Algorithm of oNe-stAge correctioN oF breAst Asymmetry iN PAtieNts with hyPomAstiA

Abstract

We analyzed the results of one-stage correction of the asymmetry of mammary glands in 51 patients with a diagnosis of hypomastia. On the basis of obtained results the algorithm of one-stage restoration of symmetry at the given category of patients, based on the type and severity of the asymmetry was developed, which contributes to the achievement of good aesthetic results in 94.1% of cases.

SPECIAL APPROACHES TO BREAST CANCER HORMONAL THERAPY OF YOUNG PATIENTS

Abstract

Breast cancer is the most prevalent female malignancy. When diagnosed at young age (up to 40 years), negative clinical, morphological and prognostic features are noted. A non-randomized retrospective trial (n=500) was performed to evaluate different scenarios of breast cancer hormone therapy in young patients. Ovarian suppression in young patients is shown to statistically improve prognosis. Disease-free survival rate values are observed to decrease in patients without ovarian suppression in comparison with any type of ovarian suppression, especially at a remote follow-up (after 60 months). Menstrual function resumption and no amenorrhea after chemotherapy significantly decrease disease-free survival rate values in young patients.

EFFECT OF ANTICANCER DRUGS ON PRIMARY CELL CULTURES OF PATIENTS WITH COLORECTAL CANCER

Abstract

The goal of the research is to estimate the effect of 5-Fluorouracil, Cisplatin, Tomudex, Leucovorin and their combinations on colorectal cancer cells in vitro. Cell response to the effect of cytotoxic compounds and immunomodulator was estimated by the change in chromosomal aberrations, cell growth rate, proliferative activity and colonyforming unit. It was detected that antineoplastic drugs affected the mutagenesis and fission rate of cultured cells.

EXPERIENCE OF THE PROCEDURE ADVANCED REAL-TIME AND SHEAR WAVES SPLEEN ELASTOGRAPHYIN MULTIDISCIPLINARY HOSPITAL

Abstract

64 patients were examined to improve technique of shear wave and real-time elastography. There were 11 patients with hematologic diseases, 19 - with infectious diseases, 24 patients with gastroenterological pathology and 10 patients with cardiologic diseases, with primary results of spleen stiffness indicators. According to the results of the studies have been made conclusions about improvement the reproducibility and efficiency of research using advanced techniques. The sensitivity amounted to 93,1%, specificity - to 89,3% and precision - 91,3%, while by using the routine method these indices amounted to 68,2%, 91,4% and 79,4% accordingly.

In the article the modern controlled miniinvasive techniques of acute destructive pancreatitis diagnostic and treatment and its complications are discussed. Perspective miniinvasive techniques as many-frequent bioimpedancemetry and crystallography are described. Different controlled miniinvasive interventions were used in the particular sequence in dependence on clinico-morphological form of pathological process. Endocavital ultrasound research is the perspective area in the development of miniinvasive techniques at the destructive pancreatitis.

MULTIPARAMETER ULTRASOUND FOR LIVER AND PANCREAS IN MODERN CONDITIONS

Abstract

The Aim. To determine the value of multiparametric ultrasonic methods of research in the frame-work of comprehensive elastography.Materials and methods. The study included 256 people: men - 145 (56,7%), women - 111 (43,3%), aged 47,4 ± 5,7, suffering from diffuse liver diseases and pancreas diseases. To determine the diagnostic and prognostic value of a comprehensive elastography use there were performed: transient edastography, compression elastography, endosonography in elastography compression, shear wave elastography.Results. According to the study it was found: the body mass index (more than 25 kg/cm2) is the restriction to conduct transient elastography, compression elastography; heart disorder - compression elastography and compression elastography at endosonography (AUROC = 0,893, [0,878 - 0,979]). Compression elastography of pancreas at endosonography is the method of choice in the differential diagnosis of benign and malignant processes, to clarify the “zone of interest” for puncture-aspiration biopsy (AUROC = 0,981, [0,891-0,982]).Conclusions. Multiparameter ultrasound elastography study complex increases the diagnostic and prognostic value of the algorithm of patients examination with liver and pancreas diseases: specificity- 97,5%, sensitivity - 95,6%, accuracy - 96,5%.

Abstract

The article presents the results of a study of water-soluble polymers effects on the “Photoditazin” activity and the course of wound process under photodynamic therapy of purulent wounds in rats. The data of macroscopic and histological study of wounds status before and after photodynamic therapy, and assessed the quality and degree of intensity of reparative processes under influence of photodynamic therapy with the drug “Photoditazin” and its complexes with water-soluble polymers of natural poly- saccharide chitosan and a triple block copolymer of ethylene- and propylene oxide (pluronic F127).It was given the rationale for Pluronic F127 joining to the drug “Photoditazine” for reducing the cytotoxic effects of the drug.

MODERN TRANSARTERIAL CHEMOEMBOLIZATION AS A STEP TOWARDS THE TREATMENT OF MALIGNANTTUMORS OF THE UTERI AND CERVIX

Abstract

Introduction: Advanced cases of carcinomas with parametrial and fornix infiltration often cause massive genital bleeding, with severe anemia, fast deterioration, and a high risk of death for patients. One method of stoped bleeding is transarterial chemoembolization, which has been studied. Methods: Seven women with advanced uterine and cervix cancer or the pelvic locoregional recurrence withgenital massive bleeding were treated using an endovascular therapy (transarterial chemoembolization (TACE) using cisplatin or doxorubicin or gemcitabine). Ware used HepaSphere (Bioshere Medical, France) filled with saturated 50 mg doxorubicin or 500 mg gemcitabine or 50 mg cisplatin. Results: On average, the total operative time for the procedure was 52 minutes. In all cases, the achieved control bleeding. By the 7th day were signs of reduction in tumor size, 1/3 of its original size. The maximum effect on the tumor size was achieved within 14-20 days. In 2 cases, chemoembolization performed twice at an interval of 21 to 28 days, due to a good picture of the ultrasound tumor vascularization. All operations are carried out under the I/V anesthesia and we had no adverse reactions or complications. Conclusions: Arterial chemoembolization dominant vessels tumor is a reliable method of stopping the genital bleeding at any time during of the planned treatment. It is to improve the quality of life of patients. Antitumor efficacy of drug has a lasting effect. The method is applicable as a minimally invasive, effective for treatment of cancer patients.

RETROPERITONEAL SCHWANNOMA (CLINICAL CASE)

Abstract

Schwannoma - a tumor growing from Schwann cells of the peripheral nerve sheath. Retroperitoneal schwannoma is a rare tumor accounting for approximately 1 to 5% of all retroperitoneal neoplasms. Retroperitoneal localization occurs for a total in 0.75-2.6% of cases, often they localize in the extremi- ties, the head and neck. Despite the significant progress made currently in the field of instrumental studies for non-organ retroperitoneal tumors, diagnosis and differential diagnosis remain a complex problem requiring further study. Presented clinical case illustrates difficulties in the diagnosis of retro- peritoneal schwannoma. Precise preoperative determination of the tumor relationship with all the nearby organs and vessels according to the different methods of ray diagnostics gives an opportunity to define the tactics of treatment.
