THE ROLE OF DIGITAL MEDICINE IN THE DEVELOPMENT OF PERSONALIZED PREVENTIVE REHABILITATION

Abstract

Average life expectancy has reached in Russia in 2017 the value of 72,4 years which overcome the historical maximum of all Russian healthcare system period statistics. The new official protocol of mass screening of definite age groups of Russian population has been approved by Russian ministry of Health on 26.10.2017 (order 869n). The goal of the document is prevention of non inflectional age-related diseases in cardiology, neurology, oncology. In 2017 the leaders of Russia, ministry of health, public and innovation platforms (Agency of strategic initiatives - ASI, Center for strategic policy-CSR) has payed much attention to the digital aspects of healthcare industry development (program HealthNet). At the same time the decrease of Russian population, far beyond position for the average life expectancy among world countries (116 position in 2015, UN data) may assume that not enough efforts has been made for economic investments and the development of new methodology of personalized screening of age-related diseases. The new experimental innovation algorithm for personalized control of age-related Biomarkers and corresponding interventions of correction is suggested which can play a significant role in the future development of personalized preventive rehabilitation. It may be used as a tool for the monitoring of patients quality of health and lifestyle environment.

ROLE OF MITOCHONDRIA IN PREVENTIVE, RESTORATIVE AND SPORTS MEDICINE

Abstract

Cells of adult human contain up to 1016 mitochondria, which play a fundamental role in the generation of 90% of cellular energy (ATP) through the transformation of energy substrates in the presence of oxygen. Simultaneously, the mitochondria produce various destructive oxidative radicals and actively involved in the metabolism of calcium, in the implementation of the cells' tolerance to different stress agents, in the proliferation and apoptosis of cells, initiate inflammation, in the synthesis, intra- and intercellular transfer of signal information and in many other functions. Mitochondrial proteins are encoded by genes localized both in nuclear and in mitochondrial DNA (mtDNA). The variety of effects produced by mitochondria have served as the basis to consider "mitobiota" and “microbiota” as a united whole functional structure that regulates homeostasis of the host organism via bioenergy, epigenetic, metabolic, endocrine, immune and neuro-humoral communication. Violations of the number, ultrastructural changes, genetic integrity, of mitochondria and disorder of their homeostasis are associated with many metabolic diseases and accelerated aging. To prevent and treatment mitochondria-associated disturbances and diseases many functional foods, diets and molecular techniques have been suggested.

WHY WE AGE AND HOW CAN WE INFLUENCE THIS PROCESS

Abstract

Identification the considerable plasticity of aging rate and ability of stem cells not to age is a significant scientific achievement in recent years. The systems approach is useful for understanding the causes and mechanisms of aging, because the organism represents a complex system consisting of a hierarchy of cooperating subsystems. Since the functioning of subsystems is subordinated to general purposes and is coordinated and managed by the control systems of the organism a body functions as a single unit. The main idea proposed here is very simple. Even a potentially ageless body must start to age under inadequate condition (like a non-melting piece of ice taken out from the deepfreeze inevitably starts to melt at the temperatures above zero Celsius). This conclusion is consistent with patterns of mortality and with agelessness potential of somatic stem cells. Therefore, there is no need to build up and explore too complicated “systems models of intrinsic aging” to understand the origin of this mainly extrinsic root cause of natural aging. In our case a simple phenomenological black-box approach with Input-Output analysis is ample. Here Input refers to the environmentally dependent initial force of mortality, whereas Output is a rate of age-related increase of mortality force. If one takes into account the fact that robustness of really “sustainable” systems and modes is possible only in a certain limited range of ambient conditions, then the control theory and systems approach are sufficient both to discover the root cause of aging and to understand the underlying mechanisms of its implementation. After all, it is when leaving the area of adequate functioning modes determined by the environment that even potentially ageless immortalized cells and hydras start aging with the exponential growth of mortality, the mode typical for humans.

ENERGY EFFICIENCY AND AGE. CHRONIC TISSUE HYPOXIA AS THE CAUSE OF DEVELOPMENT OF OXIDATIVE STRESS

Abstract

It is well known that mitochondrial dysfunction is directly related to many age-depending noninfectious chronic diseases accompanied by the critical reductions in the aerobic synthesis of ATP. The ability of mitochondria to comply with the environmental conditions and proper mitochondrial functioning occur only in the presence of adequate partial pressure of oxygen in the inner mitochondrial membranes, which results in the most effective energy production while meeting the cellular needs in energy. The reduced protons and free electrons accepted in the oxygen molecules are end products of aerobic metabolism in healthy mitochondria, leading to production of ATP and formation of such physiological metabolites as water and carbon dioxide. The increasing subclinical cellular hypoxia caused by interdependent complex factors - dietary intake of energy metabolites and tissue oxygenation - contributes to oxidative stress, which is commonly accepted as one of the main causes of age-dependent noninfectious chronic diseases.

