PRINTsIPY PERSONALIZATsII I PREDSKAZATEL'NOSTI V VOSSTANOVITEL'NOY MEDITsINE

Bobrovnitskiy I.P., Vasilenko A.M.

Abstract

The maintenance personalized and predictive medicine has been updated at the expense of the modern data regarding molecular-genetic bases of illnesses. General practical realization of prospects of genomics, proteomics and metabolomics is problematic for some reasons. Thus along with the detection of the genes associated with any disease and its molecular markers – targets for pharmacological medical products, the application of traditional personalized methods of an estimation of risks, early revealing, the forecast of development and preventive maintenance of the most widespread diseases is appropriate. Methodological bases and concrete examples of the personalized approach in the field of the rehabilitation medicine focused on primary application of non-drug means and methods are resulted.

Abstract

We study the pathogenetic significance of microcirculatory disorders in neuro dystonia of hypertensive type. Found that in this condition decreases the level of capillary perfusion mainly due to the nutritional component of the microcirculation. It is shown that the exchange application of pulsed electric field in patients with neuro dystonia of hypertensive type has a marked effect on the correctional system microhemodynamics. Microcirculatory changes were due to increased oscillations of myogenic and endothelial ranges. Result from activation of local mechanisms of tissue blood flow was adequate modulation of microvascular focused on increasing the bulk characteristics microhemodynamics, increased transport function of blood, improving transcapillary exchange and trophic tissue.

INTERVAL'NAYa GIPOKSIChESKAYa TRENIROVKA V LEChENII NEYROIMMUNOENDOKRINNYKh NARUShENIY PRI AUTOIMMUNNOM TIREOIDITE

Abazova Z.K.

Abstract

Efficiency of normobaric interval hypoxic training in treatment of hypothyroidism with autoimmune genesis in children and adolescents was shown. The therapeutic effect of hypoxic therapy realized not only through amplification of compensatory mechanisms for the oxygen delivery to the tissues, but also through inhibition of humoral immune responses and the stimulation of T-cell immunity in patients with autoimmune thyroiditis. Increase of function and quantity of CD8 +- cells after a course of hypoxic therapy prevents the progression of the autoimmune process and helps to restore the function of the thyroid gland, which in turn leads to positive changes in the neurological status of patients: improved mental performance indicators and fine motor coordination. Complications of therapy or deterioration of the patients were not observed. Follow-up monitoring of patients conducted after 6-8 months after treatment showed that the positive effect of hypoxic therapy maintained throughout this period. Positive hormonal and immunological and neurological dynamics in children and adolescents with autoimmune thyroiditis after the interval hypoxic training suggests its inclusion in the scheme of pathogenetic treatment of patients with this pathology.

VEGETATIVNYY KONTROL' SERDEChNOGO RITMA V DINAMIKE NAGRUZOChNOY TOLERANTNOSTI

Pokhachevskiy A.L.

Abstract

An ingenious method for determination of the heart rate vegetative regulation is used to study its dependence on the loading tolerance level. The maximal loading test for the mixed population (232 persons of 19±2 years old) and analysis of the resulted cardiac rhythmgrams revealed 3 clusters by tolerance level, each of which with its own defining characteristics. At that, the entire studied population showed the incremental decrease of the heart rate responsiveness to the vegetative stimulus in the loading period and the incremental increase in the recovery period, and the sensitivity to parasympathetic stimulus as distinct from sympathetic one was decreasing more slowly at loading and increasing faster at rest. The loading tolerance growth developed in the vegetative control extension (duration and intensity) in the loading period and its acceleration at recovery, accompanied in both cases by strengthening of the parasympathetic domination.

Abstract

Now the standards of integral estimation of muscle tone for different patient’s groups have not been established and the most appropriate, in terms of purposed dosed efforts for a massage is estimation of the maximum force. Integral estimation, especially for automated massage, can be used as an additional objective estimation that characterizes the direction of change in the patient's state.

Abstract

The present study was designed to evaluate efficacy of the combination of laserpuncture and tenoten for the management of the autonomic nervous system disorders. The Bashkirian State Medical University students with a mean age of 20±3,5 years were enrolled into the study. Autonomic nervous system disorders were detected in 73,1% of students. An individual approach was essential for choosing acupuncture points and methods of laserotherapy impact. With predominant sympathicotonia, we applied the method of inhibition, with vagotonia, - that of stimulation and with eutonia – the method of harmonization. The study results have shown that acupuncture performed properly regarding its indication and dose exerts a beneficial effect on the functional state of the autonomic nervous system and corrects the vegetative tonus. We have noted that the combination of laserpuncture and tenoten as well as hydrotherapy enhance the positive dynamics and improve attention selectiveness along with psychoemotional condition.

BIOPOLITIChESKIY PODKhOD K REABILITOLOGII: POTENTsIAL'NAYa ROL' MIKROBNOY NEYROKhIMII. OBZOR

Oleskin A.V., Shenderov B.A.

Abstract

In this review, brain neurochemistry and the rehabilitation of patients with neurohormonal disorders, predominantly those related to biogenic amines (norepinephrine, dopamine, serotonin, and histamine) are considered from the biopolitical perspective. In light of the data available in the literature and the authors’ own findings, the effects of human microbiota-produced neuromediators on human psyche and behavior are discussed. The question is raised whether it is potentially possible to develop “new generation” probiotics aimed at exerting a neurochemical influence on the human organism and particularly on the brain.

Reabilitatsiya s Ispol'zovaniem gipoksii pri rake molochnoy zhelezy (OBZOR)

Abstract

The gland mammary cancer rate of morbidity is getting higher and on. That is why the prophylaxis, treatment of complications, and consequences of its complex treatment become more and more actual. If the patients have undergone a radical treatment of mammary gland cancer, the medical rehabilitation of them is a complicated task. At the same time, the idea to use the hypoxemia therapy to help this group of patients seems to be not in a really sufficient degree. However, we have information concerning the influence of mountain climate on a decrease of the incidence of malignant tumors when the height above sea level is as more as possible. A number of authors records that there is a malfunction of oxygen exchange in different tissues. First of all, it concerns bearers of tumors in the process of malignant growth of them. There is a basic mechanism of their growth. We mean an action of gas hypoxemia mixtures to intensify transportability of oxygen to tissues as well as training of enzymatic processes to realize biological oxidation. So, if we use an intermittent hypoxia training everything gets better. We mean that all the components of patients’ breathing functionally improve. That includes external respiration, blood circulation, as well as mechanisms responsible for tissue respiration. So the idea is to follow: the perspectives of investigations in this realm of science are irresistible, especially, if we take into account the simplicity of treatment procedure as well as absence of any side effects.
