Analysis of the use of psychoactive drugs in the Northwestern region of Russia

Abstract

The review presents an analysis of the tendencies of alcohol, narcotics and psychoactive drugs use in the Northwestern region of Russia during the last decade. The normative documents regulating the work of psychiatrists and narcologists are considered, as well as the registration data of drug addicts in the respective public health institutions, and morbidity and distribution indices in different parts of the region. A special attention is given to the analysis of medical and statistical indices characterizing the work of narcological institutions in comparison with other regions of Russia and with foreign countries. A separate section is devoted to the data on narcological diseases morbidity among women and adolescents, on the complications encountered in drug addicts (alcohol psychoses, alcohol myocardiopathy, acute alcohol and surrogates intoxications, other somatic and psychic manifestations in alcohol and drug addict patients), as well as the data of forensic medicine studies in the region. I conclude that the official medical statistical data need to be taken into consideration when designing special programs of diagnostics, therapy and rehabilitation of drug addicts.

Influence of antiarhythmic drugs bradizol and amiodaron on ionic currents of neurons Lymnaea stagnalis

Abstract

Used a method of an endocellular dialysis and fixing membrane potential and studied influence antiarhythmic prepatations: a new preparation - bradizol (derivative of 2-mercaptobenzimidazol) and amiodaron in concentration 1, 10, 100 and 1000 mcM on transmembranic calcium, sodium, kalium slow and fast ionic currents of isolated neurones Lymnaea stagnalis. It is established, that both preparations doze-depending and it is convertible suppress ionic currents and accelerate kinetic of inactivation of kalium slow current. Bradizol suppresses ionic currents in the greater degree than amiodaron. Changing nonspecific currents of outflow, they influence on stability of a membrane. High membranic activity of bradizol can underlie it antiarhythmic action.

Genetic defects of the coagulation cascade and homocysteine metabolism in the development of cerebral thrombosis

Abstract

The main goal of the present study was to evaluate the genetic impact of some genes operating in coag­ulation system in the development of the inherited predisposition to ischemic stroke (IS) in men and women before 45 year old. For this purpose we investigated the allele distribution of the 7 following genes - F V Leiden, F2 G20210A, G/A-455 FGB, GP Illa PIA1/A2, A1/A2 5’F7, 4G/5G PAI-1, C677T MTHFR. The plasma level of total homocysteine was analyzed too. The significant prevalence of T 677 MTHFR frequency in the patients with IS compared with controls was found (33% and 27%, respectively, p=0.03). The T 677 allele was associated with high plasma level of total homocysteine. The significant decrease of protective A2 5’F7 allele frequency in IS patients compared with controls was found also (9% and 13%, respectively, p=0.04). The gene-gene interaction between CT or TT genotypes of MTHFR gene and A1A1 genotype of F7 in IS group was showed (OR=1.9 Cl 95% 1.4-2.6). The allele frequencies of other investigated genes were not dif­ferent in patients and control group. These results allowed us to make a conclusion that the inherited predis­position to IS depends on mutations in MTHFR and factor VII genes.

The in vitro influence of immunomodulator likopid on the peripheral blood granulocytes of tuberculosis patients

Abstract

Phagocytes are suggested to be the likeliest targets for immunomodulator likopid in human blood because of their expression of Toll -like receptors, which possibly can mediate its effects. Clinical efficiency of likopid was proved in tuberculosis infection, but its direct effects on phagocytes are less studied. The aim of this work was to compare direct effects of likopid on granulocytes from healthy donors and tuberculosis patients. We studied the influence of likopid in different concentrations in vitro on the oxidative burst and phagocytosis of the blood granulocytes in healthy donors (n=14) and tuberculosis patients (n=17). There were some persons with granulocytes responding to likopid by activation in the group of healthy donors and in the group of tuberculosis patients, in particular as it was seen by increased spontaneous NBT-test. Such persons were predominant in the group of healthy donors. Granulocytes of other persons responded to likopid by the decreasing of spontaneous NBT-test. Such persons were predominant in the group of tuberculosis patients, where likopid was revealed to inhibit induced NBT-test also. Effect of likopid on NBT-test induction in tuberculosis patients was characterized by dependence on the basal levels of NBT-test. Though likopid stimulated phagocytosis, its ability to increase number and index of phagocytosis in some cases was not showed. On the basis of our results we speculated, that humans, including tuberculosis patients, have individual susceptibility of granulocytes to likopid.