HOW TO IDENTIFY REVERSIBLE DEMENTIA: SURGICAL TREATMENT, EVALUATION OF RECOVERY OF LOST FUNCTIONS

Abstract

Treatment (surgical and conservative) is very effective in idiopathic normotensive hydrocephalus (iNPH), characterized by the expansion of the ventricles of the brain with normal or slightly increased intracranial pressure. Most often, iNPH observed in the elderly over 65-70 years with clinical manifestations of the triad of Hakim-Adams (memory, gait and urination). For the diagnosis of INH and the resolution of the question of adequate therapy, it is essential to study the pathophysiological mechanisms of this clinical form. However, the question of the mechanisms of the appearance of the iNPH remains to this day unclear. Among the various etiological factors that lead to the development of iNPH, the leading role is played by violations of the venous circulation of the brain. Given that the cerebral volume of blood is 80% of venous blood, three quarters of which is in small veins and venules, the evaluation of the venous circulation of the brain is of the greatest importance, in which gravitational influences play a leading role. The purpose of this study was to study cerebrovenous orthostatic reactivity (CVOR) in idiopathic normotensive hydrocephalus.

Abstract

The study was undertaken to assess the opportunities of use of the indicators of mental efficiency, sensorimotor coordination and psycho-emotional state as markers of bioage. Analysis of age-related trends is made on the basis of experimental data obtained by computer system “SOPR-monitoring” in three experiments - a longitudinal one, a crosssectional one and a mixed one. Age-related decline in the counting productivity, reaction to the moving object, somatic state, tone and mood was detected at both individual and group level. It is shown that indicators of performance depend heavily on individual abilities to certain activities and on professional experience. Taking into account individual trends of those indicators can significantly improve the accuracy of bioage estimation, however, it requires a sufficiently long monitoring.

METHODOLOGY OF ESTIMATION OF SUBJECTIVE QUALITY OF LIFE OF PATIENTS WITH OPHTHALMOLOGICAL DISEASES

Abstract

The purpose of the study is to approbate the developed methodology for assessing the subjective quality of life of people with severe ophthalmic diseases. Materials and methods of research. The collection of empirical data was carried out using e-mail, the Skype program and the social network Vkontakte. As the diagnostic tool used the author's methodology "Assessment of the subjective quality of life of persons with profound visual impairment". The questionnaire consists of a block with personal data and 11 questions / statements reflecting the subjective quality of life. Ball score was made on an ordinal scale (from -2 to 2 points). Results. In the course of the study, the features of the subjective evaluation of the quality of life and the nature of the relationship to its individual aspects in two groups of respondents are revealed: in the totally blind and in patients with ophthalmologic diseases that have residual vision. It has been established that the totally blind assess their quality of life higher than those with residual vision: the total quality of life measures were 6.5 points in the group of totally blind and 1.6 points in the group of visually impaired people with residual vision respectively (p<0.01). Conclusion. Statistically significant differences in the quality of life indicators of patients with different severity of visual disturbances are revealed, which testify to the criterial validity of the developed technique.

OZONOTHERAPY AS A FACTOR HAVING AN IMPACT ON THE RESULTS OF CELL THERAPY OF EXPERIMENTAL LIVER CIRRHOSIS

Abstract

The purpose of our study was to evaluate the impact of ozonotherapy on the results of cell therapy of experimental liver cirrhosis. White Wistar male rats (n=45) were used as an object of our study. After modeling of liver cirrhosis a course of ozonotherapy for one group of rats (group 3) was performed. Than animals of groups 2 and 3 were treated with single intra-portal injection of autologous mesenchymal stem cells (MSCs). Animals had been sacrificed before cell therapy, 30 days and 90 days after cell transplantation, pathohistological state of liver, prooxidant and antioxidant markers of blood serum had been examined with further statistical analysis. Administration of autologous MSCs in experimental liver cirrhosis leads to statistically significant consistent reduction of thickness of connective-tissue septa in liver (р=0,0041), decreasing of prooxidant markers concentration and increasing of antioxidant markers activity (p=0,009) 30 days and 90 days after cell transplantation in comparison with control animals. Course of ozonotherapy before cell therapy leads to statistically significant improvement of mentioned indicators in comparison to those in case of monotherapy with stem cells (р<0,05). Transplantation of autologous MSCs leads to consistent improvement of histological state in liver cirrhosis 30 days and 90 days after cell therapy. Ozonotherapy facilitates the improvement of the results of cell therapy of experimental liver cirrhosis. The mechanism of the positive impact of ozonotherapy is probably in its antioxidant effect, which occur in statistically significant decreasing of prooxidant markers concentration and increasing of antioxidant markers activity.

