Theory and practice of general ecological reproductology

Abstract

The article carries out generalization and analysis of data, which make it possible to form basic principles of general ecological reproductology. Parameters оf female reproductive system are taken as an integral criterion of evaluating the influence of environmental unfavorable factors, both constant and changeable, periodical, and at the same time as a criterion of estimating its recreational capacities. On the basis of evaluation of reproductive health and therefore health of future generations, using ecological maps we may strive more successfully for adequate financing of public health taking account of level of well-being, stability or degradation of environment in a given region and ecological tendencies and prognosis of people’s health in a given district, city, region and country.

Gestosis as a course of maternal mortality

Abstract

In the present article analysis of maternal mortality of gestosis is undertaken. The study was carried out from 1996 till 1999 in Saint-Petersburg. During this period nine patients died of gestosis (7.0: 100.000 childbirth). The share of gestosis in the structure of maternal mortality increased from 2 7.9 ±5.7% in 1988- 1995 to 40.9±10.5% in 1996-1999. At the same time the tendency towards increasing of frequency of this complication is displayed: 13.2% in 1986-1990, 24.0% in 1991-1995 and30.5% in 1996-1999. The analysis made it possible to determine two variants of gestosis clinical picture and to expose some mistakes in treatment of patients at the prenatal stage, during the period of delivery and postnatal period.

Medico-ecological aspects of female reproductive health protection

Abstract

The article describes the influence of ecological factors upon reproductive system of women working at wood-shaving plate complex and women residing in settlements situated in industrial zone. Unfavorable ecological situation is brought about by influence of formaldehyde and wood powder. The article points at high level оf gynecological and extra genital morbidity among residents оf ecologically unfavorable regions. The main pathology was inflammatory processes of womb and uterine appendages, the frequency of which came to 91,85% as to workers of the complex and 73,8 % as to residents of the settlement. With the purpose of sanitation and prophylaxis magneto-infrared-laser therapy was applied. 94 patients with chronic salpingooophoritis passed therapy course with apparatus “Milta”. 82 % of patients were cured, 18% demonstrated considerable improvement of the state.

Influence of ecological factors of Tumen region on female reproductive health and results of extracorporal impregnation

Abstract

The aim of present research is to study integral influence of ecological factors of Tumen region on female reproductive health and results of extracorporal impregnation. There was examined the state of reproductive health of women residing in two districts of Tumen region - in the Far North and South of the region. The results of analysis show that hard ecological situation of the Far North of Tumen region oppresses female reproductive function. Residing in ecological conditions of the Far North of Tumen region is a negative prognostic factor as to outcome of sterility treatment by method of extracorporal fertilization.

Features of endocrinal status dynamics of women subjected to the influence of radiation

Abstract

The authors investigated the influence of integrated factors of Chemobol accident upon endocrinal system of 37 women who took part in liquidation of its consequences in remote period of observation. 4-6 years later after work in 30 km zone most women got hypermenstrual syndrome, on the background of which underfurther observation hyperplastic processes of organs which serve as targets of reproductive system (womb, ovaries, mammary glands) appeared. Less of liquidators demonstrated hypomenstrual syndrome, hyperprolactinemia. As for the group о f patients о f perimenopausal age, premature development оf climacteric syndrome and relatively premature menopause was observed.

The state of women's reproductive health and health of their posterity in regions with unfavourable ecological situation

Abstract

The aim of the research is to study abnormalities of women’s reproductive health and health of their posterity, three generations of which reside in regions with unfavourable ecological situation. Indices of reproductive health of women in two generations and health of neonatal population in the second and the third population are examined. On the basis of regressive analysis considering one or many factors the research reveals highly trustworthy dependence оf frequency of general, reproductive and neonatal health abnormalities upon gravity of ecological loading with xenobiotics and duration of their influence. The method of clinico-mathematical forecasting of frequency of all nosological forms of extra genital, gynecologic and obstetrical and neonatal pathology for five years ahead is elaborated. Complex of treatment-and-prophy lactic measures for decreasing abnormalities of reproductive health of women of the second population and health of their posterity in the third population are suggested, pathogenetically substantiated and clinically tested.

Abstract

During the last period there appeared a considerable amount of scientific researches, elaborations and sanitary regulations as to female labour and protection of workers’ reproductive health. On the basis of studying the state of reproductive health of two groups of women engaged at aviacosmic industry (483patients) the system of evaluating the degree of chemical compounds toxicity was suggested depending on content of harmful substances in the air of working zone (chiefly organic solvents).

Chemical substances as a risk factor causing abnormalities of female reproductive function

Abstract

There were examined 74 pregnant women residing in regions with advanced chemical industry, 47 of which worked on chemical plants, 27 were engaged in other spheres. Analysis of immunological and metabolical status was held twice - at the term of 15-17 and 30-32 weeks. Study of state of reproductive function revealed high level of risk for women under observation.

