Hormonal replacement therapy for postovarioectomic syndrome
Abstract
The forming specifies of Postovarioectomic syndrome (POES) after extirpation of uterine with both adnex in transitive age (after 45 years) in regular menstrual cycle was investigated. The high frequency of POES (78,1%), rapid appearance of early and late climacteric symptoms is found out. The role of premorbide surgical background in the domination of hard and middle degree disturbances (56%) was showed. The hormonal profile POES was investigated in dynamics HRT estradiol. The role of large estrogen doses and potentioning gestogen effect in the rapid elimination of early climacteric symptoms was shown. We carried out the comparative analysis of modern drugs HRT (Livial, Klimonorm, Klimen, Estraderm-TTS) after ovarioectomy with or without the uterine extraction.
About some basic questions of surgical treatment of urinary stressincontinence in women
Abstract
Symptom of urinary stress incontinence can be observed in clinic at various condition. In patients with variants of neuromuscular disfunctions of urinary bladder and uretra the surgical treatment is not shown. Surgical treatment of urinary stress incontinence is a method of a choice only at the patients with anatomic variant of disease caused as by irreversible damages of uretra and urinary bladder, ligaments, supporting them in a physiological position. In a basis of success of surgical treatment of urinary stress incontinence there is the restoration of transmission mechanism of intraperitoneal pressure on uretra lays.

The application of augmentin for prophylaxis of pyo-septical complications after caesarean section
Abstract
The frequency of caesarean section in St. Petersburg is 11,3 ~ 13,4 % from the number of parturitions. Antibiotic treatment is the specific method of prophylaxis of pyo-septical complications after operation. The comparison data on prophylactic efficiency of Cefalosporins and Augmentin infused during a surgical treatment and during the first day after a surgical treatment was made.
It is known that intravenous injection of 1,2 grams of Augmentin during and double injection of 0,6 grams at interval of 12 hours after an operation reduces the risk of postoperative complications up to 7 %, reduces the length of staying of the puerperiums in the hospital.
Danoval and omnadren effect on the patients with dysfunctional uterine bleedings during the premenopause
Abstract
Initial level of gonadothropic, steroid hormones in the blood, receptors of estradiol and progesteron of the endometrium in the patients with dysfunctional uterine bleedings during the premenopause, and their dunamics during and after hormone therapy were studied.
Danoval and oxy prog esterone-capronat in combination with omnadren had different effect on the parameters of the hormonal homeostasis and receptor apparatus of the endometrium. Antigonadоthropic effect of danoval results in FSG level reduction, estradiol and progesterone decrease in the blood and that of steroid receptors in the endometrium, that did not take place with other therapies. The effectiveness of the treatment with danoval was 88,24%, and it was 62,50% when oxy prog esterone-capronat with omnadren therapy was used.
It is concluded that danoval prescription to the patients with dysfunctional uterine bleedings during the menopause is more expedient for the wide spectrum of antigonadothropic effect.
Application of perftorcarbons for blood loss restoration during pregnancy
Abstract
The experimental way proves an opportunity of perftorcarbons emulsion application for blood loss restoration at acute massive blood loss during pregnancy. The features of effects and outcome of labor at experimental animals after perftorane transfusion are investigated. The positive p erf tor carbons emulsion influence on oxygen miometrium maintenance is investigated.
Influence of iron preparations on pregnancy
Abstract
At 156 pregnant women with threatened abortion we carried out the research of parameters of red blood on a background of iron preparations application. Amount of erytrocytes, hemoglobin and colour parameter at them authentically grew is revealed. However the statistically meaningfull increase of number of infectious complications and pathological increase of weight of a body is seen. The judgement for biological advantage of deficiency of iron expresses at pregnancy as factor of natural preventive maintenance of infectious complications of the mother and fetus.
Positive experience of replacement hormonal therapy in patients with climacteric syndrome with Proginova, Cyclo-Proginova and Climen
Abstract
At 75 patients (average age 57,2 years, average postmenopausal period 3,5 years) the hormonal replacement therapy is carried out because of climacteric syndrome with Proginova (29 women), Cyclo-Proginova (25 women) and Climen (21 women). The preparations were nominated in a cyclic mode (average duration of treatment has made 5,6 cycles).
There is marked a high therapeutic effect as reduction of frequency of vegetovascular climacteric frustrations in Proginova application (83 % ), Cyclo- Proginova (84 %) and Climen (96%).
The application of Proginova, Cyclo-Proginova and Climen resulted in normalization of lipid spectrum of blood by 3 month of treatment, that was expressed in authentic decrease in initially raised cholesterin and cholesterin of low density, with increase of the contents of cholesterin fraction of high density.
The marked normalization of lipid spectrum as a result of hormonal replacement therapy can be considered as pathogenic influence directed on normalization in age - associated metabolic infringements in female organism.
