NEW CRITERIA FOR RHEUMATIC FEVER DIAGNOSIS (REVISED STATEMENT OF THE JONES CRITERIA FROM THE AMERICAN HEART ASSOCIATION)

Abstract

The present article considered the modified Jones Criteria proposed by AHA. The main purpose of the revision was to increase the sensitivity of Jones Criteria by including Doppler echocardiography as a tool to diagnose cardiac involvement. Another reason to update them was current epidemiology of rheumatic fever, which should be taken into account while diagnosing RF. Advantages and disadvantages of the revised criteria were discussed. On the basis of the performed analysis one can conclude that the revised Jones criteria could not be applied in Russian clinical practice without any preliminary evaluation. Moreover, the revised Jones criteria should be thoroughly discussed by Russian national experts so as to produce consensus regarding the usefulness of these modifications and their inclusion in the national guidelines for diagnosing acute rheumatic fever.

NONALCOHOLIC FATTY LIVER DISEASE AS A MULTISYSTEM DISEASE

Abstract

Non-alcoholic fatty liver disease (NAFLD) is the most common cause of chronic liver disease. Over the last decade, it has been shown that the clinical burden of NAFLD is not only confined to liver-related morbidity and mortality, but there is now growing evidence that NAFLD is a multisystem disease, affecting extra-hepatic organs. For example, NAFLD increases risk of type 2 diabetes mellitus (T2DM), cardiovascular (CVD) and cardiac diseases, and chronic kidney disease (CKD). The review discusses the factors involved in the progression of liver disease in NAFLD and the factors linking NAFLD with other extrahepatic chronic diseases, such as CVD and CKD.

DEVELOPMENT AND INITIAL VALIDATION OF A QUESTIONNAIRE TO STUDY DISEASE ACTIVITY IN SYSTEMIC LUPUS ERYTHEMATOSUS

Abstract

Current work presents the results of development and initial validation of a questionnaire for self-assessment of disease activity in systemic lupus erythematosus (SLE) patients. Three-stage process provided formulation of an initial questionnaire, further improvement after application in different SLE patients and assessment of external, content and divergent validity as well as reproducibility in a cohort of 30 SLE patients with various disease activity. Good correlation with valid SLE activity indices was demonstrated and no correlation with damage was seen. Reliability analysis have shown acceptable internal consistency of a new questionnaire.

Abstract

Haloperidol is used to control the use of alcohol; however, dose adjustment often results in serious side effects. CYP2D6 enzyme, which is encoded by CYP2D6 gene, has a highly variable activity caused by CYP2D6, a genetic polymorphism. The study included 20 males with alcohol addiction. We evaluated the efficacy and safety of haloperidol using international psychometric scales. A real-time PCR method was used for CYP2D6 genotyping. The study revealed statistically significant patterns between CYP2D6 polymorphism and haloperidol efficacy and safety.

NARINGIN AND APIGENIN IMPROVE ENDOTHELIAL VASODILATOR FUNCTION OF BRAIN VESSELS IN EXPERIMENTAL ANIMALS WITH FOCAL ISCHEMIA

Abstract

The study describes the effects of naringin and apigenin on endothelial vasodilator function (EVF) of cerebral vessels after ischemia. The experiment was carried out on Wistar male rats. Focal ischemia was induced by occlusion of the middle cerebral artery. We used Doppler sonography to assess EVF through modifying NO synthesis. We used sulodexide (Vessel Due F, Alfa Wasserman, Italy) and mexidol (Pharmasoft, Russia) as reference drugs. Our findings suggest that both naringin and apigenin can protect EVF in rats after brain ischemia. The animals which received naringin and apigenin showed that administration of acetylcholine improved vasodilation by 1,6 and 2.2 (p < 0.05) times, respectively, as compared with negative controls. Moreover, the use of naringin and apigenin resulted in diminished vascular response (increased blood flow) caused by L-arginine administration. L-NAME-mediated vasoconstriction was more pronounced in rats treated with naringin and apigenin than in negative controls. Nonetheless, apigenin showed pharmacological effects superior to those of naringin and comparable to those of the reference drugs, sulodexide and mexidol.

Abstract

High prevalence of periodontal disease and increased treatment needs pose a challenge for dentists. The prevalence of vitamin D deficiency ranges 1 % - 78 % in different age groups. High vitamin D-deficiency levels associated with increased prevalence of oral diseases need to be considered when selecting an appropriate therapy with vitamin D metabolites. We studied blood supply to the oral cavity in patients with mild chronic generalized periodontitis accompanied by vitamin D deficiency The oral mucosa showed hyperemic microcirculation characterized by increased microvascular blood flow, increased vascular permeability, stagnation areas in periodontal tissues.

SINGLE-DOSE TOXICITY STUDY OF RU-185 (ENOXIFOL) IN RATS

Abstract

LD50 values were 1920.96 mg/kg (1858.47÷1985.55 mg/kg) for male rats and 2260.3 (2294.69÷2226.39) mg/kg for female rats after intragastric administration of toxic doses of RU -185. Based on classification criteria for acute toxicity (GOST 12.1.007-76), we allocated this substance to a moderately toxic drug category. We evaluated functional and behavioral effects of RU-185 on rats and established that its minimum pharmacological activity was observed at a dose of 1.9 mg/kg, while the maximum - at a dose of 59-119 mg/kg. Taking into account the LD50 values and the range of pharmacological activity recorded for RU-185, we calculated the therapeutic index of enoxifol and it equaled 1000. Within the given therapeutic index, the range of pharmacological activity that is safe equals 240, while the range of toxic activity equals 475-1000. We suggest that the data on enoxifol toxicity and the ranges of its pharmacological and toxic activities could be used for dose selection/adjustment in subsequent preclinical toxicity studies.

A mathematical model for predicting the time of QTc interval prolongation with the debut of type 1 diabetes, according to microcirculatory disorders diagnosed by LDF has been developed.

THE USE OF SODIUM HYPOCHLORITE IN NON-ALCOHOLIC STEATOHEPATITIS

Abstract

31 patients with non-alcoholic steatohepatitis (NASH) received monotherapy with sodium hypochlorite infusions. We measured the levels of lipid peroxidation, antioxidative enzyme activity, citolytic and mesenchymal inflammatory syndrome at baseline and upon completion of the treatment. All indices as well as the general condition of the patients improved after treatment with sodium hypochlorite. We conclude that treatment with sodium hypochlorite appears to be an effective option in treating NASH.
