Issues on preparation of specialists for medical radioactive and chemical safety in the Armed Forces

Abstract

The unique domestic system of training military doctors for medical provision of radiation and chemical safety has repeatedly proved its effectiveness. The current state of the system of training military medical personnel on the issues of medical provision of chemical and radiation protection is analyzed. It is shown that the provision of medical service needs in such specialists is closely connected with the improvement of the system of rendering medical assistance in the event of radiation and chemical damage. The authors proposed ways to solve the problem. The basis for the preparation of the mobilization reserve for the medical service of the Armed Forces can be training programs for specialists that are united for all military medical educational organizations, including questions of preventive and medical-evacuation measures in emergency situations of civil and military time.

Ways to improve the system of medical support for preparation of young men for military service.

Abstract

The article is devoted to the problem of improvement of the system of medical support for preparation of civilians for military service, raising troops with healthy contingent under complicated social and economic condition. The dynamics of the health status of young men aged 15-16 years was analyzed on the basis of preventive medical examination. The authors proposed measures aimed to minimize costs for additional medical examination of young men and improve prophylaxis effectiveness among them. The authors also proved the necessity of united principals of managing and monitoring within the system of health improvement, implementation of new organizational and legal norms in this sphere and specification of appropriate functions of specialists from different ministries and departments.

Massive recurrent thromboembolism of the pulmonary artery in young people.

Abstract

The description of successful surgical treatment of massive bilateral recurrent pulmonary thromboembolism (with lesions of both major, upper-lobar, lower-lobe and mid-lobe pulmonary arteries) in a young patients with hereditary thrombophilia is described. It is emphasized that the onset of pulmonary embolism (especially repeated) at a young age is often associated with the presence of a congenital defect in the blood coagulation system. It is always justified to search for the cause of pulmonary embolism, which affects the tactics of the patient’s dynamic observation. The modern approach, the availability of high-tech equipped automobile and air transport, highly qualified medical and nursing team allows evacuating a patient with acute acute pulmonary embolism with a high risk of early death from any region of Russia to a specialized medical institution for the provision of timely highly qualified medical care. In this case, it is possible to achieve a satisfactory result of treatment, to reduce the risk of early death and to level out the risk of disability of patients.

State of the hemostasis system in acute massive blood loss.

Abstract

The article examines the changes that arise in the hemostasis system with massive blood loss, and their effect on the severity of the patient’s condition. Acute blood loss leads to activation of the hemostasis system, which is manifested by hypercoagulable changes in the blood. After a few hours, hypercoagulation is replaced by hypocoagulation of varying degrees of severity, which is accompanied by increased intravascular coagulation. Excessive intravascular activation of the hemostasis system leads to the deposition of fibrin in the microcirculatory bed and self-sustaining process - disseminated intravascular coagulation. Severe and extremely severe hemorrhage is accompanied by its development in 38 and 92% of cases, respectively, which indicates the need for laboratory monitoring of the hemostatic system and targeted treatment of hematologic changes in blood.

High resolution ultrasound in the differential diagnosis of malignant skin and subcutaneous tissue.

Abstract

The use of ultrasound linear sensors of high resolution and efficiency of sonoelastography in the diagnosis of malignant tumors of the skin and subcutaneous tissue has been studied. A comparative statistical analysis of ultrasound signs of benign and malignant skin and subcutaneous diseases has been performed. The possibilities of using qualitative and quantitative criteria for the rigidity of formations in the differential diagnosis of this pathology are determined. It is established that the use of sonoelastography improves the accuracy of ultrasound diagnostics. It is proved that the coefficient of deformation of malignant diseases is significantly higher. It was found that to improve the diagnostic efficiency of the study, it is expedient to calculate the boundary values of this indicator separately for skin and subcutaneous foci. A conclusion is made about the prospective inclusion of sonoelastography in the algorithm for the study of patients with suspected malignant pathology of the skin and subcutaneous tissue.

Multidisciplinary approach to correction of computer visual syndrome in military specialists

Abstract

A study was conducted to substantiate and evaluate the clinical effectiveness of a multidisciplinary approach to the correction of computer visual syndrome in military specialists. The essence of the method consists in the combined use of multidirectional technologies of physical impact (low-energy laser radiation, cranial osteopathic effect and special training of the cervicothoracic spine). The received results testify that application of the multidisciplinary approach provides effective correction of computer visual syndrome in military specialists - operators of visually intensive work. This is confirmed by the expressed statistically significant positive dynamics of clinico-functional, ophthalmic-ergonomic and subjective indicators of the visual system after the course of treatment and recovery measures.

Issues on decompression safety of flight crew.

