Heart transplantation: pathophysiologic aspects of the main problems in modern cardiac surgery

Abstract

In modern transplantology, what used to seem unthinkable is nowadays gradually becoming routine. This happened with heart transplantation, which today has become the operation of choice among the methods of surgical treatment of terminal stages of pathology of this organ. Despite the rapid growth and development of transplantology, a number of problems remain at the moment, starting from the criteria of optimal donor hearts and ending with the legal side of this issue. As research into heart transplantation continues unabated, in time these questions will also be answered. In this connection it became necessary to highlight the main problems of heart transplantation and possible ways of their solution so far at the experimental level.

Myocardial infarction of the second type - current state of affairs

Abstract

Literature data of recent years on comparative characteristics, diagnosis and prognosis of myocardial infarction of the first and second types are analyzed. The data on the epidemiology of myocardial infarction of the second type (IM2t) are presented, which at the moment seem to be rather contradictory. The combination of several modern methods of instrumental and laboratory diagnostics, for example, the use of intracoronary imaging with optical coherence tomography or intravascular ultrasound in combination with highly sensitive methods of cardiac troponin determination may be of special diagnostic significance. Most authors currently support the idea of an individualized approach in the treatment of patients with IM2t. But the ongoing randomized trials comparing modern approaches in diagnostics with conservative treatment in relation to 2-year total mortality in patients with IM2t should soon give an idea of treatment efficacy.

Diabetes mellitus: present and future

Abstract

The main method of treatment of type 1 diabetes mellitus is insulin therapy. This method is associated with certain difficulties: frequent subcutaneous injections, inaccurate calculation of insulin dose, insufficient glycemic control, high cost of insulin pump maintenance. These problems of diabetes mellitus treatment at the present stage are trying to solve with the help of the latest technologies. For this purpose they study different ways of insulin delivery, create algorithms for the operation of devices that synchronize the level of glycemia and drug release, use in experiment the accumulated knowledge in the field of transplantology, stem cells, vaccines. The paper presents a review of the most promising methods of treatment and control of diabetes mellitus.

Current issues of treatment of lower limb deep vein thrombosis and prevention of pulmonary embolism

Abstract

In this literature review the problem of treatment of such an urgent disease in surgery as deep vein thrombosis (DVT) of the lower extremities was considered. The main subject of discussion in the article was the choice between conservative therapy and surgical correction in the treatment of DVT in order to prevent the development of thromboembolic complications, as well as consideration of the advantages of various methods of surgical intervention. The aim of the work is to analyze Russian and foreign articles and monographs published over the last few years and devoted to this problem. According to the literature sources, the relevance of the problem in surgical practice is confirmed due to the lack of standardized treatment and high rates of lethal outcomes from thromboembolic complications.

Abstract

This literature review describes the components of the intestinal barrier that maintain its integrity and evaluates their contribution to the development of increased intestinal permeability syndrome. Data on the relationship of intestinal permeability syndrome and excess bacterial translocation with autoimmune diseases - celiac disease, inflammatory bowel disease, type 1 diabetes mellitus, autoimmune hepatitis, and systemic lupus erythematosus - are analyzed. The reasons for the occurrence of this syndrome are considered, its significance in the occurrence of these diseases and in the development of therapeutic and preventive measures is determined.

Obesity and inflammatory bowel disease: is there a relationship?

Abstract

Inflammatory bowel diseases (IBD) are an "enigma of modern gastroenterology" because their etiology remains unknown. The prevalence of ulcerative colitis (UC) and Crohn's disease (CD) has increased in parallel with the rise in obesity. At first glance, the association between UC and obesity is questionable. However, when the predisposing factors and prevalence of these diseases are analyzed in detail, many similarities can be found. Obesity is becoming increasingly common among patients with ICD and plays an important role in both the development, course and treatment of UC and CD. The involvement of adipocytes and the release of proinflammatory cytokines, leptin and adiponectin imbalance, decreased mucin synthesis, induction of intestinal permeability, and bacterial translocation in the pathogenesis of CD is suggested. In the presented review, we focused on studies conducted in recent years. Literature search was conducted in PubMed and Google Scholar.

