PATHOLOGICAL CHANGES IN THE LUNGS DURING TUBERCULOSIS AT DIFFERENT STAGES OF HIV INFECTION

Byhalov L.S., Smirnov A.V.

Abstract

We revealed pathological changes in the lungs of patients who died from tuberculosis and HIV infection considering the immunological stage of the process. The structural changes were multidirectional and with CD 4+ > 200 cells/mcl (mediana is 356,2 cells/mcl) they were more similar to the pathomorphology of pulmonary tuberculosis without HIV-infection with limited foci of inflammation. As the secondary immunodeficiency progressed the number of CD 4 + ranged from 200 to 100 cells/mcl (mediana is 148,4 cells/mcl), clinical and morphological forms of pulmonary tuberculosis with destructive component prevailed and there was a tendency to generalization and inflammatory changes with exudative component. At the terminal stage CD 4+ lymphocyte count was less than100 cells/mcl (mediana is 49,4 cells/mcl), generalized forms of 2-sides milliary tuberculosis as well as alterative reactions in lung parenchyma and massive vasculitis in the microvasculature prevailed.

Abstract

The effects of immunomodulating therapy with polyoxidonium on the immune system of elderly patients with odontogenic maxillofacial phlegmon has been studied. The authors have shown that the use of polyoxidonium in the treatment of odontogenic maxillofacial phlegmon allows for a progressively increasing resistance through stimulating the mechanisms of innate immunity and accelerating up-regulation of immune cytokines. The authors suggest that polyoxidonium boosts the effectiveness of treatment and reduces the length of hospital stay.

RELATIONSHIP OF MITOCHONDRIA REACTIONS AND DISTRIBUTION OF GAP JUNCTIONS IN CONTRACTILE CARDIOMYOCYTES IN POSTNATAL ONTOGENESIS INDUCED BY EXPERIMENTAL CHRONIC INTRAUTERINE HYPOXIA

Petruk N.S., Ivanchenko M.V., Tverdokhleb I.V.

Abstract

The aim of this study was to determine the relationship between mitochondrial reactions and the distribution of gap junctions in contractile cardiomyocytes in postnatal ontogenesis experimentally induced by chronic intrauterine hypoxia. Electron-microscopic analysis of rats' ventricular myocardium showed that more than 40 % of the length of gap junctions conjugated with local mitochondrial clusters. Subsarcolemmal mitochondria were closely linked with gap junctions forming gap junction-mitochondria complexes. The interaction of gap junctions and mitochondria is conditioned by their morphological and functional features and their spatial relationship both in a mature myocardium and in the developing heart.

OPPORTUNITIES OF PRENATAL ULTRASONOGRAPHY IN DIAGNOSTICS OF THE RIGHT ARCH OF THE AORTA

Altynnik N.A., Shatokha Y.V., Medvedev M.V.

Abstract

The ultrasound findings were retrospectively evaluated in 38 fetuses with the right aortic arch (RAA) diagnosed at 16- 34 weeks of gestation. The ultrasound examination revealed that all 38 fetuses with RAA had a U-shaped confluence of the ductus arteriosus with the descending aorta on the three vessels and trachea view. The trachea was located between the vessels. In 63,2 % of cases the diagnosis of PDA was made before 22 weeks and in 36,8 % the diagnosis was made after 22 weeks of gestation. In 36 (94,8 %) cases abnormal position of the cross section of the thoracic aorta was identified; however, abnormal position of the axis of the heart were reported only in 4 (10,5 %) out of 38 fetuses.

ON THE POSSIBILITY OF DEVELOPING A NEW DRUG-ELUTING BILIARY STENT

Beburishvili A.G., Zaporotskova I.V., Spiridonov E.G., Mandrikov V.V., Shinkarev R.A.

Abstract

On the basis of experimental studies we proposed a technology of developing thin-film coated polymer stents for bile duct replacement based on a polymeric substance of polivinillpirrolidon (PVP), saturated with doxorubicin. The developed thin-film coating of an experimental model slows down the bile crystallization process on the surface of a stent sample. We proposed and studied the potential mechanism of formation of a stable PVP-doxorubicin adsorption complex for gradual delivery of doxorubicin into the body and its prolonged action. We analyzed the different types of molecule orientation of doxorubicin in relation to PVP adsorbent (during their interaction), to calculate the main energy characteristics of adsorption processes. We found that the interaction through an oxy group of the doxorubicin molecule is energetically a more favorable option. The proposed technology of applying thin-film drug coating can be introduced as quantum chemical calculations were made and it can become a basis for further research.
