Modern international and national guidance on a strategy for treatment and prevention of bronchial asthma distinguish the role of the respiratory viral infection in the strengthening of the inflammatory process in bronchial asthma. Respiratory viral infection is the most common trigger of asthma in childhood. Acute respiratory viral infection is often precede appearance of bronchial asthma. The majority of children with bronchial asthma the occurrence of the first attack suffocation due to the accession of acute respiratory infection. In 60-85 % of cases the cause of exacerbations of asthma are respiratory viruses. Recurrent respiratory infection is often caused by lack of control of asthma symptoms, despite the basic therapy. Effective anti-inflammatory drugs for the prevention of exacerbations of virus-induced bronchial asthma in children and adults at the present stage are leykotrienovyh receptor antagonists.

Zakharova I.N., Kasatkina E.N.

Abstract

One of the most frequent reasons for the visiting a pediatrician by parents with children in the first months of life is intestinal colic (IC). The article discusses the problem of the pathogenesis, diagnosis, and treatment of IC. The use of Lactobacillus reuteri DSM strain in 17938 children with IC reduces their manifestation. It is possible that this effect is associated with changes in the intestinal microbiota. These data make an important contribution to the understanding of the role of violations of bacterial microflora in the pathogenesis of infantile colic and enable their overcoming with the use of probiotics.

Abstract

Developmental dysphasia (alalia) is one of severe disorders of speech formation in children. During follow-up in 50 children 3-5 years old with developmental dysphasia, therapeutic effect of nootropic drug Pantogam (homopanthothenic acid) was analyzed. Within 2 months, 30 patients in Group 1 have received Pantogam (syrup, daily dose - 500-600 mg), 20 patients in Group 2 (control group) were evaluated. Significant improvement in the expressive and impressive speech, and speech attention were achieved against the background of treatment. The amount of active language and the number of phrases in the conversational speech was increased more than 3 times, while in the control group - by 1,5 times. Questioning the parents has demonstrated a decrease of cerebrastenic, psychosomatic disorders, motor clumsiness, hyperactivity, and improvement of attention, emotional and behavioral control in children. Assessment of performance of tasks for the motor development has shown a positive effect of Pantogam on the formation of motor skills.

Abstract

The use of different physical methods in the treatment and rehabilitation of children with chronic respiratory diseases (CRD) along with baseline drug therapy improves the quality of life of the patients, has a positive effect on the individual clinical symptoms, and increases the adaptive reserves. The development of modern technology allows to expand potentialities of physical therapy with the use of new factors and optimization of previously used factors. Therefore, development of optimal criteria for evaluating their therapeutic effectiveness in pediatric patients is actual. Dynamics of indicators of oxygen-transport function of blood can be considered as one of the such criteria for children with CRD, often accompanied by different degree of oxygen deficiency. The article presents the results of research on the effectiveness of inclusion of course of enteral oxygen therapy (EOT) in the complex rehabilitation of 66 children with CRD. These data show a marked beneficial effect of single dose of oxygen cocktail and course of EOT on oxygen-transport function of blood and blood oxygen saturation.

Abstract

The problem of medical tactics in the treatment of acute respiratory diseases (ARD) and its complications in children is discussed. The article provides information on the etiology, pathogenesis and clinical manifestations of acute respiratory diseases, complicated acute respiratory viral diseases and bacteria-associated diseases of the upper respiratory tract which emerge against this background. The need for a balanced approach to the choice of treatment strategy for acute respiratory diseases is emphasized, especially in relation to the antibiotic therapy of bacterial respiratory infection. In addition to the complex of causal treatment, the therapeutic measures aimed to key components of the pathogenesis of infection are presented. In particular, it is referred to a so-called. vaccine-like medicines containing lysates of the most common pathogens of respiratory infections, - immunomodulators of microbial origin.

SOVREMENNYE PREDSTAVLENIYa O TERAPII MOChEVYKh INFEKTsIY U DETEY

Chashchina I.L.

Abstract

The problem of diagnosis and treatment of acute urinary tract infection (UTI) among febrile children remains relevant in connection with a large prevalence of the nosology and the growth of antibiotic resistance. The need for antibiotics in the treatment of acute UTI in children is also practically assured. The duration of therapy remains debatable, since the exact topical diagnosis in infants is usually difficult. The effectiveness of treatment of acute UTI in infants does not depend on the method of starting administration of antibacterial drug, but depends on the dose that ensures the achievement of minimum inhibitory concentration. Protected aminopenicillins, as well as II and III generation cephalosporins are drugs of choice. In children with acute UTI who received antibiotic therapy during the previous period of the disease, as well as in suspected obstructive uropathy, uropathogens with high resistance to aminopenicillins, including protected aminopenicillins, are most likely causative agents. In such cases, the II-III generation cephalosporin should be the drug of choice.