Chronic endometritis - new data, treatment perspectives

Abstract

Today it is proved that chronic endometritis and the underlying complex of morphofunctional changes of the endometrium is the cause of key problems of obstetrics and gynecology, realized in menstrual cycle disorders, infertility, pregnancy failure, IVF failures. The available data on the pathogenesis of the disease and treatment options cannot fully satisfy clinicians. This review presents new information about the regulatory role of genes of HOX family, lipopolysaccharides and inflammasomes in the development of chronic endometritis, considers innovative methods of therapy of chronic endometritis, which have shown their effectiveness.

Triple negative breast cancer: modern achievements and perspectives

The review highlights the problem of triple negative breast cancer (TNRMBC) as the most aggressively occurring, heterogeneous and the most studied subtype of breast tumors. The existing classifications of TNRML are analyzed with an assessment of their application in clinical practice. The review also focuses on available therapeutic options and discusses the possibilities of targeting key molecular mechanisms of carcinogenesis. Abstract

Every year about 100 thousand premature babies are born in Russia. The cardiovascular system of the premature infant is subjected to adaptive stress due to unpreparedness for extrauterine life, limited compensatory mechanisms and hypoxic myocardial damage. These factors lead to cardiac dysfunction up to the development of heart failure. At present, one of the directions of scientific search in neonatal cardiology is the establishment of early biomarkers of myocardial dysfunction in premature neonates for prognosis and timely correction of cardiovascular system disorders.

Peculiarities of the course of COVID-19 in children

Abstract

Infectious diseases every year attract more and more attention of the scientific community because of high lethality and lack of means of etiotropic therapy of some of them. Among the total number of pediatric pathologies, acute respiratory infections are of special interest because of high morbidity, constant change of etiologic factor, and a wide range of viral agents. This year in Wuhan city there was an outbreak of pneumonia caused by a new, previously unreported virus. The rapid spread and severity of clinical manifestations caused by the new infection has posed challenges to physicians related to the development of means of diagnosis of infection, therapy of manifestations and specific prophylaxis. The aim of this work is to study and summarize the data on the severity of clinical manifestations and peculiarities of the course of a new coronavirus infection in children.

Craniosynostosis in children

Abstract

Craniosynostosis (CS) is a serious pathology that not only affects the child's appearance and further social adaptation, but can also lead to severe consequences - increased intracranial pressure (ICP), mental retardation, visual disturbances. While the incidence of this craniofacial anomaly is quite high - it is observed in 1 out of 2000-2500 newborns, it is not always possible to recognize it in time, and various methods of surgical treatment to date are controversial and require further improvement. The paper considers the most relevant practical issues of diagnostics and surgical treatment of children with CS, taking into account the latest scientific data. The possibilities of pre- and postnatal diagnosis of CS are presented. Approaches to the determination of indications for surgical treatment are considered. New modalities in treatment are presented: cranial orthotization, 3D-modeling.

Presepsin: diagnosis, risk stratification and prognosis in surgical patients

Abstract

Presepsin is a new promising early marker of bacterial infection. The paper considers the significance of presepsin monitoring in surgical patients. Both the isolated determination of presepsin and the utilization of presepsin along with the use of integrated severity rating scales and other current widely used biomarkers such as procalcitonin, C-reactive protein, interleukins and others are evaluated. The causes of non-infectious elevation of presepsin levels are reviewed. Conclusions are drawn about the prospective use of presepsin in surgical patients for diagnosis, therapy monitoring, risk stratification and prognosis.

Congenital cystic-adenomatous malformation of the lung is a rare congenital pathology characterized by the formation of cysts of different diameters, which manifests as respiratory distress syndrome in the neonatal period and as chronic inflammatory lung diseases in older age.  Cystic adenomatous malformation of the lung has several variants of outcomes, including malignization and regression, and can be combined with other anomalies. This paper reviews the embryogenesis of the lung, histopathologic classification, antenatal and postnatal diagnosis of the malformation.