Consequences of prenatal effects of hypoxia on developing brain

Abstract

The paper presents data on the effects of hypoxia on the development of brain and revealing of consequences of this effect during postnatal ontogenesis. Experiments with hypoxic stress revealed clearly the critical period of development of brain cortex, which has a special sensitivity to damage. The first response consists in lesioning of developing brain (apoptosis, necrosis, changes in membrane properties), while modification of histogenetic processes, alteration in NO-system, gliosis can be considered as adaptive response. Protective antihypoxic responses of the brain related to development of blood circulation, initiation of anti-apoptotic processes, elevation of NO synthase activity in brain vessels and it’s decrease in neurons are formed to the end of embryonic period.

Total saliva radical scavengers: photometric assay

Abstract

The method for determination of total antioxidant capacity of oral fluid (mixed saliva) has been developed. Dipyridyl served as an optical indicator reagent. The validity of the method was proved by theoretical calculations; by the data concerned the the capacity of such radical scavangers as querticin, morin, rutin and trolox to inactivate the active forms of oxygen as well as by measuring of their ferric-reduction ability. This methodical approach has allowed to confirm the literary data connected with sum antioxidants in human and rat serum (1,73 and 0,54 mM, accordingly). With help of this method the effective antioxidant concentration in saliva of volunteers (age 19-26 yeas old: female and male )was determined in ranges 0-0.6 mM as trolox Its average value appeared to be 6-fold lower than that of human plasma and 2-fold lower than that of rat serum.

Neuroimmunoendocrinological mechanisms of thymocytes proliferative activity regulation in postnatal ontogenesis

Abstract

The immunocytochemical study of the localization of hormones in thymic cells has been performed to clarify possible correlations of their expression with proliferative activity of thymocytes. We used commercial antibodies to serotonin, melatonin, somatostatin, glucagon, gastrin, beta-endorphin and histamine, and ABP or BSP kits for visualization of reaction. Computer image analysis was used to find correlations between hormone production and proliferative activity of thymocytes. Different subpopulations of thymocytes are able to produce hormones: precursors of T-lymphocytes (CD4¯CD8¯) contain serotonin and melatonin, immature cortical cells (CD4+CD8+) produce only serotonin, mature medullar cells (CD4+CD8¯) show immunoreactivity to serotonin, melatonin, beta-endorphin and histamine. The expression of serotonin, somatostatin and gastrin is localized in thymic epithelial cells, formating Gassal’s bodies. Proliferative activity of thymocytes depends from the expression of serotonin and somatostatin in thymic cells. The data received testify the expression of different hormones in human thymic cells and showing by this fact high endocrine activity of thymus. The presence of correlations between hormonal expression and cell proliferative activity could be considered as the bright illustration of important role of neuroimmunoendocrine mechanisms in the paracrine regulation of thymic functions.

Transcutaneous electrical stimulation (TES) is found to be capable of invoking latent abilities of the vision physiology for restoration of damaged functions occurred in 51-76 % of cases under study. The data obtained suggest a dependence of the TES effect on initial damage of vision and a possibility of using the TES for restoring some visual functions.

The role of intracellular infection in the pathogenesis of arterial hypertension

Abstract

Eighty four patients with an arterial hypertension (AH) of 1-2 degrees and 50 healthy persons were examined. At all surveyed were defined Chlamydia pneumoniae, Chlamydia psitaci, Chlamydia trachomatis, Toxoplasma gondii, Virus herpes 1, 2, Epstein-Barr virus, Cytomegalovirus and as antibodies to them. With the purpose of definition of possible pathogenetic mechanisms of influence of infections on development AH had been analysed correlation communications with well-known risk factors, parameters of the metabolic status, factors of an inflammation and endothelial functions. At patients with AH the authentic increase in frequency of revealing Chlamydia pneumoniae, Toxoplasma gondii and antibodies to them and as antibodies to Virus herpes 1, 2 was observed, Epstein-Barr virus. The association of an intracellular infection with various parameters of the metabolic status, endothelial function, factors of an inflammation and well-known risk factors AH (age, BMI) it has been revealed.