Index of the natural killer cells cytotoxic activity and values in patients with uterine myoma
Abstract
State of the system of immunobiological control is of great importance for pathogenesis of uterine myoma as of any process of tumorous growth.
56 patients were examined to study their clinical and laboratory data, total serum interferon, the ability of peripheral blood leucocytes to synthesize a/b- and g -interferons, cytotoxic activity of natural killer cells.
The ability of peripheral blood leucocytes to synthesize a/b -interferon was found to be low in 89,5% of patients, g -interferon - in 51,6%. In women with very low g -interferon level (max 28, min 14) the size of myomatic nodes exceeded 5 cm in diameter, fast growth of tumors was noted. Cytotoxic activity of natural killers was low in 27% of women, and all of them had either fast growth of the uterine myoma (with nodes more than 5 cm in diameter) or myoma recurrence after myomectomy.
High positive correlation was found between the patient’s age and the ability of peripheral blood leucocytes to produce g -interferon (correlation coefficient = 0,44), between the size of a myomatic node and indices of the functional activity of natural killers (c.c. = 0,35), between the ability of peripheral blood leucocytes to produce a/b -interferon and functional activity of natural killer cells (c.C. — 0,32).
So, the growth of uterine myoma can be predicted indirectly by the showings of cytotoxic activity of natural killer cells and by the ability of peripheral blood leucocytes to produce g-interferon.
Peripheral nervous system diseases and pregnancy
Abstract
113 pregnant women with periferal nervous system diseases(PNSD) are examined. 84 of them had radiculoneuropathy: 48 - lumbosacral segment (LSS), 36 - cervicothoracal segment (CTS), 29 had single and multiple neuropathy (SMN).
Almost half cases (49,6%) the symptoms of PNSD first appeared in 2nd trimester. In the 1st trimester it took place in 28,3%, in 3rd - in 22,1%. SMN more frequently appeared in 21-28 weeks of gestation.
The pregnancy was complicated by threatened miscarriage in CTS more frequently in early gestational age, and in LSS - in 2nd trimester. SMN complicated gestosis. The correlation with professional factors is revealed.
Complex étiopathogénie treatment was carried out with use of medications, physiotherapeutic procedures, physical exercises and psychotherapy. Complete nervous function restoration was noted in 80,5%, partial - in 17,5% women.
Reproductive health disturbance caused by cervix pathology
Abstract
165 women at the end of the first trimester of pregnancy were evaluated by 5 th points’ scale of abortion risk, stimulated by cervical incompetence, with following of cervical incompetence had done by a submucosal circular suture. As the result of in time therapy 96% viable newborns were born (in anamnesis the number was 38% only), in other cases abortions couldn’t been prevented.
Hofitol in EPH-gestosis
Abstract
The authors carried out the examination of 33 pregnant women with EPH- gestosis, in which complex therapy with the use of vegetative preparation «Chofitol» was included. The clinico-laboratory efficiency of a medication is shown.
Effects of two monophasic oral contraceptives containing 150 mcg of desogestrel in combination with 20 or 30 mcg of ethinylestradiol on adolescent girls with oligomenorrhea and ovarian hyperandrogenism
Abstract
Adolescent girls in the early years after their first menstruation are characterized by frequent irregular menstrual cycles and absence of ovulation, often associated with clinical signs of hyperandrogenism, including hirsutism and acne. Often in these adolescent girls, both the ovarian structure and endocrine characteristics do not allow to distinguish the disease from polycystic ovarian syndrome (PCOS) observed in adults.

Perimenopause - from contraception to hormone replacement therapy
Abstract
Perimenopause is a period of age-related decline in ovarian function, mostly after age 45, including premenopause and one year after menopause or 2 years after the last independent menstruation. Menopause is the last independent menstruation due to the function of the reproductive system. The date of menopause is set retrospectively, that is, after 12 months of no menstruation. Menopause occurs on average around the age of 50.
Femoston as a hormone replacement therapy for ovarian failure
Abstract
The article presents the results of a number of hemostasis parameters (coagulation, vascular and thrombocytic) and lipoprotein spectra, including apo A-1 and apo B100 apoproteins against the background of hormone replacement treatment with Femostone (Solvay, Germany). The results were compared taking into account menstrual cycle phases in perimenopausal patients (with a preserved cycle) and conditional estrogenic, progestagenic phases of Femostone administration. The advantages of the progestagen didrogesterone included in the drug in its effect on the lipid spectrum, apoproteins and some parameters of vascular thrombocyte hemostasis are shown.
Peculiarities of the course of the menopausal period in women with endocrine system diseases
Abstract
In recent decades, there has been a clear trend towards an increase in the life expectancy of the population. In the 1990s it reached 78.9 years for women. The global population of postmenopausal women today is 10%. According to WHO projections 46% of women are expected to be over 45 years old by 2015. The age of menopause remains fairly stable and in different regions of Russia is 49-50 years. Consequently, a woman spends more than a third of her life in a state of deficiency of female sex hormones.