Chronic pelvic pain syndrome in adolescents

Abstract

Chronic pelvic pain syndrome (CPPS) is an urgent problem in gynecology, as well as in other fields of medicine. The etiology of this syndrome is diverse and quite individual.  Differential diagnosis is difficult due to the close anatomical and functional relationship of the pelvic organs. At the moment, ultrasound (ultrasound) acts as a first-line tool, but often the data obtained are not enough to make an accurate and complete diagnosis. In order to determine the cause of the SCTB, it is then necessary to use additional clarifying diagnostic methods. Correct diagnosis leads to the appointment of timely adequate treatment, which contributes to the early recovery of patients and improvement of their quality of life.

Abstract

Nonsteroidal anti-inflammatory drugs (NSAIDs) as one of the leading groups of drugs by volume of consumption are an important component of modern therapy. However, numerous literature data and clinical studies draw attention to the relationship between analgesic intake and reproductive health. Prolonged systematic use of NSAIDs negatively affects fertility and functional status of reproductive organs in both female and male. The use of this group of drugs during pregnancy also has a pathological effect on the formation of the reproductive system of the fetus, hence on the reproductive potential of future generations. Consequently, NSAID therapy should be used with caution by persons of childbearing age.

Demyelinating diseases (DD) are the most common cause of severe physical limitations among adult patients of working age. Blockade of host cell TMPRSS-2 as an effective strategy for the treatment and prevention of novel coronavirus infection caused by SARS-CoV-2

Abstract

The worsening epidemiologic situation and the increasing spread of a new coronavirus infection caused by SARS-CoV-2 requires an early search and development of an etiologic and pathogenetic drug for possible use not only in the treatment of clinical manifestations of the new coronavirus infection, but also as a prophylactic agent. Since April 2020, there has been increased interest in the drug Bromhexine hydrochloride as a TMPRSS-2 inhibitor. The introduction of bromhexine hydrochloride into medical practice for the treatment and prevention of COVID-19 may help to slow the increase in incidence. Worldwide recognition of the inhibitory effect of bromhexine on TMPRSS-2 will allow it to be repurposed and recommended not only as a prophylactic agent to prevent infection but also for the treatment of patients with COVID-19. The question of the drug dosing regimen and duration of course administration, as well as possible positive effects in combination with other groups of drugs remains insufficiently studied.

Antibiotic therapy of acute intestinal infections: the problem of antibiotic resistance in Russia and in the world

Abstract

Acute intestinal infections (AIE) play a major role in morbidity and mortality of the population, especially among children. Antibiotic therapy is of great importance in the therapy of bacterial intestinal infections; however, the list of antibiotics to which antibiotic resistance of pathogens develops is currently increasing. The literature review presents data on the sensitivity of the main bacteria, causative agents of intestinal infections to the most commonly used in medical practice antibacterial drugs. Decrease in sensitivity of the most common in Russia and in the world pathogens of AKI to fluoroquinolones, co-trimoxazole, nitrofurans and rifampicin is revealed.

Reference intervals of human body temperature

Abstract

To date, the data on reference intervals of human temperature, for the analysis of both pathological processes and normal deviation, have not been systematized to the end. Terminology from various sources is summarized to address this issue. The difference between body and skin temperature is presented. In the course of literature search among domestic and foreign scientific publications, the dependencies of skin and/or body temperature on sex, age, race, circadian rhythms, physical activity, and physique are highlighted and discussed in detail. Conclusions are made about a wider range of normal human body temperature taking into account the considered dependencies. This indicates that so far the ranges of normal human body temperature in different points of the body remain uncertain and there is an opportunity for further research and creation of a unified methodology for measuring body temperature.

