Epidemiology and prophylactics of HIV-infection in the Armed Forces of the Russian Federation
Abstract
The HIV-infection epidemic spreading in Russia also affects those in military service, compromising the state's defence power. A system of HIV-infection prevention has been developed by the medical service of the Armed Forces, and is functioning successfully. Military groups are a unique model for conducting scientific research, as well as for carrying out prophylactic programs. A closer interaction of civil and military scientists is one of the ways to improve the effectiveness of HIV-infection epidemic prevention.

Molecular determinants of lung cancer susceptibility
Abstract
The data on the influence on lung cancer (LC) predisposition of so-called «unfavourable» alleles of polymorphic genes, which products take part in tobacco smoke carcinogens metabolism are provided in this article. We showed that the mutant allele with T6235C substitution in CYP1A1 gene, which codes arylhydrocar-bonhydroxylase (АНН), is associated with lung cancer susceptibility, especially with squamose cancer (SCLC). Also the obtained date on the prevalence of GSTM1 gene deletions in patients with LC allows the above-mentioned polymorphism on the LC risk. The evidence of the increased LC risk in the carriers of the mutant alleles in both genes are provided.



State and perspectives of vaccine development against potential pandemic flu viruses
Abstract
Data were summarized on the vaccines development against avian influenza viruses with high priority for pandemic threat. The overview of global surveillance of avian influenza outbreaks caused severe human diseases is provided. Several approaches to develop modern inactivated and live influenza vaccines are discussed. Comparative analysis of different influenza vaccines for future pandemic preparedness was done.

Adenomyosis and leiomyoma of the uterus: the clinical and morphological aspects of combined pathology
Abstract
According to the published data, the frequency of the combination of adenomyosis (AM) and leiomyoma (LM) of the uterus is 50-85% of cases of AM. Recognition of such a combination makes a complex problem to the clinician and linked with large diversity of features of these diseases with the absence of typical symptoms, and various forms of changes.
We performed a detailed comparative clinical and morphological analysis of 50 cases of AM and 150 cases of AM/LM combination. This analysis revealed the considerable hypodiagnostics of AM, especially by its combination with LM and/or by the presence of perifocal leiomyomatous hyperplasia. The regularity of perifocal leiomyomatous hyperplasia with microscopic forms of LM and/or the diffusive thickening of myometrium has lead to the increase of uterus size in AM. The immunohistochemical investigation revealed the correlation of expression of the estrogen and progesterone receptors in glandular epithelium and stromal cells of the AM foci, and in the leiomyocytes in perifocal hyperplasia areas of myometrium. This indicated the uniformity of the pathogenetic mechanisms of AM and LM. Differences in the symptomatology and clinical course of AM and of AM/LM combination were also established in our work. The diagnosis of AM demands the comprehensive investigation of patients taking into consideration the frequency of the combined forms and distinct features of their clinical and morphological parameters.



The role of lipoprotein lipase activity, hyperinsulinemia and free fatty acid concentration in genesis of dislipidemias
Abstract
We studied fasting insulin concentrations, free fatty acids (FFA) and glucose level in plasma and postheparin lipoprotein lipase activity in patients with combined dislipidemia (DLP) and hypertriglyceridemia (HTG). The subjects with combined DLP or HTG had increased plasma insulin level (11,62±1,36 mU/ml, p<0,001; 12,21 ±2,13 mU/ml, p<0,001 v.s. 5,01 ±0,51 mU/ml), FFA level (0,87+0,07 mmol/l, p<0,001; 0,65+0,07 mmol/l, p<0,05 v.s. 0,49±0,04 mmol/l) and high body weight compared with the control persons. The HTG patients characterised by decreased lipoprotein lipase activity in postheparin plasma (5,57±0,98 mmol/l/h v.s. 9,03±0,71 mmol/l/h; p<0,01) too. We observed the significant relationships between fasting concentrations of TG, FFA, insulin in plasma and body mass index in all patients.
Obviously the some part of combined DLP and HTG phenotypes result from metabolic syndrome and have the like biochemical mechanisms that differ no more.



Mathematical model of cell-cell interaction in erythroblastic islands culture
Abstract
The mathematical model of development of an erythropoiesis in erythroblastic islands culture has been built for the first time. At realization of experiments on cultivation of erythroblastic islands the sequential analysis of Wald has been applied. At constant values of parameters of cultivation regression model allows to reveal frame of connection of a erythropoitin dose, duration of cultivation and an amount of erythroblastic islands in culture, and also to predict productivity of erythroblastic islands culture depending on preset values (a dose of erythropoietin and duration of cultivation).