CHARACTERISTIC OF POSTURAL BALANCE AFTER SURGICAL TREATMENT IN PATIENTS WITH DORSOPATHIES OF THE LUMBAR SPACE DIVISION WITH DISORDER OF PATIENT COMPLETION

Abstract

Study population was divided into one group of patients (30 people) of patients underwent rehabilitation activities started on the 2nd day of an early post surgery period (duration of the last one was 7±2 days after the surgery). Outpatient rehabilitation measures were not carried. In the long-term period, in the absence of continuation of rehabilitation measures, in patients who underwent rehabilitative measures in the early postoperative period, no statistically significant changes in the amplitude of the center of pressure fluctuations were observed for all tested parameters. There is a tendency to increase the amplitude of oscillations in the sagittal plane, the total volume of fluctuations of the pressure center, the average velocity of displacement of the pressure center and the area of oscillations in testing with open eyes and amplitude of oscillations in the frontal and sagittal planes, the total volume of oscillations of the center of pressure and the area of oscillations in testing with eyes closed in a remote period. The level of pain syndrome according to the VAS scale significantly decreased in the early postoperative period in the back and in the lower extremities, in the long-term period there is a further decrease in the level of pain in the lower extremities, with a certain increase in pain in the back. As a result of the surgical treatment, there was a more than two-fold decrease in the average value of the ODI from baseline to 22.9 [20,0; 28,0] points in the studied population of patients This suggests that a comprehensive rehabilitation treatment is advisable to continue at the outpatient stage, to improve the postural balance in patients that underwent decompressive-stabilizing surgery of the lumbar spine.

DYNAMICS OF FUNCTIONAL STATE OF CHILDREN'S JUNIOR SPORTS SCHOOL AT THE COMPETITION ACTIVITY PERIOD

Abstract

The article presents the results of the study of the dynamics of the functional state of the pupils of the Youth Sports School during the period of competitive activity. After a full survey of athletes with the definition of physical development and comparison of the obtained indicators with the average indicators, the period of intensive training and competitive activity followed. The study involved 300 young athletes - students of the Youth Sports School, 150 of which - boys 11 years old, and 150 -12 years. The received results testify that all sportsmen at the time of the beginning of the research have good physical data and a high adaptation to physical loads. Boys of both age groups recover quickly after training, shifts of hemodynamics and biochemical blood parameters at the same time correspond to the loads and quickly return to the initial ones without harmful influence on the state of organs and systems. In the study of the nervous system, it was found that in the development of static and motor functions, conditioned reflex activity and language, children of both age groups did not lag behind their peers (from a pediatric history). The results of the survey after the competitive period indicate a significant decrease in endurance and resistance to hypoxia of young athletes of both groups. Despite the fact that the indicators of physical development of athletes have remained the same, the endurance rates have significantly decreased, ке. there was a discreption of these indicators. After the competitive period, the hemodynamic parameters of all subjects significantly worsened, and the increase in the concentration of lactic acid in the blood suggests that the organisms of athletes do not have time to recover after intensive physical exertion, and this does not lead to training and adaptation, but, on the contrary, worsens the general condition of the young athletes and reduces its ability to achieve professional success. Thus, comparing the data at the time of the beginning of the study and after six months of intensive training and the competition period, the necessity of developing a complex of restorative measures with the purpose of blocking the development of the overtraining syndrome among young athletes was confirmed.

INTEGRATED INTERDISCIPLINARY METHODOLOGY OF TREATMENT OF VERTEBROGENIC DORSOPATHY

Abstract

The problem of treatment of discogenic category of patients is of great medical and social importance and has acquired a pronounced interdisciplinary character. Ranked first in the world to reduce the quality of life among all medical reasons. Leads to the loss of 4 million working days a year. Included in the 5 leading causes of hospitalization. For the period from November 2014 to December 2017 in the clinics ’’Medical center for Tar” (Moscow) and ”St. Andrew's hospital. NEBOLIT” (MO, Krasnogorsk) were treated 180 patients with discogenic dorsopathy. All patients underwent a comprehensive examination before and after treatment: a study of neuro-orthopedic status (somatoscope, anthropometry, manual muscle testing and testing of mobility systems), testing by visual analogue pain scale (VAS), ultrasound of the fixation structures PDS, neuroimaging techniques (MRI, CT). The methodology of neurorehabilitation consisted of local application of gel and liquid carmolis, the method proprioceptive neuro-muscular re-education and natural metabolic “Greens With Flax Seed”. The proposed methodology showed high efficiency for characteristics such as dynamics of pain syndrome, the evolution of the stages of change stato-dynamic stereotype and the type of the disease.
