Organizational and structural model of medical rehabilitation to children of the early age after surgical treatment congenital heart diseases

Abstract

Lack of organizational model of rehabilitative patients care with congenital heart disease determines the relevance of the scientific justification for the creation of a rehabilitation centers network for patients after surgical treatment of congenital heart diseases in the Russian Federation. Following methods were used: questionnaires, monographic description, analytical, statistical data processing. A questionnaire survey of parents 8940 patients discharged from hospital of Scientific center of cardiovascular surgery named after A.N. Bakuleva Russian Academy of medical sciences in terms from October 2004 to January 2012, legislative acts relating to the field of study, the sources of domestic and foreign literature, technical documentation, design solutions for construction of children’s rehabilitation center. Most respondents noted the lack of availability of cardiac care in the life area. At the optimal structure of the organizational model of rehabilitation services to children with congenital heart disease after inpatient treatment for congenital heart disease, or the patient is discharged at the place of residence or, if there is evidence to the follow-up care, is sent to the second stage of rehabilitation in children’s rehabilitation center. After the end of the second phase of the rehabilitation of the patient is discharged home under medical supervision in the life area. In the future, when indicated, as well as for preventive child is sent to 3rd stage of rehabilitation in spa cardiology center..

Evaluating the effectiveness of physical rehabilitation of infants with perinatal lesion of the central nervous system

Abstract

The fundamental parameters that determine the effectiveness of physical rehabilitation are: - the results of applying the method of «decision tree»: a motor factor, the general movement, gross motor, vestibular stability, tension index, the total power spectrum of neuro-humoral regulation of heart rate, amplitude of the mode, the indicator vagosympathetic balance, the heart rate in a state of relative calm, and its growth in response on the orthostatic test. - According to the discriminant analysis: variables reflecting the response of the organism to carry out functional tests (% HF-,% LF-waves) which allow to evaluate the adaptive capabilities of the child. Using cluster analysis identified the integral index - the index of the effectiveness of rehabilitation, the magnification that characterizes the positive effect of reducing technologies.

Role of screening studies using computer and video technologies in diagnostics of abnormalities in children development and in rehabilitation organization

Abstract

Purpose of the study is to identifyleadingabnormalities in the developmentof young childrenand to develop a system of diagnosticscreening of their early development. Statistical data on the neuro-psychological development of 545, 2-16 months old children from Moscow, with the following parameters: «cognitive», «motor», «speech», «social», «self-help». Data obtained for the period of 2009-2013 from 5 pediatric city polyclinics, based on the results ofa parents surveyusingthe computer version of the KID Scale (Russian version of the Kent Infant Development Scale). Overall development of 48.6 % of the children corresponds with their age; 21.9 % of children have stunted growth; 29.5% are ahead of their age-specific development. The largest group of children with developmental disabilities belongs to «motor development « ( 23.3%) , and « self-help» ( 24.0 %).According to the «cognitive development» 20.5 % of the children have a delay, 36.0 % are ahead of their peers; for the “speech development” - 20.7 % of the children are behind their peers, 40 % are ahead; for the ‘social development ‘ - 19.6% are behind, 37.4% are ahead of standard values. All parameters of development are affected by the duration of pregnancy. The second place in terms of impact belongs to the indicators of the general physical health of children, affecting cognitive, motor and social development of children, on the third place is the child’s parenting, affecting cognitive and speech development. Treatment of children with disorders of the musculoskeletal system in terms of rehabilitation center

Abstract

The article is devoted to the organization of specialized orthopedic care for children. The authors analyzed two decades of experience of the St. Petersburg Children’s Rehabilitation Center of Orthopedics and Traumatology. The best achievements in the early diagnosis and treatment of musculoskeletal lesions were considered at organization of the Center in 1994. The purpose of organizing this type of institutions is to provide a comprehensive diagnosis and non-surgical intensive inpatient treatment with simultaneous full pedagogical support. Therefore the standards of rehabilitative hospital were applied to the medical diagnostic unit recruiting, while the standards of children’s sanatorium - to the educational service recruiting. The generalization of the experience convincingly demonstrates a high socio-economic efficiency of the institution, especially in patients with diseases and injuries of the spine. Acquired experience allows us to recommend an expanding of such institutions network and not only orthopedic ones.

