ERECTILE DYSFUNCTION IN MEN WITH LOWER URINARY TRACT SYMPTOMS

Abstract

The prevalence of both erectile dysfunction (ED) and lower urinary tract symptoms (LUTS) in men older than 40 years is high and tends to increase. In recent years, there were more evidence for origin and/or progression of ED in connection with the presence and severity of LUTS in men. There are several hypotheses for the relationship of ED with LUTS. The major prerequisite for the study of the relationship of ED and LUTS are data for the positive effect of phosphodiesterase-5 inhibitors on LUTS. The importance of advanced study of this problem is emphasized. It is highly relevant for understanding the etiology, pathogenesis, diagnosis and treatment of ED in men with LUTS.

Abstract

There are more than 17% of infertile couples in Russia, and this figure tends to increase. About half of cases of infertility are caused by male infertility. Infertility is idiopathic in 31 to 75% of men. These patients are mainly empirically treated using drugs of different groups (antiestrogen, aromatase inhibitors, luteinizing hormone, follicle-stimulating hormone, gonadotropin-releasing hormone, antioxidants and biologically active dietary supplements, etc.) that are frequently ineffective. As possible, obesity is one of the factors that adversely affect sperm parameters. According to the WHO data (2003), 1.7 billion people worldwide are overweight, a significant portion of them are men of reproductive age. This article discusses the main ways of pathogenesis, diagnosis and treatment of infertility in men with obesity.

LACTOBACILLUS AND ESTRIOL IN MAINTENANCE OF VAGINAL BIOCENOSIS

Abstract

The present article is dedicated to modern concepts of vaginal microbiocenosis, the aspects of the impact of women hormonal status on biocenosis are presented. It was found that the severity of dysbiosis depends not only on the etiology of the inflammatory process, but also the nature of hormonal disorders. Resistance to vaginal infection is largely determined by estrogens: low estrogen level leads to the abnormality in glycogen formation, and decrease in the number of lactic acid bacteria. This results in increase in the number of facultative microflora, which creates favorable conditions for the development and chronicity of the inflammatory process. The possibilities of restoration of normal vaginal microflora with the drug Gynoflor containing lactobacilli and estriol are discussed.

Abstract

Cryptorchidism is one of the most common abnormalities of sexual maturation. In adults, the disease often leads to serious complications such as infertility, testicular hypotrophy, and testicular cancer. By assumptions of some authors, the frequency of occurrence of these complications depends not only on the form of cryptorchidism, but also period to surgical treatment. The present review is concerned with the analysis of published data for the prevalence, causes, and pathogenesis of complications of cryptorchism. In addition, comparisons between different methods of treatment of cryptorchidism, particularly, in relation to the period of treatment, are presented.

Abstract

The present article is concerned with the problems of anovulatory infertility secondary to adiposity, the restoration of reproductive function after weight loss, course of pregnancy and pregnancy termination in women with overweight and adiposity. The authors present results of the study, including 241 women with infertility and adiposity. Prior to the pregnancy all women have corrected body weight with hypocaloric diet and optimum physical activity, and 114 women received Xenical in addition to weight management. Pregnancy occurred in 81 cases among 114 women's of Xenical groups, who had significant loss of body weight, and in 58 cases among 127 women of second group.

Abstract

Multicenter epidemiologic DIALOGUE II study was aimed to assess the tactics of management of pregnant women with arterial hypertension (AH) and pharmacoepidemiology of antihypertensive drugs used in pregnant women in Russia in comparison with the results of DIALOGUE I study in 2005-2006. The study was conducted in 27 cities of Russia, 2033 doctors managing pregnant women with arterial hypertension were interrogated. Used terminology, diagnostic criteria, the spectrum of laboratory tests used for diagnosis and monitoring of the status of pregnant women with arterial hypertension, antihypertensive therapy during pregnancy were analyzed. The results showed that in routine clinical practice there are still no well-established diagnostic criteria and common principles for the formulation of the diagnosis of AH in pregnant women; there are no well-established criteria for the start of antihypertensive therapy. A number of necessary laboratory tests are not used adequately, 7.4% of physicians continue administrating ACE inhibitors to pregnant women, as well as other non-approved drugs, and almost 80% of physicians use drugs that are not strictly antihypertensive for the treatment of pregnant women with arterial hypertension.

