DRUG DEVELOPMENT BASED ON PHARMACEUTICAL ALTERNATIVES

Abstract

The article discusses the development of drugs using technology of pharmaceutical alternatives. One of the possible directions is the synthesis, investigation and implementation of the new salts, esters or complexes of original drugs. Examples of the introduction of drugs, which are the pharmaceutical alternatives to previously registered drugs, are amlodipine maleate, clopidogrel besylate, mexidol, perindopril arginine salt, and others. This makes the development of drugs using alternative salts of the same component perspective by creating new drugs, with special attention is required to study the bioequivalence and therapeutic equivalence of newly developed salts.

Abstract

Administration of medicines is inextricably linked to the risk of adverse reactions (ADR). A significant increase in the number of spontaneous reports about ADR requires new technologies to improve the power of pharmacovigilance (PV) systems. The history of development of such databases started by VigiBase of Uppsala International Drug Monitoring Center of WHO is presented in the article. Special attention is paid to the need for the systems, which collect and analyze ADR reports on marketing authorization holder“s (MAH) side. Introduction of special PV software allows MAHs to optimize and effectively manage safety systems as well as ensure systemic approach to quality of PV systems and their updates.

Abstract

The article presented data on the role of selenium in the human body was shows the significance of its deficiency in the occurrence of various pathologies, including inflammatory periodontal diseases. The perspective of salivadiagnostics was shown. The experience of selenium determination in biological objects was generalized. Data were provided on the main methods for the determining trace elements, including selenium, in biological substrates are given, and their comparative analysis was carried out. General methodological approaches to the determination of chemical elements in biological environments were discussed. The fluorimetric method for the determination of selenium in biological objects is considered as the preferred method, taking sensitivity, selectivity and cost into account.

Abstract

The article describes the study of anxiolytic activity of a new series of diazepinobenzimidazole derivatives having in their structure two preferential substructures - diazepine and benzimidazole. According to the results of the study a new series of compounds has anxiolytic activity of varying severity, which may indicate a high prospect for further in-depth study of the neuropsychotropic potential of new compounds. The anxiolytic activity of the compound under the DAB-19 cipher, which is higher than the diazepam.

Abstract

Based on the known participation of serotonin receptors in the pathogenesis of the development of various anxiety disorders and the previously established ability of the compound of the azole series of RU-31 to inhibit the 5-HT2A-subtype of these receptors, it suggested that this compound might influence the behavioral activity. The study conducted the behavioral activity of animals under the action of RU-31 compound in the «Open field» test and studied the anxiolytic profile of this compound in the tests «Elevated plus maze», «The punishable taking of water by Vogel». According to the results of the study we can talk about the ability of the RU-31 compound to reduce feelings of anxiety and fear.

POSTNATAL DEVELOPMENT OF RATS' OFFSPRING FROM FEMALES GETTING NEUROGLUTAME DURING THEIR PREGNANCY

Abstract

The two month long observation of offspring from female rats, which were getting neuroglutame in the dose of 650 mg/ kg (25-fold therapeutic dose) from the 6th to the 20th day of pregnancy, showed the absence of changes in physical development. At the same time comparison of the tested rats' offspring with the group of control ones showed effects: activation of body weight increase; surpassing of the terms of muscular strength maturation and opening of eyes, as well as formation of reflex of turning over in the free fall and cliff avoidance. Descent of testicles into the scrotum of the male rats and vagina opening of the female rats matched the physiological standard and did not differ from the control indices. According to the research results there was made a conclusion about the absence of damaging influence of neuroglutame in the dose of 650 mg/kg on the processes of organo- and fetogenesis registered in the postnatal period of the offspring's development.

ENDOTHELIOPROTECTIVE AND ANTIAGGREGANT EFFECT OF PRIMULA VERIS L. SOLID HERBAL EXTRACT IN EXPERIMENTAL CHRONIC HEART FAILURE