Application of robotic mechanotherapy in rehabilitation of children with consequences limb injuries

Abstract

The article examines the scientific basis and efficacy of robotic mechanical therapy in children with postimmobilizatsionnoy contracture due to injury of the upper and lower extremities. Clinical and functional studies were performed in 60 children aged 6 to 18 years. According to the research found that the use of robotic mechanical therapy increases the effectiveness of medical rehabilitation of children with trauma of the upper and lower extremities as compared with the control group. The work received the positive impact of robotic mechanical therapy on the functional state of the musculoskeletal system. According to the laser Doppler flowmetry found a beneficial effect on the robotic Mechano microcirculatory bed, helps to clarify the mechanism of therapeutic action robotic Mechano. Dynamic evaluation of the ASP indicates the improvement of health of children against the background of course robotic Mechano. The obtained results allow us to collectively justify the possibility and necessity of including robotic mechanical therapy in medical rehabilitation of children with limb trauma and its consequences on the outpatient phase. Abstract

Evelopment of preventive direction is one of the most significant and socially important issues in pediatrics. In recent years negative tendencies in children’s health characterised by increases in chronic diseases, neuropsychiatric disorders, ecologically determined status. Emphasis should be placed on children tending to have acute respiratory infections frequently due to high risk of chronic pathology development, various forms of allergy and autoimmune processes, dramatical violations of the functional state of the organism, adverse effect on children. Currently, different natural and artificial physical factors are used to restore and preserve children’s health. Among natural physical factors climatotherapy, the main methods of which are aerotherapy, heliotherapy and thalassotherapy. Speleotherapy is one of the forms of children recovery by a long stay in microclimate of caves,salt mines , grottoes etc. Various kinds of hydrotherapy are used widely in children’s recovery: baths, showers, dousing, rubdowns, compresses. Balneotherapy is one of the principal methods and bases on the effects of mineral and gas water for external and internal use. In recent years among drug-free technologies method of halotherapy, which is carried out using halochambers, halorooms, individual haloinhalators. Moreover, aromaphytotherapy, biocontrolled aeroionotherapy, normobaric hipoxytherapy and different forms of phototherapy are included actively in programmes of medical rehabilitation. Leading method of nonspecific resistance improvement is hardening.The new technology of hardening the child’s body by feet cryomassage was developed and scientifically validated. It causes release of biologically active substances by stimulating feet reflex zones, causing in the subsequent wide range of physiological changes. Wide range of drug-free health tecnologies allows to elaborate individual prophylaxis programmes and implement them in all stages of recovery ( therapeutic and prophylactic, educational institutions; sanatoriums and health resorts, different types of recreational camps, domestic conditions..

Habilitation of children with perinatal risk in health centers

Abstract

Actions aimed at the development deviations prevention of children with severe perinatal medical history or explicit perinatal pathology are proved to be complex by nature and should be simultaneously aimed at the primary development categories of a child. Advanced approach which prevents development deviations appearance serves for timely and correct formation of experiences and skills of babies. Participation of family in medical correction program enables portioned and prolonged impact on the baby’s organism. Collaboration of family and Healthcare Center, parents’ concern in treatment results contribute to the abilitation effectiveness. Courses for parents ‘Healthy Child’ were set up within the framework of Children Healthcare Center in order to tackle their problems. Prophylaxis of neuro-psychic development deviations of perinatal risk group children is the concept of the Courses’ sphere of actions. Teaching work with pregnant women falls into perinatal pathology prophylaxis, over diagnosis and correction of CNS perinatal damage at home immediately after birth. As soon as the child is one month old the work with the young family in Healthcare Center is continued. Young mothers regularly attend the Courses ‘Healthy Child’ with babies depending on individually built abilitation program. It is not only a medical correction program but also an educational one since further abilitation work is carried out by parents at home. Abilitation program of children with perinatal pathology or risk factors of its formation is at the same time prophylactic as it prevents the appearance of formed deviations in the development categories of children. Representatives of various specialists participate in the program for the sake of comprehensive help to perinatal risk group children: neonatologist, neurologist, masseur and therapeutic physical training specialist. The program also involves non-medical specialists (perinatal psychologist, speech therapist, educational psychologist) who play a significant role in the development of a child. The following non-medical methods are used in the abilitation program: dry immersion, massage, fine finger motor skill training, kinesitherapy, conductive pedagogics, therapeutic physical training, senses development method, articulation gymnastics and finger gymnastics.

Cardiac arrhythmias in children: indications for interventional treatment and main principal of postoperative rehabilitation and follow-up

Abstract

Clinically significant cardiac arrhythmias are among the most common cardiovascular pathologies in children. In all age groups, tachyarrhythmias are the most prevalent heart rhythm disorders often leading to severe health problems and development of heart failure in children. So far, treatment of tachyarrhythmias is based on therapeutic and interventional (catheter radiofrequency ablation) modalities. Criteria for positive decision regarding interventional treatments in children with arrhythmias include high efficacy, low recurrences rate, and low complications rate. Bradyarrhythmias in children comprise two main nosological entities: sick sinus syndrome and atrioventricular blocks. Main method of treatment of bradyarrhythmias is implantation of an electric cardiac pacemaker. Indications for implantation depend on bradycardia severity, clinical manifestations, and prognosis of underlying disease. Treatment of primary electric heart diseases, associated with the high risk of sudden cardiac death, is also based on implantation of antiarrhythmic cardioverter-defibrillators. Combination therapy implies the use of implantable cardioverter defibrillators and antiarrhythmics, sometimes accompanied by the left side sympathectomy, which dramatically reduces the risk of sudden cardiac death compared with patients who do not receive adequate therapy. After invasive interventions, patients with cardiac arrhythmias require follow up by a cardiologist aimed at timely diagnosis of complications caused by therapy and their correction. Taking into account the association of arrhythmia with high risk of sudden cardiac death, many patients need help from psychologist/psychotherapist and correction of daily regime.