GENITAL MYCOPLASMOSIS AND NONCARRYING OF PREGNANCY

Abstract

1120 pregnant women were examined in order to determine the impact of urogenital mycoplasmosis on pregnancy. In 56.5% of cases urogenital mycoplasmas (Ureaplasma urealyticum, Mycoplasma hominis, or both of them) were identified: in 31.7% - only the urogenital mycoplasma, in 64.2% - urogenital mycoplasma in conjunction with other pathogenic microflora, in 4.1% - in conjunction with chlamydia. The analysis of the data showed that urogenital mycoplasma is implicated in the development of a number of pathological processes during pregnancy. Timely etiopathogenetic antibiotic therapy at 17 to 18 weeks of gestation led to reduction of inflammation in the placental system, the normalization of its function, which was reflected in a significant reduction in the frequency of pregnancy complications, optimization of perinatal outcomes.

Abstract

The adjustment of optimum treatment of premature ejaculation (PE) remains controversial due to the wide range of drugs used in treatment of this disease. It is important to remember that PE can exist both separately and in combination with erectile dysfunction (ED). In case of combination of these disorders, it is often difficult to determine which of them occur first, resulting in problems in diagnosis and treatment. This study was aimed to evaluation the efficacy of different treatment regimens in patients with PE in combination with and without ED, as well as evaluation of long-term results of treatment.

COMBINATION THERAPY OF BENIGN PROSTATIC HYPERPLASIA USING DRUGS PROSTERID AND SONIZIN

Abstract

The results of the study aimed to evaluation of efficacy and safety of combined use of drugs finasteride (Prosterid 5 mg) and tamsulosin (Sonizin 0.4 mg) for the treatment of patients with benign prostatic hyperplasia (BPH) are presented. The study included 75 patients with BPH aged 50 years, with moderate and severe symptoms of disease, moderate intravesical obstruction, prostate volume ≥ 30 cm3, and the level of prostate-specific antigen ≤ 10 ng/ml. The length of treatment was 275 ± 9 days in all groups. Patients were randomized into three groups, comparable in age, clinical manifestations and laboratory and instrumental parameters: 35 patients of Group 1 received a combined therapy with Prosterid 5 mg/day and Sonizin 0.4 mg/day, 20 patients of Group 2 - monotherapy with Prosterid 5mg/day, and 20 patients of Group 3 - monotherapy with Sonizin 0,4 mg/day. The clinical study showed that prolonged combination therapy with Prosterid and Sonizin leads to reduction of the prostate gland volume in addition to reduction of severity of intravesical obstruction caused by BPH. It makes it possible to use this treatment to reduce the risk of acute urinary retention and reduce the need for surgical intervention.

NEW POTENTIALS FOR PREDICTION AND EVALUATION OF THE EFFECTIVENESS OF TREATMENT OF IRON DEFICIENCY ANEMIA

Abstract

This study was aimed to development of prognostic criteria for efficacy of treatment of iron deficiency anemia based on erythrocyte indices and causes of anemia. For the purpose of assess the effectiveness of therapy of iron deficiency anemia (IDA), authors have analyzed results of a survey of 60 patients (9 men, 51 women, including nine pregnant women), treated by hematologist. Three prognostic factor of efficacy of treatment of IDA were defined. The treatment is ineffective in case of combined pathology of female reproductive system and gastrointestinal tract, no significant decrease in red count in peripheral blood, depression of red cell lineage of the bone marrow. The hemoglobin level is not a prognostic factor in the treatment of IDA. The transfusion therapy with erythrocyte concentrate or administration of recombinant erythropoietin in relation to the severity of patient's condition is required in patients with severe IDA or in the case of ineffective therapy with iron supplements. Correction of erythrocyte indices should be carried out even in case of with minimal decrease of hemoglobin level in all pregnant women.

The history of Reiter's syndrome description, the main clinical symptoms (urethritis, conjunctivitis, and arthritis), and the course and treatment principles are discussed. Men aged 20 to 40 years are more susceptible to Reiter's syndrome. The disease tends to recurrence, especially in the case of absence of inadequate sanitation of focus of infection stimulating the development of disease.