Gastric mucosal atrophy: from morphologic phenomenon to diagnostic pattern detection in the aspect of gastric cancer prediction

Abstract

Chronic atrophic gastritis precedes the development of intestinal-type gastric cancer (adenocarcinoma). The modern paradigm of cancer prevention focuses on the assessment of an integral index of gastric mucosal atrophy, allowing to determine the personalized risk of cancer development. However, assessment of the severity of atrophic changes is difficult due to their heterogeneity. The question of possible ways to improve the quality of diagnosis of gastric mucosa atrophy remains relevant. Within the framework of this literature review the modern ideas about atrophy as a morphological phenomenon will be considered, as well as possible ways to improve the interstudy diagnostic agreement by modifying the preanalytical stage of histologic material processing.

Possibilities of low-dose radiation therapy in the treatment of intrapulmonary lesions caused by COVID-19 infection

Abstract

This paper considers the relevance of the use of single lung irradiation in the treatment of pneumonia caused by a new coronavirus infection. Clinical observations demonstrating the prospect in the treatment of this disease are presented. Patients with severe pneumonia who received LD-RT (low-dose radiation therapy) at a dose of 0.5-1.5 Gy showed shorter recovery time and absence of complications. This treatment method has been shown to be effective in a number of studies from different countries, predicting success and economic benefit with further use and study. A literature search for relevant studies was conducted in PubMed, EMBASE, Web of Science and Google Scholar using the keywords: SARS-CoV-2, COVID-19, pandemic, coronavirus pneumonia, low-dose radiation therapy, cytokine storm, acute respiratory distress syndrome. The focus was on full-text articles given their general availability in a pandemic setting.

Selfie as a socio-cultural phenomenon

The dead body: the aestheticization of death

Abstract

The body today has become the focus of attention of philosophy, science, politics, art, and man himself, which makes modern culture "body-centered" (G. Tulchinsky). The modification of the body with the help of modern biotechnology has led to the absolute dissociation of the "I" (Spirit) and the body, and their subsequent transformation into irreconcilable antagonists. This is followed by the dissociation of the "I" from the event of death, the topos of which is the same body; this leads to a change in the attitude towards the dead body in contemporary European culture and art. Traditionally, the body was seen as a moral narrative. With the advent of biotechnology, the living body and the dead body represent an aesthetic object that is not only studied, but also played with as puppets that can be made to look appropriate for play through exsanguinating aestheticization. The dead body increasingly resembles "play equipment," and access and options for "playing with it" vary. The access and availability of the dead body and its associated publicity is an element of contemporary play with the body.

The relationship between cognition and freedom in the idea of human purpose (according to I.G. Fichte)

Abstract

In the philosophy of I.G. Fichte the formation of methodology from subjective idealism to objective idealism is realized. The existence of one's own self is made evident by the discovery of its boundary. Knowledge of the boundary frees a person from the feeling of unfreedom. Therefore, the process of cognition is thought of as a form of existence, becoming, and destiny of man. The self that is aware of itself and the boundary of its existence, i.e. the world, is the absolute self. The existence of the absolute I in being is the condition for the beginning of the process of cognition, and at the same time the absolute I in human consciousness is the highest form of self-consciousness, on which the attribute of human existence is freedom. Freedom is morality itself, to achieve it, it is necessary to identify the understanding of freedom and will. The will is "the law of the supersensible world." Since the will is inseparably connected with action, then, understanding freedom as a due, man becomes free, in other words, for him the goal-setting, rather than the value of the act performed, becomes in the first place. Thus man becomes a step above nature and directs it.

“Gastr versus ratio?”: gastrosophy through the prism of rationality

Abstract

The purpose of this article is to analyze the confrontation between two trends in modern culture associated with food, food traditions, and modern gastronomic consumption practices, expressed by the images of Gastr and Ratio. This analysis is carried out within the framework of a new scientific historical, cultural and philosophical direction, called “gastrosophy”. It is noteworthy that it is precisely at the present time that the philosophical formula ““to be” means “to eat”” acquires such an obvious meaning, creating a new philosophical discourse about food and nutrition, in which they are presented as special aspects of existence and cultural phenomena. The article offers a broad interpretation of the neologism “gastrosophy” as the metaphysics of food. The author shows that the reasons for the opposition between Gastr versus Ratio are connected precisely with the dynamics of the metaphysical principles of food and nutrition.