Vibro-compressive effect from airway clearance device usage in children with bronchitis

Abstract

The article provides the scientific rationale for the use of high-frequency chest wall oscillation (HFCWO) in the treatment of bronchitis in children. Clinical observations and special studies conducted in 50 children aged 6 to 15 years. Proved the possibility of the use of HFCWO in children with bronchitis, defined its therapeutic efficacy. Established tolerability of procedures and absence of adverse reactions. Found a beneficial effect on clinical symptoms, mucociliary clearance, which was characterized by increased sputum discharge, rapid regression and disappearance of cough, wheezing, normalization auscultation picture at an earlier date than in the control group. Combined therapy with the inclusion of high frequency chest wall oscillation improves lung function, as evidenced by the curve «flow-volume» parameters, especially when initially discounted rates. Program of management for children with duchenne muscular distrophy as an effective way to increase quality of life and rehabilitational potential

Abstract

In this article the outpatient and inpatient algorithm of observation of patients with Duchenne muscular dystrophy (DMD) has been discussed, according to latest publication in 2010-2013. The emphasis has been put on determination of severity of muscular involvement. Hammersmith Muscle Manual Testing and Adopted Hammersmith scale for Duchenne muscular dystrophy has been used for that. In addition, indications for continuous noninvasive ventilatory and videofluoroscopia of swallowing phases to exclude aspiration and define indications for gastrostoma has been described. This actual information would be helpful for specialists to evaluate rehabilitation potential for such kind of patients and program for prophylaxis of DMD complications.

Influence of balneophysiotherapy factors on the quality of life of children with functional dyspepsia

In Udmurt Republic was derived chloride bromine-iodine brine, which high efficiency has been proven in the treatment of digestive diseases in adults with by sinusoidal modulated currents.This medical factor has stable chemical mineral content, a large daily output, and suggests the possibility of its application in the treatment of gastrointestinal diseases in children. It was conducted the evaluation of quality of life in 92 school age children with functional dyspepsia and their parents were treated in the Republican Children’s Clinical Hospital in Izhevsk on treatment with balneology and physiotherapy factors. The study was performed using a questionnaire PedsQL (version 4.0). Complex of medical actions included diet, standard medical therapy. As the main factor used SMT-phoresis chloride bromine-iodine brine. In the comparison group used SMT-therapy. After the treatment aspects of quality of life based on self-assessment above all questionnaire PedsQL categories rate were upward then before treatment. This regularity significantly was expressed in the group of patients treated with CMT-phoresis brine. aspects of quality of life «physical functioning» and «emotional functioning» reacted At the spa therapy. During research were found that children living in rural areas have lower quality of life (scale of emotional and social functioning) than their peers residing in the city. During therapeutic measures rural children are catching up, and by some measures (physical functioning) and redistilled from their peers. It was establish a high informative importance of quality assessment life of children as a reliable non-invasive test the effectiveness of spa therapy.

Reflex therapy in pediatric practice

Abstract

Health of the children’s population is one of the most sensitive indicators of a condition of all society. The urgent need for more widespread introduction in pediatric practice of safe, effective and economically available methods of treatment is dictated by high level of perinatal pathology, the general incidence of children and the teenagers, the proceeding growth on all groups of diseases. Medical rehabilitation by methods of reflexotherapy is an important component at all stages of a medical support, includes the entire periods of development of children and teenagers. The modern reflexotherapy is the universal method directed on regulation of reactions of a stress and adaptation in all variety of their physiological and clinical manifestations. Activity of an office of reflexotherapy as a part of office of rehabilitation of child care treatment-and-prophylactic facility raises level of medical care to children and teenagers owing to reduction of volume of applied basic and symptomatic pharmacotherapy, decrease in risk of emergence of its undesirable collateral action, reduction of terms of recovery of health.

The article examines the opportunities and the need for medical rehabilitation of children with obsruktivnoy uropathy considering the pathogenesis of the disease. Differentiated application of physical factors at all stages of medical rehabilitation, their use in complex therapy of children with obstructive uropathy can reduce adverse drug treatment times, improve renal function, restore urodynamics and prevent disease progression.