The results of this work served as the basis for elaborating and inculcation of wide complex of sanitary-technological, ecologo-hygienic and treatment-and prophylactic measures directed to decreasing unfavourable influence of chemical substances of environment, including industrial one, on female reproductive function.

Risk factors of neuroendocrine abnormalities of female reproductive health

Abstract

The study displays the most vulnerable periods of forming and risk factors оf hurting hypothalamo-hypophyseal system regarding to women. Analysis of the data revealed marked neuroendocrinal abnormalities as to mothers, fathers and nearest relatives of patients examined. The results of the study made it possible to elaborate mechanisms of predicting abnormalities of reproductive function as to girls, that may help to carry out timely rehabilitative measures on every stage of forming оf girls’ reproductive system.

Industrial factors of abnormalities of reproductive health as to women employed at petrochemical industry

The research deals with working conditions of employees of petrochemical plants (PCP) and factory of rubber technical pro ducts (RTP). The analysis was based on medical documents of maternal hospitals and women’s dispensaries. 2350 women employed at petrochemical industry were examined. The results of the study testify to the necessity of total liberation of women from labour activity for the period of gestation.

The aim of the research is to study hormonal status of primaparae of different age during the period of gestation. Content of estriol, progesterone, and placental lactogen (PL) was studied in dynamics of gestation. Diverse levels of these hormones are ascribed to fetoplacental insufficiency and abnormalities of placental hormonal function that brings about the necessity of timely proper therapy.

Abstract

Painful menses - dysmenorrhea is the most frequent complaint of gynecological patients of the most able-bodied age. The causes of painful menses are diverse but they often come to existence in pubertal period. Sometimes dysmenorrhea leads to disablement and loss of social status of young patients. Modem diagnostics and revealing of dysmenorrhea etiology makes it possible to choose adequate therapy and to keep up basic functions of female organism.

Contemporary view of treatment of girls with pathology of mammary glands

Mammology оf girls and adolescents takes its first steps. The data drawn in the article will help to make difference between natural age changes which don’t go beyond the limits of “pubertal scenario” and those which are to be qualified as extreme deviation or obvious pathology. Results of observation and treatment of girls with functional deviations and abnormalities of mammary glands are analyzed. Schemes of conservative treatment of diffusive mastopathies in adolescent period are brought; for the first time phytospecimen mastodynon is suggested.

On the basis of data available and of original investigations some cytogenetic aspects of influence of unfavourable ecologic and industrial factors upon female organism were studied. Women-liquidators of Chemobol accident’s consequences demonstrated non-stable genome state displaying through high frequency of chromosomal aberrations. Proceeding from several indices of genetic instability of their children born after radiational influence the authors admitted high risk of development of leukosis upon them.

Etiological and pathogenetic factors of chronic pelvic pains

Abstract

The article proves that gynecological diseases fall under the category of most often organic causes of chronic pelvic pains along with such states as hurt of abdomen muscles and backbone morbus, syndrome of large intestine irritation and infection of urinary tracts. At the same time a number of researches convincingly shows that intensity and localization of pains as to commissural disease and endometriosis doesn’t correlate with gravity and localization of pathologic process. Besides that, the review generalizes various psychological and social factors, which are to be taken into consideration in evaluating of chronic pelvic pains (ChPP) genesis. A number о f principles are elaborated regarding to the role of psychiatrical help in complex examination and treatment of patients with ChPP, including psychotherapeutic methods and treatment with antidepressants.

"Mother and child". II Russian forum

Abstract

II Russian forum “Mother and child” took place in Moscow within the walls of Presidium of Russian Academy of sciences on the 18-th - 22-ndof September. Its work started with seminar “Modem technologies in protection of maternity and childhood”. In the course of the forum leading Russian scientists delivered the following lectures: “Microbiological and immunological aspects of intrauterine infections” by E.K. Ailamazyan, “Antiphospholipid syndrome and its significance in obstetrical practice” by A.D. Makartzia, “Perinatology: ways of decreasing perinatal morbidity and mortality” by G.M. Saveljeva. Report of member of RAMS V.N. Serov consistently developed the ideas set forth in this lecture. He paid attention to the role of perinatal centers, the number of which is obviously insufficient. Development of set of perinatal centers must contribute to the decreasing of perinatal loss and maternal mortality. In the scientific part of the forum the attention of its participants was drawn to the reports of member of RAMS A.N. Strizhakov and corresponding member of RAMS V.I. Krasnopolsky, dedicated to the problem of cesarean section. Within the limits of the forum a number of symposiums was held on the problems of carrying out regional programs in the sphere of maternity and childhood protection, actual issues of neonatology, clinical genetics, hormones and hormone-dependent organs, problems of cervix uteri pathology. Taking things all around, the forum made valuable contribution to science and practice and there is every reason to believe that forum “Mother and child” will become traditional.