Abstract

The basic approaches to ensuring the decompression safety of the flight crew are outlined. The forms of high-altitude decompression disorders and a list of unfavorable factors that can cause the development of high altitude decompression sickness are given. New views on the prevention of high-altitude decompression disorders are presented. The requirements of compulsory use of protective equipment during the entire flight are justified and occasionally - when the cabin is depressurized at altitudes of more than 12 000 m, oxygen-respiratory apparatus failures, and aircraft abandonment. The algorithm of the medical service’s actions in relation to the flight crew, who suffered a depressurization of the aircraft at high altitudes and in the stratosphere, is given. To maintain decompression safety, it is necessary to perform high-altitude flight crew training. Practical recommendations for the prevention of decompression disorders are given.

Activities of military commissariats and protection of public health.

Abstract

The article presents the results of a study of the participation of military commissariats in the work on protecting public health. Anonymous questioning of 838 medical organizers, including 34 experts, was conducted. It is established that the military commissariat is one of the main 13 sectors interested in protecting public health. A wide range of 37 determinants of health and the importance (on a 10-point scale) of the influence of the military commissariat on each of them have been determined 38 informative indices of the evaluation of the success of public health protection and the possible impact on them of the Military Commissariat are substantiated, which allows implementing targeted corrective optimization activities, including the formation of special preventive programs.

Formation of neuro-emotional stability in cadets of an aviation university during the training

Abstract

The results of the experiment on increasing the effectiveness of the formation of professionally important qualities of the pilot of highly maneuverable aviation with the help of a complex of psychological and pedagogical influence on 87 cadets are presented: 58 - experimental (EG) and 29 - control (KG) group. Modern pedagogical technologies of problematic, computer and modular training were used. At the end of the experiment, the indices in the EG significantly exceeded those in the KG for all personal characteristics, most significantly - for the component «motivation for success». The increase in this indicator in the EG was 82%, in the KG - only 30%. The increase in the indicator «neuropsychic stability» was respectively 38 and 13%. The formation of a complex of professionally important qualities was studied by the estimation of flight training during the flight practice: for the cadets of the EG, it was 4,6 points, which is significantly higher than for the CG students (3-7 points).

Issues on outpatient medical care for the female soldiers with uterine myoma.

Abstract

The experience of medical rehabilitation of patients with uterine myoma at an outpatient and polyclinic stage in St. Petersburg is presented. When conducting a sociological survey of women, including those who are on military service, there are significant psychological problems arising from the established diagnosis of uterine leiomyoma, as well as a lack of information on the prevention and treatment of this disease. The authors of the article proposed a technique for the early medical rehabilitation of patients with uterine leiomyoma, the use of which is advisable when observing female soldiers in departmental medical organizations. The method is based on the individual and public information of patients on the prevention, diagnosis and treatment of uterine fibroids, as well as on providing professional psychological assistance by medical personnel.

Adaptation reactions and their dependence on the period of sanatorium treatment

Abstract

Violation of adaptation makes it difficult to conduct sanatorium treatment and reduces its effectiveness. The article presents the results of studying the development of adverse reactions in response to sanatorium treatment, revealed the links between the periods of sanatorium treatment and the frequency of development of adverse reactions: their highest frequency is noted in the first week of stay in the sanatorium, a slight increase in the second and a new rise in the beginning of the third week. In the early period acute upper respiratory tract virus infections, hypertensive crises and cardiac rhythm disturbances, late acute cerebrovascular disorders, acute coronary syndrome. Data allow planning preventive measures to prevent the violation of adaptation mechanisms.

District military clinical hospital in Khabarovsk celebrates 150th anniversary

Abstract

The history of the 301st district military clinical hospital is inextricably linked with the history of the Khabarovsk Territory, the history of the Far Eastern (now Eastern) military district. The path of 150 years, from the infirmary of the Khabarovsk Postal Command to the modern military medical organization, represents an important fragment of the history of the formation and development of Far Eastern military medicine. Currently, the hospital has in its composition 4 branches and 4 separate structural units located in two regions of the Russian Federation, with a total bed capacity of more than 1 thousand beds. The Military clinical hospital (military district) is a multidisciplinary specialized medical and preventive institution of the medical service of the Eastern Military District, as well as the clinical, educational and scientific base of the Far Eastern State Medical University. After the opening and licensing in 2016 of the department of endovascular methods of diagnosis and treatment, the hospital began to provide high-tech medical care.

First military hospitals in the Russian Far East (end XVIII - middle XIX)

Abstract

Historical facts are presented, revealing little-studied questions of the history of military medicine in the Russian Far East, using the example of the creation and functioning of military hospitals. As far as the development of the Far Eastern lands in the Russian Empire at the end of the XVIII - the middle of the XIX century. On this territory, elements of the military medical service are being created. The role of military doctors in hospitals in the process of developing medical care for civilians was noted. The author concludes that the remoteness of the Far Eastern ports from the supply points and the limited material resources complicated the protection of the health of soldiers and sailors. In the wartime conditions, the organized professional work of military doctors to restore the health of soldiers and sailors made it possible to increase their fighting efficiency, reduce losses and eventually defeat the enemy in the defense of Petropavlovsk in 1854.
