POSTURAL FUNCTION EVALUATION IN CLINICAL PRACTICE

Abstract

For balance dysfunction assessment in rehabilitation practice three main methods are used: clinical, clinical scales and objective instrumental methods. Clinical scales are easy to use and does not require additional equipment. However, their value is relatively small. Instrumental methods provides diagnosis as well as differential diagnosis, including early and even preclinical forms of diseases. Appropriate standards for practical work are reviewed.

PSYCHOLOGICAL STUDY OF CHILDREN COMMUNICATION CAPABILITIES IN DEEP ALTERED STATE OF CONSCIOUSNESS AFTER SEVERE BRAIN DAMAGE

Abstract

The authors have developed a scale, which evaluates signal possibilities of children who are in the deeply changed consciousness state (vegetative, minimal consciousness, etc.). The Scale of Communicative Activity by Bykova-Lukjanov (SCABL) has an important correlation with both Glasgow Coma Scale (GCS) when GSC is used to fix the level of consciousness depression and Dobrokhotova-Zaitzev Scale for assessing the consciousness recovery. SCABL is aimed for psychologists who work with seriously ill patients. It fixes a large number of communicative signals as well as their intensity. The article also shows the different recovery scenarios bodily, vegetative, mimic and gesture, verbal signals after severe brain injuries, as well as the change in the level of contact of children with the surrounding world.

BASING FOR EMERGENCY REHABILITATION IN THE PREVENTION AND TREATMENT OF «POST INTENSIVE CARE» SYNDROME (PIC SYNDROME)

Abstract

The totality of limiting everyday life of the patient somatoneurologic consequences of a stay in the Intensive Care Unit (ICU) in the united syndrome (PICS - Post Intensive Care Syndrome). The bed-rest is recognized as the main pathogenetic factor of PICS-syndrome Combined with aggressive methods of intensive therapy (sedation, myoplegia) it promotes the development of immobilization syndrome. There are two main mechanism of this syndrome: muscle inaction and orthostatic failure (core). In a series of special studies have shown that this condition occurs in all patients with acute cerebral insufficiency with the first day in the ICU. The main strategy of prevention and treatment of PIT syndrome is expected early kinesiotherapy, in particular, plumbing and passive cycle velobike training.

META-ANALYSIS: ISCHEMIC PRECONDITIONING AFTER TRANSIENT ISCHEMIC ATTACK PROTECTS AGAINST STROKE IN HUMANS

Abstract

We hypothesize that transient ischemic attacks (TIA) can afford a protection against stroke in humans as well as ischemic preconditioning (IPC) can reduce ischemic brain injury in experiments. We used 16 studies. For statistic analysis we used Mantel Hanzel test. To study role of TIA in favorable outcome we included six reports with equal design in meta-analysis. They were 7912 patients with only stroke and 703 patients with TIA before stroke. This meta-analysis provided evidence that patients with prior TIA had more favorable outcome, than patients with only stroke (p=0,0002). To test the influence of TIA on the stroke mortality we analysed only two trials. They were 3108 patients with only stroke and 361 patients with TIA before stroke. In contrast, TIA didn’t lead to decrease of mortality after stroke (p=0,30). Conclusions: TIA may have IPC effect in the human brain, which leads to elevating of favorable outcomes after stroke.

NEUROPHYSIOLOGICAL PREDICTORS OF EFFICIENCY ROBOTIC MECHANOTHERAPY OF PATIENTS WITH ISCHEMIC STROKE

Abstract

The paper analyzes the findings of a comprehensive survey of 628 patients (346 men and 282 women, average age 51,4 ± 1,23 years) with ischemic stroke in the middle cerebral artery, which were used in the rehabilitation of locomotor robots - Erigo and Lokomat. Complex clinical and neurophysiological (TMS and EP) survey conduction whether the 6th , 21th and 42th day of stroke and in the long term - 6 months. It was estimated, that the main predictors of the effectiveness of robotic mechanical therapy for stroke is stroke severity, severity of paresis and the activity of the motor cortex at TMS forecast with accuracy from 68 to 96%. The proposed integrated clinical and neurophysiological approach can be used to determine the rehabilitation potential of patients with ischemic stroke.