Ontogenous mediastinitis: pathogenesis, clinics, diagnostics, outcomes

Abstract

On the example of 143 patients with odontogenic mediastinitis is shown that this complication accounts for about 0,3% of all acute odontogenic infection. In 80% mediastinitis attack people with chronic somatic diseases, in 51,7% - from 31 to 50 of age. Mediastinitis may occure in two cases: pus can spread to the mediastinum from the primary inflamed zone; or as a result of generalization of infection and sepsis. Manifestation of mediastinitis include 3 symptoms: pain, toxic - that are most important in the course of disease; the 3-d - constraintion of nerves and vessels, that can make the diagnosis difficult in the period of disease.

Prognosis and outcome depend on opportune hospitalization and appropriate therapy. Life quality after mediastinitis depends on adequate therapy of concomitant somatic disease.

Thechnical details of laparoscopy in the early post-operation period

Abstract

Early post-operation intra-abdominal complications represent one of the most burning problems in surgery. Laparoscopy is a promising method of diagnostics and treatment of early post-operation pathology. We have analyzed the results of 277 laparoscopy procedures performed in the early post-operation period in 199 patients with the purpose of diagnostics and treatment. They were carried out after operations on the abdominal and retroperitoneal organs. The objective of the study was to define the peculiarities of laparoscopy in the early post-operation period and to work out the ways to perform it.

Considering the results of laparoscopy performed in the early post-operation period, we suggest an algorithm of early post-operation intra-abdominal complications diagnostics, which allows the efficient use of additional diagnostic methods. The main principle of the algorithm is the choice of the method most informative for the respective intra-abdominal complication. The use of laparoscopy enabled us to minimize the number of complications (0.36%).

The influence of blood flow redistribution in the hip arteries on the regeneration of the femoral head following its experimental necrosis

Abstract

The partial occlusion (≈70-80%) of the canine femoral artery below its bifurcation with the deep femoral one has been shown to result in a redistribution of blood flow in favor of the latter along with a formation of collateral vessels in the direction of the hip joint. This leads to a revascularization of the femoral head, in particular, after modeling of its traumatic aseptic necrosis. The results of the morphological analysis provide some evidence of activation of osteogenesis and gradual replacement of the necrotic area with a newly formed osteal tissue, with the apparent implications for clinical use.

Sodium-calcium-chloride mineral water intake in complex therapy of the patients with functional dyspepsia

Abstract

The reason of our study was to estimate the efficacy of sodium-calcium-chloride mineral water intake in complex therapy of the patients with functional dyspepsia. Object of research: the patients with dyskinetic and ulcer-like variants of functional dyspepsia (179). In the study the clinical, psychological, functional, laboratory, instrumental, morphological methods and also assessment of life quality were used. For the first time is proved, that the drinking treatment of sodium-calcium-chloride mineral water intake essentially contributes to the medical effect of complex therapy in the patients with dyskinetic and ulcer-like variants of functional dyspepsia, which showed by decreasing of the dyspeptic complaints, normalization of the nonspecific resistance parameters and motility function of the stomach, reducing of local inflammation in stomach mucosa, by enriching of patients life quality and is kept during one year after a course of treatment. The circuits of drinking treatment in complex therapy of the patients with functional dyspepsia are scientifically proved, and the new system of drinking treatment, which not dependent on a diet regimen, is developed and clinically tested. The efficacy of new system of drinking treatment is shown from the positions of evidence - based physiotherapy.

Abstract

This article is devoted to the experience of establishing and using information systems in organizing medical provision of preparation of the citizens for the Military service on the territory of the Republic of Karelia. Over eight-year-experience of the employees of Petrozavodsk State University and Central Military - Medical Commission of the Ministry of Defence of the Russian Federation has been taken into account in this article. The article generalizes the work results of establishing three medical information systems: «Expertise», «GIS»(Geographical Information System), and «Monitoring». It also estimates the situation from the point of view of personal experience and prognosis of the medical (expert) information science development.

Social and psychological characteristics of injection drug users

Abstract

The executed research includes the analysis of the social - demographic and psychological characteristics of 249 injection drug addicts of the Moscow Oblast and comparison of these characteristics with control group of 200 men who haven’t previously had any experience in using narcotic substances. The research provides a statistical social - psychological portrait of an injection drug addict, the distinctions of the analyzed characteristics among the men and women, the features of character of different age groups. The questions of applicability and interpretation of the psychological tests are considered. The revealed social - psychological features of the injection drug addicts are supposed to be used in the analysis of psychotherapeutic activity of the psychiatrist - narcologist.
