulse conduction in peripheral nerves and the reinnervation process, ipidakrin has analgesic effect. The good drug tolerability enables to recommend it to a wide range of patients, including elderly with concomitant diseases.

Relief of acute pain syndrome in the patient with spondylogenic dorsalgia

Abstract

Dorsalgia (particularly, lumbalgia) is a serious medical problem with involvement of significant economic losses. We consider the possibility of non-steroidal anti-inflammatory drugs (NSAIDs) ketorolac administration, which is widely used in several countries for the relief of acute pain caused by various pathological conditions, in treatment of dorsalgia. According to numerous studies, ketorolac is an effective and sufficient safety drug for the treatment of acute lower dorsalgia. At least, it equal to other NSAIDs, in some cases surpassing their efficacy and tolerability. Comparable analgesic effect of ketorolac with opioids in the absence of many undesirable effects of the latter is also showed.

Abstract

Data for evaluation of efficacy of magnesium-containing drugs in 185 patients experiencing emotional stress, with tension headaches, hyperventilation syndrome, vegetative dystonia syndrome, asthenia, and other forms of neurotic disorders associated with lower adaptive capacity are presented. Along with the clinical neurological and vegetative assessment, a third of patients underwent to analysis of magnesium and other of essential micronutrients (calcium, sodium and potassium) content in biological material (hair). Initially, most patients have magnesium deficiency, and other essential elements in absolute measures were kept within normal limits. Course Magne B6 administration (8 weeks) significantly increased the magnesium level, favorably impacted on its relations with other important minerals that leaded to regression of clinical manifestations, autonomic dysfunctions relief and increase of tolerance to stress in the orthostatic stress.

Abstract

The results of a pilot clinical study of vasoprotective and antiatherosclerotic effect of simvastatin in 60 patients with carotid atherosclerosis confirmed by ultrasonic duplex scanning are presented. Clinico-instrumental patients assessment included the evaluation of the lipid profile, endothelial function (endothelium-dependent vasodilation of the brachial artery in the reactive hyperemia test), the thickness of the intima-media complex and the morpho-functional state of the atherosclerotic plaques of carotid arteries. The study performed within 12 months. Simvastatin administration in low dose (25 mg daily) leads to improvement of vasodilation capacity, reduction of proinflammatory cytokines production, regression of the thickness of the intima-media complex and increase the volume of atherosclerotic plaques. Secondary to anti-inflammatory action, simvastatin administration caused the conversion of soft plaques to the hard plaques that characterizes their greater stability and reduces the risk of atherothrombosis.

The effects of cytoprotectors Meksikor and Mildronate in combination antihypertensive therapy with metoprolol on the level of serum lipid peroxidation products, daily blood pressure profile, morphofunctional state of left ventricular myocardium and the state of the vessel wall in patients with arterial hypertension are evaluated. The results indicate the perspective use of Meksikor and Mildronate in combination with metoprolol in the treatment of hypertension. Evaluation of the beta-adrenoblocker atenolol efficacy in patients with arterial hypertension depending on renin-aldosterone blood profile

Abstract

Efficacy of beta-adrenoblocker atenolol use in mono- and combination therapy in patients with arterial hypertension (AH), distributed on the blood renin-aldosterone profile is evaluated. The study enrolled 656 patients with essential hypertension. Plasma renin activity (PRA; normal - 1.0-3.0 ng/ml/h) and aldosterone plasma concentration (APC; normal - 0.18-0.83 nmol/L or 5-23 ng/dL) were identified by radioimmunoassay technique. The PRA/APC ratio amount 5-23 is allowable. It is concluded that atenolol in monotherapy or in combination with nifedipine retard, enalapril and losartan has a pronounced hypotensive effect, leads to a normalization of all parameters of the renin-angiotensin-aldosterone system, regardless of the initial activity sympathoadrenal system in patients with PRA > 3.0 ng/ml/h. In the event of PRA ≤ 3.0 ng/ml/h, atenolol administration is effectually in combination with enalapril or losartan in the case of the initial high activity of sympathoadrenal system.

Possibilities for the correction of carbohydrate metabolism disturbances and 24 hours blood pressure profile in patients with chronic heart insufficiency and metabolic syndrome

Abstract

The data for evaluation of effect of the drug Dibikor (taurine) on carbohydrate metabolism, insulin resistance (IR) and the parameters of the 24 hours blood pressure monitoring in patients with metabolic syndrome (MS) and chronic heart failure (CHF) are presented. The study included 65 men at the age of 31-66 years. Dibikor was added to the main group therapy with enalapril and indapamide. Dibikor administration leads to positive dynamics of anthropometry parameters, carbohydrate metabolism, IR and 24 hours blood pressure monitoring. Thus, the Dibikor use in the treatment of patients with MS and chronic heart failure significantly improves their carbohydrate status, reduces the severity of IR, and promotes more effective control of pre-existing blood pressure disturbances.

Mexidol in the rehabilitation of elderly patients survived after ischemic stroke

Abstract

The effect of antihypoxant and antioxidant Mexidol on clinical neurological, biochemical parameters and cerebral hemodynamic in 20 elderly patients for recovery phase of ischemic stroke is evaluated. We used Meksidol in dose of 4 ml (200 mg) per 100 ml of saline intravenously once daily (7 days), then 2 tablets bis in day (15 days). Meksidol administration leads to sense of well-being, memory and mnestical function improvement, reduction of depression level. We noted social activity range extension, spasticity reduction, cerebral circulation improvement and activation of relationship between the hemodynamics in extra- and intracranial vessels in the carotid circulation in patients. This goes to prove the activation of metabolic processes in the brain.

Neurodiclovit in the treatment of dorsalgia and artralgia in patients with arterial hypertension

Abstract

The results of evaluation of the effecacy and safety of Neurodiclovit, combined drug containing 50.00 mg of diclofenac sodium, 50.00 mg of thiamine hydrochloride, 50.00 mg of pyridoxine hydrochloride and 0,25 mg cyanocobalamin, in 36 patients with hypertension suffering from dorsalgia and/or arthralgia. Neurodiclovit administration leads to significantly relief of low-back pain and joint pain severity without action on blood pressure. The drug was well tolerated; rare side effects did not require its cancellation.

Dynamics of quality of life in patients with different forms of epilepsy against a background of therapy with topiramate

Abstract

Assessment of therapeutic efficacy and tolerability of topiramate (Topamax), its effect on emotional state, sleep and quality of life is presented. We examined 45 patients (18 men and 27 women) with average age 30.7 ± 11.7 years. Topiramate administration started with a dose of 25-50 mg daily, titration step was 3-5 days. The effective dose was 50-500 mg daily (1-5 mg/kg). The study included history taking, general clinical and neurological examinations, analysis of EEG, X-ray and/or magnetic resonance imaging of the brain, comparative evaluation by next scales: subjective sleep characteristics score, sleep apnea syndrome screening, Epwort sleepiness scale, Hospital Anxiety and Depression Scale, British National Hospital Scale severity of epileptic seizures, QOLIE-89 scale. Remission was achieved in 22.2% of cases. Side effects occurred in 17.7% of patients. The greatest effect was marked in cases of secondary generalized seizures. The link between quality of life and side effects of therapy, psychoemotional and sleep disorders is noted.