POSSIBILITIES OF MODERN MECHANICAL THERAPY IN THE CORRECTION OF MOTOR DISORDERS OF NEUROLOGICAL PATIENTS

Abstract

The objective of this review is to summarise the data of literature regarding use of robotic mechanical therapy in different periods of recovery after damage of the nervous system. It is apparent that a recovery of motor function lesion as a result of illness or injury of the nervous system, can only be the result of intense repetitive prolonged training. This review highlights the need to use robotic technology in the early recovery period, provides a list of the most commonly used devices and describes technical features essential in mechanical apparatus for fait restoration. Also, this review highlights methods of robotic movement therapy as a method of choice, especially in patients with severe motor deficits, as well as describes the most common protocols for use of rehabilitation training simulators. Additionally, the review provides characteristics of exoskeletons, which will expand the use of motor capabilities and independence of patients in everyday life.

MECHANICAL STIMULATION OF THE PLANTAR SUPPORT ZONES IN ACUTE PERIOD MODERATE AND SEVERE STROKE

Abstract

The study included 45 patients aged 61 (55,0; 66,0) years, an average of 1 (1,0; 2,0) day from the time the development of moderate and severe stroke. Study group comprised 24 patients who received mechanical stimulation of the plantar support with the use of a device «Korvit» (75 steps in 1 minute) and conventional therapy in the first hours of stroke. The comparison group consisted of 21 patients who received only conventional therapy. All patients on admission and at 21 days after stroke onset were evaluated according to international clinical scales (NIHSS, Rankin, Barthel, Fugl-Meyer, Ashworth). It is shown that the application of mechanical stimulation of support zones of soles leads to normalization of muscle tone in the paretic leg and prevents the development of severe spasticity in the extensors of the foot, and also contributes to an earlier stance and develop skills in independent movement.

EFFECTIVENESS OF THE REHABILITATION PROGRAM OF PATIENTS WITH MOVEMENT DISORDERS IN CEREBRAL CATASTROPHE

Abstract

Objective: to assess the effectiveness of rehabilitation programs for patients with movement disorders in cerebral accident. Material and methods. We examined 997 patients 23-84 years old who have undergone extended rehabilitation at the clinic Medical University TMGI Health Ministry. There were conducted a comprehensive survey of patients. Results. Rehabilitation process include diagnostic and rehabilitation units. In assessing the effectiveness of rehabilitation course obtained improvement of the cardiovascular and respiratory systems, blood , blood pressure , attention, sleep , emotions , as well as activity and participation in the parameters of interpersonal interactions and relationships ( p <0.05). Improved ability to use upper extremities, personal hygiene (p < 0.05). Stabilometry and complex Primus RS allowed to objectify increased resistance and an increase in muscle strength of the extremities. Conclusion. Using the complex rehabilitation of patients improves performance of the cardiovascular and respiratory systems, the emotional-volitional and cognitive function , as well as activity and participation in interpersonal interaction, the use of hands , the implementation of personal hygiene.

Spasticity follows up such pathologies, as multiple sclerosis (MS), spinal cord injury, stroke, cerebral palsy. Despite the whole arsenal of medical therapy, there are a lot of patients who unresponsive to therapy. Transcranial magnetic stimulation (TMS) is a new method of noninvasive brain stimulationin the treatment of spasticity This review describes the basic mechanisms underlying spasticity, methods of its measurement, the main approaches to therapy and also existing trials about the effectiveness of treatment spasticity using TMS.

Abstract

In the article with the pathogenetic positions analyzes the efficiency of application of photophoresis пантовегина combined with пелоидотерапией. It is shown that корригирующий effect of the combined effects of factors is a variant of additive interaction, implemented through common points of regenerative activity of mud applications, and metabolic potentials пантовегина, reinforced by the influence of the infrared laser. Application photophoresis пантовегина in the complex of peat with «TOMED-APPLICATE» accompanied by growth regulator-the metabolic capacity of the organism, providing persistent clinical effect. Used it is a very effective method of regenerative correction of patients suffering from chronic inflammation of the gum tissues, creating favourable conditions for the relief of inflammation and regeneration of gum tissue.