Abstract
In the sharp period of posttraumatic disease (period of shock) systemic hypoperfusion caused to ishemia and damage of target-organs (organs with high level of methabolism): intestinum, marrow, hepar, kidneys.
In the early period of posttraumatic disease reperfusion damages of this organs is manifest syndrome of systemic inflammatory response (SIRS). Damages of mucous tissue on intestinum lead to translocation endogeneous bacteria in blood.
The feature of late period of posttraumatic disease is the presence of dead tissues, bacteremia and depression of immune defense. All this create the conditions on transformation aseptic SIRS to sepsis



Modern methods of diagnostics of myocard function
Abstract
The present article is devoted to the modern methods of the identification of the viable myocardium. The pathogenesis of the hibernating myocardium and the stunned myocardium is described. The article reviews possibilities and limitations of stress echocardiography, single-photon emission tomography, positron emission tomography.



Clinico-immunological criteria of estimating specific immune therapy complications in seasonal allergic rhinitis patients
Abstract
By the purpose of work was the retrospective comparative research of a condition immune system at two groups of the patients (having complication of treatment and without those) with seasonal allergic rhinit prior to the beginning treatment and at early stages of realization allergovaccination. 125 patients with sensibilisation to pollen of weed grass were surveyed. The patients complications immunotherapy more often have not allergic chronic diseases. Before initiation allergovaccination the level IgM is higher and less quantity common IgE. Allergovaccination is accompanied by earlier change of mechanisms regulation of levels common IgE and deferred reactions cells and phagocytic of parts immune system. On the basis of given anamnes, change of a ratio CD4/CD8, quantity CD19, phagocytosys of a parameter and levels IgE, IgM per the first month of realization allergovaccination forecasting risk of occurrence of system complications is possible.



Results of the prospective examination of diabetes mellitus patients with trophic ulcers of the feet
Abstract
This paper presents the results of long-term (5,5±1,4 yers) follow-up of 356 diabetic patients with feet ulcers. Total mortality, cardiovascular mortality, mortality due to amputation, amputation rates and development of new ulcer were separately evaluated for patients with neuropathic (group 1) and neuroischemic ulcers (group 2). Incidence of all studied endpoints except new ulcer development was significantly higher in group 2: relative risk for total mortality 1,95, for cardiovascular mortality-2,12, for mortality due to amputation -4,1, for amputation rates - 2,01. The probability of new ulcer was slightly higher in neuropathic patients: relative risk 0,96 (p=0,03). Age, ischemic heart disease and severity of diabetic nephropathy were independent factors for mortality in both groups. Significance of duration of diabetes mellitus for mortality and amputations was noticed only for group 1. We concluded that low extremity ischemia worsens the prognosis for life and for foot. Duration of diabetes and severity of renal impairment due to nephropathy are important negative prognostic factors in neuropathic patients comparing to neuroischemic ones.



The melatonin expression in placentas with the chronic insufficiency
Abstract
It was investigated expression of melatonin in placentas in comparison with them morphological condition in mothers, who born newborns with intrauterine growth retardation. Our data showed that high melatonin expression was identified only in placentas with the subcompensated placental insufficiency. Melatonin expression increased in cases when the adaptation mechanism exhausted.



Significance of the immunohystochemical research in the diagnostics of chronic viral hepatitis B and C
Abstract
Diagnostics of chronic viral hepatitis В and С is based on the detection of viral proteins and nuclein acids in the blood and in the liver tissue by serological, polimerase chain reaction, immunohistochemical methods. The aim was comparative analysis of foregoing methods of viral markers detection in chronic hepatitis В, С and mixed B+C hepatitis diagnostics. 58 chronic viral hepatitis patients have been observed by clinicolaboratory, instrumental, morphological methods. The whole spectr of serological markers of HBV, HCV, PCR HBV and HCV in the blood and in the liver tissue were performed. HbcorAg HBV and NS3 HCV were examined by immunohistochemical method (Kit «Novocastra»).
By serological and PCR methods HBV-infection was diagnosted in 39,7% of patients. HCV-infection- in 22,4%, mixed B+C-infection- in 27,6% and etiology was not determined in 10,3% of patients. By immunohystochemical method HBV-infection was defined in 15,5% of patients, HCV-infection- in 10,3% and mixed B+C-infection - in the majority of patients - in 74,1%.
Immunohystochemical method of detection HbcorAg HBV and NS3 HCV is more sensible method of viral markers testing in comparison to PCR and serological method, allowing to diagnose mixed viral B+C infection in 74,1% of chronic hepatitis patients, that in 2,6 times exceeded the frequence of this form of hepatitis by PCR and serological method.

Reducing psycho-emotional stress in emergency unit employees by functional biocontrol with feedback connection
Abstract
The researches were carried out with medical staff (volunteers) of a first aid station in St. Petersburg. A mobilizing method of cardiorhythm functional control with visual biofeedback was used for normalizing of human state and decreasing of stress-induced influences. The examinations were carried out at the first hours of 24 hours' watch. The checkup session for the comparative analysis of the volunteers’ state fulfilled after watching. The every participating in cardiotraining procedures marked some improvement of own general state, mood and increment of capacity for work. The arterial pressure and the heart rate normalized with the biofeedback control procedures. The anxiety reduced, the perception of individual time has come nearer to metric estimation. The mobilization version of the oscillatory cardio-dependent biofeedback technology can be applied for adaptive correction and improvement of operator’s condition.
