Drug interactions of directly acting antivirals in the treatment of chronic virus hepatitis C

Abstract

In recent years, in Russia epidemic situation on the VCH becomes apparent. Increase in morbidity of chronic hepatitis C (CHC) from 31.8 cases per 100 000 in 2005 to 39.9 per 100 000 in 2011 is observed; VHC often affects young persons, with a high Level of chronization and such possible outcomes as cirrhosis and primary liver cancer. The processes at key stages of hepatitis C virus life cycle are the main targets in the development of drugs for the treatment of chronic hepatitis C. The treatment is indicated for patients with hepatitis C with compensated and decompensated chronic liver disease, previously treated or not to receiving treatment, ready to receive therapy and with no contraindications. Through to 2011, the combination of pegylated interferon (PegIFN)-α and ribavirin for 24 or 48 weeks was officially recommended CHC treatment. After approval of three new DAAs for the treatment of CHC in Europe in 2014, wide interferon-free treatment regimens come into common use. These schemes have been used mainly for the treatment of patients with severe liver damage (stage 3-4 liver fibrosis according METAVIR scale (F3, F4)). Combination of drugs requires careful use and dose adjustment, alternation in drug intake time, and additional monitoring. Drug interactions should be especially considered when choosing a combination therapy in HCV and HIV co-infection.

The article presents the literature review devoted to the problem of polymorbidity, differential diagnosis, and treatment of combined pathology: coronary artery disease (CAD) and gastroesophageal reflux disease; rational pharmacotherapy of cardiovascular diseases against the background of the pathology of the gastrointestinal tract (GIT). It is emphasized that an individual, rational choice of therapy and correction of the regimen for the treatment of CAD and associated gastrointestinal diseases is an important task for the clinicians, which requires a full range of knowledge about the features of pathogenesis, manifestation and clinical course of diseases, as well as the pharmacokinetics of drugs.

Abstract

The article considers the rote of hyperendotoxinemia and systemic inflammation response syndrome as risk factors for the progression of chronic heart failure (CHF). The necessity of joining the gastrointestinal continuum to the existing cardiovascular continuum for expanding and deepening the possibilities of etiopathogenetic correction of CHF is substantiated. It is emphasized that chronic indolent inflammation is an important risk factor for the development of CHF and the progression of atherosclerosis against the background of elevated btood endotoxin tevets.

Deficiencies of magnesium and pyridoxine and the pathogenesis of the metabolic syndrome

Abstract

Metabolic syndrome (MS) represents a set of metabolic abnormalities that increase the risk of diabetes, cardiovascular and cerebrovascular disease. The components of MS are abdominal obesity, hypertension, disturbances of lipid and carbohydrate metabolism. The pathogenesis of the MS involves complex disorders of carbohydrate and fat metabolism caused by physical inactivity, a load of bad habits as well as by macro- and micronutrient imbalances in diet. Magnesium is one of the main factors supporting the physiological metabolism of sugars and fats as magnesium ion is required for the active sites of a number of enzymes. The paper presents the results of a systematic analysis of the molecular mechanisms of effects on magnesium characteristic of MS disorders of carbohydrate and fat metabolism. The results of experimental and clinical research and the evidence-based data indicate feasibility of using organic salts of magnesium with pyridoxine in the prevention and treatment of MS.

Dorsopathy (DP) is the most common variant of musculoskeletal pain syndromes. Taking into account the chronic course of the disease with a tendency to recurrence, the high incidence of comorbid conditions, the need for a large number of medications, a serious problem is the choice of an effective drug with sufficient safety for use in individual patient. The article presents data on the possibility of the use of flupirtine, an original drug with the ability to activate potassium channels, providing analgesic and anti-spasmodic effects. The safety of the drug in patients with DP is analyzed.

Urinary tract infections in outpatient practice

Abstract

The most frequent urinary tract infections (UTIs) encountered in outpatient practice include asymptomatic bacteriuria, cystitis and pyelonephritis. Treatment of these diseases should be comprehensive and include antibacterial therapy NSAIDs, immunotropic therapy, and phytotherapy. The choice of antibacterial therapy is influenced by the nature of the infection (AB, cystitis, pyelonephritis) and the type of pathogen. During pregnancy, antibacterial drugs of certain groups and in a certain trimester can be used. With the recurrent nature of the infection, immunotropic therapy is indicated. Phytotherapy is used only as a component of complex treatment of UTIs. Choice of a phytopreparation should be based on the presence of pregnancy, urine acidity, and the chemical composition of salts or stones.
