The main approaches and methods to manage the process of rehabilitation injured at work
Abstract
The article is devoted to one of the urgent problems related to health and working capacity of the working population -the creation of a system of medico-social rehabilitation of injured at work. The results of the analysis of the current system of rehabilitation of injured at work. From the standpoint of the International classification of functioning, disability and health identified, described and systematized the main obstacles and shortcomings of the system, which are marked as: organizational, personnel, educational, technological, methodological and motivational factors. Organizational improvement of the system proposed by creating a system of rehabilitation management, implementing strategic, tactical and operational-coordinating the management of the rehabilitation process. The main aim of the activities should be the creation of the rehabilitation system focused on the needs of a particular injured at work. Asked to identify the main subprocesses in the system of tactical management process of rehabilitation of injured at work such as: the provision of inpatient care; early rehabilitation; "late" rehabilitation and "passive" rehabilitation. Identified key activities within each subprocess. The proposed structural changes should increase the effectiveness of rehabilitation of injured at work and significantly increase the percentage of returnees to employment, which in turn will help to reduce the financial burden on the working population as a whole.

Assessment of the method of functional taping on bioelectric activity of muscles of shoulder patients with pain syndrome
Abstract
The article presents the research results of influence of the method of functional taping on the contractile ability of muscles in patients with humeroscapular pain syndrome. In the study participated 20 patients with diagnosed reliably humeroscapular pain syndrome in the acute stage. In the main group (13) were subjected to functional taping with a moderate degree of tension of elastic bandages (25-50%) in the control group (7 people) - the degree of tension was maximum (100%). The degree of tension of the patch was estimated from the increase in original length at 140% with a maximum tensile and 120% with moderate. To analyze the efficiency of the method of functional taping were compared with quantitative changes in bioelectrical activity of the shoulder muscles using surface EMG. As a result of the study revealed that the functional application of the tape lead to the decrease of quantitative indexes of contractility of muscles. The degree of tension of the elastic bandage had no effect on changes in the indices of electromyography signal. Thus, functional taping contributes additional support from interested muscle groups and can be used as an alternative to other elastic bandages.

The effectiveness of dynamic electroneurostimulation (DENS) In the pain management in knee osteoarthritis (results of a multicenter randomized study)
Abstract
Objective to investigate the effectiveness and safety of the DENS therapy in knee osteoarthritis. Subjects and methods. The present study is a multicenter randomized double blind placebo-controlled investigation. The study include 132 subjects. Inclusion criteria were: the diagnosis of the knee osteoarthritis according to the ACR (1987) criteria, the pain intensity more than 40 mm by the visual analogue scale, the Lequenne index between 4 and 12. The DENS group (66 subjects) received the DENS treatment on the target knee, by the applicator, connected to DENAS-PCM device, with the stimulation parameters - 77 Hz frequency and 15 mA intensity of the current. Patients in the placebo group was carried out identical to the procedure included AP-drugs-placebo, which by external signs did not differ from working. All audio signals and the display were similar. The placebo did not have any electrical impulses, therefore, had no impact on the patient. The treatment duration was 30 minutes at the 10 sessions. The outcome measures were: a pain knee intensity measured by the visual analogue scale (VAS). Results. In both groups there was a statistically significant reduction in pain in VAS by the end of treatment (p<0.0001). A statistically significant difference in two groups in VAS pain was noted after the treatment on the first (p=0,014), third (p=0,018), sixth (р=0,047) therapy visits, and after the two weeks of the end of the treatment (p=0,003). There was not statistically significant difference in the use frequency of NSAIDs. Both the physician and the patient significantly positively evaluated the effectiveness of the DENAS-PCM treatment in the DENS group (the evaluation by the patients p=0,004; by the physician p <0.0001, in comparison with the placebo group). Conclusion. Dynamic electroneurostimulation is an effective method of immediate and prolonged pain relief in patients with knee osteoarthritis.