Abstract

Were studied endothelioprotective and antiaggregant properties of Primula veris L. solid herbal extract (PVSHE) in animals under experimental chronic heart failure (CHF) caused by isoproterenol administration in a dose of 2,5 mg / kg twice a day for 21 days. It was shown that in the control group of animals with CHF, the introduction of acetylcholine increased by 21,2 %, while the non-selective NO-synthase inhibitor L-NAME reduced the rate of blood flow in the carotid artery of rats by 27,5 %, which was less than the same values in the intact group: 45,2 % and -46,8 %, respectively (p < 0,05). The introduction of acetylcholine caused an increase in the rate of blood flow in the carotid artery in rats with CHF who received PVSHE at a dose of 30 mg / kg by 43,4 % and mildronate at a dose of 50 mg / kg - by 43,8 %, which was significantly higher than in animals of the control group (p < 0,05). The non-selective inhibitor of NO-synthase L-NAME reduced blood flow in the animals with CHF receiving study medications: -40,4 % and -39,5 % (p < 0,05), respectively It was found that the rate and degree of aggregation of platelets was higher in rats with CHF than in intact animals (29,6 % / min versus 20,6 % / min and 27,3 % vs. 18,8 %, p < 0,05, respectively ). In rats with CHF receiving PVSHE, the rate of blood flow in the carotid artery was 20% / min, mildronate -23,2 % / min, the degree of aggregation was 19 % and 21,9 %, respectively which was significantly lower in comparison with animals of the control group (p < 0,05). It was found that the level of von Willebrand factor (vW) was higher in animals with CHF than in the intact group by 91,1 % (p < 0.05) and significantly lower in animals with CHF who received PVSHE by 31,4 % (p < 0,05) and mildronate 21,2 % relative to the control group of rats (p < 0,05). The obtained data testify to the endothelioprotective and antiaggregant effect of the PVSHE comparable to the comparative preparation mildronate.

THE RESULTS OF THE RESEARCH ACTIVITY OF SILVER. NANOPARTICLES IN VITRO AND IN VIVO AGAINST DRUG- RESISTANT MYCOBACTERIUM TUBERCULOSIS STRAINS

Abstract

The paper presents the results of experimental studies on the antitubercular activity of silver nanoparticles. In vitro experiments on 117 MBT strains with resistance to isoniazid and other drugs with preserved sensitivity to rifampin (group 1) and 108 ml of MBT strains (group 2), bactericidal action of nanoparticles was demonstrated in 48,1 and 41,7 % of cases, respectively. With the combined use of nanoparticles with isoniazide, similar indicators were 65,3 and 60,5 %, respectively (p < 0.05). In the model of experimental tuberculosis in BALB/C mice, the best survival and seeding rates of the pathogen from the lung tissue were also observed with the combined use of nanoparticles and isoniazide. The results obtained allow us to offer silver nanoparticles as adjuvants of chemotherapy for clinical trials.

APPLICATION OF ARTIFICIAL NEURAL NETWORKS FOR DIFFERENTIAL DIAGNOSIS AND PROPHYLAXIS OF THE DISEASES OF HEPATOPANCREATODUODENAL ZONE

Abstract

The purpose of the study was to evaluate the possibilities of neuronet differential diagnosis of hepatopancreatoduodenal zone diseases and their potential for the formation of an individualized preventive strategy. The work was performed on the basis of materials of 488 patients with the pathology of the hepatopancreatoduodenal zone. Neural network analysis of information on risk factors (sex, age, eating habits, stress, family status, bad habits) was used. Results. The sensitivity of the differential diagnostic model reaches Se = 84,7, m = 1,66 for peptic ulcer, Se = 81,4, m = 1,8 for pancreatitis and Se = 92,1, m = 1,24 for cholecystitis. Specificity levels equaled respectively to Sp = 91,5, m = 1,29, Sp = 90,1, m = 1,38 and Sp = 82,7, m = 1,75. The article also presents a method of developing an individualized diagnostic strategy using the artificial neural network.

Abstract

The article provides an overview of the literature information and normative documentation describing the methods for the qualitative and quantitative determination of latanoprost, betaxolol hydrochloride, as well as the determination of latanoprost in combined ophthalmic dosage forms. It was revealed that the predominant method of identifying betaxolol hydrochloride is IR spectrophotometry and reaction to chlorides. For latanoprost, we apply the HPLC method and UV spectrophotometry. Both substances are quantitatively determined primarily by HPLC.

