INTEGRAL METHODS OF PROGNOSTICATING THE DEVELOPMENT OF ACUTE PANCREATITIS IN ENDOSCOPIC TRANSPAPILLARY INTERVENTIONS

Beburishvili A.G., Zubina E.N., Turovets M.I., Mandrikov V.V., Vedenin Y.I.

Abstract

Introduction: Many researchers point out the most common complication of endoscopic transpapillary interventions (ETI) is acute pancreatitis. It is necessary to assess the risk of its development in patients to apply the most effective methods of preventing this complication (stenting the main pancreatic duct and thoracic epidural anesthesia). Materials and methods: The present cohort study included all patients (n = 783) who underwent ETI in the period from January 2008 to December 2014. Results: On the basis of mathematical processing of the incidence rate of acute pancreatitis depending on demographics, severity of comorbidity, the nature of the disease, baseline serum amylase and bilirubin of blood in patients we worked out two ways to identify patients at high risk for this complication. Conclusion: The use of the proposed forecasting methods will not only significantly reduce the risk of developing severe complications, but will also help to expand the indications for endoscopic correction of bile hypertension syndrome in patients with diseases of hepatopancreatobiliary zone.

ADMINISTRATION OF THE AGENTS FROM THE GENERAL SALES LIST IN CHILDREN'S SPORTS (OUTCOMES OF THE COMPARATIVE ANALYSIS OF THE VIEWS OF SPORTS MEDICINE DOCTORS, YOUTH COACHES AND FULL-AGED SPORTSMEN)

Vershinin E.G.

Abstract

The survey of 120 sports medicine doctors, 89 youth coaches and 197 full-aged sportsmen showed that administration of agents from the general sales list aiming to enhance the functional capacity of sportsmen often begins under the age of 10 and later keeps increasing. It is difficult to assess their negative effects on health. The parents of youth sportsmen are ill-informed about it and the administration of agents from the general sales list is a risk factor leading to the administration of prohibited drugs. However, youth coaches have a positive attitude towards the general sale list medicines, while sports medicine doctors don't pay proper attention to it.

RELATIONSHIPS BETWEEN EPITHELIAL AND STROMAL CELLS IN DUCTAL BREAST CARCINOMA

Mnikhovich M.V.

Abstract

An increasing number of studies have examined the relationships between epithelial and stromal cells which play a key role in determining a dedifferentiated phenotype of epithelial tumours. The relationships between epithelial and stromal cells in ductal breast cancer are responsible for its aggressive clinical course, resistance to chemotherapy and varying survival rates depending on tumour cell differentiation. The purpose of the present study is to evaluate morphological characteristics of the discrete loci within the tumour with evidence of anaplasia and the surrounding stroma in ductal breast cancer. The study showed that heterogeneity of ductal breast cancer manifests primarily in sarcoma-like or anaplastic component, in which epithelial-mesenchymal transition occurs. Mesenchymal cells with no stroma are considered to be potential precursors for aggressive cancer.

ANALYSIS OF THE RISK OF DEVELOPING POSTOPERATIVE THROMBOEMBOLISM IN ELDERLY PATIENTS UNDERGOING JOINT REPLACEMENT

Ryazantsev D.I., Prokhorova M.Y., Chensky A.D., Petrov N.V., Kavalersky G.M., Zarov A.Y., Katunyan P.I.

Abstract

The analysis included 141 patients aged 60-90 years who underwent 78 hip and 63 knee joint replacements. 12 patients in the early postoperative period were found to have thrombosis of the superficial and deep veins of the lower extremities. 2 out of 12 patients required surgical treatment. To prevent venous thromboembolism, the patients received new thrombolytic agents which have been successfully used in many European clinics. The analysis showed that the likelihood of thrombosis is directly proportional to the duration of surgery and patient's body weight. Blood clotting factors including INR, PTT, thrombin time, prothrombin and fibrinogen as well as the extent of intraoperative blood loss are not suited to predict postoperative thrombotic complications.

CHANGES IN CELL-MEDIATED IMMUNE REACTIONS IN GINGIVAL FLUID IN PATIENTS WITH CHRONIC GENERALIZED PERIODONTITIS ACCOMPANIED BY METABOLIC SYNDROME WHEN MEMBRANE PLASMAPHERESIS IS ADDED TO COMPLEX THERAPY

Starikova I.V., Piterskaya N.V., Trigolos N.N., Radyshevskaya T.N., Aleshina N.F., Chaplieva E.M.

Abstract

We studied the changes in local immune reactions in gingival fluid samples obtained from 40 patients aged 35- 55 years with chronic generalized periodontitis (CGP) and metabolic syndrome (MS) to add membrane plasmapheresis (PA) to standard care. The patients were divided into two groups. The first group received standard treatment. The second group received membrane plasmapheresis in addition to standard therapy. The analysis of the local immunogram obtained from patients with chronic generalized periodontitis of moderate severity accompanied by metabolic syndrome showed significant changes in nonspecific immunity. After completing the standard treatment, the immune parameters of the patients in the first group failed to reach the same levels as in the control group, which was accounted for by compromised immunity and especially its local component. Long-term results show that patients in the first group are more likely to develop acute inflammation in the periodontal tissues. In the group of patients where membrane plasmapheresis was used, cellular immunity did not significantly differ from that in the control group.

ASSESSMENT OF THE EFFECT OF PYATD1, THE NEW DERIVATIVE OF DIAZINON, ON SOME PERIPHERAL BLOOD INDICATORS IN RATS OF BOTH SEXES

Voronkov A.V., Luzhnova S.A., Osychenko S.A., Gabitova N.M.

Abstract

The experiment on 60 Wistar line rats of both sexes was conducted to perform a comparative analysis of the effect of the administration of the new derivative of diazinon encoded as PYaTd1 and the drug of dapsone on the peripheral blood hemogram and platelet parameters. The obtained findings demonstrated that the course administration of the drug of dapsone had a toxic effect on hematologic parameters of peripheral blood, morphological and functional characteristics of red blood cells and megakaryocytopoiesis in rats of both sexes, whereas when the PYaTd1 compound was administered these adverse effects were registered, which makes its further development promising.