Rehabilitation of elderly patients with systemic osteoporosis through intraosseous reinforcement of the proximal part of the femur to prevent pathological fractures. Experimental study
Abstract
The work is devoted to the problem of rehabilitation of elderly people suffering from various diseases, causing destructive-dystrophic changes in the bone such as osteoporosis, cancer and complications that cause pathological fractures, as well as the study of experimental methods of preventive reinforcement of the proximal part of the femur. We have developed the methodology of preventive reinforcement and construction of the original implants for her conduct, to which the patents of the Russian Federation (No. 2316280, 91845, 98901, 101,351, 121,725, 136,703, 136,704, 140684). Manufactured prototypes of implants of nano-titanium and set of tools for the operation of preventive reinforcement. For the study of biomechanical properties of bone tissue, as well as changes in strength characteristics as a result of the introduction of implants in bone us studies by mathematical modeling, which proved that the voltage at the load in the area of the proximal part of the femur occurs unevenly. This leads to the formation of zones started destroying the bones as the load increases, and after exceeding the critical voltage rise to fracture. In points of destruction reinforced systems bone-implant studies have shown that when critical loads, the voltage is reduced by 11.6% -12.1 in contrast to the intact samples of the bone. Bench testing for the purpose of studying the changes of strength reinforced system of bone-implant in comparison with healthy samples of bones show increased resistance to fractures of the proximal femur reinforced on 23-93% depending on the form and combination used implants. As a result, tests revealed that bone reinforced even after the appearance of the fracture is not accompanied by the fragment offset, which held vnutrikostnymi implants. Studies are entitled to argue that when there is a low-energy trauma, bone-implant system less prone to fracture.

Computer stabilometry in the diagnosis of postural disorders in children with uncomplicated compression fractures of the spine
Abstract
Investigated postural dysfunction in children with compression fractures of the thoracic spine using computer stabilometry. Analyzed stabilometric characteristics in different periods from the date of injury. Comparison of indicators with similar in children with conditional norm. In the study group, children with a date of injury until 12 months, significant differences from conventional norms the increase in the area of the ellipse, the standard deviation in the frontal plane in the test "Target", reducing the value of the quality of the equilibrium function. In the study group, children with a date of injury more than 12 months, significant differences from conventional norms: the area of the ellipse and the reduction of the compression ratio. Stabilometry allows you to appreciate the depth of postural disorders in children with compression fractures of the spine, and can be used to predict and assess the effectiveness of medical rehabilitation.

Treatment of pain in the heel area by means of extracorporeal shock wave therapy
Abstract
In the article there is given an example of diseases with the similar clinical pattern, with the pain in calcaneus area. The differencial analysis of occasion the pain in calcaneus was done. The success of the treatment the pain in calcaneus area depends directly from the right diagnostics. Shock-wave therapy, corrective shoes, dosed load on foot prevent acute and chronic pain in calcaneus area.

Medical rehabilitation for victims of terrorism
Abstract
Each year, an increasing number of acts of aggression, leading to loss of life and causing instability in the society. According to the provisions of the Military Doctrine of the Russian Federation, the world is expected to increase the frequency of the use of precision-guided weapons. At the same time one of the main external threats is the rapid spread of international terrorism, because of which suffers mostly civilians. The purpose of this work - the identification of symptoms of acute stress disorder in victims of terrorist attacks and the analysis of the model Brigadier providing psychological and psychiatric assistance. Results. In this article the necessity of beginning a medical rehabilitation of victims directly at the scene of the terrorist attack and proved the effectiveness of the brigade model of providing psychological and psychiatric assistance. The features of the diagnosis of acute stress reactions among victims and witnesses of acts of terrorism, stressed the importance of this knowledge for specialized doctors. Conclusion. The results of the psychological and psychiatric teams have proven the effectiveness of the model Brigadier emergency psychological assistance.

Working with children who are in profound changes state of consciousness after severe trauma brain. Clinical anthropocentric approach
Abstract
This article explains the importance and the necessity of a conceptually new method of rehabilitation of children in reduced state of consciousness after severe brain injury. During the period from 2014 to 2015 29 children in a reduced state of consciousness have been treated in the Clinical and Research Institute of Urgent Pediatric Surgery and Trauma. 14 children ages 7 to 17 have been passed neurorehabilitation course in an approach of clinical anthropocentrism. Using comparative method the authors present the results of the recovery process of working with children in reduced state of consciousness, with the use of clinical and anthropocentric approach and standard comprehensive rehabilitation. The results of the two groups of patients were evaluated on the parameters: 1. Level of consciousness - GCS, Scale of Dobrokhotova-Zaitsev, Scale communicative activity (SCABLn). 2. Motor opportunities - The scale of the Committee for Medical Research (for R.Van der Ploeg, 1984), Scale of spasticity Ashfort. The methodology of the clinical anthropocentrism is based on: A. The priority of the individual necessities and possibilities of the patient B. Identification and registration of "The Zone of Proximal Development" of the patient (by Vygotsky) C. The primary principle of the work is "the processuality but not situationality." D. No strict instructions in training. The dialogue between the rehabilitation team and the patients in altered states of consciousness in the paradigm of clinical anthropocentrism allows us to see, to understand and to "return" the missing puzzles in the mosaic of destroyed and/or lost patient’s internal integrity. The summary table shows the data of two children from each group. Both children were discharged from the hospital with the improvement in their condition and in the level of consciousness, and in motor abilities. At the same time, the authors have shown that the use of clinical anthropocentrism in dealing with patients, who are in deep altered states of consciousness, is not only possible but also necessary.