CYTOKINE PROFILE IN PATIENTS WITH GOUTY ARTHRITIS IN COMBINATION WITH METABOLIC SYNDROME

Abstract

There were 60 patients with reliable PA under observation (the diagnosis was established according to the classification criteria ACR / EULAR, 2015). All patients were divided into two groups: Group 1 - 40 patients with gout without MS signs, the second group included PA patients with signs of MS (75 % for women, 25 % for men). The average duration of the disease was (8,2 ± 3,5) years. In 25 patients there was a family history. Despite the differences in etiology and pathogenesis, the variety of clinical manifestations of rheumatic diseases (RH), the basis of most of them is chronic inflammation. A significant role in the formation of inflammation, which has a number of common immune mechanisms in some of the most common RH (rheumatoid arthritis, psoriatic arthritis, gout), belongs to cytokines, including tumor necrosis factor-а (TNF-а). It is with the hyperproduction of cytokines associated with an increased risk of atherosclerosis and associated with it vascular complications. Of particular importance is the metabolic syndrome, which is often observed in patients with gout. On the one hand, the metabolic syndrome is reversible, and on the other - significantly increases the risk of such socially significant diseases as type 2 diabetes and atherosclerosis.

In the light of the growing number of neurotic disorders, health care reforms and the socioeconomic component of the country, the development of short-term and effective methods of psychotherapy is becoming especially topical. The aim of the study was to determine the indications for short-term individual analytical-cathartic therapy (ACTA) of patients with anxiety neurotic disorders. The article analyzes the dynamics of clinical and psychological characteristics of 90 patients with anxiety neurotic disorders (F40, F41, F42, F45, F48) who have undergone short-term individual analytical cathartic psychotherapy Predictors of the ineffectiveness of the proposed treatment option are identified and a mathematical model for calculating the prognosis of the effectiveness of the short-term individual ACTA is presented. Based on the obtained data, indications and contraindications for the application of this method were determined.

STRUCTURE OF UTERINE AND CERVICAL FACTOR OF INFERTILITY IN WOMEN OF EARLY AND ACTIVE REPRODUCTIVE AGE

Abstract

Aim of research. The aim of this research is determining the structure and clinical peculiarities of uterine and cervical factor of female infertility in early and active reproductive age. Material and methods of research. A prospective outpatient and inpatient study of 222 patients was conducted in the Department of Gynecological Endocrinology of PI «SRI OG and P» in the period from 01.03.2015 to 01.03.2018. Conclusion. High frequency of combined forms uterine and cervical factor of infertility in relation to isolated forms (73,6 % versus 27,3 %, p < 0,01) was revealed in infertile women at early and active reproductive age. High frequency of uterine myoma (34 %), endometrial polyps (26,4 %), endometrial pathology (22,6 %) and congenital anomalies of the uterus (11,3 %) was determined in the structure of uterine factor. Diseases with following frequency of occurrence were diagnosed in the combined form of uterine and cervical factor of infertility: chronic metroendometritis (77,6 %), hypoplasia of the uterus (46,7 %), cervical erosion (50,5 %), congenital abnormalities of the uterus and cervix (32,7 %), endometrial polyp (31,8 %) and pathology of the endometrium (22,4 %) which contributes to the increase of infertility rates.

Abstract

An anonymous survey of 66 pediatricians from Volgograd was conducted. It was found that 100 % of the respondents recognize the importance of preventive work. Most of pediatricians (86 %) actually aware of effective methods of preventing and educate parents prevention methods. The main obstacle to carrying out preventive work is the lack of time at the reception (71,2 % of the respondents indicated this fact). Pediatricians working in private clinics and hospitals are involved in prevention more time than doctors in municipal medicine.

The aim of the study was to study the expression of matrix metalloproteinase-9 (MMP-9) and its tissue inhibitor 1 (TIMP-1) in hepatocytes and nonparenchymal liver cells in neonatal mice of two opposing lines of CBA and C57B1/6 after chronic intrauterine hypoxia. In The total expression level of MMP-9 and TIMP-1 in hepatocyte cells was more pronounced in CBA mice, in compared with mice of the C57B1/6 line.

FREQUENCY OF METABOLIC SYNDROME AND OTHER COMPLICATIONS OF OBESITY IN PRACTICE OF A CHILD ENDOCRINOLOGIST

Abstract

The analysis of 121 medical history of children with obesity at the age from 5 to 17 years is carried out. The leading complications and concomitant diseases are: gastropathology (96 %), cardiac disorders (50 %), metabolic disorders (47 %), hypothalamic syndrome (46 %), accelerated or accelerated physical development (85 %), delayed sexual development in boys 25 %), accelerated puberty (60 %) and hyperandrogenism in girls (5 %). It was found that the metabolic syndrome was 1,5 times more common in children with a 1-2 degree of obesity than in children with a 3 degree of obesity. The high morbidity of children with obesity is revealed, which reflects the reduced immunological reactivity of the organism.

THE EFFICIENCY OF THE LIGATION OF INTERSPHINCTERIC FISTULA TRACT IN THE TREATMENT OF TRANSSPHINCTERIC ANAL FISTULAS

Abstract

Modern treatment of chronic paraproctitis is aimed at preserving the integrity of the anal sphincter in order to reduce the risk of incontinence. One of these techniques is the operation of the ligation of intersphincteric fistula tract (LIFT). The data are presented in the article about the results of sphincter-saving operation of the LIFT and comparison of the effectiveness with the traditional method of treatment-fistula excision with the sphincter suture. The main and control group included 36 patients with transsphincteric fistula-in-ano. Primary recovery in the first group was 77,7 %. Disease recurrence occurred in 16,7 % cases as intersphincteric fistula, 5,6 % developed a «complete» fistula. In the second group, the relapse occurred in 25 % cases and exclusively by the type of «complete» fistula. The phenomena of incontinence were noted only in the control group.

Abstract

The article presents a comparative analysis of the results of questioning of patients and medical personnel using the tools of lexical analysis of medical records. The questioning was conducted simultaneously in a 24-hour in-patient clinic. A survey was conducted of all patients discharged from the hospital during the week, and all medical workers who worked during the period. The questionnaire for staff includes questions about patient satisfaction. The formulation of questions is «mirror» to the questions of the questionnaire of patients. The results of the questionnaire convincingly testify to a higher assessment of the conditions and the provision of medical care to patients compared with the assessment by medical personnel.

Abstract

From the inventing of the secondary bilateral synchrony (SBS) by KenanTukel and Herbert Jasper in 1952, it becomes clear, that the part of the seizures, which were classified and described as generalized (tonic, atonic, myoclonic, absence, etc.) in their mechanism of appearance are focal, are evolving from the frontal and temporal lobes with this phenomena. The actual ILAE classification (2017) has proved their conclusions. It was shown, that part of the seizures, which classically were described as generalized ones, could have a focal onset. In our study we were trying to mark most important clinical-encephalography features of the patients with focal epilepsy and SBS phenomena on EEG.

Abstract

There were examined 136 patients with progressive myopia (192 eyes) in the main group and 113 patients (113 eyes) with stationary myopia. The age of patients is from 8 to 17 years, mean age is (12,5 ± 0,07) years. The observation period is from 1 to 7 years. We also examined 46 patients with emmetropia (46 eyes) and 30 patients with low-grade hyperopia (30 eyes) from 6 to 17 years. It was established that the progression of myopia is characterized by an increase in the anteroposterior size of the eye, a decrease in the thickness of the sclera, a decrease in the corneal hysteresis and rigidity of the corneoscleral membrane is accompanied in 83,3 % of cases by an increase in intraocular pressure to the upper norm and above, which eventually leads to an increase in the strain of the corneoscleral membrane, a decrease in the biomechanical properties of the sclera and the progression of myopia.

Abstract

A complex examination of 110 patients with chronic generalized periodontitis was performed on the background of metabolic syndrome (MS) with various degrees of obesity. Glucose, immunoreactive insulin, C-peptide concentration, low density lipoprotein (LDL), high density lipoprotein (HDL), total cholesterol (OX) were determined. The high LDL-cholesterol levels were detected against the background of the HDL decrease, in proportion to the increase in the MT index, there is an increase in metabolic disorders, which is manifested by an increase in the level of basal, stimulated insulin.

Abstract

In the course of the study the variability of the linear parameters of the cerebral part of the skull was determined in the brachicranial skull. The work was performed on 64 preparations of skulls of people of both sexes of mature age with physiological occlusion of teeth. The analysis of linear indices of the cerebral part of the brachicranial skull of men and women revealed a statistically significant difference in width of the base of the skull. The other parameters did not differ significantly from each other. The variability of all parameters of the cranial vault was weak.

DATABASE OF 3D MODELS OF RELEVANT PROTEIN-TARGETS OF THE RAGE-NF-KB SIGNSLING PATHWAY

Abstract

For 14 main nodes of the RAGE-NF-kB signaling pathway 34 types of monofunctional proteins, subunits and isoforms have been identified that are related to the development of pathies in diabetes mellitus and Alzheimer's disease. For human, 583 experimental X-ray and 1668 theoretical 3D models of these biotargets were found, a primary verification of their validity was performed. For the RAGE-NF-kB signal pathway a database of 826 adequate 3D model of protein-targets of Homo sapiens has been formed.