Priority issues patients with post-traumatic lover limb amputations
Abstract
In figurative expression Rene Baumgartner and Pierre Botha, 2002 "Treatment does not end with amputation, and only begins with it." The consequences of traumatic amputations of the lower limbs have a long list of early complications such as traumatic disease, visceral diseases, psychological disorders, malformations and diseases of the residual limb, associated head injury, the contralateral limb, arms, torso. The severity of the herbs and then determines the number of complications, such as metabolic disorders, ateroskroticheskie vascular changes, coronary heart disease, pustular, and fungal diseases of the stump, etc. As a result of the severity of life progresses restrictions formiru4yutsya risks for even more late complications. This reduces the quality of life of these patients. The introduction of the system of expert and rehabilitation diagnostics, which allows not only to identify with the anatomical and functional disorders, but also prevent the development of long-term complications, eliminate or minimize the consequences of their manifestations. In this regard, urgent is the development programs of the priorities of the rehabilitation treatment and social adaptation at different stages of the rehabilitation process with emphasis on the determination of their volume, duration, rehabilitation goals, personification to improve the quality of life.

Among these methods, it is necessary to include physical therapy, implementation of which, according to clinical studies, increases the regenerative function of bone tissue, as well as increase the periods of remission after complex treatment. The effectiveness of development and application of these methods is largely determined by organizational and economic forms of organization studies and their introduction into clinical practice.

Efficacy of non-pharmacologic treatment of psycho-emotional disorders in the third trimester of pregnancy (literature review)
Abstract
There is a growing number of women worldwide, who experience anxiety disorder during the whole period of pregnancy and take psychotropic medications. Obstetricians, pediatricians, neonatologists and general practitioners are seriously concerned and are interested in alternative methods of psycho-emotional state correction in pregnant women, as pharmacological treatment is considered unsafe for the fetus. The assessment of clinical-pathological characteristics and personality traits during normal pregnancy revealed an increase of trait and state anxiety by the third trimester. High anxiety level during pregnancy has a widely accepted adverse influence on maternal and fetal physiologic state and correlates, according to many researchers, with intense fear of childbirth. It is well-known that women with anxiety and depressive disorders are prone to childbirth fear (tokophobia), compared to women with no mental disorders. There arises an intensive need for the evaluation of non-pharmacologic treatment efficacy of psycho-emotional states during pregnancy and for the development of psychotherapy efficacy criteria in women during the third trimester of pregnancy. The article reviews different non-pharamcologic treatment modalities aimed at the improvement of psychological functioning of pregnant women. Authors emphasize lack of evidence-based data on comparative efficacy of different methods of prenatal care. Authors postulate there is a need for the development of efficacy criteria for prenatal care methods, including objective and subjective, patient-oriented. The main efficacy criterion should be based on comparative data, demonstrating significant differences in subjective well-being and behavior of pregnant and parturient women both according to obstetricians of maternity wellbeing centers, delivery rooms and on the retrospective self-assessment made by women 3-4 days and 1-2 months after delivery.

The influence of alcohol abuse on the hair content of chemical elements in pregnant women
Abstract
The purpose of the study was to investigate the effect of alcohol on the hair content of chemical elements in pregnant women. The hair content of spectrum of minerals and trace elements (22 elements) were analyzed by mass spectrometry with inductively coupled plasma. The study involved pregnant women with alcohol abuse, as well as control; hair samples were taken in the III trimester. Statistically significant two-fold increase in the level of lithium in the hair samples of women from the risk group compared with control values was noted. The biological role of lithium still contradictorily characterized: on the one hand it is a toxic metal having pharmacological activity, on the other hand is shown its essentiality. Thus, changes in the hair level of lithium in the pregnant women with alcohol abuse should be assessed as a problem that requires careful medical supervision. No significant differences in hair content of essential trace elements between the two groups as a result of alcohol abuse has been identified. The correlation analysis showed the presence of a number of parameters relating to alcohol consumption and the content of certain chemical elements. Most interesting is the significant correlation of potassium concentration with the age of first use of alcohol in the both risk group and group of healthy pregnant women, it confirms the data obtained by other authors. Further studies with involvement of larger number of persons will help to identify more close relationships between elemental status of the organism and the abuse of alcohol by pregnant women. Detailed data allows more effectively prevent the negative effects of alcohol abuse during pregnancy on the fetus health.
